  • VASCULAR RECONSTRUCTION OF INFECTED PSEUDOANEURYSM IN 17 DRUG ADDICTS

    Objective To evaluate surgical treatment of infected pseudoaneurysm (PA) caused by injection of addictive drugs. Methods The clinical data of 17cases of infected PA caused by drug injection were reviewed retrospectively. Ofthem, 1 case was female, and the other 16 cases were male, aging 24-38 years. The locations were brachial artery in 1 case and femoral artery in 16 cases. Fourteen cases were treated by artificial blood vessel transplantation after resection of the aneurysm and radical debridement, 1 case by amputation for serious infection, and 2 cases by non-operation for different reasons. Results Among 14 cases treated by prosthetic grafts, 13 cases achieved satisfactory results, no blood drawback of lower extremity occurred after operation; 1 case had to be amputated for serious infection. After 3 months of operation, the color Doppler examination showed that the 9 artificial vessel grafts were patent and the pulse of arteria dorsal pedis was good. Conclusion Though artificial vessel grafting has high risk in the vicinity of infected PA, it is still an effective procedure for revascularization in condition that there is no suitable autogenous conduit available for auto transplantation. Sufficient drainage, anti-infection and anti-coagulation therapy should be the key factors after the operation.

  • Rapid Assessment of Health Needs after Disasters: A Systematic Review

    Objective To provide references for fast assessment after disasters in China by assessing articles on fast assessment of health needs after disasters. Methods We systematically searched CNKI, VIP, CBM, and PubMed from their establishments to Jan. 2011. The main results of the eligible articles were described. Results A total of 65 articles were included, of which introductions for methodology and cross-sectional studies accounted for 52.31% and 44.62%, respectively. Articles which focused on natural disasters made up 86.15% of the total. Post-disaster rapid assessment should include seismic death estimate and on-site public health assessment which contained the public health status in affected areas, health coping capacity, health condition of victims and health needs. Rapid assessment of seismic death mainly used methods based on GIS and ANN model. On-site public health assessment mainly used epidemiological field investigation. Questionnaires, interviews and surveillance of diseases were adopted. Conclusion Rapid post-disaster health needs assessment is one of the most important tasks after disasters, and the current rapid assessment methods need to be refined and improved.

  • Comparative analysis of high ligation and foam sclerotherapy or spot stripping in treatment of great saphenous varicose veins

    Objective To compare the effect of high ligation and spot stripping (HLSS) and high ligation and foam sclerotherapy (HLFS) in the treatment of great saphenous varicose veins (GSVV). Methods A retrospective analysis was conducted on the clinical data of 385 patients with GSVV who received HLFS or HLSS treatment at Guizhou Provincial People’s Hospital from January 2018 to June 2022. The differences in surgical time, intraoperative bleeding, complications, and postoperative hospital stay between the two groups were compared. Results The surgical time, intraoperative bleeding, postoperative hospital stay, visual analog scale for pain, incidence of postoperative hematoma, skin sensory disorders, and infection in the HLFS group were less or lower than those in the HLSS group (P<0.05), but there was no statistically significant difference in recurrence rate between the two groups (P>0.05). Compared with preoperative indicators at the same group, the modified venous clinical severity score (VCSS) decreased and the chronic lower limb venous insufficiency questionnaire (CIVIQ) score increased (P<0.05). However, there was no statistically significant difference in the degree of improvement in VCSS and CIVIQ between the two groups at three months after surgery and before surgery (P>0.05). Conclusions HLFS has a definite therapeutic effect on GSVV, with smaller trauma, faster recovery after surgery, and lower incidence of postoperative complication compared to HLSS. It can be used as a surgical option for GSVV patients in clinical practice.

  • Retrospective comparative analysis of unilateral neck exploration and direct minimally invasive parathyroidectomy in the treatment of primary hyperparathyroidism

    ObjectiveTo compare the clinical effects of unilateral neck exploration (UNE) and direct minimally invasive parathyroidectomy (DPT) in the treatment of primary hyperparathyroidism (PHPT). MethodsThe clinical datas of 87 PHPT patients treated with intraoperative parathyroid hormone monitoring (IPM) -assisted UNE and DMIP in Guizhou Provincial People’s Hospital between January 2016 and December 2021 were analyzed retrospectively. ResultsThe average age of patients was (47.5±13.1) years, and there were 24 males (27.6%) and 63 females (72.4%). The pathological types of 87 cases were parathyroid adenoma in 78 cases (89.7%), atypical parathyroid adenoma in 5 cases (5.7%), parathyroid hyperplasia in 4 cases (4.6%), and no parathyroid carcinoma. Two cases (2.3%) developed recurrent laryngeal nerve injury, and 3 cases (3.4%) developed postoperative persistent hyperparathyroidism, of which 1 (1.1%) case underwent secondary surgery. There were no significant differences in the incidence of recurrent laryngeal nerve injury (2.0% vs. 2.6%, P=0.855) and postoperative persistent hyperparathyroidism between the UNE group and the DMIP group (2.0% vs. 5.3%, P=0.713). However, the intraoperative bleeding volume, operative time, hospitalization time, incision length, and pain score in the DMIP group were significantly less than or shorter than or lower than those in the UNE group (P<0.001). ConclusionThere is no significant difference in the efficacy between the DMIP and UNE in the treatment of PHPT, but DMIP can significantly shorten the operative time and hospitalization time, and reduce the trauma.

  • Comparative study of catheter-directed thrombolysis and anticoagulation alone in the treatment of acute proximal deep venous thrombosis

    ObjectiveTo compare the effect of catheter-directed thrombolysis (CDT) combined with anticoagulation (AC) and AC in the treatment of acute proximal deep venous thrombosis (APDVT) of the lower extremities. MethodsThe clinical data of 184 APDVT patients treated in Guizhou Provincial People’s Hospital from January 2017 to December 2022 were retrospectively collected. According to the treatment methods, the patients were divided into CDT group (n=82, CDT combined with AC) and AC group (n=102, AC alone). The prognosis indicators such as the incidence of bleeding events, the incidence of post-thrombotic syndrome (PTS), Villalta score, venous clinical severity score (VCSS) and chronic lower limb venous insufficiency questionnaire (CIVIQ) score were compared between the two groups. ResultsCompared with the AC group, the CDT group had a higher incidence of bleeding events [11.0% (9/82) vs. 2.9% (3/102)], a shorter time to detumescent [(2.8±1.2) d vs. (7.2±1.9) d], and lower VS score [3 (2,4) vs. 3 (2, 7)], VCSS score [2.0 (1.7, 4.0) vs. 3.0 (2.0, 5.2)] and postoperative venous patency score [1 (1, 2) vs. 2 (1, 3)], and higher CIVIQ score [80.0 (77.0, 86.0) vs. 71.5 (68.0, 78.0)], P<0.05. However, there were no significant differences in the incidence of PTS [28.2% (22/78) vs. 36.5% (35/96)] and thrombosis recurrence rate [9.0% (7/78) vs. 11.5% (11/96)] between the two groups (P>0.05). ConclusionCDT can relieve the symptoms and improve the quality of life of APDVT faster than AC, but it is necessary to strictly grasp the indications of thrombolysis to reduce the risk of bleeding.

  • Research on the Relationship between Mental Health Status and Perceived Social Support among Middle School Students from Wenchuan Earthquake Hit Area

    Objective To investigate the mental health status, perceived social support, as well as the relationship between them among middle school students in Wenchuan Earthquake region, so as to provide references for the relevant department to formulate appropriate strategies and intervention measures. Methods A cluster sampling method was adopted to select all 1698 students (excluded the students in Grade 3) as the research subjects from three middle schools in the disaster regions. All students were investigated with self-designed basic information questionnaire, Symptom CheckList 90 (SCL-90) and Perceived Social Support Scale (PSSS) through self-administered questionnaire. Results The overall positive rate on the SCL-90 was 46.3% among 1690 students, of whom 66.6% had mild psychological problems and 26.5% had moderate to severe psychological problems. The positive rate on the SCL-90 and the score for each factor of SCL-90 were statistically higher in the female students than male students (Plt;0.05). Compared with the senior middle school students, the score on the phobic anxiety subscale of the SCL-90 was statistically higher in the junior middle school students (Plt;0.05), while the score on the obsessive-compulsive subscale was otherwise statistically lower (Plt;0.05). With the exception of the hostility subscale, the score on each subscale of the SCL-90 in the urban students was statistically lower than the rural students (Plt;0.05). The median score on the perceived social support subscale was 60.00, with significant differences between the students of different sexes and grades (Plt;0.05). The overall score on the perceived social support subscale was negatively correlated with the SCL-90 score (r= –0.261, Plt;0.05). The score was negatively correlated with the SCL-90 score in the students regarding different sexes, grades and living areas prior to the earthquake (Plt;0.05). Conclusion The middle school students in the disaster regions have prominent psychological problems; perceived social support is helpful to promote the development of their mental health. In conducting psychological intervention, the difference of individuals’ mental health among different student groups should be concerned, so as to carry out targeted counseling and education.
