  • Iris-Registration in Wavefront-Guided LASIK versus Conventional LASIK for Correction of Myopia and Myopic Astigmatism: A Meta-Analysis

    Objective To systematically evaluate the efficacy and safety of iris-registration in wavefront-guided LASIK (IR+WG LASIK) versus conventional LASIK for correction of myopia accompanied with astigmatism. Methods Such databases as PubMed, EMbase, The Cochrane library (Issue 2, 2012), CBM, CNKI, VIP, and WangFang Data were searched to collect the randomized controlled trials (RCTs) and quasi-RCTs about IR+WG LASIK versus conventional LASIK for correction of myopia accompanied with astigmatism. The retrieval time was from inception to February 2012, and the language was in both Chinese and English. Two reviewers independently screened the literature, extracted the data and assessed the quality of the included studies. Then the meta-analysis was performed by using RevMan 5.1 software. Results A total of 9 studies involving 3 903 eyes were included. The results of meta-analysis showed that, compared with the conventional LASIK group, the IR+WG LASIK group had a higher ratio in patients with postoperative uncorrected visual acuity no less than 1.0 (RR=1.03, 95%CI 1.01 to 1.05, P=0.002), as well as in patients with best-corrected visual acuity gained over 1 line (RR=1.75, 95%CI 1.49 to 2.16, Plt;0.000 01); it was smaller in the postoperative high order aberration RMS (WMD=−0.16, 95%CI −0.21 to −0.11, Plt;0.000 01), coma-like RMS (WMD=−0.05, 95%CI −0.11 to 0.00, P=0.07), spherical-like RMS (WMD=−0.15, 95%CI −0.23 to −0.07, P=0.000 2), and residual astigmatism (WMD=0.14, 95%CI 0.10 to 0.18, Plt;0.000 01); moreover, it was lower in the incidence of postoperative glare (RR=0.27, 95%CI 0.15 to 0.50, Plt;0.000 1), and it was higher in the subjective satisfaction of patients (RR=1.08, 95%CI 1.04 to 1.13, P=0.000 3). Conclusion Compared with conventional LASIK, IR+WG LASIK can more effectively reduce astigmatism, postoperative high order aberration RMS and spherical-like RMS. It can also get visual function including uncorrected visual acuity and best-corrected visual acuity, consequently increase patient’s satisfaction. But further studies are still required for its long-term effect.

  • Investigation of the Exercise Situation in the Elderly Diabetic Patients

    【摘要】 目的 了解老年糖尿病患者运动锻炼情况。 方法 以成都某教学医院2009年2-8月住院的老年糖尿病患者106例为调查对象,采用自行设计的问卷对老年糖尿病患者进行运动锻炼和相关知识及自我效能调查。 结果 35.8%的患者近1个月有运动锻炼,仅有25.5%坚持运动锻炼,43.4%的患者运动锻炼效能低,不同年龄、性别、文化水平、婚姻状况、收入、糖尿病病程与是否有锻炼行为无明显差异。 结论 老年糖尿病患者运动锻炼行为较差,运动锻炼相关知识缺乏,需要提高老年糖尿病患者对运动锻炼相关知识的认识,增加患者运动锻炼行为,提高患者的自我效能,提高老年人的生活质量。【Abstract】 Objective To understand the situation of exercise in the elderly diabetic patients.  Methods At a teaching hospital in Chengdu, adopting the self-designed questionnaire, we investigated 106 elderly diabetic patients on their situation of doing exercises, the related knowledge and the self-potency, from February to August 2009. Results A total of 35.8% of the elderly diabetic patients did exercises within the recent month. Only 25.5% insisted doing exercises, and 43.4% of the patients’ exercise potency was low. There was no correlation between the exercise situation and age, sex, educational level, marital status, income, and duration of diabetes (Pgt;0.05). Conclusions The elderly diabetic patients′ exercise behavior is poorer, and they lack the related knowledge. Consequently, they need to enhance their exercise related knowledge, increase their exercise behavior to improve their self-efficacy and the quality of life.

  • Prognostic prediction models based on peripheral biomarkers for non-small cell lung cancer: a systematic review

    ObjectiveTo systematically review the prediction models of blood-based biomarkers for non-small cell lung cancer (NSCLC). MethodsThe PubMed, Embase, Cochrane Library, Web of Science, VIP, WanFang Data and CNKI databases were electronically searched to collect studies related to the objectives from inception to June, 2023. Two reviewers independently screened literature, extracted data and assessed the risk of bias of the included studies. Meta-analysis was then performed by using RevMan 5.4.1 software. ResultsA total of 8 studies were included and all of them were retrospective cohort studies. The models were internally validated in 2 studies and externally validated in 4 studies. The performances of the eight predictive models were stable, which was measured by the area under the curve of receiver operating characteristic curve lying between 0.664 and 0.783. However, the risk of bias was high, which may mainly be reflected in data processing, model validation and performance adjustment. Meta-analysis showed that LDH (HR=1.86, 95%CI 41.32 to 2.63, P<0.01), dNLR (HR=2.15, 95%CI 1.56 to 2.96, P<0.01) and NLR (HR=1.71, 95%CI 1.08 to 2.69, P=0.02) were independent factors of prognosis for NSCLC patients. Conclusion Current evidence shows that the NSCLC prediction models based on peripheral blood biomarkers are still in the development stage, and the models have a high risk of bias.

  • Comparison of Endoscopic Dacryocystorhinostomy with Mitomycin C versus without Mitomycin C for Nasolacrimal Obstruction: A Meta-Analysis

    Objective To systematically review the effectiveness of endoscopic dacryocystorhinostomy (En-DCR) with versus without Mitomycin C (MMC) for nasolacrimal obstruction. Methods Databases such as PubMed, EMbase, CENTRAL (Issue 12, 2012), VIP, WanFang Data, CBM and CNKI were electronically searched to collect the randomized controlled trials (RCTs) which investigated the comparison between En-DCR with and without MMC for nasolacrimal obstruction. The searched data was updated to December 31st, 2012. According to the inclusion and exclusion criteria, literature was screened, data were extracted, and the methodological quality of the included studies was also assessed. Then, meta-analysis was performed using RevMan 5.2.0 software and the quality of evidences was graded using GRADEpro 3.6 software. Results A total of 9 RCTs were included in the meta-analysis. The results of meta-analysis showed that, the recovery rate in the MMC group was significantly elevated (RR=1.13, 95%CI 1.04 to 1.22, P=0.003), the area of ostium in the MMC group was bigger at 1, 6 and 12 months than in the control group, postoperatively (MD=6.68 mm2, 95% 5.43 to 7.94, Plt;0.000 01; MD=11.61 mm2, 95%CI 4.67 to 18.55, P=0.001; MD=15.65 mm2, 95%CI 2.95 to 28.34, P=0.02), respectively, but the area of ostium in the MMC group at the third month was bigger than that in the control group (MD=8.20 mm2, 95%CI –6.67 to 23.08, P=0.28). The operative time was significantly prolonged in the MMC group (MD=10.1 min, 95%CI 8.00 to 12.20, Plt;0.000 01). Conclusion En-DCR combined with MMC could improve the recovery rate and prevent the over shrinkage of ostium area effectively due to nasolacrimal obstruction.
