  • CT and MRI diagnosis of rare pancreatic tumors

Objective To investigate the CT and MR imaging manifestation of rare pancreatic tumors in order to deepen the understanding of their imaging characteristics and improve the diagnostic accuracy. Methods Clinical and image date of 34 cases with rare pancreatic tumors proved by surgery and histopathologically were analyzed retrospectively. Including neuroendocrine tumors of the pancreas (NETP,n=13), solid-pseudopapillary tumors of pancreas (SPTP,n=10), intraductal papillary mucinous neoplasms (IPMN,n=2), serous cystadenoma (SCA,n=7), and mucinous cystadenoma (MCA,n=2). Examined by CT in 19 cases, by MRI in 11 cases, examined by CT and MRI at the same time in 4 cases. The characterized imaging features of each kind of tumors were analyzed emphatically. Results Of the 13 cases of NETP, solid lesions in 6 cases, predominantly soild in 4 cases, predominantly cystic in 3 cases. Homogenous enhancement in 6 cases, heterogeneous enhancement in 7cases, the soild constituent of all cases were showed moderate to marked enhancement. Of the 10 cases of SPTP, predominantly soild in 2 cases, soild and cystic in 5 cases, predominantly cystic in 3 cases. The solid part of 10 cases presented as gradually enhancement, 2 cases appeared hemorrhage, 1 case appeared stippled calcification. Of the 2 cases of IPMN, both of them were combined type, showed multilocular cystic tumors due to the dilated of the pancreatic duct. Of the 7 cases of SCA, microcystic partten in 3 cases and single cyst partten in 4 cases, showed unilocular or multilocular cystic with clear boundary. The 2 cases of MCA, showed unilocular cystic with clear boundary. Conclusion Different histological types of pancreatic rare tumor appeared different kinds of imaging characteristic, we may improve the diagnostic accuracy by analyzing their features.

  • Laparoscopic spleen-preserving distal pancreatectomy: a report of 17 cases

Objective To evaluate feasibility and clinical application value of laparoscopic spleen-preserving distal pancreatectomy (LSPDP). Method The clinical data of 17 patients underwent LSPDP from January 2015 to June 2017 in this hospital were retrospectively analyzed. Results The LSPDP was successfully completed in the 17 cases, with Kimura procedure and Warshaw procedure were performed in the 12 cases and 5 cases, respectively. The operative time was (218±60) min, the intraoperative blood loss was (136±114) mL, the time to get out of bed after surgery was (1.4±0.6) d, the postoperative fasting time was (2.0±0.8) d, and the postoperative hospital stay was (13.4±5.7) d. The rate of the postoperative pancreatic fistula was 17.6% (3/17). The spleen infarction occurred in the 2 cases following the Warshaw procedure. The pathologic examination showed that there were 2 patients with the serous cystadenoma, 7 patients with the mucinous cystadenoma, 3 patients with the solid pseudo-papillary tumor, 3 patients with the intraductal papillary mucinous cystadenoma neoplasm, and 2 patients with the insulinoma. All the patients were followed-up for 5 to 26 months (average 13 months), and the perigastric varice occurred in 1 patient, no recurrence or spleen infarction occurred during the following-up. Conclusion LSPDP is a safe, feasible and effective method with less injury and rapid recovery.

  • Combination therapy of ionizing radiation and lipofectin-mediated cytosine deaminase/5-flurocytosine system for pancreatic cancer cell

ObjectiveTo investigate the synergistic antitumor effects of ionizing radiation and the cytosine deaminase (CD)/5-flurocytosine (5-FC) system therapy in human pancreatic cancer cell.MethodsThe expression vector containing CD was transfected into the human pancreatic cancer cell line PC3. The clones were picked out after G418 selection. The CD gene integration and expression were confirmed by the RT-PCR. The cytotoxicity to the cells with or without CD and (or) ionizing radiation under the treatment with 5-FC was measured by the MTT assay. The clonogenic assay was used to investigate the radiosensitizing effect of 5-FC on the PC3 cells transfected or untransfected with CD gene.ResultsThe CD gene was stably expressed in the PC3 cells transfected with CD gene. The cytotoxic effect of 5-FC was superior on the PC3 cells transfected than that of untransfected with CD gene (P<0.05) and which were enhanced in combination with the ionizing radiation (P<0.05). The CD/5-FC enhanced the radiosensitivity of PC3 cells transfected with CD gene (P<0.05). The change in the radiosensitivity was quantified by calculating the sensitization enhancement ratio (SER) at the clinically relevant dose of 2 Gy. The SER was 1.5 in the PC3 cells transfected with CD gene by giving ionizing radiation of 2 Gy.ConclusionsCD/5-FC system is a potenial radiosensitizer in PC3 cells transfected with CD gene. Ionizing radiation and CD/5-FC system is more effective for killing effect of PC3 cells than ionizing radiation or CD/5-FC system alone.

  • Application of pancreatic duct stent in common pancreas diseases

ObjectiveTo investigate the application scope and effects of pancreatic duct stent in common pancreas diseases.MethodThe literatures on application of pancreatic duct stent in common pancreas diseases at domestic and foreign were reviewed.ResultsPancreatic duct stent had been used in acute and chronic pancreatitis and pancreatic tumor, which could the relieve symptoms, reduce the complications of pancreatitis, improve the prognosis, shorten the hospital stay, and reduce the hospitalization costs. Meanwhile it had also been used in pancreatic tumor surgery, palliative treatment, and brachytherapy that some achievements had been made.ConclusionAlthough application of pancreatic duct stent in some common pancreatic diseases is still controversial, effect is better and has some advantages with small trauma and low incidence of complications, which also provides a new treatment option for some common pancreatic diseases.

  • Application progress of mixed reality technology in hepatobiliary and pancreatic fields

Objective To investigate the application progress of mixed reality (MR) technology in hepatobiliary and pancreatic fields. Method The relevant literatures on the application of MR technology of the hepatobiliary and pancreatic field in recent years at home and abroad were reviewed. Results MR technology had been widely used in the hepatobiliary and pancreatic field, including preoperative diagnosis and evaluation, surgical plan formulation, doctor-patient communication, intraoperative navigation precision surgery, teaching practice and many other aspects, which had the advantages of shortening the operation time, reducing the difficulty of surgery and improving the success rate of surgery. To some extent, it had promoted the innovation of clinical diagnosis and treatment in the field of liver, gallbladder and pancreas. Conclusions The application and development of MR related techniques are of great significance to the operation and teaching in the hepatobiliary and pancreatic field. With the development and progress of MR technology and modern medicine, MR technology will give full play to its advantages in intelligent real-time navigation hepatobiliary and pancreatic surgery system and promote the further development of hepatobiliary and pancreatic surgery.
