  • Research Status of Prosthesis-patient Mismatch

    Abstract:Since 1978 the definition of prosthesis-patient mismatch(PPM) was first introduced by Rahimtoola, it has attracted the attention of foreign researchers. The PPM phenomenon is concerned to be an important factor which will affect patients' survival rate and quality of life. Till now, a lot of effort should still be put on this problem. But unfortunately, this problem is still being neglected in our country. This paper is a review of PPM from it's many aspects, such as definition, current research status, prevention, and it's research prospect. We hope this paper will intensify the clinicians' knowledge on PPM.

  • Expression Change and Its Implication of Cell Adhesion Molecules CD11b/CD18, sPselectin in Perioperative Period of Cardiopulmonary Bypass

    Abstract: Objective To observe the significance of the changes of cell adhesion molecules (CAM) CD11b/CD18 and sPselectin during the perioperative period of open heart surgery under cardiopulmonary bypass (CPB), and investigate the roles of CD11b/CD18 and sPselectin in systemic inflammatory response triggered by CPB. Methods Thirty patients including 18 males and 12 females, age ranged from 29 to 55 years (45.3±8.1 years) having undergone valvular replacement for rheumatic heart disease in our hospital were selected as the subjects of this research. After anesthesia induction, radial arterial blood sample was collected at six different time points including the time prior to skin incision, and 30 min, 1 h, 6 h, 12 h and 24 h following the start of CPB. The expression levels of CD11b/CD18 were tested by flow cytometry, and concentration of sP-selectin in the plasma was measured with enzymelinked immunosorbent assay(ELISA). Results The expression of CD11b/CD18 was elevated at 30min after CPB, and it reached the peak (581.44±215.26) at 6 h after CPB with significant differences (Plt;0.05). Its expression started to drop at 12 h after CPB, but it was still higher than the expression level before CPB. The expression returned under the level before CPB at 24 h after CPB with insignificance differences (Pgt;0.05). The expression of sPselectin in the peripheral blood started to rise evidently at 30 min after CPB, reaching the peak (51.44±10.06 ng/ml) with significant differences (Plt;0.05). Its expression level decreased at 12 h after CPB and fell back below the level before CPB with insignificant differences (Pgt;0.05). Conclusion CPB can cause the expression of CD11b/CD18 and sPselectin to rise in the peripheral blood, which may play an important role in the systemic inflammatory response triggered by CPB.

  • Surgical Treatment of Aortic Dissection

    Objective To summarize the experience of surgical treatment for 84 patients with aortic dissection, investigate the surgical technique and perioperative treatment, and to improve surgical results. Methods The operations were performed in 50 patients with aortic dissection of Stanford A, Bentall or Cabrol operations were performed in 24 patients, graft replacement of ascending aorta in 8 patients, Trusler operation in 5 patients, Wheat operation in 5patients, operation of ascending aorta and aortic arch in 8 patients.Stentgraft were used in 34 patients with aortic dissection of Stanford B. Results There were 11 hospital death,the hospital mortality was 13.1%.There were 3 operative deaths.One patient with ascending aortic and hemiarch grafting died of aortic root bleeding;one patient with replacement of ascending aorta died of failure to restart heart beating;one patient with ascending aortic and hemiarch grafting died of rapture of aortic dissection. There were 8 postoperative deaths, including low cardiac output syndrome in 2 patients, lung infection in 2 patients, renal failure in 2 patients, respiratory failure in 1 patient and permanent mental anomaly in 1 patient. The complications were occurred in 16 patients. The followup period was 3 months to 10 years, and carried out in 62 patients(84.9%,62/73).One died of endocarditis, another one died of sudden death. Conclusion The surgicaltreatment of aortic dissection could be carried out safely based on the accurate diagnosis, specific surgical strategy and fine surgical technique.

  • A Modified Method for Aortic End Strengthening in Acute Type A Aortic Dissection Operation

    Objective To modify the method for aortic end strengthening in acute type A aortic dissection operation, and investigate its clinical efficacy. Methods We modified the method for aortic end strengthening in acute aortic dissection operation based on ‘Sandwich method’ in the department of thoracic and cardiovascular surgery of West China Hospital. From January 2006 to December 2008, twentyeight patients with acute type A aortic dissection underwent modified aortic end strengthening operation. We made adventitia turn over and enfold to strengthen the aortic end in 10 cases, and placed stripshaped felt or pericardium belts between dissection (between adventitia and intima)and inner intima and strengthened the aortic end by suture in 18 cases. The hemorrhage of anastomotic stoma and the postoperative early prognosis were observed. Results No bleeding complication was found in all the cases. Two cases died, one died of severe low cardiac output syndrome and another died of multiple organ failure. No nervous system complication was found except that 2 cases had delayed revival. No sternum and surgical incision related complication was found. The rest 26 cases were cured and discharged. Conclusion The modified method for aortic end strengthening can not only strengthen the aortic end but also make people be able to find the petechia of anastomotic stoma clearly, then stitch hemostasia could be done effectively. The method is easy to implement and effective, it should be extend in clinic.

  • A Clinical and Imageological Study on Endovascular Repair of Thoracic Aortic Aneurysm, Thoracic Pseudoaneurysm and Aortic Dissection

    Objective To summarize the critical point of diagnosis and endovascular repairment (EVR) to thoracic aortic aneurysm (TAA), thoracic pseudoaneurysm (TPA) and aortic dissection (AD), by comparison the computerized tomography angiography (CTA) images before and after EVR to observe effects, so as to explore a unique index of imageology to assess the pathological development and evaluate therapeutically effect in dynamic and systemic reviews in pre, intra, postEVR and followup period. Methods Fortyeight patients involving aneurysm or dissection of thoracic aorta were treated with EVR based on the preoperative CTA imaging. Before and after the introducing of stentgraft, digital substation angiography (DSA) was taken place and sequential enhanced CTA was followed to evaluate the effects of the treatment. All imagings of CTA and DSA were collected and induced into e-FilmTM database to select key sections for analyses and measurement. Results Fortynine EVR were preformed and 54 stent grafts were implanted in 48 cases, with endothelial tears sealed in 42 cases of dissection, aneurismal cavities excluded in 2 cases of aortic aneurysm, and rupture site closed in 4 cases of pseudoaneurysm. Endoleakage happened in 9 cases, which were treated successfully by appropriate measures. One case suffered hemorrhage from introducing artery (iliac) which was controled by surgery, but he died of disseminated intravascular coagulation and then multiple organs failure. Fortyseven cases were followed up in 6-51 months with a satisfied clinical effect. Conclusion EVR is favorable in the effect of repairment to true, false and dissection of thoracic descending aorta. Chest pain and CTA scan is the key of early diagnosis of aortic dissection. Certain sections and leftanterior oblique viewing are the crucial profile for assessment and evaluation before and after operation.

  • Skeletonized Internal Mammary Artery for Coronary Artery Bypass Grafting

    Objective To summarize the early outcomes and clinical experience in the use of skeletonized internal mammary artery(IMA) for coronary artery bypass grafting(CABG). Methods From January 2004 to June 2007, a total of 139 patients underwent CABG and received skeletonized arteries in this hospital. Results The number of distal anastomoses was 3.6±1.7,there was no sternal wound infection or thoracic cavity effusion. Two patients died (1.4%), the complications incidence was 5.8%(8/139) lung infections 3 cases, incision infections 2 cases, and low cardiac output syndrome 3 cases.All complications were well treated by using antibiotics, dressing change and positive inotrope, and the follow-up period was 2 to 34 months(20.6±5.9 months); 110 patients were followed up (80.3%). All living patients were free from angina after operation and showed I-II class heart function (New York Heart Association). Conclusion Using skeletonized IMA is? a safe and effective method in CABG.

  • The Effective Orifice Area and Haemodynamics of 19mm St. Jude Regent Valve——An Early in Vivo Study

    Abstract: Objective To investigate the in vivo effective orifice area (EOA) and whether prosthesispatient mismatch (PPM) is going to happen after 19mm St.Jude Regent valves replacement were performed. Methods Twentythree patients with valvular heart diseases were divided into 2 groups according to aortic annular diameter. 19mm St. Jude Regent valves were replaced in aortic valve place (Regent valve group), and 21mm other doubleleaflet mechanical valves were replaced in aortic place (other valves group). All of the operations were accomplished under cardiopulmonary bypass in West China Hospital. All of the patients were followed up in 3 to 6 months after their surgery processes. Color Doppler echocardiography was used to measure the hemodynamic parameters in their followup. Then left ventricular mass (LV mass), EOA and effective orifice area index (EOAI) etc. were calculated and compared by using SPSS 12.0. Results No cardiac episodes were detected during the perioperative period and 36 months after operation. LVmass reduced in 36 months after operation in both groups. No statistical significance of EOA, EOAI and LVmass reduction were detected between the two groups. Conclusions (1)The EOA and in vivo hemodynamic effects of 19mm Regent valve are similar to 21mm other double leaflet mechanical valve. (2)It is safe to use 19mm Regent valve in those patients whose aortic annular diameter are small and need double valve replacement. Using 19mm Regent valve will not cause short time cardiac episodes and PPM. (3)More further works should be done to make sure what is the standard of PPM in Chinese people, such as enlarging the patients number and prolonging the followup time. (4) Further clinical and followup works should be done to make St.Jude Regent valve’s feature out.

  • Correlation of Clinical Characteristics and Outcomes with Organ Injury Scaling in Penetrating Cardiac Trauma

    Objective To evaluate the relationship of the organ injury scale (OIS) with clinical characteristics and survival in penetrating cardiac trauma patients. Methods A retrospective review identified 224 patients with penetrating cardiac trauma. A grade assigned on the basis of the American Association for the Surgery of Trauma OIS was assigned to each case studied. Clinical data, including Glasgow coma scale(GCS), revised trauma score(RTS), injury severity score (ISS), penetrating thoracic trauma index (PTTI), penetrating trauma index (PTI), and outcomes were analyzed for association with OIS grade. Results The majority of patients were either grade Ⅳ/ or Ⅴ , with grade Ⅵ having the highest mortality. Patients of grade Ⅰ to Ⅲ were mostly Sub-clinical type patients, and grade Ⅳ to Ⅵ were mostly pericardial tamponade type and hemorrhagic shock type patients. The OIS grade was significantly negative correlated with postinjury duration and GCS (r=-0. 18, -0.16), and positive correlated with transfusion, ISS, PTTI, PT1, and death(r=0. 17,0. 22,0. 40,0. 41,0. 29). Conclusion The OIS appears to correlate with severity of injury and survival, and it may be useful in predicting outcomes in patients with penetrating cardiac injury.

  • Correlation between Perioperative Brain Glucose Metabolism Disorder and Postoperative Cognitive Dysfunction after Heart Valve Replacement Operation

    【摘要】 目的 了解心脏瓣膜置换术后认知功能障碍(POCD)的危险因素,明确围手术期脑糖代谢异常是否为术后POCD的可能发病机制之一。 方法 选取2004年3月-2009年7月心脏瓣膜置换术患者70例,根据术后第7天患者认知功能测定结果分为POCD组(n =31)和非POCD组(n =39),比较两组患者年龄、性别、体外循环(CPB)时间、主动脉阻断时间、主动脉阻断后平均动脉压和平均灌注流量,并且分别于麻醉后手术前(T1)、CPB稳定的低温期(T2)、复温至33℃(T3)、CPB结束后30 min(T4)4个时点采动脉及颈内静脉血,测定血气、血糖浓度,计算出脑摄氧率(CEO2)、脑糖摄取Glu(a-v)。 结果 发生POCD的患者年龄较非POCD组大,并且CPB时间和主动脉阻断时间也更长,两组之间比较有统计学差异(Plt;0.05),但是两组在性别、主动脉阻断后平均动脉压和平均灌注流量比较无统计学差异。两组的CEO2变化趋势一致,各时点的值无明显变化,两组间比较无差异;非POCD组的Glu(a-v)各时点间比较无统计学差异,POCD组的Glu(a-v)T1、T2时点也无明显变化,但POCD组的Glu(a-v)在T3、T4明显降低,并且与非POCD组比较有统计学意义(Plt;0.05)。 结论 年龄、CPB时间和主动脉阻断时间是POCD发生的危险因素。脑糖代谢异常是POCD发生的可能机制之一。【Abstract】 Objective To understand the risk factor of postoperative cognitive dysfunction (POCD), and identify whether perioperative brain glucose metabolism disorder was or not pathogenesis of POCD after heart valve replacement operation. Methods From March 2004 to July 2009, 70 patients with heart valve replacement were divided into POCD group (n=31) and non-POCD group (n=39) according to cognitive function status tested on the seventh day after operation. The age, sex, cardiopulmonary bypass (CPB) duration, aortic crossclamp duration, mean arterial blood pressure and mean priming volume after aortic crossclamp of two groups were compared. Blood samples were drawn from the radial artery and jugular bulb catheters for determination of blood gas and glucose after induction of anaesthesia (baseline), at the time of the lowest CPB temperature, rewarming to 33℃, and 30 minutes after termination of CPB, CEO2 and Glu (a-v) were calculated: CEO2= (CaO2-CvO2) /CaO2. Results In POCD group, the average age was larger and CPB duration and aortic crossclamp duration was longer than in non-POCD group (Plt;0.05) , but there were no statistical differences in sex, mean arterial blood pressure and mean priming volume after aortic crossclamp. The change tendency of CEO2 of two groups were similar and without significant difference. Glu (a-v) of non-POCD group had no difference in every point, and Glu (a-v) of POCD group had no difference in T1 and T2 points, but value of Glu (a-v) of POCD group decreased obviously and had distinct difference with that of non-POCD group in T3 and T4 points (Plt;0.05). Conclusion Age, CPB duration, and aortic crossclamp duration were risk factors of POCD. Brain glucose metabolism disorder was possibly one of pathogenesis of POCD.

  • Research progress on acute lung injury and lung protective strategies by cardiopulmonary bypass

    Acute lung injury is a kind of common complication after cardiopulmonary bypass. Acute lung injury is attributed to the ischemia-reperfusion injury and systemic inflammatory response syndrome. Several factors common in cardiac surgery with cardiopulmonary bypass may worsen the risk for acute lung injury including atelectasis, transfusion requirement, older age, heart failure, emergency surgery and prolonged duration of bypass. Targets for prevention of acute lung injury include mechanical, surgical and anesthetic interventions that aim to reduce the contact activation, systemic inflammatory response, leukocyte sequestration and hemodilution associated with cardiopulmonary bypass. We aim to review the etiology, risk factors and lung protective strategies for acute lung injury after cardiopulmonary bypass.
