  • Progress of reconstruction in bone tumor surgery

    Bone tumor surgery involves tumor resection and subsequent reconstruction. With the development of surgical technique and new material, there is a great step toward bone and joint reconstruction in bone tumor surgery. Generally speaking, there are two major reconstructive methods including bio-reconstruction and mechanical reconstruction. In addition, three-dimensional printed prosthesis has been widely applied in the field of bone tumor surgery. The short-term result is encouraged; however, long-term results and related complications are seldom reported.

  • Effectiveness and mechanism of pure platelet-rich plasma on osteochondral injury of talus

    Objective To explore the effectiveness and mechanism of pure platelet-rich plasma (P-PRP) on osteochondral injury of talus. Methods Thirty-six patients with osteochondral injury of talus selected between January 2014 and October 2017 according to criteria were randomly divided into control group (group A), leukocyte PRP (L-PRP) group (group B), and P-PRP group (group C), with 12 cases in each group. There was no significant difference in gender, age, disease duration, and Hepple classification among the three groups (P>0.05). Patients in the groups B and C were injected with 2.5 mL L-PRP or P-PRP at the bone graft site, respectively. Patients in the group A were not injected with any drugs. The American Orthopaedic Foot and Ankle Society (AOFAS) score and visual analogue scale (VAS) score were used to evaluate the effectiveness before operation and at 3, 6, and 12 months after operation. Study on the therapeutic mechanism of P-PRP: MC3T3-E1 cells were randomly divided into control group (group A), L-PRP group (group B), and P-PRP group (group C). Groups B and C were cultured with culture medium containing 5% L-PRP or P-PRP respectively. Group A was cultured with PBS of the same content. MTT assay was used to detect cell proliferation; ELISA was used to detect the content of matrix metalloprotein 9 (MMP-9) protein in supernatant; alkaline phosphatase (ALP) activity was measured; and real-time fluorescence quantitative PCR (qRT-PCR) was used to detect the expression of osteopontin (OPN), collagen type Ⅰ, and MMP-9 in cells. Western blot was used to detect the expression of MMP-9 in supernatant and phosphoinositide 3-kinase (PI3K), phosphorylated protein kinase B (pAKT), and phosphorylated c-Jun (p-c-Jun) in cells. ResultsAll patients were followed up 13-25 months, with an average of 18 months. No complication such as wound infection and internal fixation failure occurred. MRI showed that the degree of injury was similar between the three groups before operation, and patients in the three groups all recovered at 6 months after operation. Moreover, group C was superior to groups A and B. Compared with preoperation, AOFAS scores and VAS scores in the three groups were all significantly improved at each time point after operation (P<0.05). AOFAS score of group C was significantly higher than that of groups A and B at 3, 6, and 12 months after operation (P<0.05); there was no significant difference in VAS score between the three groups (P>0.05). Study on the therapeutic mechanism of P-PRP: The absorbance (A) value, ALP activity, the relative mRNA expression of OPN and collagen type Ⅰ in group C were significantly higher than those in groups A and B (P<0.05), and those in group B were significantly higher than those in group A (P<0.05). The relative expression of MMP-9 protein and mRNA and the content of MMP-9 protein detected by ELISA in group B were significantly higher than those in groups A and C, while those in group C were significantly lower than those in group A (P<0.05). Western blot detection showed that the relative expression of PI3K, pAKT, and p-c-Jun protein in group B was significantly higher than those in groups A and C (P<0.05), but there was no significant difference between groups A and C (P>0.05). Conclusion P-PRP is superior to L-PRP for osteochondral injury of talus, which may be related to the inhibition of PI3K/AKT/AP-1 signaling pathway in the osteoblast, thereby reducing the secretion of MMP-9.

  • EXPERIMENTAL STUDY ON SIMVASTATIN IN PREVENTION AND TREATMENT OF ASEPTIC LOOSENING OF PROSTHESIS

    Objective To evaluate the mechanisms of p42/p44 kinase phosphorylation in cell models and to investigate the effect of simvastatin on the prevention and treatment of aseptic loosening of prosthesis by observing the influence of simvastatin on the levels of tumor necrosis factor α (TNF-α) and monocyte chemoattractant protein 1 (MCP-1) of human peri pheral blood mononuclear cell (PBMC) challenged with titanium particles. Methods PBMC from 45 mL peripheral blood of healthy adult voluntary donators, were separated and cultured, and divided into 5 groups according to different culturemedium: group A, PBMC and titanium particles; group B, PBMC and titanium particles with 1 × 10-5 mol/L simvastatin; group C, PBMC and titanium particles with 1 × 10-6 mol/L simvastatin; group D, PBMC and titanium particles with 1 × 10-7 mol/L simvastatin; and group E, PBMC and titanium particles with the extracellular signal-regulated kinase (ERK1/2) inhibitor U0126. The contents of TNF-α and MCP-1 were tested by ELISA after 24 hours of culture. PBMC were pretreated with different medium grouping as groups A, B, C, D, and E for 60 minutes, and were challenged with titanium particles for 30 minutes and 60 minutes, then the level of ERK1/2 expression was tested by Western blot. Results In groups A, B, C, D, and E, the absorbance (A) values of TNF-α were 1.115 5 ± 0.243 6, 0.693 6 ± 0.354 3, 0.695 7 ± 0.387 3, 0.716 4 ± 0.478 9, and 0.263 5 ± 0.101 6, respectively; and the A values of MCP-1 were 1.421 0 ± 0.105 3, 0.915 1 ± 0.411 3, 1.003 5 ± 0.464 2, 1.102 0 ± 0.353 9, and 0.271 3 ± 0.145 1, respectively. The levels of TNF-α and MCP-1 in group A were significantly higher than others, showing significant differences (P lt; 0.05). There were significant differences between group E and groups B, C, and D (P lt; 0.05), between group B and groups C, D (P lt; 0.05); no significant difference between group C and group D (P gt; 0.05). Western blot results showed the expression of ERK1/2 in all groups at 30 minutes and 60 minutes of culture. The levels of ERK1/2 expression were 1.612 1 ± 0.068 2, 1.078 1 ± 0.072 8, 1.268 7 ± 0.223 1, 1.439 7 ± 0.180 1, and 0.732 0 ± 0.110 4 in groups A, B, C, D, and E, respectively; showing significant differences between groups (P lt; 0.05). Conclusion ERK1/2 is a phosphorylated protein after stimulated by wear particles; it is also one of the most important cell signal ing activation of macrophage. Simvastatin can inhibit the expression of bone absorptive factors induced by wear particles and may be used in the prevention and treatment of aseptic loosening of prosthesis.

  • ALLOGRAFT OF CLAVICLE FOR RECONSTRUCTION OF BONE DEFECT AFTER TUMOR RESECTION

    【Abstract】 Objective To evaluate the effects of allogeneic bone graft for reconstructing bone defect of clavicle aftertumor resection. Methods Nine patients received the surgical procedures for repairing and reconstructing bone defect afterresection of clavicle tumor from January 2000 to January 2006. There were 4 males and 5 females aged 12 to 68 years. Two patientswere diagnosed as having benign tumor and tumor-like lesion(stage Ⅲ according to Campanacci grading), and 7 as having primarymalignant tumor and metastatic cancer. Three lesions were located in acromial end, 2 in sternal end and 4 in shaft of clavicle. Twocases was accompanied by pathological fracture. According to Enneking’s functional evalutation, the results were excellent in 3 cases,good in 3 cases, fair in 1 case and poor in 2 cases. Based on the visual analogue scale (VAS), pain was rated as medium rest-pain in4 cases, as medium move-pain in 3 cases and heavy pain in 2 cases preoperatively. The tumor was resected with en bloc. Defectedclavicles were reconstructed by a part of allograft clavicle and plate. The patients with malignant tumor received neo-adjuvant chemotherapyand radiotherapy postoperatively. The results were evaluated according to control of tumor, rel ief of pain, bone healingand shoulder joint function. Results These patients were followed up 12 to 70 months (mean 34.6 months). There were no localrecurrence and metastases in all patients during the follow-up. No complications of immune rejection and deep infection wereseen. The normal shape of the shoulders was restored. Eight of all patients showed primary bone healing between allograft boneand host bone. The mean healing time was 5.4 months (3.2-6.8 months). Loosening of the plate and nonunion were seen in 1 case;and bone grafting was given again. Fracture of allograft occurred in 1 case after removing internal fixation. According to Enneking’s functional evulation, the results were excellent in 6 cases and good in 3 cases at 3 months after operation; excellent in 4 cases, goodin 1 case and fair in 1 case among 6 patients who were followed up for 24 months. The VAS results of postoperation showed l ightmove-pain in 2 cases at 3 months, l ight move-pain and medium move-pain in 1 case respectively and no pain in 7 cases 12 months after operation. Conclusion Allogeneic clavicle is a useful and rel iable reconstructive material for clavicle defect due to tumor resection. The satisfactory shape and favorable function of the shoulder may be recovered after this procedure.

  • FABRICATION OF CUSTOM-MADE ARTIFICIAL SEMIKNEE JOINT BASED ON RAPID PROTOTYPING TECHNIQUE:COMPUTER-ASSISTED DESIGN AND MANUFACTURING

    Objective To design a new custom-made artificial semi-knee joint based on rapid prototyping(RP) technique and to explore a method to solve necroses of allocartilage in hemi-joint allotransplantation. Methods Based on the extracted 3D contour image of the articular cartilage of femoral condyle, the custom-made artificial semi-knee joint was designed with Surfacer 9.0 image processingsoftware. The artificial semi-knee joint design used the femoral condylar 3D contour of the patient as the outer face and the subchondral bone 3D contour of allograft bone as inner face. One dado for medullary nailand two for special designing cages which were used to fix the cartilage into the allograft were added on the inner face. After being converted into RP data format, the computerassisted design was imported into the LPS600 rapid prototyping machine, and the prototype was achieved. Furthermore, the prototype could be modified by hand according to the design. Then the RP model was used as a positive mould to build up a silica gel negativemould, and the negative mould was sent to the factory to manufacture Ti-6Al-4V alloy articular cartilage through ordinary mould-melted founding process. Finally, the whole metal cartilage was completed after melting two special cages on it andpolishing it. Results A new custom-made artificial semi-knee joint was made ad used to treat a 14-year old patient. The custom-made artificial semi-knee joint and the subchondral bone were a perfect match. The operative result was satisfactory. The patient could walk 5 weeks after operation. The bone healing of the auto-bone and allo-bone was achieved 6 months later. A follow-up period lasting 1 yearshowed that the knee joint played a good function. Conclusion The artificial semi-knee joint is a good match for the allograft boneand a good idea to solve necroses of allocartilage in hemijoint allotransplantation. 

  • IMMUNOLOGICAL RESEARCH OF RAT OSTEOBLAST XENOTRANSPLANTATION

    OBJECTIVE To detect the immunoreaction after osteoblast xenotransplantation and to investigate the possibility of heterogenic osteocyte transplantation and tissue engineered bone reconstruction. METHODS: Rat osteoblasts were isolated by two-part bony digestion/elements in culturing, and incubated in vitro at 37 degrees C, 5% carbon dioxide for 5 days until they multiplied and formed a monolayer on the bottom of dish. Twenty-eight rabbits were divided into 3 groups. Autograft of osteoblasts(group A), xenograft of osteoblasts(group B) and normal saline(group C) were implanted into the rectus abdominus muscle. The immunological and histological observations were performed after 1, 2, 4 and 8 weeks of transplantation. RESULTS: Cultured cells reached confluence within 5 days and was identified as osteoblasts by ALP staining and Bon kossa staining. The result of host versus graft reaction was negative. In group B, specific antibody reaction was detected 2 weeks and 4 weeks after transplantation. Cell mediated cytotoxicity was detected after 2 weeks, reached the peak value 4 weeks later, and then began to decline 8 weeks later. HE staining showed mass inflammatory cells and no ectopic ossification after 8 weeks. CONCLUSION: Heterogenic osteoblast transplantation will lead to an obvious change in host humoral and cellular immunity and lost the ability of bone formation. So, it can not be used for the reliable cell sources for osteocyte transplantation or tissue-engineering bone reconstruction.

  • A Randomized, Double-blind Controlled Trial of Indigenous Leflunomide in the Treatment of Rheumatoid Arthritis

    Objective To assess effectiveness and safety in rheumatoid arthritis (RA)patients treated with the indigenous leflunomide in comparison with Airohua produced by Cinkate Corp. Methods A double blind and double dummy randomized controlled trial was conducted in the outpatient clinic of rheumatology. Fifty-six Chinese RA patients were divided into two groups who received either indigenous Leflunomide or Airohua of 20 mg daily and the two matching placebo tablets of Airohua or indigenous Leflunomide with the constant dose of Oxaprozin (0.4 g, qd) in the first 6 weeks. Clinical and laboratory data were collected every 6 weeks during 24 weeks of follow-up. Results This study showed that the patients of both groups significantly improved on main variables except erythrocyte sedimentation rate (EXR) at the 12th week . The variables such as ESR, and the level of c-reaction protein in both groups at 24th week showed no statistical difference compared with those at the baseline. There were also no statistical difference in the clinical characteristics and laboratory findings between the two groups at the 12th week and 24th week (P 〉0. 05). The effcacy of Airohua(37% ) was lower than that of indigenous Leflunomide(64% ) at the 12th week (P 〈0. 05 ) and slightly better than indigenous Leflunomide group at the 24 week (81.5% vs 77.3% , ( P 〉0. 05). The administration of Airohua and indigenous leflunomide presented a similar pattern and frequency of adverse events. Serious adverse events such as interstitial pneumonitis, pulmonary infiltration, and liver impairment etc. were not reported in either group. Conclusions The study shows that the indigenous Leflunomide has a superior therapeutic effect. Its efficacy and safety are similar to Airohua in the treatment of acfive RA patients in China.

  • A retrospective survey on epidemiological characteristics of emergency department patients

    ObjectiveTo investigate the epidemiological characteristics of patients in the emergency observation room of Beijing Shijitan Hospital during the last 5 years, to enhance the local and regional construction of education discipline and improve the quality of medical services.MethodsA retrospective study was carried out on selected patients admitted to the emergency observation room of the Capital Medical University Affiliated Beijing Shijitan Hospital between January 1st 2014 and December 31st 2018. Statistical analysis was performed to identify associated epidemiological features in these patients.ResultsAmong the 19 518 patients admitted to the emergency observation room, 49.08% were males and 50.92% were females, with a male to female ratio of 1:1.04. The 46-65 age group had the lowest proportion of females of 47.28%, while this proportion reached to 55.38% in the over-80 age group, which was higher than that in the 46-65 and 66-80 age groups, showing statistical significance (P<0.05). During the last 5 years, 59.66% of the patients were aged above 65; the proportion of over 65 years old patients admitted to the emergency observation room of the hospital each year was 53.86%, 53.19%, 57.00%, 65.40%, and 68.79%, respectively, showing an increasing trend (P=0.011). Among the patients diagnosed, 24.20% had respiratory system diseases, 23.67% had circulation system diseases, 14.47% had digestive system diseases, 12.75% had neurological system diseases, and 7.08% had endocrine system diseases. According to the primary diagnosis, the top 10 most predominant diseases were bacterial pneumonia (14.72%), acute and chronic heart failure (7.86%), hypertension (5.00%), arrhythmia (4.02%), coronary atherosclerotic heart disease (3.92%), chronic obstructive pulmonary disease (3.01%), cerebrovascular disease (2.68%), gastrointestinal hemorrhage (2.66%), acute diabetes (2.43%), and celiac infection (1.73%), which accounted for 48.02% of the total patients admitted. In the winters (December to February) of these 5 years, the total number of patients admitted was 6 145, which contributed to 31.48% of the total admission number, and hence making winter the season with the highest number of patients seeking for medical services.ConclusionsAccording to the spectrum of the disease features, there is a need to enhance the training of specialty and emergency clinics associated with respiratory, circulation, neurological, digestive, and endocrine diseases. Special attention should be paid for treatment and health care targeted at old-aged patients. During winter, the peak hospital visiting time, appropriate medical care service, which addresses the plan of working time, should be provided in order to optimize diagnosis and treatment processes.

  • Impact Assessment of Health Education on Schistosomiasis among High School Students in Wanzhou Area of Three Gorges Reservoir

    Objective To learn the impact of health education on schistosomiasis among high school students in Wanzhou area of Three Gorges Reservoir, and to evaluate its effects. Methods The stratified multi-level cluster randomized sampling was used to survey 566 high school students in Wanzhou and Yunyang areas, and the questionnaire was used to investigate their recognition of schistosomiasis. Results After one-year health education on schistosomiasis, the passing rate of the knowledge about schistosomiasis prevention among high school students in demonstration area raised from 9.43% to 98.87% (χ2=463.46, P=0.000), which was higher than that of the inside and outside control areas (χ2=312.92, P=0.000); the passing rate of the attitude and belief of schistosomiasis prevention were 98.12%, which was higher than that of the inside and outside control areas (χ2=109.49, P=0.000); 56.02% of high school students in demonstration area had no susceptible behavior of schistosomiasis, which was higher than that of the inside and outside control areas (χ2=237.53, P=0.000). Conclusion Health education on schistosomiasis can efficiently improve the awareness of schistosomiasis prevention among high school students, and has important meanings for controlling potential epidemics and spread of schistosomiasis in Wanzhou area of Three Gorges Reservoir.

  • Treatment for resectable cervical esophageal carcinoma: A surgical perspective

    Definitive chemoradiotherapy (dCRT) is the general recommendation for the treatment of cervical esophageal cancer for organ preservation. However, the long-term survival of dCRT is not satisfactory. Surgical resection alone is not superior to dCRT in the treatment of cervical esophageal cancer. Surgical resection is often combined with laryngectomy, which will affect the quality of life. Recent evidence suggests that neoadjuvant therapy combined with surgery improves the long-term survival of cervical esophageal cancer. On the other hand, the development of technologies such as laryngeal preservation surgery and minimally invasive esophagectomy has reduced the risk of operation and improved the quality of life. This article will review the new progress in the comprehensive treatment of cervical esophageal cancer from the perspective of surgery.
