  • Body Compositions of Patients with Carcinoma Who Had Undergone Chemotherapy

    【摘要】 目的 利用人体成分分析仪测定肿瘤化疗患者人体组成,进而探讨肿瘤化疗患者人体成分组成特点及与营养状况的关系。 方法 用人体成分分析仪于2008年4-5月对50例肿瘤化疗患者行人体成分测定并分析。 结果 肿瘤化疗患者人体细胞内液、外液,体内水分、蛋白质、矿物质、骨骼肌含量等成分存在不足,且有30%的肿瘤化疗患者体脂肪含量过剩,40%体脂肪百分比超标。 结论 肿瘤化疗患者由于疾病自身的高消耗,化疗药物对机体的影响,导致摄入不足,营养状况较差,各成分含量均异常。其营养问题应受到重视,并采取积极有效的营养支持以改善营养状况。【Abstract】 Objective To detect the body compositions of patients with carcinoma who had undergone chemotherapy, and to analyze the features of the composition and its relationship with nutritional status. Methods Bioelectrical impedance analyzer was used to measure and analyze the body compositions of 50 patients who was underwent chemotherapy from April to May 2008. Results The compositions such as intracellular and extracellular water, total body water, protein, minerals, skeletal muscle mass were insufficient in patients who had undergone chemotherapy; 30% of the patients had excess body fat, and 40% of the patients′ body fat percent was over standard. Conclusion Because of the high consumption of disease itself and the effects of chemotherapy drugs on the body, the intake of the patients who have undergone chemotherapy is insufficient and the nutritional status is poor. We should actively evaluate their nutritional status and do some effective nutritional supports to improve the nutritional status of patients with carcinoma who have undergone chemotherapy.

  • Mid-term Clinical Effect of Posterior Annular Decompression for Thoracolumbar Burst Fractures and Related Problems

    目的 观察后路环形减压治疗胸腰椎爆裂骨折术后2~5年的临床疗效及并发症发生情况。 方法 回顾性分析2007年1月-2011年3月23例胸腰椎椎体爆裂骨折患者资料,23例患者存在骨折压迫硬膜合并神经症状,均予后路环形减压。术后定期随访,采用日本骨科协会评估治疗分数、美国脊髓损伤协会脊髓损伤分级评定临床疗效及神经功能改善情况,通过影像学资料观察脊柱Cobb角变化情况。 结果 23例患者手术顺利,经过2~5年的随访,出现术后脑脊液漏3例,尿路感染5例,经对症处理后好转。 结论 经椎弓根内侧行椎体后壁切除,可良好减压,避免神经挤压继发加重损伤,有利于神经功能恢复。

  • Dynamic decrease of platelet count predicts poor outcome of acute respiratory distress syndrome

    ObjectiveTo investigate the relationship between platelets changes and outcomes of acute respiratory distress syndrome (ARDS) patients.MethodsA total of 275 ARDS patients treated in Zhongshan Hospital of Fudan University were retrospectively enrolled from 2008 to 2015. Their clinical characteristics, experimental test results and disease outcomes were obtained from the archived medical records. The correlation between the decreasing of platelet within three days and the prognosis of ARDS and in each subgroup were analyzed by statistical methods, including COX analysis and Kaplan-Meier curve.ResultsThere were 233 patients validly selected through eliminating those with exclusion criteria. They were divided into a decreasing group and a non-decreasing group according to their platelet counts in three days. There was significant difference in the ontcome between the two groups with the univariate analysis, the COX analysis and Kaplan-Meier curve (all P<0.05). According to the initial platelet count and change of platelet in 3 days all the patients were categorized into 9 subgroups. The mortality among them was compared and two risk groups were defined, including a persistently low platelet group (the initial platelet count <139×109/L with an increase less than 16×109/L during the first three days after the diagnosis), and a decreased platelet group (the initial platelet count ≥139×109/L and platelet count decreased more than 30×109/L during the first three days after the diagnosis). The other subgroups made up a non-risk group. Merging two risk groups as one risk group and comparing with the non-risk group, there were significant differences in the outcome between two groups with the univariate analysis, the multiplicity COX analysis and Kaplan-Meier curve (all P<0.05), the differences of coagulation function indexes were not significant (allP>0.05). The platelet count of the risk group was also an independent risk factor for ARDS mortality in the surgery subgroup (P=0.003), the non-hypertension subgroup (P=0.018) and the pneumonia subgroup (P<0.001).ConclusionLow platelets and declining platelets are closely associated with poor prognosis in most ARDS patients, which might be applied in clinical prognosis evaluation.

  • Correlation of treatment compliance analysis with lung function and control level in asthma

    ObjectiveTo analyze the relationship between medication compliance of patients with uncontrolled asthma and lung function,airway inflammation level, asthma control level and quality of life so as to obtain important references for improving patient compliance and asthma control level in the future. MethodsQuestionnaires were performed in asthma patients who did not achieve asthma control and had poor compliance in 32 third-class hospitals in 28 provinces of China mainland. All patients were tested for lung function and airway inflammation levels. So the relevant data of asthma compliance was investigated and analyzed. ResultsA total of 923 patients were investigated and the questionnaire recovery rate was 100%. Two hundred and forty-three (26.33%) answered cognitive related questions about asthma completely correctly. Treatment compliance in asthma patients was positively correlated with lung function and significantly negatively correlated with exhaled nitric oxide. Better treatment compliance in asthma has higher level of asthma control and quality of life. Poor compliance in asthma patients will lead to decreased lung function and elevated levels of airway inflammation, resulting in decreased asthma control and quality of life. ConclusionAsthma treatment compliance is related to lung function, airway inflammation, asthma control level and quality of life.
