  • Cyclooxygenase (COX)-2 Expression and the Clinicopathologic Characteristic of Cervical Cancer: A Meta-analysis of Case-control Studies

    Objective To investigate the expression of COX-2 in human cervical cancer and explore their relationship between the COX-2 expression and the clinicopathologic characteristic of cervical cancer. Methods The published studies were searched in the CBMdisc (1979 to 2009), CNKI (1979 to 2009), VIP (1989 to 2009) and WANFANG Database (1982 to 2009), and other relevant journals were also hand searched, to identify all the relevant case-control trials. The quality of the included studies was assessed. The Cochrane Collaboration’s software RevMan 4.2.10 was used to test the heterogeneity, overall effect and publication bias of the combined studies. Results A total of 9 studies were recruited. As for the positive rate of COX-2 expression, significant differences was tested between cervical cancer vs. normal cervical tissues, lymph node metastasi vs. non-lymph node metastasi, clinical stages I-II vs. clinical stages III-IV, cell differentiation G1 vs. cell differentiation G2-G3 and cervical squamous cell carcinoma vs. adenocarcinoma with OR (95%CI) at 28.03 (9.53 to 82.50), 5.16 (3.36 to 7.93), 0.53 (0.33 to 0.84), 3.11 (1.86 to 5.22) and 5.00 (2.68 to 9.35) respectively. Conclusions According to the domestic evidence, higher COX-2 expression might be associated with cervical cancer. However, more high quality case-control studies are expected for further study.

  • MODIFIED SINGLE-STAGE TRANSPEDICULAR DECOMPRESSION, DEBRIDEMENT, AND POSTERIOR INSTRUMENTATION IN TREATMENT OF THORACIC TUBERCULOSIS

    Objective To investigate the effectiveness and feasibil ity of modified single-stage transpedicular decompression, debridement, and posterior instrumentation in treatment of thoracic tuberculosis. Methods Between January 2005 and December 2009, 22 cases of thoracic tuberculosis were treated with modified single-stage transpedicular decompression, debridement, and posterior instrumentation. There were 12 males and 10 females with an average age of 39.4 years (range, 22-52 years). The mean disease duration was 1.2 years (range, 3 months to 10 years). The involved vertebral bodies were T5-12, including 2 segments in 17 cases and 3 segments in 5 cases. The kyphosis Cobb angle was (31.2 ± 14.5)° before operation. According to Frankel score system for neurological deficits, 2 cases were classified as grade A, 1 case as grade B, 8 cases as grade C, 5 cases as grade D, 1 case as grade E, and 5 cases had no neurological deficits before operation. Results All incisions healed by first intention. All patients were followed up 22.2 months on average (range, 12-65 months). Pain in low back was rel ieved in varying degrees 2 weeks after operation. Fusion was achieved in the bone implant area at 3 months after operation. According to Frankel score system, 1 case was rated as grade B, 2 cases as grade C, 4 cases as grade D, 7 cases as grade E, and 8 cases had no neurological deficits at last follow-up. The kyphosis Cobb angle was (16.2 ± 3.6)°, showing significant difference when compared with the value before operation (t=5.952, P=0.001). No loosening, emersion, breakage of internal fixation or pneumothorax occurred 1 year after operation. Conclusion Single-stage transpedicular decompression and posterior instrumentation is an effective and safe method in treatment of thoracic tuberculosis.

  • Analysis of Psychological States of New Nurses and Their Coping Styles

    【摘要】 目的 了解新护士的心理健康状况及应付方式。 方法 采用心理卫生自评量表(SCL-90)和应付方式量表对2008年1月-2009年12月新上岗的64名护士进行测查。 结果 新护士强迫症状、抑郁、焦虑、敌对、恐怖、偏执、精神病性因子分、总均分及阳性项目数与全国常模比较偏高,差异有统计学意义(Plt;0.05);SCL-90阳性者与阴性者应付方式比较,阳性者自责、幻想、退避、合理化因子均分均高于阴性者,差异有统计学意义(Plt;0.01);新护士应付方式各因子均分、退避、幻想、自责因子均高于吴超等人的研究(Plt;0.05)。 结论 新护士心理健康状况低于一般人群;较其他年龄段护士更多采用不成熟应付方式;成熟应付方式有益心理健康。护理管理者应重视新护士心理健康,引导新护士多采取成熟应付方式,提高心理健康水平。【Abstract】 Objective To evaluate the psychological states of new nurses and their coping styles. Methods Sixty-four nurses starting work between January 2008 and October 2009 in our hospital were investigated by Symptom Checklist 90 (SCL-90) and Coping Style Questionnaire. Results The scores of compulsion and mental disorder, average score, and the number of positive items were statistically higher than the national norm of China (Plt;0.001). The scores of athymia, anxiety, hostility, and phobia factors were higher than the national norm (Plt;0.01). The score of paranoid factor was also statistically higher than the national norm (Plt;0.05). Based on the results of SCL-90, we found that the scores of factors of self-accusation, fantasy, back-off (Plt;0.05) and the factor of rationalization (Plt;0.01) in the positive group were significantly higher than those of the negative group. Compared with the result of the research made by WU Chao and his colleges, the average scores of such factors as back-off (Plt;0.001), fantasy (Plt;0.01) and self-accusation (Plt;0.05) were statistically higher in the study group than those in the control group. Conclusion The psychological states of new nurses are worse than the general population. Compared with older nurses, new nurses are more inclined to use immature coping styles. Mature coping styles are favorable to their psychological states. The nursing regulators should pay more attention to the psychological states of new nurses and guide them to use mature coping styles to improve their psychological states.

  • Analysis of Knowledge and Attitude Related to Evidence-Based Medicine Course in Clinical Medical Students

    Objective To investigate the effects of evidence-based medicine (EBM) course on clinical medical students and to propose teaching advice. Methods Using a predesigned questionnaire, we conducted an investigation on the literature retrieval, knowledge of EBM terms, and subject attitude of clinical 5-year and 7-year medical students before and after EBM course, and then an interview was performed to collect the opinions of the students. Data was statistically analyzed. Results After the course, average reading time and frequency of literature retrieval increased significantly compared to the situation before EBM course (Plt;0.05). Knowledge levels of main EBM terms related to practice increased significantly (Plt;0.05). 5-year medical students’ ability of literature appraisal also increased (Plt;0.05). In this interview, these students suggested that course time of literature retrieval and screening should increase and medical statistics should be reviewed. Conclusion Through the study of EBM course, both knowledge and attitude of students changed a lot, and combining case teaching with EBM course has a better teaching effect. Learning the concepts and techniques of EBM for clinical medical students can help them apply medical research evidence correctly in clinical practice, and train their self-learning ability.

  • Prevention of hemodynamics response to tracheal extubation by administration of lidocaine prior to extubation: a meta-analysis

    ObjectivesTo systematically review the efficacy of lidocaine injected prior to tracheal extubation in preventing hemodynamic responses to tracheal extubation in general anesthesia.MethodsPubMed, Ovid, Web of Science, EMbase, The Cochrane Library, CBM, CNKI, VIP and WanFang Data databases were electronically searched to collect randomized controlled trials (RCTs) on the efficacy of lidocaine administrated prior to extubation in preventing hemodynamic responses to tracheal extubation in patients undergoing general anesthesia from inception to October, 2018. Two reviewers independently screened literature, extracted data and assessed risk of bias of included studies, then, meta-analysis was performed by using RevMan 5.3 and Stata 13.0 software.ResultsA total of 10 RCTs involving 525 patients were included. The results of meta-analysis showed that: compared with control group, lidocaine could reduce mean arterial pressure in 5 min after extubation (MD=–5.10, 95%CI –9.41 to –0.79, P=0.02), weaken the increase in systolic blood pressure caused by extubation from the moment before extubation to 5 minutes after extubation (before extubation: MD=–7.22, 95%CI –10.34 to –4.11, P<0.000 01; at extubation: MD=–14.02, 95%CI –19.42 to –8.62, P<0.000 01; 1 minutes after extubation: MD=–15.82, 95%CI –22.20 to –9.45, P<0.000 01; 3 minutes after extubation: MD=–12.55, 95%CI –20.36 to –4.74, P=0.002; and 5 minutes after extubation: MD=–12.05, 95%CI –20.35 to –3.74, P=0.004), and weakened extubation-induced increase in diastolic blood pressure at extubation (MD=–9.71, 95%CI –16.57 to –2.86, P=0.005). In addition, lidocaine inhibited heart rate in all time points except the moment of before and at 10 minutes after extubation.ConclusionsCurrent evidence shows that lidocaine can inhibit the increase in blood pressure and heart rate caused by extubation at certain times. Due to limited quality and quantity of the included studies, more high-quality studies are needed to verify above conclusions.
