  • The New Concepts and Challenges of Surgery for Colorectal Cancer

    ObjectiveTo explain the latest concepts of colorectal surgery, and predict the future direction of it. MethodsA review and summary based on the clinical experience of our hospitals and theses over the past years and new advances on the researches in home and abroad were performed. ResultsDoctors should attach more importance to anal preserving operation; and there should be more usage of fast track in colorectal surgery. Besides, predicting low risk of postoperative complications and digitizing colorectal surgery also needed more attention. ConclusionThose aspects of colorectal surgery in the result need further development.

  • Construction and Identification of Eukaryotic Expression Vector of pEGFP-N3-TFPI-2

    ObjectiveTo construct eukaryotic expression vector of pEGFP-N3-TFPI-2, and to provide the base of studying the function of TFPI-2 gene. MethodsExtraction of total RNA from placental tissue was extracted at first, and then reverse transcriptase synthesis of cDNA was carried out. The cDNA fragment of TFPI-2 gene which was obtained by real time PCR (RT-PCR) was inserted into eukaryotic expression vector of pEGFP-N3. After double digestion with XhoⅠand KpnⅠ, the recombinant vector of pEGFP-N3-TFPI-2 was identified in 1% agarose gel electrophoresis and was tested by the sequence analysis. Then, the recombinant vector of pEGFP-N3-TFPI-2 (transfection group) and vector of pEGFP-N3 (blank control group) were transfected into Top10 competent cells with LipofectamineTM 2000, but no transfection-related treatment was performed in cells of untransfection group. Western blot method was used to test the expression of TFPI-2 protein in cells of 3 groups. ResultsThe purity of total RNA which were analysis by agarose gel electrophoresis and spectrophotometry were fit for PCR. After coding of TFPI-2 gene fragment and eukaryotic expression vector of pEGFP-N3, the recombinant plasmid of pEGFP-N3-TFPI-2 were got double digestion with XhoⅠand KpnⅠ, and was identified in 1% agarose gel electrophoresis, of which showing that there were 2 specific amplification of strips at 708 bp and 4 700 bp. Result of sequence analysis confirmed that the size of recombinant vector was consistent with the theoretical value. Results of Western blot showed that the expression of TFPI-2 protein in transfection group (0.657 3±0.032 5) was higher than those of blank control group (0.301 7±0.028 7) and untransfection group (0.314 3±0.026 6), P < 0.01. ConclusionsThe eukaryotic expression vector of pEGFP-N3-TFPI-2 has been constructed successfully, which laiding the foundation for the analysis about function of TFPI-2 gene.

  • The Impact of The Preoperative Nutritional Status on Quality of Life of Colorectal Cancer Patients

    ObjectiveTo explore the impact of the preoperative nutritional status on quality of life of colorectal cancer patients. MethodsIn this study, patients diagnosed definitely as colorectal cancer were enrolled from September 2012 to December 2012 in this hospital.NRS-2002 was used to assess the preoperative nutritional status, and QLQ-C30 was used to assess the quality of life. ResultsForty-four patients were enrolled in this study and divided into two groups based on NRS-2002 preoperative nutritional status evaluation scores:mild nutritional risk group had 27 patients who got 1 point, and moderate-severe nutritional risk group had 17 patients who got 2 point or over it.Patients in moderate-severe nutritional risk group were significantly older than the mild nutritional risk group (P=0.001), but there were no statistical differences between the two groups in gender (P=0.718), TNM stage (P=0.302), differentiation degree (P=0.564), preoperative medical complications (P=0.070), postoperative complications[NS (not significant)], and stoma (P=0.786).There were no statistical differences between the two groups in quality of life, too (P > 0.05). ConclusionsThis study don't get the conclusion that there is any relationship between nutritional status and quality of life of the colorectal cancer patients.And maybe it is effected by the too small sample size and single time of accessing.Therefore, the further research by expanding sample size, using more kinds of assessment tools, and selecting different time to explore the impact of the preoperative nutritional status on quality of life of colorectal cancer patients are necessary.

  • Effect of High Platelet Count on Short-and Long-Term Outcomes for Patient with Colorectal Cancer Following Operation

    ObjectiveTo study the short-and long-term outcomes of high platelet count in patients with colorectal cancer after surgery. MethodsA total of 398 patients with colorectal cancer who complied with the requirements for this study and operated in professional group on colorectal surgery of Gastrointestinal Surgery Center of West China Hospital in Sichuan University from January 2009 to October 2009 were retrospectively included in this study. Based on the last preoperative platelet count, these patients were divided into normal platelet group(100×109/L-300×109/L)and high platelet group( > 300×109/L). The short-and long-term outcomes were analyzed and compared between two groups. Results①The gender, age, histological type, degree of differentiation, diameter of tumor, site of tumor, and operation method had no significant differences between two groups(P > 0.05), but the ratio ofⅢ, Ⅳstages of TNM in the high platelet group was significantly higher than that in the normal platelet group(P < 0.05).②The first exhaust time and defecation time of the normal platelet group was significantly shorter than those of the high platelet group(P < 0.05). The time of the first oral intake, the time of pull out the drainage, ureter, and nasogastric tube, the time of the first ambulation, and hospitalstay had no significant differences between two groups(P > 0.05).③The total complications rate and single complication rate had no significant differences between two groups(P > 0.05).④All the patients were followed-up with average following-up time 58 months, there were 7 patients with recurrence and 35 patients with death in all the followed-up patients, the recurrence rate and death rate in the high platelet group were significantly higher than those in the normal platelet group(3.5% versus 1.1%, P=0.013; 15.7% versus 6.0%, P=0.002). ConclusionsThere is a relationship of platelet count to colorectal malignant degree. The recovery time of gastrointestinal function is longer, and the local recurrence rate and the postoperative death rate are higher for the patients with high platelet count.

  • Expression of miR21 in Rectal Cancer of Different Tumor Stages

    ObjectiveTo quantitate expression of miR21 in rectal cancer of different tumor stages and discuss their significances. MethodThe expression of miR21 was detected and quantitated in the rectal cancer tissues and corresponding adjacent normal tissues of 40 patients with rectal cancer in this hospital from August 2012 to October 2012 by Taq Man microRNA assay. ResultsThe significant overexpression of miR21 was observed in the rectal cancer tissues (4.122±1.973 versus 1.825±0.661, P=0.000)as compared with the corresponding adjacent normal tissues. The expressions of miR21 in the rectal cancer tissues of N1-N2 and Dukes C-D stages were significantly higher than those in the rectal cancer tissues of N0(4.852±2.344 versus 3.391±1.171, P=0.019)and Dukes A-B stage(4.787±2.304 versus 3.386±1.203, P=0.021). From N0 to N2 stage, the expression of miR21 increased progressively in the rectal cancer, and the expression in the rectal cancer tissues of N2 stage was significantly higher than that in the N0 stage(5.556±1.500 versus 3.391±1.171, P=0.010). And receiver operating characteristics curve analysis showed that miR21 could discriminate N 0 stage from N1-N2 stage with a 0.698 area under curve(AUC), 50.0% sensitivity and 90.0% specificity, Dukes C-D stage from A-B stage with a 0.689 AUC, 42.9% sensitivity and 94.7% specificity. ConclusionmiR21 appears to have a potential correlation with N and Dukes stages of rectal cancer, which cautiously and optimistically suggests that it could be a potential novel biomarker for predicting tumor stage preoperatively.
