  • Medical Education Model with Core Competency as Guide, Evidence-Based Medicine as Carrier and Lifelong Learning as Purpose (3): Current Status of Information Literacy on Medical Students

    Objective To investigate the information literacy status of medical students, so as to provide references for further developing information literacy education. Methods A total of 764 medical undergraduates and postgraduates in Sichuan University were investigated by using “information literacy questionnaire”. Results A total of 744 feedbacks among 764 distributed questionnaires were qualified, accounting for 97.4%. Among then, 37.2% students could easily tell the key words for required information, 26.1% students could make a plan before searching information, 25.5% students could choose appropriate databases for searching, 14.7% students could critically appraise the information, and only 12.4% students had received systematic trainings on intellectual property rights and copyrights. Conclusion Most students of West China School of Medicine of Sichuan University possess good information consciousness, but their critical appraisal for information and information morality need to be improved further.

  • Current situation and influencing factors of health literacy among follow-up patients with chronic kidney disease

    ObjectiveTo investigate the health literacy level and its influencing factors among follow-up patients with chronic kidney disease (CKD).MethodsFrom March to August 2018, 248 patients from the CKD Follow-up Management Center, West China Hospital, Sichuan University were included. Basic information questionnaire and chronic diseases health literacy scale were used. Analysis of variance and t test were used in univariate analysis, and multiple linear stepwise regression was used in multivariate analysis, to explore the influencing factors of health literacy score.ResultsThe average health literacy score of the 248 CKD patients (97.24±12.22) were in medium to low level. Listed from high to low, the score of each dimension was: ability to obtain information (4.24±0.50), willingness to improve health (4.17±0.66), competence to communicate and interact with others (3.95±0.59), and willingness to support financially (3.41±1.10). The result of multiple linear stepwise regression showed that whether followed up on time, families’ monthly income per capita, and the patients’ age were independent influencing factors of health literacy score (P<0.05).ConclusionsThe health literacy level of follow-up patients with CKD remains to be improved. Medical personnel should pay attention to whether patients with CKD are followed up on time, make targeted intervention, and improve the self-management of patients so as to delay the disease progress of CKD.

  • Investigation on the status of medical information literacy of medical students

    ObjectiveTo investigate the capability and demands of medical students of medical information retrieval, and to provide information for medical information literacy education.MethodsWe conducted an online questionnaire survey among medical students from 15th to 21st July, 2019, aiming to analyze basic characteristics of the subjects and their ways and capability to acquire medical information, and evaluated their demands and satisfaction about relevant courses.ResultsFour hundred and nine valid questionnaires were collected. The average age of the respondents was (21±3) years old. Among the respondents, 279 (68.2%) were female, 290 (70.9%) were undergraduates, and 187 (45.7%) majored in clinical medicine. The mostly accessed ways to obtain medical information were textbooks (87.8%) and search engines (84.4%). Among Chinese literature databases, the most commonly used was China National Knowledge Infrastructure (67.7%), followed by Wanfang (54.3%). As for English literature databases, the most commonly used was PubMed (66.0%), while the proportions of respondents who used other databases was less than 1/4. As for the problems when obtaining information, 84.8% of the respondents supposed that no access to some online databases was the main problem. Obtaining information from search engine scored the highest [(3.21±1.00) points, 5 points totally] when they were asked to self-evaluate their ability to obtain medical information by different methods. In terms of training demands, the need for courses about literature authenticity and accuracy evaluation was the highest [(4.05±1.07) points, 5 points totally]. The trainings were expected to be conducted within 1-2 years after enrollment (83.9%). Training through professional courses (86.8%) was the most welcomed training form, followed by courses on MOOC (51.3%), an online course platform. More than 50% of the respondents were satisfied with the current relevant courses and trainings.ConclusionBoth the students’ capability of medical information retrieval and the design of relevant trainings should be improved to better prepare the medical students for further clinical practice and scientific research.

  • Current situation of “digital divide” among the elderly under the background of intelligent medical treatment and countermeasures for aging adaptation

    With the rapid development of information science and technology, the development of internet medical service is irresistible, forming a new situation of digital ecological environment of medical cloud service. However, reform and innovation of medical service mode have brought many problems and challenges to the elderly with low level of information literacy and difficult operation of information and intelligent applications on mobile phones, forming a “digital gap” with intelligent medicine. This paper analyzes the status quo of the digital divide in the elderly group, and classifies the elderly according to the pain and difficulty of the elderly wisdom, and then implements a series of online and offline aging adaptation to build a harmonious and humanistic care Internet plus medical service ecosystem for the elderly.   • Electronic health literacy status and influencing factors of maintenance hemodialysis patients

    Objective To understand the current situation of eHealth literacy of maintenance hemodialysis (MHD) patients and analyze the influencing factors to improve the eHealth literacy of MHD patients. Methods Using cross-sectional survey method, MHD patients in West China Hospital of Sichuan University were selected as the subjects between February and April 2022. eHealth Literacy Scale (eHEALS) was used to investigate the status of eHealth literacy, and the influencing factors of MHD patients were analyzed. Results A total of 194 MHD patients were investigated. The total eHEALS score of MHD patients was 24.55±9.14. The results of generalized linear regression showed that education level, medical payment method, age, frequency of internet forums, interest in internet health knowledge search, and Baidu search engine (a common channel for searching disease information online) were the influencing factors of MHD patients’ electronic health literacy. Conclusion The electronic health literacy level of MHD patients is low, so renal medical staff should pay attention to the electronic health literacy of MHD patients and develop targeted interventions to help patients better manage their own health problems.

  • Research on electronic health literacy and online health information search behavior of maintenance hemodialysis patients

    Objective To explore the current status of electronic health (eHealth) literacy and online health information seeking behavior of maintenance hemodialysis (MHD) patients, and to explore the relationship between them. Methods MHD patients in the Hemodialysis Center of West China Hospital of Sichuan University between January and April 2022 were selected by convenient sampling method. The patients were investigated with general condition questionnaire, eHealth Literacy Scale and internet health information retrieval behavior questionnaire. According to the score of eHealth literacy, patients were divided into high eHealth literacy group and low eHealth literacy group, to compare the differences between the two groups, and analyze the factors that affected the internet health information retrieval behavior. Results A total of 194 MHD patients were included. Among them, 112 were male and 82 were female. The average score of eHealth literacy was 24.84±9.24. There were 136 cases in low eHealth literacy group, and the average score of eHealth literacy was 20.76±7.91. There were 58 cases in high eHealth literacy group, and the average score of eHealth literacy was 34.36±3.01. Multivariate analysis showed that eHealth literacy and education level were the influencing factors of health information search behavior (P<0.05). Conclusions The overall eHealth literacy of MHD patients is low, and the online health information search means is single and behaviorally inconsistent. It is necessary to improve the eHealth literacy of hemodialysis patients, and at the same time, provide various health information publicity and education for patients with different levels of eHealth literacy, which will help them better carry out disease management.

  • The impact of medical humanities literacy on job competency

    ObjectiveTo construct a structural equation model of the mechanism of the role of medical humanities literacy in job competency and to conduct empirical analysis to verify the supporting role of medical humanities literacy in cultivating job competency. MethodsLiterature research was conducted to select the initial indicator system for medical humanities literacy and job competency, and then a conceptual model was constructed. Questionnaire data from four hospitals in Chengdu were collected. Through exploratory factor analysis, an indicator system for medical humanities literacy and job competency more suitable for the research population was obtained and the conceptual model was adjusted. Finally, the positive mechanism of medical humanities literacy on job competency was verified using confirmatory factor analysis (structural equation model). ResultsThe Cronbach's alpha coefficient of the overall questionnaire involved in the exploratory and confirmatory factor analyses was greater than 0.9, and the KMO values of the questionnaire were greater than 0.8, with a Bartlett's sphericity test P value less than 0.01, indicating good reliability and validity of the questionnaire. The model fit of the structural equation model met the requirements, with a chi-square degree of freedom ratio (CMIN/df) of 2.768, root mean square error of approximation (RMSEA) of 0.077, comparative fit index (CFI) of 0.891, normalized fit index (NFI) of 0.840, and incremental fit index (IFI) of 0.891. ConclusionThe improvement of individual characteristics in job competency is mainly influenced by medical humanities knowledge and spirit. The improvement of cognitive characteristics in job competency is mainly influenced by medical humanities knowledge and ability. The improvement of achievement characteristics in job competency is mainly influenced by medical humanities ability and spirit. The improvement of management characteristics in job competency is mainly influenced by medical humanities spirit.

  • Nutritional literacy status and influencing factors of maintenance hemodialysis patients

    Objective To explore the current status of nutritional literacy in maintenance hemodialysis (MHD) patients and analyze its influencing factors. Methods MHD patients in Wenjiang Hemodialysis Center of West China Hospital, Sichuan University between January and April 2022 were selected by convenient sampling method. A questionnaire survey was conducted using the Nutrition Literacy Evaluation Scale for end-stage renal disease dialysis patients, and the current status and influencing factors of nutritional literacy in MHD patients were analyzed. Results A total of 214 patients were included, with an average nutritional literacy score of 19.14±5.78 for MHD patients. Among them, there were 60 cases (28.04%) of highly likely low nutritional literacy, 124 cases (57.94%) of moderate nutritional literacy, and 30 cases (14.02%) of highly likely high nutritional literacy. The correlation test results showed that hemoglobin, albumin, cholesterol, triglyceride, calcium, phosphorus, potassium, creatinine, estimated glomerular filtration rate, and nutritional literacy scores were not correlated. The results of multiple linear stepwise regression analysis showed that education level [non-standardized partial regression coefficient (b)=1.821, 95% confidence interval (CI) (1.143, 2.498), P<0.001], age [b=−2.460, 95%CI (−4.247, −0.672), P=0.007], hypertensive kidney damage [b=2.233, 95%CI (0.428, 4.039), P=0.016] were all factors affecting the nutritional literacy of MHD patients. Conclusions In clinical work, more attention should be paid to MHD patients with low educational level, older age, and more primary diseases. And targeted interventions should be adopted to improve the level of nutritional literacy in order to improve the quality of life of MHD patients.

  • Impact of literacy on long-term prognosis in colorectal cancer patients: a real-world study based on DACCA

    ObjectiveTo analyze the relation between the literacy and prognosis in the patients with colorectal cancer (CRC) in the current version of the Database from Colorectal Cancer of West China (DACCA). MethodsThe version of DACCA selected for this data analysis was updated on September 12, 2022. The data items analyzed included age, gender, literacy, tumour site, nature of tumour, pathological TNM (pTNM) stage, survival status, and survival time. The overall survival and disease-specific survival of the CRC patients with different literacy (illiteracy, primary, secondary, and tertiary educations) after radical resection were compared, and then which were analyzed in the patients with different pTNM stages. ResultsA total of 3 692 data eligible for the study were screened, of which 202 were illiteracy, 1 054 were primary education, 1 809 were secondary education, and 627 were tertiary education; And there were 13 of stage 0, 406 of stage Ⅰ, 1 193 of stage Ⅱ, 1 139 of stage Ⅲ, and 941 of stage Ⅳ. The differences in the comparison of the pTNM stage and the nature of the tumour among the patients with the four levels of literacy were not statistically significant (P>0.05), while the differences in the comparison of the gender, age, and tumour site were statistically significant (P<0.001). The overall survival and disease-specific survival curves of the CRC patients with different literacy had no statistical differences (χ2=1.982, P=0.576; χ2=2.618, P=0.454), and the stratified overall survival curves had no statistical differences among the patients with pTNM stages Ⅰ to Ⅳ (stage Ⅰ: χ2=1.361, P=0.715; stage Ⅱ: χ2=3.507, P=0.320; stage Ⅲ: χ2=3.144, P=0.370; stage Ⅳ: χ2=4.993, P=0.172), and the stratified disease-specific survival curves had no statistical differences (stage Ⅰ: χ2=0.723, P=0.868; stage Ⅱ: χ2=3.295, P=0.348; stage Ⅲ: χ2=4.767, P=0.190; stage Ⅳ: χ2=6.177, P=0.103). ConclusionsThe results of this study based on real-world big data analysis suggests that the differences of overall survival and disease-specific survival of CRC patients with different literacy levels (illiterate, primary, secondary, and tertiary education) are not statistically significant, and the results of stratified analysis based on pTNM staging are consistent with this. In the future, limitations of this study can be excluded and further analysis can be conducted by combining treatment details or expanding sample data to seek more realistic results.
