  • Clinical analysis on the influencing factors of cognitive impairment in 65 alcohol dependent patients

    Objective To explore the characteristics of cognitive impairment in patients with alcohol dependence, and analyze the related influencing factors. Methods The Montreal Cognitive Assessment Scale (MoCA) was used to evaluate the cognitive function of 65 alcohol dependent patients hospitalized between January 1st and December 31st, 2014. The features of cognitive impairment and related influencing factors were analyzed. Results The differences of MoCA attention and delayed recall between different drinking year groups had statistical significance (P<0.05). The correlations of drinking year with MoCA attention (r=–0.250,P=0.044), and with delayed recall (r=–0.326,P=0.008) were both negative. MoCA scores, naming, attention and delayed recall were different statistically among different age groups (P<0.05). The correlations of ages with MoCA scores (r=–0.429,P<0.001), naming (r=–0.261,P=0.035), attention (r=–0.391,P=0.001) and delayed recall (r=–0.461,P<0.001) were all negative. MoCA scores, the visuoconstructional skills, language, abstraction and delayed recall were significantly different among different education level groups (P<0.05). The correlations of education level with MoCA scores (rs=0.650,P<0.001), the visuoconstructional skills (rs=0.540,P<0.001), language (rs=0.486,P<0.001), abstraction (r=0.602,P<0.001) and delayed recall (rs=0.593,P<0.001) were all positive. Ages had an effect on MoCA scores by multiple linear regression analysis (P<0.01). Conclusions For alcohol dependent patients with cognitive impairment, cognitive function is correlated with drinking year, age and education level. The cognitive function is much serious in patients with older age and longer drinking years. This kind of patients should be focused on and intervened early.

  • Application of different types of congenital heart disease occluder in bronchopleural fistula

    ObjectiveTo introduce implantation methods of different types of congenital heart disease occluder for the treatment of bronchopleural fistula (BPF) and its preliminary efficacy.MethodsThree patients who diagnosed with BPF and treated by congenital heart disease occluder were reviewed. The clinical data was analyzed after comprehensively reviewing of relevant literature.ResultsAll the three patients were treated with postoperative BPF and empyema. The diameter of the fistula ranged from 3 to 8 mm. We used occlusive devices for congenital heart diseases such as atrial septum (ASD), ventricular septum (VSD) defect or patent ductus arteriosus (PDA), respectively. After treatment, all three patients were cured of BPF and empyema caused by BPF in a short time, and the thoracic drainage tube was successfully removed. During the follow-up period from 7 to 25 months, no significant long-term complications were observed.ConclusionThe use of ASD, VSD and PDA occluder for the treatment of BPF with a fistula more than 3 mm is effective and safe.

  • Effect of oxidative damage on expression of myocardin and NF-E2-related factor 2 in myocardial cells

    With the oxidative damage model established in rat myocardial cells by hydrogen peroxide (H2O2), the expression of myocardin and nuclear factor erythroid 2-related factor 2 (Nrf2) during oxidative damage and effect of myocardin on Nrf2 were preliminarily explored. The expression of the target gene was increased or decreased by transfection of plasmid DNA or shRNA, respectively. Cell proliferation was detected by sulforhodamine B (SRB) assay. The expression of myocardin mRNA and Nrf2 mRNA was detected by Real-time PCR, and their protein levels were detected by Western blot. The results showed that oxidative damage was induced by H2O2 with an optimized incubation condition of 200 μmol/L H2O2 for 24 hours. H2O2 inhibited expression of myocardin in mRNA and protein levels, and increased expression of Nrf2 in mRNA and protein levels. The overexpression of myocardin or the knockdown of Nrf2 significantly decreased cell viability compared with the control group, while the knockdown of myocardin or the overexpression of Nrf2 significantly increased cell viability. The overexpression of myocardin significantly down-regulated the expression of Nrf2 in mRNA and protein levels, while the knockdown of myocardin dramatically up-regulated the expression of Nrf2. Thus, it is deduced that myocardin may inhibit cell proliferation and Nrf2 may promote cell proliferation. Oxidative damage induced by H2O2 in rat myocardial cell might activate Nrf2-related signaling pathway through down-regulation of myocardin.

  • Clinical application of cone beam CT guided technique in diagnosis of pulmonary nodules

    ObjectiveTo explore the clinical application of the comprehensive guidance technologies, such as cone beam computed tomography (CBCT), virtual bronchoscopic navigation (VBN), and superimposed high-frequency jet ventilator for respiratory control in the biopsy of peripheral pulmonary nodules (PPNs). MethodsThe clinical information of 3 patients with PPNs diagnosed by CBCT combined with VBN and superimposed high frequency superposition jet ventilator in Shanghai Changhai Hospital were retrospectively analyzed. Results Clinical data of 3 patients were collected. The average diameter of PPNs was (25.3±0.3) mm with various locations in left and right lung. The first nodule was located in the apex of the left upper lung, and the biopsy was benign without malignant cells. The lesion was not enlarged during the 5-year follow-up. The second one was located in the left lingual lung, and the postoperative pathology was confirmed as mucosa-associated lymphoma. The third one was located in the anterior segment of the right upper lung. After the failure of endobronchial procedure, percutaneous PPNs biopsy under CBCT combined with VBN was performed, and the pathological diagnosis was confirmed as primary lung adenocarcinoma. Postoperative pneumothorax complication occurred in the third patient with right lung compression rate approximately 20%. ConclusionsThe application of CBCT, combined with VBN and the superimposed high frequency jet ventilator for respiratory control can potentially improve the accuracy and safety in the diagnosis of PPNs. Multi-center clinical trials are needed to verify its further clinical application.
