  • Detection of serum homocysteine levels in retinal branch vein occlusion patients with hypertension or non-hypertension

    Objective To observe the serum homocysteine (Hcy) levels in retinal branch vein occlusion (BRVO) patients with with hypertension or non-hypertension. Methods A total of 120 patients (120 eyes) with BRVO were divided into hypertension group [72 eyes, blood pressure 140 - 175/90 - 105 mmHg (1 mmHg=0.133 kPa)] and non-hypertension group (48 eyes, blood pressure 100 - 139/70 - 88 mmHg). According to the sex and age, 78 patients with hypertensive non-retinal vascular diseases and 48 patients with non-hypertensive and non-retinal vascular diseases were collected by a way of same-size ratio as hypertension control group and non-hypertension control group, respectively. Fasting venous blood was collected from all patients in the morning and serum Hcy levels were measured by rate method. The total Hcy concentration over 15.0 μ mol/L was defined as high level Hcy. Fasting serum glucose and fasting serum lipid were also measured. Measurement data among groups were compared with t test. Results The serum Hcy levels were (26.82±28.0), (8.39±3.11), (21.37±4.24), (9.25±3.31) μmol/L in the hypertension group, hypertension control group, non-hypertension group and non-hypertension control group, respectively. The serum Hcy levels of patients in the hypertension group was significantly higher than that in the hypertension control group (t=3.324, P=0.004). The serum Hcy levels of patients in the non-hypertension group was significantly higher than that in the non-hypertension control group (t=2.216, P=0.049). The serum Hcy levels of patients in the hypertension group was significantly higher than that in the non-hypertension group, but the difference had not statistical significance (t=0.581, P=0.566). Among 120 patients, there were 68 patients (56.67%) with high level of Hcy (40 patients in the hypertension group and 28 patients in the non-hypertension group). Among the 40 patients with high levels of Hcy in the hypertension group, 36 patients were older than 50 years old (90.00%) and 4 patients were less or equal than 50 years old (10.00%). Among the 28 patients with high levels of Hcy in the non-hypertension group, 16 patients were older than 50 years old (57.14%); 12 patients were less or equal than 50 years old (42.86%), whose indexes of serum glucose and serum lipid were not abnormal. There was significant difference in age distribution of patients with high level of Hcy between the hypertension group and the non-hypertension group (χ2=9.882, P=0.002), but there was no significant difference in sex distribution (χ2=2.052, P=0.216). Conclusions The level of serum Hcy increased both in BRVO patients with hypertension and non-hypertension. The indexes of serum glucose and serum lipid were not abnormal in BRVO patients aged less or equal than 50 years old with non-hypertensive except for the increase of serum Hcy level.

  • Characteristics and diagnostic value of temporal retinal thinning in young patients with Alport syndrome

    ObjectiveTo observe the features of temporal macular thinning and its value for the diagnosis of Alport syndrome (AS) in young patients.MethodsEighty-one young patients with AS (81 eyes) from Peking University First Hospital during January 2016 and July 2017 were included in this study. There were 67 males (67 eyes) and 14 females (14 eyes),the aged from 3 to 17 years, with the mean age of 9.6 years. Among 81 patients (81 eyes), there were 64 patients with X-linked AS (XLAS, including 53 males and 11 females), 17 patients with autosomal recessive AS (ARAS, including 14 males and 3 females). One hundred healthy subjects aged 4 to 17 years were included as controls. Clinical data were retrospectively evaluated, including visual acuity, slit-lamp microscopy, dilated fundus photography, and OCT. Retinal thickness was measured with an OCT scan and the temporal thinning index (TTI) was calculated as stated in a previous study. The TTI values of each group was compared by One-way ANOVA or independent sample t test. The receiver operating characteristic (ROC) curve was used to evaluate the diagnostic effectiveness for AS.ResultsThe TTI of the control group, XLAS and ARAS patients were 6.46±1.58, 10.93±3.77, 12.14±4.05, respectively. Compared with the control group, the TTI value of males were larger in the XLAS and ARAS group (F=45.056, P<0.001), the TTI value of females were larger in the ARAS group (F=26.541, P<0.001). The difference of TTI value in females was significant between the XLAS and ARAS groups (F=26.541, P<0.001). In males, the area under the ROC curve was 0.896 (95%CI 0.837−0.955, P<0.001). The optimal cutoff value of the TTI was determined as 9.47, with a sensitivity of 73.1% and a specificity of 100%.ConclusionsTTI is a common ocular finding in young patients with AS. In males, a TTI > 9.47 may differentiate AS from normal males.

  • Clinical characteristics of vascular neuro-ophthalmology in patients with central retinal artery occlusion

    ObjectiveTo investigate the clinical characteristics of vascular neuro-ophthalmology in patients with central retinal artery occlusion (CRAO). MethodsA single-center, prospective clinical study. From January 2018 to December 2020, 49 eyes of 49 CRAO patients of The Neuro-ophthalmology Department of Xi'an First Hospital were included in the study. Data on patient demographic characteristics, vascular risk factors, disease characteristics, digital subtraction angiography (DSA) imaging characteristics of internal carotid arteries, treatment, treatment-related adverse events, and 1-month follow-up vascular events were collected. All patiens were examined by visual acuity, head CT and or magnetic resonance imaging. At the same time, 35 cases of internal carotid artery vascular DSA were examined; 14 cases of head and neck CT angiography were examined. The anatomical variation of the extracranial segment of the internal carotid artery was divided into tortuous, tortuous, and coiled; the aortic arch was divided into type Ⅰ, type Ⅱ, type Ⅲ, and bovine type. Intravenous thrombolysis, arterial thrombolysis, conservative treatment were performed. The follow-up time was 1 month after treatment. Functional vision was defined as vision ≥20/100. Vascular events were strokes, cardiovascular events, deaths and neovascular glaucoma during follow-up. ResultsAmong 49 eyes of 49 cases, 40 eyes were male (81.6%, 40/49), and 9 eyes were female (18.4%, 9/49); the average age was 60.7±12.9 years. There were 33, 17, and 16 cases with hypertension, type 2 diabetes, and cerebrovascular disease, respectively; 27 and 34 cases had a history of smoking and tooth loss, respectively. Taking antihypertensive, hypoglycemic, antiplatelet aggregation/anticoagulation, and hypolipidemic drugs were 15, 5, 8, and 5 patients, respectively. There were 11 cases of transient amaurosis before the onset, and 17 cases of CRAO after waking up. There were 33 cases (67.3%, 33/49) with infarction of the affected side of the brain tissue. DSA was performed in 35 cases, and the stenosis rate of the internal carotid artery on the affected side was 70%-99% and 100% were 3 (8.6%, 3/35) and 4 (11.4%, 4/35) cases, respectively. The ophthalmic artery on the affected side originated from the external carotid artery in 5 cases (14.3%, 5/35). There were 17 (54.8%, 17/31) and 2 (6.5%, 2/31) cases of tortuousity and kinking in the extracranial segment of the internal carotid artery. There were 15 (42.9%, 15/35), 6 (17.1%, 6/35), and 2 (5.7%, 2/35) cases of aortic arch type Ⅱ, type Ⅲ, and bovine type, respectively. Intravenous thrombolysis and arterial thrombolysis were performed in 13 and 29 cases, respectively. Complications occurred in 2 cases during treatment; 3 cases of symptoms fluctuated after treatment, and 10 cases of asymptomatic new infarcts occurred in imaging studies. Forty-eight cases were treated with antiplatelet aggregation/anticoagulation and hypolipidemic treatment. At discharge and 1 month after treatment, the recovery of functional vision was 7 and 17 cases, respectively. One month after treatment, 1 case died because myocardial infarction; 2 cases of neovascular glaucoma occurred. ConclusionThe proportion of CRAO patients with vascular risk factors and internal carotid artery abnormalities on the affected side is relatively high; the prognosis is relatively good after intravenous thrombolysis and/or arterial thrombolysis and secondary stroke prevention.
