  • Advances of relationship between p53 gene family and thyroid cancer

    ObjectiveTo investigate the relationship between p53 gene family and thyroid cancer.MethodThe related literatures in the database were reviewed and analyzed.ResultsThe p53 gene family included p53, p63, and p73. The p53 played an important role in the development of thyroid cancer, especially in the development of undifferentiated thyroid cancer. The different subtypes of p63 might have different roles in the thyroid cancer, so it couldn’t be generally said that the p63 was an oncogene or an anti-oncogene, and the function of its specific protein needed to be further studied. The biological role of p73 in the thyroid cells might be contradictory, depending on the interaction of many different factors, and the interaction between various p73 subtypes and members of the p53 molecular network was still unclear.ConclusionThere is still some controversy about role of p53 gene family in development of thyroid cancer.

  • Use of Harmonic Scalpel and Bipolar Coagulation Forceps Versus Harmonic Scalpel and Conventional Clampand-Tie Technique in Open Thyroid Surgery —— A Randomized Controlled Trial

    Objective To compare the advantages and disadvantages of using the harmonic scalpel and bipolar coagulation forceps versus harmonic scalpel and conventional clamp-and-tie technique in open thyroid surgery.Methods One hundred patients indicated for thyroid surgery were randomly divided into two groups:the bipolar coagulation forceps group underwent surgery with harmonic scalpel and bipolar coagulation forceps,and the conventional clamp-and-tie group with harmonic scalpel and conventional clamp-and-tie technique,respectively.All operations were performed by the same group of doctors.The total operation time,intraoperative bleeding,mass diameter,postoperative drainage,and surgical complications (postoperative bleeding, postoperative recurrent laryngeal nerve paralysis,seroma,and permanent postoperative hypoparathyroidism) were compared.Results There were 48 valid cases in the bipolar coagulation forceps group, and 49 cases in the conventional clamp-and-tie group. There were no significant differences between two groups patients of age,gender,disease composition,and mass diameter(P>0.05).With the same operative approach, the total operation time,intraoperative bleeding,and the incidence of transient postoperative hypoparathyroidism in the bipolar coagulation forceps group were significantly lower than those in the conventional clamp-and-tie group (P<0.001).The postoperative drainage in the bipolar coagulation forceps group was more than that in the conventional clamp-and-tie group (P<0.05).There was no single case of postoperative bleeding,postoperative recurrent laryngeal nerve paralysis,seroma,and permanent postoperative hypoparathyroidism in both groups.Conclusions The combination of harmonic scapel with bipolar coagulation forceps provides significant advantages over the combination of harmonic scapel with conventional clamp-and-tie technique in open thyroid surgery.

  • Chyle Fistula after Neck Radical Dissection Using Harmonic Scalpel

    ObjectiveTo investigate the cause and treatment for chyle fistula after neck radical dissection using harmonic scalpel. MethodsFrom January 2005 to April 2009, 105 patients with thyroid carcinoma underwent thyroidectomy by harmonic scalpel (harmonic scalpel group) and 110 patients with thyroid carcinoma by conventional procedures (conventional group). Postoperative chyle fistula in all the cases was studied retrospectively. ResultsThe incidence of chyle fistula was 5.71% (6 of 105 patients) in the harmonic scalpel group and 0.91% (1 of 110 patients) in the conventional group. The difference was significant between two groups (Plt;0.05). ConclusionsHarmonic scalpel increases the risk of chyle fistula in neck radical dissection. The conventional procedures with exposing and preserving or ligating the thoracic duct can reduce the risk significantly.

  • Clinical Significance of Internal Mammary Nodes in Diagnosis and Treatment of Breast Cancer

    Objective To explore the clinical significance of internal mammary nodes in diagnosis and treatment of breast cancer. Methods The research papers on internal mammary nodes at home and abroad were analyzed in order to summarize the distribution, metastasis, detection and the effects of clinical treatment of the internal mammary nodes in breast cancer. Results The internal mammary nodes mainly locate near the sternum, along with the internal mammary artery and vein in the thorax. They were prone to metastasize at early stage and the metastasis rate also increased with the increased number of axillary lymph nodes in breast cancer. It could influence the recovery of the breast cancer patients by taking biopsy and treatment to the internal mammary nodes. Conclusion The biopsy and treatment of internal mammary nodes are useful for the patients with breast cancer to avoid the recurrence and helpful to stage, treatment and prognosis judgement of the breast cancer patients.

  • Study on The Use of Harmonic Scalpel vs Conventional Knot Tying in Open Thyroid Surgery

    Objective To compare the disadvantages and advantages of open thyroid surgery when using the harmonic scalpel (HS) vs conventional hemostasis (CH). Methods One hundred and fifty-one patients and 70 patients underwent thyroid surgery with HS or CH respectively. The tumor diameter, incisional length, the total operative time, thyroidectomy time, intraoperative bleeding volume, postoperative drainage volume and surgical complications were compared. The incisional length and the surgical complications in and after learning curve with harmonic scalpel were compared. Results There was no significant difference between the HS and CH group at baseline. Incisional length, total operative time, thyroidectomy time, intraoperative bleeding volume and postoperative drainage volume in HS group were significantly lower than those of CH group (P<0.001); the surgical complications showed no significant difference between the HS and CH group (Pgt;0.05); the incisional length after learning curve was shorter than that in learning curve in HS group (P<0.05); the surgical complications after learning curve were significantly lower than that in learning curve in HS group (P<0.001). Conclusion HS provides significant advantages over conventional hemostasis in open thyroid surgery. However, it can’t be used in a blindly exclusive way but be combined with conventional knot tying appropriately.

  • Relation Between Iodide Transporters and Thyroid Diseases

    Objective To investigate the clinical significance of the three iodide transporters in thyroid diseases. Methods Literatures about the Na+/I- symporter (NIS), pendrin and human apical iodide transporter (hAIT) in recent years were reviewed and their expressions in different thyroid diseases were also analyzed. Results NIS proteins express at the basolateral membrane of thyrocytes in normal thyroid tissue, while pendrin and hAIT proteins are limited to the apical membrane of thyrocytes lining in the follicular lumen. In the tissues of thyroid carcinomas, it was found that the NIS proteins expressed in the cytoplasm and their expressions decreased; The mutation of NIS gene may be one of the main causes of congenital hypothyroidism. The expression of prendrin protein may be related to the function of follicles: its expression level increased significantly both in Graves diseases and toxic adenomas, but significantly decreased in differentiated thyroid carcinoma. However, the correlation between the decrease and the degrees of differentiation of carcinoma cell line are still disputable. The expression of hAIT protein does not significantly altered in hyperfunctioning tissues. It only slightly decreased occasionally in hypofunctioning adenomas, but it decreased significantly in thyroid carcinomas. Conclusion The abnormal expressions of the three iodide transporters may be related closely with the type of thyroid diseases. However, their pathogenic mechanisms and the causes of their abnormal expression are still unknown, which need to be studied further.

  • EXPERIENCE IN DIAGNOSIS AND TREATMENT FOR HASHIMOTO′S DISEASE (REPORT OF 78 CASE)

    Objective To sum up experiences in diagnosis and treatment for Hashimoto′s disease (HD). Methods Clinical records of 78 patients who underwent operations and were diagnosed as Hashimoto′s disease by histologic examination in our hospital from Jan. 1988 to Dec. 1998 were analyzed. Results Seventy females and 8 males, aged 9 to 70 years (average of 41.6 years). HD was coexistent with 10.3% of thyroid gland malignant tumor, 23.1% of adenoma and 30.8% of other thhroid gland diseases. The misdiagnosis rate was 35.9% and missed diagnosis rate was 46.2%. The clinical feature of HD and most common cause of misdiagnosis and missed diagnosis have been discussed. Conclusion It is emphasized that patients with diffuse goiter, palpable nodules, lighty color on scintillation scintigraphy, elevation of antimicosomiaux and antithyroglobuline but no finding on Bus should be highly suspected of having Hashimoto′s disease.

  • Advancement of long non-coding RNA in papillary thyroid carcinoma

    Objective The aim of this study is to review the association between long non-coding RNA (lncRNA) and papillary thyroid carcinoma (PTC). Method The relevant literatures about lncRNA associated with PTC were retrospectively analyzed and summarized. Results The expression levels of noncoding RNA associated with MAP kinase pathway and growth arrest (NAMA), PTC susceptibility candidate 3 (PTCSC3), BRAF activated non-coding RNA (BANCR), maternally expressed gene 3 (MEG3), NONHSAT037832, and GAS8-AS1 in PTC tissues were significantly lower than those in non-thyroid carcinoma tissues. The expression levels of ENST00000537266, ENST00000426615, XLOC051122, XLOC006074, HOX transcript antisense RNA (HOTAIR), antisense noncoding RNA in the INK4 locus (ANRIL), and metastasis-associated lung adenocarcinoma transcript 1 (MALAT1) in PTC tissues were upregulated in PTC tissues, comparing with the non-thyroid carcinoma tissues. These lncRNAs were possibly involved in cell proliferation, migration, and apoptosis of PTC. Conclusion LncRNAs may provide new insights into the molecular mechanism and gene-targeted therapy of PTC and become new molecular marker for the diagnosis of PTC.

  • Advancement of transoral endoscopic thyroidectomy vestibular approach

    ObjectiveTo summarize the advancement of transoral endoscopic thyroidectomy vestibular approach (TOETVA).MethodThe relevant literatures about comparative study btween TEOTVA and other thyroidectomy were retrospectively analyzed and summarized.ResultsCompared with the conventional open thyroidectomy or other endoscopic thyroidectomy, even though TOETVA requires a longer operative time, it provides similar surgical outcomes and no scar on the body.ConclusionTOETVA is a safe and effective procedure with excellent cosmetic results for patients with thyroid disease.

  • Research Progress of Electrical Impedance Spectrum Technology in The Diagnosis of Thyroid Carcinoma

    Objective To summarize the basic principle of electrical impedance spectrum technology and the latest progress in the diagnosis of thyroid carcinoma. Methods By the domestic and overseas literatures review, medical application and diagnostic prospect in thyroid carcinoma of electrical impedance spectrum were summarized. Results Electrical impedance spectrum was a kind of somatic function imaging, whose measurement results was objective, and it could diagnose thyroid carcinoma in early stage effectively. In addition, it could be used as a complementary form of fine needle aspiration biopsy, improving the diagnostic accuracy rate of thyroid carcinoma, thereby reducing the unessential thyroid operations. Conclusion Electrical impedance spectrum technology is a potentially useful imaging modality for diagnosing thyroid carcinoma in early stage, and functions as a auxiliary clinical diagnosis method for fine needle aspiration biopsy.
