  • Research progress on risk predictors of vascular dementia

    Vascular dementia is one of the most common types of dementia in China. How to better prevent and treat vascular dementia is still an unresolved problem, and the risk predictor of vascular dementia may help provide clinical targeted prevention measures to intervene in the development process of vascular dementia early. This article reviews the current research status of vascular dementia predictors from four aspects: blood markers, predictors based on disease characteristics, predictors based on assessment tools and neuropsychological tests, and predictors based on activity dysfunction. It aims to provide a basis for establishing a risk prediction model for patients with vascular dementia suitable for China’s conditions in the future.

  • Current research status of patient-reported outcomes in the field of venous thromboembolism

    ObjectiveTo summarize the applied research status on the evaluation tools of patient-reported outcome at home and abroad in patients with venous thromboembolism (VTE). MethodBy searching and analyzing the literatures, this paper summarized the concept, evaluation tools and application status of patient-reported outcome in the field of VTE. ResultsThe patient-reported outcomes can more comprehensively and accurately evaluate the disease burden and treatment effect of patients with venous thromboembolism, and can help doctors better understand patients' needs and guide individualized treatment and rehabilitation plans. ConclusionsPatient-reported outcome has a broad application prospect in the field of venous thromboembolism. Further promotion and application of patient-reported outcome can promote the development of medical research and provide reference guidelines for improving the management of patients with venous thromboembolism.

  • Epidemiology of Intracranial Infection after Craniotomy: A Meta-analysis

    目的 总结近十年来颅脑手术后颅内感染的发病率、病死率及病原菌谱,为制订预防颅内感染措施提供依据。 方法 检索中国学术期刊网全文数据库(CNKI)、万方数据库、重庆维普中文科技期刊全文数据库,并辅以文献追溯、手工检索等方法收集2001年-2012年国内正式刊物上公开发表的有关颅脑手术后颅内感染的中文文献。并对颅脑手术后颅内感染的流行病学调查资料进行Meta分析。 结果 共有27篇论文进入Meta分析,其中,25篇论文用于颅脑手术后颅内感染发病率的统计。共调查40 343例,发生颅内感染1 712例,感染率为4.24%。6篇论文提供了开颅术后颅内感染死亡的数据,在268例颅内感染患者中死亡39例,病死率为14.55%。15篇论文统计显示,颅内感染细菌培养阳性率为54.48%;15篇论文给出细菌培养结果,其中金黄色葡萄球菌占23.16%,表皮葡萄球菌占17.85%,铜绿假单胞菌占8.85%,大肠埃希菌占8.70%。 结论 国内颅脑手术后颅内感染以G+球菌为主,病原菌分布相对集中,临床上应予以重视。

  • Bioinformatics and functional analysis of key genes and pathways in tuberculosis

    ObjectiveTo explore the pathogenesis of tuberculosis and provide new ideas for its early diagnosis and treatment.MethodsGSE54992 gene expression profile was obtained from the gene expression database. Differentially expressed genes (DEGs) were screened using National Center forBiotechnology Information platform, and GO enrichment analysis, pathway analysis, pathway network analysis, gene network analysis, and co-expression analysis were performed to analyze the DEGs.ResultsCompared with the control group, a total of 3 492 genes were differentially expressed in tuberculosis. Among them, 1 686 genes were up-regulated and 1 806 genes were down-regulated. DEGs mainly involved small molecule metabolic processes, signal transduction, immune response, inflammatory response, and innate immune response. Pathway analysis revealed chemokine signaling pathway, tuberculosis, NF-Kappa B signaling pathway, cytokine-cytokine receptor interaction, and so on; gene signal network analysis found that the core genes were AKT3, PLCB1, MAPK8, and NFKB1; co-expression network analysis speculated that the core genes were PYCARD, TNFSF13, PHPT1, COMT, and GSTK1.ConclusionsAKT3, PYCARD, IRG1, CD36 and other genes and their related biological processes may be important participants in the occurrence and development of tuberculosis. Bioinformatics can help us to comprehensively study the mechanism of disease occurrence, which can provide potential targets for the diagnosis and treatment of tuberculosis.

  • Research progress of nanomaterials for intra-articular targeted drug delivery in treatment of osteoarthritis

    ObjectiveTo review the research progress of intra-articular targeted delivery of nanomaterials in the treatment of osteoarthritis (OA). MethodsThe domestic and foreign related literature on intra-articular targeted delivery of nanomaterials for the treatment of OA was extensively reviewed, and their targeting strategies were discussed and summarized. Results Rapid drug clearance from the joint remains a critical limitation in drug efficacy. Nanocarriers can not only significantly improve the residence profiles of drugs in the joint, but also achieve targeted delivery of drugs to specific joint tissues through active or passive targeting strategies. Conclusion With the continuous development of various emerging tissue- or cell-specific drugs, the targeted delivery of drugs with nanomaterials promise to realize the clinical translation of these drugs in the treatment of OA.

  • Genome-wide association analysis of genetic susceptibility to hepatitis B virus co-infection in patients with pulmonary tuberculosis

    ObjectiveTo explore the single locus mutation that related to hepatitis B virus (HBV) co-infection by means of genome-wide association study (GWAS) in Chinese Han patients with pulmonary tuberculosis (TB).MethodsA total of 946 patients with pulmonary TB enrolled between March 2013 and March 2018 were genotyped by Illumina Human Omni Express gene chip. After quality control, 389 972 single nucleotide polymorphisms (SNPs) of 703 patients with single TB infection and 53 patients with TB-HBV co-infection were included in the follow-up association analysis.ResultsThe SNP with the strongest statistical correlation signal was rs118122819 (P=2.923×10−12, odds ratio=7.933) located on chromosome 8p23.1. Other potential susceptibility genes included CDH4 (rs73309833), MARCH1 (rs3797020), and DNER (rs13393112), etc. In addition, a strong linkage imbalance between rs118122819 and rs4840365 (D’=0.88, r2=0.76) was found, while rs4840365 was located in the MFHAS1 gene region.ConclusionsThis study provides evidence for the presence of susceptibility gene locus for HBV co-infection in pulmonary TB patients, and provides important clues for the mechanism research, disease prevention, and treatment of co-infection. But these associations must be replicated and validated in larger studies.

  • Correlation between the polymorphisms of inhibition genes WIF1 and DKK1 in WNT signaling pathway and the susceptibility to tuberculosis in Chinese Han population

    Objective To explore the relationships between the polymorphisms of inhibitor genes WIF1 and DKK1 in WNT signaling pathway and susceptibility to tuberculosis, clinical characteristics and laboratory indexes. Methods From December 2014 to November 2016, 475 tuberculosis patients and 370 healthy controls of West China Hospital of Sichuan University were enrolled in the study, and the clinical data of the subjects were collected. High-throughput genotyping technique was used to detect the polymorphisms of WIF1 rs58635985 and DKK1 rs11001548 in WNT signaling pathway. The allele frequency distribution, genotype, genetic model, clinical features and laboratory indexes of two single nucleotide polymorphisms were analyzed by χ2 test and logistic regression analysis. Results There was no significant difference in the allele frequency distribution (P=0.275, 0.949), genotype (P=0.214, 0.659) or genetic models: additive model (P=0.214, 0.659), dominant model (P=0.414, 0.827), recessive model (P=0.227, 0.658) of rs58635985 and rs11001548 between the tuberculosis group and the healthy control group. Subgroup analysis showed no significant difference in allele and genotype distribution between rs58635985 and rs11001548 (pulmonary tuberculosis group vs. healthy control group: P>0.05; pulmonary tuberculosis groupvs. extra-pulmonary tuberculosis group: P>0.05). There was no significant difference in the clinical features (fever, night sweat, fatigue,etc.) or laboratory indexes (complete blood count, erythrocyte sedimentation rate, TB-DNA, etc.) (P>0.05). Conclusions There is no association between rs58635985 of WIF1 gene or rs11001548 of DKK1 gene and genetic susceptibility, clinical characteristics and laboratory indexes in Han population in Western China. To expand the sample size for verification and analysis in different populations is necessary.

  • Expression and diagnostic value of long non-coding RNA lnc-PAPSS2-2 in peripheral blood of active pulmonary tuberculosis patients

    ObjectiveTo evaluate the expression level and diagnostic value of lnc-PAPSS2-2 (lnc-PA) in peripheral blood of active pulmonary tuberculosis (PTB) patients.MethodsFrom January 2011 to January 2018, 798 patients with active PTB and 1 650 healthy people undergoing health examination in West China Hospital of Sichuan University and their electronic health records (EHR) were collected. Peripheral blood lnc-PA levels were quantified by quantitative real-time polymerase chain reaction method. The data of lnc-PA and EHR were modeled using nomogram, and the receiver operating characteristic (ROC) curves of lnc-PA, EHR and the combination of lnc-PA and EHR were compared to evaluate the diagnostic value of lnc-PA for active PTB.ResultsThe level of lnc-PA was lower in active PTB patients than that in healthy controls (P<0.001). The areas under ROC curve of lnc-PA, EHR and their combination were 0.619, 0.962, and 0.964 in the training set and 0.626, 0.950, and 0.950 in the validation set, respectively.ConclusionThe diagnostic ability of lnc-PA is poor and that of EHR is good, which indicates that the clinical value of lnc-PA as a biomarker of active PTB remains to be further explored.

  • Comparative study of clinical features and vascular risk factors in patients with unexplained non-acute dizziness and headache in cerebrovascular disease clinic

    Objective To analyze the difference of clinical characteristics and vascular risk factors between patients with unexplained non-acute dizziness and headache, so that to bring new perspective on prevention of vascular risk factors and enhancement of symptoms among different patients. Methods The data of patients in the cerebrovascular disease clinic of the Department of Neurology, West China Hospital of Sichuan University between July 1th and September 30th, 2021 were collected retrospectively. The included patients were divided into dizziness group or headache group according to the complained. The clinical characteristics and risk factors of the two groups were compared. Results A total of 273 patients were included. There were 198 cases (72.5%) in dizziness group and 75 cases (27.5%) in headache group. Compared with headache group, patients in dizziness group were older, with a shorter course of disease, and accounted for a higher proportion of patients with symptoms related to body position, higher proportion of patients with hypertension and carotid plaque, and more vascular risk factors (P<0.05). Logistic regression results showed that patients with symptoms related to body position [odds ratio (OR)=7.025, 95% confidence interval (CI) (1.772, 28.488), P=0.007], at least one [OR=2.461, 95%CI (1.298, 4.664), P=0.006], two or more vascular risk factors [OR=2.314, 95%CI (1.033, 5.186), P=0.042] were independently associated with dizziness. The longer course of disease [OR=0.994, 95%CI (0.990, 0.997), P=0.001] was independently associated with headache. Conclusion Patients with non-acute unexplained dizziness and headache, especially those with dizziness, should pay more attention to the inquiry and treatment of vascular risk factors and psychopsychological factors.
