To investigate the cause of septicemia in patients with obstructive jaundice,the correlationship between intra-biliary tract pressure(IBTP),portal veinous flow rate(PVFR)and interleukin-2(IL-2),soluble interleukin-2 receptor(sIL-2R),T lymphocyte subpopulation in patient with obstructive jaundice(Group A)has been studied.Group A was subdivided into A1,emergency operation group;A2,elective surgery group;A3,patient’s age over 60 years and A4,age under 60.Ninety patients with simple gallstone(Group B)were also tested as a contrast.The result showed that of all Group A,CD3+,CD4+,CD8+ before operation were much lower than those 10 days after operation(Plt;0.05 or Plt;0.01),while the postoperative sIL-2R was significantly higher than that of 10 days after operation(Plt;0.01),in Group A1,emergency surgery,the preoperative sIL-2R was much more higher than that in others of the jaundice group(Plt;0.01).Corralation analysis showed IBTP was negatively corralated to IL-2,CD3+,CD4+,CD8+,but it had positive correlation with sIL-2R(Plt;0.01).PVFR was positively correlated to IL-2(Plt;0.01).These indicate that obstructive jaundice with infection is closely related to the decreased host immunity.
Objective To compare the clinical effect between high flow rate modified ultrafiltration (HMUF) and conventional modified ultrafiltration(CMUF), and the effect on hemodynamic data and inflammatory mediators. Methods Forty children were divided into two groups with random number table, HMUF group and CMUF group, 20 cases each group. Hematocrit (HCT) and hemodynamic changes were recorded and the concentration of tumor necrosis factor (TNF) and interleukin 6 (IL-6) were measured. Results The operations were done uneventfully with moderate hypothermia cardiopulmonary bypass in 40 patients. Duration of ultrafiltration of HMUF group (7.83±0. 75 min) was less than that of CMUF group (13.86±1.95 min, P〈0.01). The volumes ultrafiltrated of HMUF group (440.00±91.86ml) was more than that of CMUF group (372.22±56.52ml, P〈0.01). There are no significant differences about the hemodynamic data, HCT, TNF and IL-6 between two groups. Conclusion The HMUF is safe and efficient,when it was used after pediatric cardiopulmonary bypass, the duration of ultrafiltration can be shortened significantly.
To evaluate the influence of different brands of syringes on performance parameters and application quality of syringe pump, we carried out matching experiment of infusion/syringe pump analyzer Rigel Multi-Flo, made by ALK Co. in Sweden, for 3 different types of syringes to measure the flow rate, occlusion alarm pressure, occlusion time and bolus volume of 26 syringe pumps. Data of quality control were analyzed with statistical method. The results demonstrated that there were significant differences in the flow rate, occlusion alarm pressure, occlusion time and bolus volume (P<0.01) between those of common syringes and original syringes. This study points out that the health care providers should select compatible syringes for syringe pumps, which provides evidence and guidance to assure the reasonable and safe application of syringe pumps in clinical practice.
The flushing pump which is applied to clean operative wound has no temperature controlling function up to now, and doctors have to prepare the flushing fluid that has previously been warmed. The flushing pump system with medical constant temperature designed in our laboratory can absorb flushing fluid at the room temperature, and then eject flushing fluid with the temperature in accordance with the requirements of operations at a controlled constant flow rate. The system combines flow rate control with temperature control functions. The flushing pump system includes flushing part, temperature controlling part, key inputting part, liquid crystal displaying part and exceptional situation monitoring part. The present paper introduces the design method and principle of each part of the system at first, and then gives the debug method of all the system parameters. Finally the paper discusses the performance of the system according to the result of the experiment.
