  • Clinical analysis of 282 patients with infectious endophthalmitis

    Objective To analyze the causes and pathogens of infectious endophthalmitis in our hospital. Methods The clinical data and laboratory findings of 282 inpatients with infectious endophthalmitis were retrospectively reviewed. There were 206 males(73.05%)and 76 females(26.95%)with a mean age of (36.0plusmn;11.2) years (range from 2 to 79 years). Except 14 patients who lost their eyeballs as of serious conditions, pathogen examinations of vitreous specimen had been performed for all remaining 268 cases. The clinical and laboratory data were statistically analyzed by Chi-square and Kappa consistency tests, when P<0.05 the difference should be considered as statistically significant. Results The 4 major causes of infectious endophthalmitis of those 282 patients included ocular trauma (177 cases, 62.77%), infectious keratitis (61 cases, 21.63%), eye surgery (32 cases, 11.35%), and endogenous endophthalmitis (12 cases, 4.25%). 19.21% of the patients in ocular trauma group was 2-13 years old, 55.74% of the patients in infectious keratitis group was 14 -50 years old, and 60.61% of the patients in eye surgery group was 51-79 years old. The major pathogens were fungus and staphylococci each accounted for 30.63% of all cases. Most of the pathogenic bacteria were sensitive to ciprofloxacin (81.81%) and gentamicin (77.92%). Conclusions The major reason of infectious endophthalmitis was ocular trauma, and the major pathogens were fungus and staphylococci. Those findings may help us to treat infectious endophthalmitis patients appropriately.

  • Infected endophthalmitis diagnosed by multiplex polymerase chain reaction

    Objective To evaluate the rapid diagnosis of bacterial and (or) fungal endophthalmitis by multiplex polymerase chain reaction (MPCR). Methods MPCR was performed to detect the DNA segment of bacteria and (or) fungi from standard strains and 41 samples of intraocular fluid or vitreous from 38 patients (3 with double eyes and 35 with single), and the results were compared with the cultured bacteria and fungi. Results Five hours after detected by MPCR, bacteria and (or) fungi in 34 out of 41 samples (82.9%) from patients were detected,in cluding bacteria in 26,fungi in 6,and both bacteria and fungi in 2. The positive rate of MPCR was obviously higher than the cultured ones(χ2=9.60, P<0.05). Conclusion With the advantages of rapidity, sensibility, and specificity, MPCR can make for the rapid and definitive diagnosis of bacterial and (or) fungal endophthalmitis.   • The clinical analysis on bacterial isolates from the aqueous humor and the vitreous body of patients with suspected endophthalmitis

    Objective To review the distribution and shifting trends of cultured bacteria from the aqueous humor and the vitreous body. Methods A retrospective analysis on distribution of Gram′s stain, the distribution and change of isolates was performed in 522 specimens (aqueous humor,261 and vitreous body,261) of patients with suspected endophthalmitis during a 10-year period (1989-1998). Results The positive cultures were 119 (aqueous humor,44 and vitreous body,75) of 522 specimens. The average positive rate was 22.8%. Gram-positive cocci constituting 45.4%(54) of total isolates followed by Gram-negative bacilli,34.5%(41);Gram-positive bacilli, 20.2%(24). In the positive bacterial cultures, enterobacteriaceae was the most common isolate, 18.5%, and the next was micrococcus, 16.0%; coagulase-negative staphylococcus,12.6%; and pseudomonas,10.9%.Comparing the data from 1989 through 1993 with the data from 1994 through 1998, the frequency of Gram-positive cocci had no significant change, while the frequency of Gram-positive bacilli was decreased and the percentage of Gram′s-negative bacilli was increased. Conclusions Gram-positive cocci and Gram-negative bacilli are the predominant pathogens of bacterial endophthalmitis. The percentage of Gram′s-negative bacilli has increased for 5 years. It is very important to comprehend the distribution and shifting trends of these pathogenic bacteria for diagnosis, prevention and treatment of bacterial endophthalmitis.   • Analysis of pathogenic factors and etiological characteristics of 531 patients with suppurative endophthalmitis

    ObjectiveTo analyze pathogenic factors and etiological characteristics of suppurative endophthalmitis.MethodsA total of 531 consecutive patients (531 eyes) with suppurative endophthalmitis who were hospitalized in Qingdao Eye Hospital of Shandong Eye Institute from January 2006 to December 2015 were included in the study. Among them, 410 patients with 410 eyes were males (77.2%), 121 patients with 121 eyes were females (22.8%). The average age of the patients was 38.62±15.36 years. The relevant medical records were collected to analyze the pathogenic factors. Samples of aqueous humor, vitreous or other intraocular samples were taken under aseptic conditions for bacterial and fungal culture and in vitro drug sensitivity test.ResultsOcular trauma was the primary pathogenic factor of suppurative endophthalmitis (60.1%), other factors included postoperative endophthalmitis (19.0%), suppurative keratitis-related endophthalmitis (17.1%) and endogenous endophthalmitis (3.8%). Postoperative endophthalmitis mainly occured after cataract surgery. A total of 224 strains of organisms were isolated, among which the predominant organisms isolated were gram-positive bacteria (54.0%) and staphylococcus epidermidis was the most common (25.0%). The other pathogenic organisms were fungi (29.5%) and gram-negative bacteria (16.5%). Among the fungi, aspergillus (10.7%) was the dominant genus, followed by fusarium (9.8%). For gram-positive organisms, susceptibilities were vancomycin 97.4%, gatifloxacin 91.8%, fusidate acid 77.9% and levofloxacin 54.6%. For gram-negative organisms, susceptibilities were gatifloxacin 85.7%, levofloxacin 77.8%, tobramycin 71.4% and ceftazidime 62.5%. For fungal isolates, sensitivities were voriconazole 88.2% and amphotericin B 84.8%.ConclusionsOcular trauma is the main pathogenic factor of suppurative endophthalmitis, followed by postoperative endophthalmitis and suppurative keratitis-related endophthalmitis. Gram-positive bacteria are the major pathogenic organisms, especially staphylococcus epidermidis followed by fungal species, among which aspergillus and fusarium were the dominating pathogenic genus.

  • The role of clinical microbiology service in antimicrobial stewardship

    Inappropriate and irrational prescription of antimicrobial agents has led to increasing bacterial resistance, which has become a major concern around the world. Clinical microbiology service provides a basis and guarantee for the rational application of antimicrobial agents and an important technical support for antimicrobial stewardship, and plays an important role in promoting the rational use of antimicrobial agents. This paper summarizes and evaluates the specific role and technical requirements of clinical microbiology service in the diagnosis of infectious diseases, rational application of antimicrobial agents, hospital infection control and training of medical staff, so as to provide a technical guidance for clinical microbiology service in the antimicrobial stewardship.

  • A survey on the current situation of equipment and information system function of microbiology laboratory for etiological submission before antibiotic treatment in 783 hospitals

    Objective To preliminarily understand the equipment configuration and information system function improvement of the microbiology laboratory for etiological submission before antibiotic treatment in Chinese medical institutions, and provide a scientific basis for further targeted action work on the rate of etiological submission before antibiotic treatment. Methods A network questionnaire was released in “Sentinel Hospital Information Reporting System of National Hospital Infection Management Professional Quality Control Center” between March 1 and June 21, 2022. The second-level and above hospitals were investigated. Results A total of 783 hospitals were included, of which 765 (97.7%) hospitals had been equipped with microbiology laboratory equipment to varying degrees, and the allocation rate of tertiary hospitals was higher than that of secondary hospitals (P<0.05). The top three items of common pathogenic test were aerobic (98.3%), bacterial smear (97.6%) and fungal smear (95.1%), and the last three items were streptococcus pneumoniae urine antigen (20.4%), silver hexamine staining (19.0%) and gene sequencing (8.9%). The comparison of relevant information system and function improvement among hospitals of different levels showed that tertiary hospitals were superior to secondary hospitals (P<0.05). In different regions, except for the allocation rate of nosocomial infection information monitoring system and rational drug use monitoring system (P>0.05), other relevant systems and functional improvement of hospitals in eastern, central and western regions, the differences were statistically significant (P<0.05). The number of hospitals that could obtain the indicators of “etiological submission rate of inpatients before antibiotic treatment”“etiological submission rate related to hospital infection diagnosis”“etiological submission rate before combined use of key drugs” were 698 (89.1%), 474 (60.5%) and 337 (43.0%), respectively. Among the factors affecting the implementation of special actions, 454 hospitals (58.0%) thought that the information system was not fully functional, 341 hospitals (43.6%) thought that the etiology testing project was not fully carried out, and 148 hospitals (18.9%) thought that the microbiology laboratory testing ability was insufficient. Conclusion All kinds of hospitals at all levels in China, especially specialized and secondary hospitals, need to further improve the configuration of equipment and information system functions in the microorganism laboratory related to aetiology inspection, strengthen the support for microbial inspection, and strengthen information management, monitoring and analysis.

  • Microbiological characteristics of airway bacteria in adult patients with bronchiectasis and their correlation with the clinical features

    ObjectiveTo analyze the microbiological characteristics of airway bacteria in adult patients with bronchiectasis and to analyze their correlation with the clinical features. MethodsPatients diagnosed with bronchiectasis in the Department of Respiratory and Critical Care Medicine of West China Hospital of Sichuan University from October 2017 to April 2018 were classified into the bronchiectasis group, while the control group was those who were found to have pulmonary nodules (diameter less than 10 mm) requiring bronchoscopy by physical examination. All subjects in both groups had not used antibiotics or hormones within 4 weeks and had no other respiratory diseases. Bronchoalveolar lavage fluid (BALF) from the lesion site of the branchial expansion group was collected, and BALF from the basal segment of the contralateral inferior bronchial lobe of the pulmonary nodule was collected in the control group. Bacterial culture and 16S rRNA gene sequencing were performed in both groups. ResultsSeventeen cases and six controls were enrolled in this study and the BALF specimens were collected. Eight cases were in stable period and nine cases were in acute period. The case group was divided into the bacteria-positive group and negative group based on bacterial culture of BALF. Shannon index in the bacteria-positive group was significantly lower than the bacteria-negative group and the control group. And Shannon index showed a negative correlation with positive bacterial culture in BALF. When Shannon index ≤4.5 was used to predict positive bacterial culture, the sensitivity and specificity were 83.3% and 90.9% respectively. The average relative abundance of bacteria was higher and the average sample distribution uniformity was lower in patients with acute period, compared with those in patients with stable period. Shannon index was negatively correlated with the acute exacerbation in patients. When Shannon index <5.0 was used to predict acute exacerbation, the sensitivity and specificity were 77.8% and 100.0%, respectively. ConclusionsShannon index in 16S rRNA gene sequencing results has certain predictive value for acute exacerbation stage. 16S rRNA gene sequencing combined with bacterial culture results can help guide clinicians to provide more precise treatment plans.

At present, the demand for medical engineering interdisciplinary in the field of medical laboratory technology is increasing, which also puts forward new requirements for the “Clinical Microbiology Laboratory Technology”. This article elaborates on the necessity and existing problems of the medical engineering interdisciplinary, reports on the overall ideas of the curriculum reform of “Clinical Microbiology Laboratory Technology” at West China School of Medicine of Sichuan University based on the medical engineering interdisciplinary, and specifically elaborates on the reform practices in three aspects: teaching philosophy, teaching content and methods, and teacher optimization, as well as the preliminary achievements obtained, so as to provide new ideas for the curriculum reform of the “Clinical Microbiology Laboratory Technology”.
