  • Relation between age of patients with colorectal cancer and decision-making and treatment effect of neoadjuvant regimens: a real-world study based on DACCA

    ObjectiveTo analyze the relation between the age of patients with colorectal cancer and neoadjuvant therapy (NAT) regimen decision-making and outcomes in the current version of the Database from Colorectal Cancer (DACCA). MethodsThe version of DACCA selected for this analysis was updated on January 5, 2022. The patients were enrolled according to the established screening criteria and then assigned to 3 age groups: ≤45, 45–65, and ≥65 years old groups. The differences in the NAT regimen decision-making and changes of symptom, imaging, and cancer markers in these 3 age groups were analyzed. ResultsA total of 4 882 data that met the screened criteria were enrolled. The results of statistical analysis showed that the difference in the constituent ratio of patients chosen NAT strategies among 3 age groups was not statistically significant (χ2=8.885, P=0.180). There was a statistical difference in the constituent ratio of patients chosen combined target drug among 3 age groups (χ2=8.530, P=0.014), it was found that the proportion of the patients with ≤45 years old adopting combined target drug regimen was higher. Although the changes of symptom (H=12.299, P=0.056), image (H=1.775, P=0.412), and cancer markers (H=11.351, P=0.183) had no statistical differences of the 3 age groups after NAT, it was found that the proportions of patients with ≥65 years old with progresses of symptom and imaging changes and elevated cancer markers after NAT were higher, and the proportions of patients with ≤45 years old with complete and partial remissions of symptom and imaging changes and with normal cancer markers after NAT were higher. ConclusionsThrough analysis of DACCA data, it is found that in the selection of NAT strategy for colorectal cancer, the lower age group, the higher proportion of patients adopting combined target drug regimen. Although it is not found that age is related to changes of symptoms, imaging, and cancer markers after NAT, it still shows a trend of better outcomes in younger patients.

  • The predictive value of preoperative NRS2002 score for survival in patients with colorectal cancer: an analysis based on the DACCA database

    ObjectiveBy mining data from the Database of Colorectal Cancer (DACCA) at West China Hospital of Sichuan University, this study aims to evaluate the relationship between nutritional risk screening (NRS) 2002 scores at initial diagnosis and long-term survival in patients. MethodsThe DACCA database version from November 24, 2023, was selected to compare the clinicopathological data of patients with NRS2002 scores <3 and ≥3, and to explore the impact of NRS2002 scores on survival. ResultsA total of 723 patients were screened, with 585 (80.9%) had NRS2002 scores <3 and 138 (19.1%) had NRS2002 scores ≥3. All 723 patients were followed up, with a follow-up period ranging from 1 to 78 months and a median follow-up time of 34 months. The median survival time for patients with NRS2002 scores <3 was 35 months, while it was 31 months for those with NRS scores ≥3. During the follow-up period, 589 patients (81.5%) survived, including 515 (71.2%) tumor-free survivors and 74 (10.2%) survivors with tumors. There were 134 deaths (18.5%), including 126 cancer-related deaths (17.4%) and 8 non-cancer-related deaths (1.1%). Multivariate logistic regression results showed that after controlling for 6 factors including age, radical surgery, adjuvant therapy, hypertension, differentation, and TNM staging, NRS2002 score was not a factor affecting the survival of colorectal cancer patients (RR=0.98, P=0.875). ConclusionNRS2002 score is not a predictive factor for the survival of colorectal cancer patients, possibly because although patients may have nutritional risks preoperatively, the long-term impact on survival is minimal following surgery and postoperative recovery.

  • Whole process management of hepatocellular carcinoma patients with high-risk of recurrence and metastasis based on multidisciplinary team mode

    ObjectiveTo summarize the experience of the whole process management of hepatocellular carcinoma (HCC) patients with high-risk of recurrence and metastasis based on the multidisciplinary team (MDT) mode, and to improve the clinicians’ understanding of the concept of whole process management, so as to improve the survival rate of patients with HCC. MethodThe clinicopathologic data of a HCC patient with high-risk of recurrence and metastasis admitted to the Division of Liver Surgery, Department of General Surgery, West China Hospital of Sichuan University were retrospectively analyzed. ResultsA 52-year-old male patient was diagnosed with HCC with intrahepatic metastasis (China liver cancer staging Ⅱ b, Barcelona Clinic Liver Cancer stage B) after admission due to “epigastric discomfort for 1+-month and liver occupying for 1+-week”. Through discussion by the MDT mode, the allogeneic liver transplantation was performed after successful downstaging following two conversion therapies. No serious complications occurred after operation, and the patient was discharged on the 23rd day after operation. Up to now, pulmonary bacterial and fungal infections and pulmonary metastases had been found during the postoperative follow-up. After anti-infective therapy and targeted therapy combined with radiotherapy, the patient was significantly relieved, had survived for 34 months after operation, and was still under regular follow-up. ConclusionsFor HCC patients with high-risk of recurrence and metastasis, MDT mode has a good clinical benefit for the whole process management of patient. Through the MDT model, the diagnosis, treatment, and follow-up of HCC are organically integrated, and the patient’ s diagnosis and treatment plans are dynamically adjusted to realize the whole process management of HCC patient, and to raise the survival rate and improve quality of life of HCC patient.
