  • Effect of Modified Sleeve Gastrectomy on Ghrelin and Glucagon Like Peptide-1 in Rats with Diet Induced Obesity

    ObjectiveTo explore a surgery of effective weight loss concentrating on gut hormone release. MethodsWistar rats were fed with high-fat diet for inducing obesity and which randomly divided into sleeve gastrectomy plus decent jejunoileal bypass (SJB) group (n=12), sleeve gastrectomy (SG) group (n=12), and sham operation (SO) group (n=11), the body weight reduction, food intake, plasma ghrelin level, and glucagon like peptide-1 (GLP-1) level were compared among three groups. ResultsThere were no differences of the body weight and food intake before operation among three groups (Pgt;0.05). Compared with the SO group, the body weight descended and the food intake decreased obviously on 1-8 weeks after operation in the SJB group (Plt;0.05), the body weight of rats on 1-8 weeks after operation in the SJB group significantly descended as compared with the SG group (Plt;0.05), and the food intake of rats on week 3, 6, 7, and 8 in the SJB group signicantly decreased as compared with the SG group (Plt;0.05). There were no differences of the levels of the plasma ghrelin and GLP-1 before operation among three groups (Pgt;0.05). Compared with SO group, the plasma ghrelin level decreased and the GLP-1 level increased in the SJB group and the SG group, meanwhile the SJB group significantly decreased level of plasma ghrelin and elevated level of plasma GLP-1 as compared with the SG group on week 8 after operation(Plt;0.05). ConclusionThe data demonstrate that SJB could represent an effective way of losing weight by interfering with food intake and obesity related hormone levels.

  • Application and effect evaluation of enterprise WeChat in the publicity and education of Party style and clean governance in public hospitals

    ObjectiveTo discuss the ways and effects of carrying out the publicity and education of Party style and clean governance in public hospitals through enterprise WeChat, aiming at improving the quality and efficiency of the publicity and education and forming an accurate and real-time pattern of discipline warning education.MethodsTaking the articles regarding the publicity and education of Party style and clean governance on the enterprise WeChat of West China Hospital of Sichuan University as the research object, the content analysis method was used to review the content from October 2017 to December 2018 and its publicity effect. The number of readers was shown in median (lower quartile, upper quartile) and the statistical analysis was done through rank sum test.ResultsFrom the content updated, medical staff read more about Internet hot spots and related clean governance news happened around them [M (QL, QU): 1 106 (691, 1 506)] than policy learning [301 (233, 408)] (P<0.05) and knowledge explanation [392 (457, 1 133)] (P<0.05). In terms of the methods of the update, medical staff read more about the update in traditional text and pictures [462 (312, 1 073)] than cartoon, video and other methods [230 (175, 315)] (P<0.05).ConclusionPaying attention to updated content, increasing the discussion function of the audience, choosing the time that the audience likes to update the article, and in the meantime, building the brand for the publicity and education of Party style and clean governance in the hospital may have a better effect on the education of Party style and clean governance toward Party members and medical staff in public hospitals.

  • Value of Dual-soure CT in Diagnosing Pulmonary Artery Embolism

    【摘要】 目的 探讨双源CT在诊断肺栓塞中的价值。 方法 2008年5月-2010年12月纳入50例可疑肺栓塞患者,使用双源CT进行肺动脉血管增强扫描,对图像行三维重建,分析栓塞部位、栓子形态、肺内、心脏及胸腔改变等。 结果 44例确诊肺栓塞,发现肺栓塞共260处,最常见于右肺下叶动脉28例(63.0%),其次于右肺动脉22例(50.0%)、左肺下叶动脉19例(43.2%);15例(34.1%)累及亚段肺动脉;右肺动脉受累较左肺动脉常见(Plt;0.05)。39例系急性肺栓塞(88.6%),5例系慢性肺栓塞(11.4%)。 结论 双源CT扫描速度快,空间分辨率高,可以准确显示肺栓塞部位,并且可以同时评价胸腔、肺内及心脏的变化。在临床上,双源CT可以作为肺栓塞的首选检查。【Abstract】 Objective To explore the value of dual-source CT in diagnosing pulmonary embolism. Methods From May 2008 to December 2010, 50 patients who were suspected of pulmonary embolism were included, and pulmonary angiography was performed on these patients by dual-source CT. The obtained images were reconstructed with three dimensional technique to analyze the location and shape of the embolism, and lesions affecting the lung, heart and thoracic cavity. Results A total of 44 patients wiht pulmonary embolism with 260 affected branches of arteries were confirmed by CT angiography. The most common affecting artery was the right lower pulmonary artery(28/44,63.0%), which was followed by the right pulmonary artery (22/44,50.0%) and the left lower pulmonary artery(19/44,43.2%). Segmental and subsegmental pulmonary arteries were involved in 15 patients (15/44, 34.1%). Pulmonary artery embolism involves the right pulmonary arteries more frequently than the left pulmonary arteries (P<0.05).Acute pulmonary embolism was found in 39 cases(39/44, 88.6%), while chronic pulmonary embolism was found in 5(5/44, 11.4%). Conclusions Dual-source CT with fast scanning speed and high spatial resolution can demonstrate the exact site of the pulmonary embolism and evaluate the lesions of thoracic cavity, lung and heart. In clinical practice, dual-source CT could be the preferred choice in diagnosing pulmonary embolism.

  • Influence of Sleeve Gastrectomy on Blood Glucose, Ghrelin, and Glucagon Like Peptide-1 Levels in GK Rats

    ObjectiveTo investigate the effect and mechanism of sleeve gastrectomy (SG) on reducing blood glucose level. MethodsThirty GK rats were randomly divided into SG group, sham operation (SO) group, pair-fed (PF) group, and blank control (BC) group. The changes of weight, fasting blood glucose, glucose tolerance (oral glucose tolerance test, OGTT), insulin tolerance (insulin tolerance test, ITT), plasma insulin, ghrelin, and glucagon like peptide-1 (GLP-1) were monitored before and 24 weeks after operation respectively. ResultsFrom the 4th week after operation, weight gain in SG group and PF group began to decrease significantly compared with SO group (Plt;0.01). From the 2nd week after operation, fasting blood glucose level in SG group was lower than that in SO, PF, and BC groups (Plt;005), and the glucose tolerance in SG group obviously improved compared with preoperation and the other 3 groups (Plt;0.01). On the 6th week after operation, the insulin sensitivity in SG group obviously improved compared with SO group (Plt;0.05, Plt;0.01). There was no significant difference of insulin level between SG group and SO group (Pgt;0.05), ghrelin level significantly decreased (Plt;0.01) while GLP-1 level significantly increased (Plt;0.01) in SG group compared with SO group during 2-24 weeks after operation. ConclusionsThe effect of SG on reducing blood glucose is definite. SG can directly lower blood glucose independent with weight loss. Postoperative decreased ghrelin level and increased GLP-1 level may be its primary mechanism.

  • Clinical study of pirenpanet as add-on therapy for intractable epilepsy

    Objective To observe the efficacy and adverse reactions of pirenpanet in patients with refractory epilepsy treated with add-on therapy. MethodsForty-one patients with medically refractory epilepsy were selected from January 2021 to December 2021 in the Department of Epilepsy of Jiamusi Central Hospital, among which 25 males and 16 females. The self-control study was conducted before and after the treatment with pirenpanet. The observation period was 1 year, and the clinical efficacy, platelet, liver and kidney function and EEG discharge index were observed at 6 months and 12 months respectively. Changes in seizure frequency after treatment were divided into the following 3 groups: effective control: no seizure; effective: seizures reduced by 50% ~ 99%; Ineffective: < 50% reduction in seizures or exacerbation. Results Among the 41 patients with drug-refractory epilepsy, 8 (8/41, 19.51% P < 0.05) were seizure-free, 17 (17/41, 41.46% P < 0.05) were effective, and the total effective rate was (25/41, 60.98%); Sixteen cases (16/41, 39.02%) were ineffective. There were 31 children in 41 cases (31/41, 75.61%), of which 18 cases (18/31, 58.06%) were effective, 16 cases (16/31, 51.61%) had mental and motor retardation, and 8 cases (8/16, 50%) were effective; There were 10 adults (10/41, 24.39%) and 7 effective patients (7/10, 70%). After one year's treatment, the EEG changes of the patients were observed, and the abnormal discharge of 6 patients was reduced according to the discharge index, 2 patients had no abnormal discharge, and the other patients had no obvious EEG changes. Conclusion Pirenpanet is effective and safe as an add-on therapy for medically refractory epilepsy.

  • Influential Factors for Graft Patency after Coronary Artery Bypass Grafting

    Abstract: Objective To evaluate graft patency at one year after coronary artery bypass grafting (CABG) and analyze the risk factors of graft occlusion. Methods We prospectively included 71 consecutive patients with coronary heart disease who underwent CABG in First Affiliated Hospital of Harbin Medical University from June to December 2010. There were 37 males and 34 females with their average age of 59.8±7.7 years. All the patients underwent CABG performed by the same surgeon, and received standard drug therapy after surgery. At 1 year after CABG, 256-slice multislice computed tomography (MSCT) angiography was performed to evaluate graft patency. All the patients were divided into two groups according to their graft patency during follow-up, including 16 patients in the occluded graft group and 55 patients in patent graft group. Preoperative, intra-operative and postoperative variables were collected. Univiariate analysis and logistic regress analysis were performed to analyze risk factors of graft occlusion. Results The graft patency was 91.0% (172/189) at 1 year after CABG. Univiariate analysis showed that pre-operative cholesterol (t=-2.389,P=0.017), diffused coronary artery disease (χ2=4.449,P=0.042), diameter of target vessels (t=5.064,P=0.000), non-adherence to prescribed antiplatelet medications (χ2=10.175,P=0.008) were potential risk factors of graft occlusion after CABG. Logistic regress analysis showed that diameter of target vessels [RR=0.014,95% CI (0.001,0.229),P=0.003] and non-adherence to prescribed antiplatelet medications [RR=13.375,95% CI (1.075,175.536),P=0.044] were significant risk factors for graft occlusion. Conclusion The graft patency is satisfactory at 1 year after CABG. Graft patency after CABG is closely related to the stenosis degree of target vessels and adherence to prescribed antiplatelet medications after surgery.

  • Treatment of Refractory Glomerular Diseases in Patients with Multitarget Therapy

    【摘要】 目的 观察激素加霉酚酸酯(mycophenolate mofetil,MMF)和他克莫司(tacrolimus,FK506)的多靶点方案治疗难治性肾小球疾病的疗效及安全性。 方法 2008年5月-2010年3月收治的15例狼疮性肾炎(lupus nephritis,LN)、3例膜增生性肾小球肾炎(membranoproliferative glomerulonephritis,MPGN)及3例膜性肾病(membranous nephropathy,MN)患者,因多种免疫抑制剂治疗无效或复发而改用多靶点疗法。泼尼松以30~40 mg/d起始,逐渐减量。MMF 和FK506起始剂量分别为0.5 g/d或1 mg/d,目标血药浓度分别为20~40 mg/(h·L)或5~8 ng/mL。定期随访观察肝肾功能、尿蛋白定量、不良反应等指标。 结果 治疗6个月时15例LN中7例(46.7%)完全缓解(complete remission,CR),5例(33.3%)部分缓解(partial remission,PR),3例(20%)无效(no response,NR)。3例MPGN均表现为NR。3例MN中2例(66.7%)PR,1例(33.3%)NR。治疗过程中呼吸道感染及脱发各1例,胃肠不适2例,肌酐逐步升高3例,无死亡或退出者。 结论 多靶点疗法对难治性LN安全、有效,可作为其他免疫抑制剂治疗无效或复发时的选择方案,但对MPGN和MN疗效欠佳,需进一步研究。【Abstract】 Objective To investigate the efficacy and safety of multitarget therapy with steroid, mycophenolate mofetil (MMF) and tacrolimus (FK506) in the treatment of refractory glomerular diseases.  Methods Fifteen patients with lupus nephritis (LN), 3 patients with membranoproliferative glomerulonephritis (MPGN) and 3 patients with membranous nephropathy (MN) who failed the previous immunosuppressive therapy from May 2008 to March 2010 in our hospital were treated with multitarget therapy. The initial dose of prednisone was 30-40 mg/d and then tapered gradually. MMF and FK506 were started at 0.5 g/d or 1 mg/d, and the target blood concentration of the two drugs was 20-40 mg/(h·L) and 5-8 ng/mL respectively. Clinical parameters such as liver and renal function, urine protein, and side effects were recorded and analyzed in the regular follow-up. Results After 6 months of treatment, 7 (46.7%) of the 15 LN patients achieved complete remission (CR), 5 (33.3%) achieved partial remission (PR), while 3 (20%) failed this treatment and had no response (NR). All of the three MPGN patents had NR to this combined therapy. Two (66.7%) of the 3 MN patents achieved PR while 1 (33.3%) had NR. No patient withdrew or died because of side effects. One patient developed upper respiratory infection, one experienced alopecia, two developed gastrointestinal syndrome and three experienced gradual increasing in the serum creatinine. Conclusion Multitarget therapy with FK506, MMF and steroid is an effective and safe therapy for refractory lupus nephritis and it can be used in patients who are resistant to the conventional immunosuppressive therapy. However, this combined therapy does not meet a satisfactory result in patients with MN and MPGN, which entails further study.

  • CONSTRUCTION OF INDUCIBLE LENTIVIRAL VECTOR CONTAINING HUMAN BONE MORPHOGENETIC PROTEIN 2 GENE AND ITS EXPRESSION IN HUMAN UMBILICAL CORD BLOOD MESENCHYMAL STEM CELLS

    Objective To construct inducible lentiviral vector containing human bone morphogenetic protein 2 (hBMP-2) gene and to study its expression in human umbil ical cord blood mesenchymal stem cells (HUMSCs). Methods hBMP-2 gene was ampl ified by PCR from a plasmid and was cloned into pDown by BP reaction. pLV/EXPN2-Neo-TRE-hBMP-2 and pLV/EXPN2-Puro-EF1A-reverse transactivator (rtTA) were obtained with GATEWAY technology, and then were sequenced and analyzed by PCR. The recombinant vectors were transfected into 293FT cells respectively through l ipofectamine, and the lentiviral viruses were harvested from 293FT cells, then the titer was determined. Viruses were used to infect HUMSCs in tandem. In order to research the influence of induction time and concentration, one group of HUMSCs was induced by different doxycl ine concentrations (0, 10, 100 ng/mL, and 1, 10, 100 μg/mL) in the same induction time (48 hours), and the other by the same concentration (10 μg/mL) in different time points (12, 24, 48, and 72 hours). The expression of target gene hBMP-2 was indentified by ELISA method. After 2-week osteogenic induction of transfected HUMSCs, the mineral ization nodes were detected with Al izarin bordeaux staining method. Results Therecombinant inducible lentiviral vectors (pLV/EXPN2-Neo-TRE-hBMP-2 and pLV/EXPN2-Puro-EF1A-rtTA) were successfully constructed. The lentiviruses were also obtained and mediated by 293FT cells, and the virus titers were 3.5 × 108 TU/mL and 9.5 × 107 TU/mL respectively. HUMSCs could expression hBMP-2 by induction of doxycycl ine. The expression of hBMP-2 reached the peak at 10 μg/mL doxycl ine at 48 hours of induction. After 2-week osteogenic induction, a lot of mineral ization nodes were observed. Conclusion The recombinant inducible lentiviral vectors containing hBMP-2 gene can be successfully constructed, which provide an effective and simple method for the further study of stem cells and animal experiment in vivo.
