  • Progress of Postoperative Fluid Administration for Colorectal Surgery in Fast Track

    Objective To investigate the application progress of postoperative fluid administration in colorectal surgery. MethodsLiteratures about the advancement of fluid administration in colorectal surgery were reviewed and analyzed. Results Compared to standard fluid management, restrictive fluid administration could reduce the incidence of complications, the length of stay in hospital and improve postoperative survival rate. Colloid-crystalloid combined therapy was better than that pure crystal therapy. Conclusion Volume and type of rehydration influence postoperative recovery, which is also considered in “fast track” colorectal surgery.

  • Research on masking effect of multiple natural sounds for monotone-like tinnitus

    This paper focuses on whether masking signal with natural sounds is an effective way for mitigating annoyance caused by monotone-like tinnitus. Four natural sounds, pink noise, and narrow bandwidth noise were applied as maskers in the present study shown in this paper. Two monotones were applied as maskees. As a basis for research, natural sounds do not necessarily mask monotones entirely. According to the results, spring water and rain noise can be selected as effective noise for masking monotones. In the case of monotones with different frequency, the same masker performed a little differently on masking. We preliminarily demonstrated the feasibility of natural sounds for masking monotone-like tinnitus, which is of great use for improving maskers in tinnitus masking therapy.

  • Clinical Effect of Restrict Rehydration Strategy for Patients with Colorectal Cancer Combined Diabetes

    Objective To explore the impact of restrictive fluid administration for patients with colorectal cancer combined diabetes. Methods The clinical data of patients diagnosed definitely as colorectal cancer with diabetes were analyzed retrospectively from January 2007 to October 2009 in this hospital, the clinical effects on postoperative early rehabilitation were studied and the differences between restrictive fluid regimen (fluid restriction group) and tradition fluid regimen (tradition therapy group) were compared. Results The time of first aerofluxus and the first ambulation in fluid restriction group were shorter than those of tradition therapy group, the differences had statistical significances (Plt;0.05). The incidence of wound infection in fluid restriction group was lower than that in tradition therapy group (Plt;0.05). The differences of preoperative hemoglobin (Hb), white blood cell (WBC), glucose (GLU) and blood urea nitrogen (BUN) were not statistically significant between two groups, but the difference of postoperative GLU was statistically significant between two groups (Plt;0.05). Conclusion Restrictive fluid regimen can reduce the incidence of common complications after colorectal surgery for diabetic, and has a certain promoter action to the early rehabilitation after rectal surgery.

  • Clinical Effect of Different Strategies for Surgical Treatment of Rectal Cancer Combined with Neo-Adjuvant Chemotherapy under Multi-Disciplinary Team

    Objective To explore the clinical effect of different strategies for surgical treatment of cancer of rectum combined with neo-adjuvant chemotherapy under multi-disciplinary team. Methods From January 2007 to December 2007, the patients diagnosed definitely as rectal cancer were analyzed retrospectively, of whom accept surgery combined with neo-adjuvant chemotherapy were included. The clinical effects were studied and observed, the differences among the strategies for different surgical treatment 〔high anterior resection (HAR), low anterior resection (LAR), out-pouching with colo-anal anastomosis (OCAA), radical resection with non-saving anus (RNSA), and palliation colostomy (PCO)〕 in the multi modality therapy were compared. Results Comparing the constituent ratio of gross type of tumor in five groups, the difference between PCO group and the other 4 groups had mainly statistical significance (P<0.05). The difference of constituent ratio of Dukes staging had statistical significance between HAR group and OCAA group, HAR group and PCO group, LAR group and PCO group (P<0.05). According to the duration of operation, the persisting time of PCO group was obviously shorter than that of the other 4 groups (P<0.05), meanwhile, the persisting time of LAR group was shorter than that of OCAA group and RNSA group (P<0.05). According to the volume of ascites discovered in operation, mainly between HAR group and LAR group, between HAR group and RNSA group, the incidence of a seroperitoneum more than 200 ml in the former was less than the latter (P<0.05). Regarding to indexes of laboratory, CEA value in PCO group was higher than the other 4 groups, serum amyloid A value in RNSA group was higher than the other 4 groups, and the differences had statistical significances (P<0.05). Conclusion Mini-invasive surgery may have certain significance in clinical effect of the comprehensive treatment for advanced rectal cancer when different operative methods were combined with neo-adjuvant chemotherapy. And by the way, better clinical effect will take place when the anus-retained operation is possibly performed.
