  • Application of Duct Endoscopy in Diagnosis of Nipple Discharge

    【Abstract】Objective To evaluate the application of duct endoscopy in the diagnosis of breast diseases characterized by nipple discharge. Methods One hundred and sixtynine consecutive cases from October 2003 to September 2004 who had duct endoscopy for nipple discharge were reviewed and followed up. Results Of 169 cases, 79 ductitis, 62 intraductal papilloma, 9 intraductal carcinoma in situ, 19 without breast diseases were diagnosed under duct endoscopy. During following up, the diagnostic accuracy of intraductal papilloma and intraductal carcinoma in situ with duct endoscopy was 89.8%(44/49) and 55.6%(5/9),respectively. There was only one patient who had breast pain and mild skin redness which was cured with oral antibiotics for 3 days. Conclusion Duct endoscopy is an only technique to explore the etiology for duct discharge under vision. It is direct, safe and effective, with a high accuracy.

  • Endoscopic Vein Harvesting in Diabetic Patients Undergoing Coronary Artery Bypass Grafting: A Prospective Controlled Trial

    Abstract: Objective To evaluate clinical outcomes of endoscopic vein harvesting (EVH)for coronary artery bypass grafting(CABG) in diabetic patients. Methods In this prospective non-randomized control study, patients with type 2 diabetes who underwent CABG from December 2010 to Febuary 2012 in West China Hospital were enrolled. Based on different vein graft harvesting technique, these patients were divided into two groups: an EVH group and a conventional vein harvesting(CVH)group. Perioperative complications were compared between the two groups. Interventional or CT coronary angiogram was used to evaluate bypass graft patency during follow-up. Results A total of 51 patients with type 2 diabetes were enrolled in this study with 24 patients in the EVH group and 27 patients in the CVH group. There was no statistical difference in age, weight, and comorbidities between the two groups. There was no statistical difference in cardiopulmonary bypass time and aortic cross-clamping time between the two groups (67.2±9.8 min versus 68.3±14.5 min, P>0.05; 62.4±11.3 min versus 65.2±10.3 min, P> 0.05). The vein graft harvesting time (35.6±6.4 min versus 45.2±11.4 min, P< 0.05)and rate of delayed leg wound healing(0.0% with 0/24 versus 18.5% with 5/27, P<0.05) of the EVH group were significantly shorter or lower than those of CVH group.There was no statistical difference in major postoperative complications with respect to venous graft failure rate and chest pain during short term follow-up(9.1 months in the CVH group and 9.4 months in the EVH group) between the two groups. Conclusion EVH is a safe, effective, minimally invasive and quick vein graft harvesting technique for CABG in diabetic patients.

  • EFFICACY OF POSTERIOR MICROENDOSCOPIC FORAMINOTOMY FOR CERVICAL RADICULOPATHY

    Objective To evaluate the cl inical results of posterior microendoscopic foraminotomy in the treament of cervical radiculopathy and cervical intervertebral disc protrusion. Methods From February 2004 to June 2007, 24 cases of cervical radiculopathy received posterior microendoscopic foraminotomy. There were 16 males and 8 females, aging 42-68 years (59 years on average), including 16 cases of cervical radiculopathy and 8 cases of cervical intervertebral disc protrusion.The course of disease was 6-15 months. The affected intervertebral discs were C4, 5 in 8 cases, C5, 6 in 12 cases, and C6, 7 in 4 cases. The radiological examinations showed that 8 protrusions included 6 soft tissue protusions and 2 rigid tissue protusions, and that cervical radiculopathy were caused by yellow l igament hypertrophy, Luschka’s joint hyperplasia, and abnormal position of facet joint. According to Japanese Orthopedic Association (JOA), the score before operation was (12.60 ± 1.52) points. Results The operation time was 90 to 120 minutes (100 minutes on average), the bleeding during operation was 100 to 150 mL (120 mL on average). Nerve root pain were rel ieved completely in 19 cases and were rel ieved partly in 4 cases. One case of calcified nucleus pulposus had neurological traction injury and recovered completely after 3 months. All cases were followed up 24-36 months (28 months on average). The radiological examinations after operation showed the intervertebral disc site was decompressed completely and the height of intervertebral disc and the cervical segmental al ignment were normal. At 24 months postoperatively, the JOA score was (16.10 ± 0.29) points, showing significant difference when compared with that of preoperation (P lt; 0.01). Conclusion The posterior microendoscopic foraminotomy can get to the operation site with miniincision, decrease tissue damage during operation, and avoid narrow intervertebral space, so it has satisfactory cl inical results.

  • EARLY OUTCOME OF ONE-STAGE TRANSBASAL SURGERY COMBINED WITH TRANSNASAL SURGERY FORCRANIONASAL TUMORS AND RECONSTRUCTION OF SKULL BASE

    To investigate the microsurgical management of cranionasal tumors and the method of the reconstruction of the skull base. Methods From June 2005 to October 2007, 20 patients with cranionasal tumor were treated. There were 10 males and 10 females, aged between 13 and 77 years (median 49 years). The disease course was 2 months to 13 years.The cranionasal tumors, proved by MRI and CT scans, located in the anterior skull base, paranasal sinus, nasal and/or orbit cavity. And their cl inical presentations were l isted as follows: dysosphresia in 14 patients, headache in 11 patients, nasal obstruction in 9 patients, epistaxis in 8 patients, visual disorder in 4 patients, exophthalmos in 4 patients and conscious disturbance in 2 patients. All 20 patients underwent transbasal surgery combined with transnasal surgery, and tumors were resected by one-stage operation. The skull base was reconstructed by surgical technique “Pull Down Sandwich” with pedicle periosteum flap. Results Tumors were resected by one-stage operation, and the anterior skull bases were reconstructed. Pathological examination showed 8 cases of mal ignant tumors and 12 cases of benign tumors. The total surgical excision was complete in 16 patients, and 4 patients with subtotal excision. There was no operative death. Eighteen patients were followed up 3 months to 2 years and 6 months. Transient cerebrospinal fluid rhinorrhea was found in 2 cases which were cured by lumbar drainage. And recurrence of tumor was observed in 5 patients 3 months to 2 years after operation. Conclusion Microsurgical operation via subfrontal approach assisted bytransnasal endoscopy is an effective method in management of cranionasal tumors, with the advantages of econstruction of the skull base with pedicle periosteum flap or “Pull Down Sandwich” and low compl ication rate.

  • Preliminary Observation of Chronic Inflammation of Submandibular Gland under Sialoendoscopy

    【摘要】 目的 观察运用涎腺镜对慢性下颌下腺炎诊断和治疗的临床效果。 方法 应用涎腺镜观察32例慢性下颌下腺炎患者导管,根据不同病因给予相应治疗。分别于手术前当天,手术后2、7 d,4周,6、12个月观察治疗效果。 结果 32例慢性下颌下腺炎患者中,28例存在导管结石。手术后2 d大部分患者胀痛症状明显缓解,之后1个月内呈逐渐缓慢缓解趋势,手术后6~12个月胀痛感略有回升表现。结论 运用涎腺镜治疗慢性下颌下腺炎是微创、有效的。【Abstract】 Objective To observe the clinical effect of chronic inflammation of submandibular gland treated by sialoendoscopy. Methods The conduit of 32 patients with chronic inflammtion of submandibular gland under sialoendoscopy, and to observe the curative effect after two, seven days, four weeks, six and 12 months. Results Of the all of 32 patients, 28 had stones in duck. Two days after surgery, the most patients has bursting pain palliation, and then relieved gradually; from six to 12 months after surgery, bursting pain rebounded slightly. Conclusions Use of sialoendoscopy on chronic inflammtion of submandibular gland is minimally invasive and effective treatment.

  • Endoscopic Endonasal Transsphenoidal Surgery for Pituitary

    摘要:目的:探讨神经内镜经单鼻腔蝶窦入路在切除垂体腺瘤中的临床应用。 方法:对58例垂体腺瘤患者进行手术切除。应用神经内镜直接自单鼻腔进入,暴露双侧蝶窦开口,打开蝶窦前壁进入蝶窦腔切除肿瘤。 结果: 肿瘤全部切除42例,约占 72%;次全切除16例,约占28%,无严重并发症。结论:神经内镜经单鼻腔蝶窦入路切除垂体腺瘤是一种更微创、暴露更好、并发症少的手术方式。Abstract: Objective: To investigate the clinic application of endoscopic end nasal transsphenoidal surgery for pituitary tumors. Methods: 58 patients were treated. A endoscope was used to open the anterior wall of the sphenoid sinus and resected tumors. Results: 42 cases (72%) underwent total resection, 16 cases (28%) underwent subtotal resection. No severe complications was found. Conclusion: Endoscopic end nasal transsphenoidal surgery for pituitary tumors can reduce the tissue trauma, improved visualization, more complete tumor removal, and reduce complications.

  • Clinical Study of Mini-Incision Anterior Cervical Approach Thyroidectomy and Modified Miccoli Endoscopy Assisted Thyroidectomy for Thyroid Benign Tumor

    Objective To compare the therapeutic efficacy between the mini-incision anterior cervical approach thyroidectomy and modified Miccoli endoscopy assisted thyroidectomy for thyroid benign tumor. Methods Clinical data of 80 patients with thyroid benign tumor treated in The First Affiliated Hospital of General Hospital of PLA from Jan.2010 to Jan. 2012 were retrospectively analyzed,and the 80 patients were divided into mini-incision anterior cervical approach thyroidectomy group (n=40) and modified Miccoli endoscopy assisted thyroidectomy group (n=40) according to operative type. The operative time,drainage volume of neck,cosmetic satisfaction score,duration of hospitalization,expense of hospitalization,and rates of postoperative complication of 2 groups were compared and analyzed. Results The operative time of mini-incision anterior cervical approach thyroidectomy group were significantly longer than that ofmodified Miccoli endoscopy assisted thyroidectomy group〔(95.5±20.3)min vs. (62.4±15.5)min,P<0.05〕,but thecosmetic satisfaction score〔(3.5±0.2) score vs. (4.3±0.1) score〕and expense of hospitalization〔(5 814.6±1 262.3)yuan vs. (9 846.7±1 080.5) yuan〕were lower (P<0.05). There were no significant differences on the drainage volumeof neck after operation〔(28.6±5.5) mL vs. (22.2±4.5) mL〕and duration of hospitalization〔(4.5±1.5) d vs. (3.8±0.9) d〕between the 2 groups (P>0.05). There were 3 cases happened transient hoarseness in mini-incision anterior cervical approach thyroidectomy group (all recovered in 1 month after operation) and 1 case in modified Miccoli endoscopy assi-sted thyroidectomy group (recovered in 3 months after operation),and there were no significant difference on incidence of recurrent laryngeal nerve injury (P=0.608). No other complications happened. Conclusions Modified Miccoli endoscopy assisted thyroidectomy has better cosmetic benefit than mini-incision anterior cervical approach thyroidectomy, but more expensive. The patients who have specific cosmetic demand or engage in special profession can choose modified Miccoli endoscopy assisted thyroidectomy.

  • Clinical Comprehensive Therapy of Peutz-Jeghers Syndrome (Report of 71 Cases)

    Objective To explore the clinical comprehensive therapy of Peutz-Jeghers syndrome. Methods From January 2000 to December 2010,71 cases of Peutz-Jeghers syndrome underwent endoscopic polyp resection firstly,and those with unresectable lesions or with severe complications underwent rescue laparotomy. After endoscopic or surgical treatment,the patients took Celecoxib capsules voluntarily for 6 to 9 months under informed consents. All cases were followed up from 6 months to 8 years. Results Twenty-nine patients had familial history of Peutz-Jeghers syndrome among the 71 patients (41 males and 30 females). Sixty-two cases underwent 94 surgeries and intussusception was the most common cause of laparotomy. Sixty-five patients underwent 169 double-balloon endoscopy (DBE) therapies,and a total of 1 714 polyps were resected by DBE polypectomy. The largest major axis of small-bowel polyp was 8 cm. No severe complications occurred after DBE polypectomy except for 3 cases of intestinal perforation. Eight patients took Celecoxib capsule,3 of them were treated more than 6 months,and DBE examination showed the gastrointestinal polyps reduced in number and size. Conclusion The comprehensive treatment (including of endoscopic therapy,operation,and drug intervention) is a safe and effective clinical model to treat Peutz-Jeghers syndrome.

  • Progress of Endoscopic Diagnosis and Therapy for Pancreatic Cancer

    Objective To summarize the progress of endoscopic diagnosis and therapy for pancreatic cancer. Methods Domestic and international publications online involving progress of diagnosis and therapy for pancreatic cancer by using endoscope in recent years were collected and reviewed. Results Recently, early diagnostic rate of pancreatic cancer increased with the development of endoscope and endoscopic technique such as endoscopic ultrasound, endoscopic ultrasound-guided fine needle aspiration, peroral pancreatoscopy, optical coherence tomography, ERCP, and cytology in pancreatic juice. Furthermore, varied therapies such as endoscopic ultrasound guided celiac plexus neurolysis, implantation of iodine 125-particles or pancreatic duct/bile duct stents were performed by endoscope for advanced pancreatic cancer. Conclusion Early diagnostic rate and novel therapeutic alternative of pancreatic cancer are supplied by digestive endoscopy.

  • Study of Multi-slice CT Portography Imaging in the Diagnosis and Evaluation of Esophageal and Gastric Varices in Cirrhosis Patients

    ObjectiveTo study the application value of multi-slice CT portography (MSCTP) in the diagnosis and evaluation of esophageal and gastric varices (EGV) caused by cirrhosis. MethodsPatients with cirrhosis diagnosed between September 2009 and December 2012 were screened in this study. And the consistency of MSCTP and digestive endoscopy in the diagnosis, classification and grading of EGV in cirrhosis were evaluated. ResultsA total of 78 patients were included in this study, and there were 55 patients with EGV diagnosed by endoscopy, including 35, 16 and 4 patients with GOV1, GOV2 and IGV1 respectively by Satin type standards; and the number of patients with mild, moderate and severe EGV by general grading standards was 2, 15, and 37, respectively. In this cohort, the findings of MSCTP examination also showed that 58 patients had EGV, including 36, 17, 4 and 1 patients with GOV1, GOV2, IGV1 and IGV2 by Satin type standards; and the number of patients with grade I,Ⅱ andⅢ EGV by Kim grading standards was 5, 16 and 37, respectively. Statistical analysis showed that there was a high consistency between endoscopy and MSCTP in the diagnosis (Kappa=0.712, P=0.000), typing (Kappa=0.732, P=0.000) or grading (Kappa=0.863, P=0.000) of EGV. ConclusionMSCTP has a high application value in the diagnosis and severity evaluation of EGV in patients with cirrhosis.
