  • EFFECTIVENESS COMPARISON OF MODIFIED PERCUTANEOUS MINI-INCISION AND OPEN Achilles TENDON LENGTHENING FOR TREATMENT OF Achilles CONTRACTURE SYNDROME

    Objective To explore the effectiveness of modified percutaneous mini-incision Achilles tendon lengthening for the treatment of Achilles contracture syndrome by comparing with traditional open Achilles tendon lengthening. Methods Between May 2008 and May 2012, 42 patients with Achilles contracture syndrome were treated and the clinical data were retrospectively analyzed according to the inclusion criteria. The modified percutaneous mini-incision Achilles tendon lengthening was used in 19 cases (19 feet, trial group); 0.5 cm incisions were designed at medial and lateral Achilles tendon according to the degree of Achilles contracture syndrome; and percutaneous double hemi-section was done according to the deformity range of varus and valgus foot. The traditional open Achilles tendon lengthening was used in 23 cases (24 feet, control group). There was no significant difference in age, gender, disease duration, motion of ankle dorsiflexion, and Hannover Achilles tendon score (P gt; 0.05). Results The operation time, postoperative hospitalization days, and intraoperative blood loss in the trial group were less than those in the control group, showing significant differences (P lt; 0.05). After operation, incision healing by first intention was obtained in the other cases except 1 case which had infection in the control group. The patients were followed up 6-35 months in the trial group and 8-34 months in the control group. Correction of varus deformity foot was not obvious in 1 case of the trial group, correction of varus and valgus deformity feet were satisfactory in the other cases. There was no significant difference in motion of ankle dorsiflexion and Hannover Achilles tendon score between 2 groups at last follow-up (P gt; 0.05), but significant differences were found between preoperation and last follow-up in 2 groups (P lt; 0.05). There were significant differences in motion of ankle dorsiflexion and Hannover Achilles tendon score between normal and affected sides in 2 groups at last follow-up (P lt; 0.05). Conclusion Modified percutaneous mini-incision Achilles tendon lengthening has similar effectiveness to the traditional open Achilles tendon lengthening for Achilles tendon contracture syndrome. And it can effectively correct varus and valgus deformities of the foot with less trauma and fast recovery.

  • COMPARISON OF EARLY EFFECTIVENESS BETWEEN TOTAL PERCUTANEOUS ENDOVASCULAR ANEURYSM REPAIR AND SURGICAL FEMORAL CUTDOWN ENDOVASCULAR ANEURYSM REPAIR FOR ASYMPTOMATIC ABDOMINAL AORTIC ANEURYSM

    Objective To investigate the early effectiveness of total percutaneous endovascular aneurysm repair (TPEVAR) in treating asymptomatic abdominal aortic aneurysm (AAAA) by comparing with surgical femoral cutdown endovascular aneurysm repair (SFCEVAR). Methods Between January 2010 and May 2011, 41 cases of AAAA were treated with TPEVAR in 26 cases (TPEVAR group) and with SFCEVAR in 15 cases (SFCEVAR group). The maximum tumor diameter ranged from 3.5 to 9.2 cm (mean, 5.7 cm) in TPEVAR group, and ranged from 3.5 to 10.0 cm (mean, 6.9 cm) in SFCEVAR group. There was no significant difference in gender or age between 2 groups (P gt; 0.05). Results All patients underwent EVAR successfully. The patients were followed up 6-23 months (mean, 13.5 months). No significant difference was found in the outer diameters of the delivery system for main body and iliac leg, operation time, contrast media dosage, hospitalization days, or postoperative hospitalization days between 2 groups (P gt; 0.05). The patients of SFCEVAR group had more bleeding volume and longer ICU stay than patients of TPEVAR group (P lt; 0.05). The incidence of minor complication was 7.7% (2/26) in TPEVAR group and 33.3% (5/15) in SFCEVAR group, showing no significant difference between 2 group (χ2=4.42, P=0.08); the incidence of major complication in SFCEVAR group (20.0%, 3/15) was significantly higher than that in TPEVAR group (0) (χ2=5.61, P=0.02). Conclusion TPEVAR shows safer and more effective than SFCEVAR in treating AAAA.

  • EFFECTIVENESS COMPARISON BETWEEN MODIFIED PERCUTANEOUS SUTURE AND CONVENTIONAL OPEN SUTURE IN REPAIRING ACUTE CLOSED Achilles TENDON RUPTURE

    Objective To investigate the effectiveness of modified percutaneous suture in repairing acute closed Achilles tendon rupture by comparing with conventional open suture. Methods Between January 2006 and October 2009, 50 patients with acute closed Achilles tendon rupture were treated with modified percutaneous suture by making 5 small incisions at both sides of Achilles tendon and zigzag suture (improved group, n=22) and with Kessler suture (conventional group, n=28), respectively. No significant difference was found in gender, age, time from injury to operation between 2 groups (P gt; 0.05). Results In improved group, the patients achieved healing of incisions by first intention after operation and nocomplication occurred; however, incision infection occurred in 1 case, Achilles tendon re-rupture in 1 case, and incision scar contracture in 2 cases in conventional group. The operation time of improved group [(38.7 ± 6.6) minutes] was significantly shorter (t= —12.29, P=0.00) than that of conventional group [(52.3 ± 6.9) minutes]; the blood loss of improved group [(4.9 ± 2.0) mL] was significantly less (t= —25.20, P=0.00) than that of conventional group [(40.7 ± 7.1) mL]. The patients were followed up 2-3 years (mean, 29.9 months). The American Orthopaedic Foot and Ankle Society (AOFAS) score was 99.6 ± 1.0 in improved group and was 98.4 ± 3.0 in conventional group, showing no significant difference between 2 groups (t=1.66, P=0.10). Conclusion Comparison with conventional open suture, modified percutaneous suture has some advantages, such as easy operation, less complications, rapid recovery of limb function, and so on. Modified percutaneous suture is one of the best choices for the treatment of acute closed Achilles tendon rupture.

  • MINIMALLY INVASIVE PERCUTANEOUS PLATE OSTEOSYNTHESIS TECHNIQUE FOR TREATMENT OF NEER II, III PARTS FRACTURE OF PROXIMAL HUMERAUS

    Objective To evaluate the cl inical significance of minimally invasive percutaneous plate osteosynthesis (MIPPO) appl ied in Neer II, III parts fractures of proximal humeraus. Methods The cl inical data were retrospectively analyzed, from 30 patients with Neer II, III parts fractures of proximal humeraus in accordance with selection criteria, who were treated with manual reduction and plaster external fixation (11 cases, non-operative group) or with MIPPO (19 cases, operative group) between January 2008 and May 2010. In non-operative group, there were 6 males and 5 females with an average age of 60 years (range, 56-80 years) and with an average time of 10 hours (range, 3-24 hours) between injury and reduction, including 8 cases of Neer II and 3 cases of Neer III. In operative group, there were 13 males and 6 females with anaverage age of 65 years (range, 45-78 years) and with an average time of 3 days (range, 1-5 days) between injury and operation, including 9 cases of Neer II and 10 cases of Neer III. There was no significant difference in gender, age, fracture type, and time from injury to operation (P gt; 0.05). The shoulder joint function before and after treatments was evaluated according to Constant-Murley Score (CMS) and American Shoulder and Elbow Surgeons’ Form (ASES) scoring systems. Results Thirty patients were followed up. In operative group, the follow-up time ranged from 11 to 18 months (mean, 12 months); all incisions healed by first intention with no compl ication of internal fixation failure, infection, or nerve injury. In non-operative group, the follow-up time ranged from 9 to 15 months (mean, 11 months). The X-ray films showed that fractures healed without humeral head necrosis in 2 groups. The bone heal ing time in operative group and non-operative group was (11.47 ± 2.48) weeks and (11.82 ± 2.44) weeks, respectively, showing no significant difference (t=0.369, P=0.889). The CMS score and ASES score at each time point after treatment were significantly better than those before treatment (P lt; 0.05); the CMS scores in operative group were better than those in non-operative group at 3 weeks, 3 months, and 1 year after treatment (P lt; 0.05); and the ASES score in operative group was better than that in non-operative group at 3 weeks and 3 months after treatment (P lt; 0.05), but no significant difference was found at 1 year after treatment (P gt; 0.05). Conclusion MIPPO fixation using the locking compression plate is an effective option for Neer II, III parts fractures of proximal humeraus. It can provide good functional recovery of the shoulder joint so that patients can get back to their normal l ife as soon as possible.

  • IMPROVED PERCUTANEOUS KYPHOPLASTY FOR DIAGNOSIS AND TREATMENT OF THORACOLUMBAR METASTATIC SPINE TUMORS

    Objective To investigate the diagnosis and effectiveness of improved percutaneous kyphoplasty (PKP) for patients with thoracolumbar metastatic tumors, who could not tolerate anesthesia and open operation. Methods Between September 2009 and September 2010, 16 patients with thoracolumbar metastatic tumors underwent improved PKP. Of 16 patients, 7 were male and 9 were female with an average age of 64.5 years (range, 60-73 years). All patients had vertebralmetastasis tumor. The disease duration was 3-6 months with an average of 4 months. The visual analogue scale (VAS) score was 8.9 ± 0.8. No spinal cord compression and nerve root compression was observed. The involved vertebrae included T7 in 1 case, T8 in 1, T12 in 1, L2 in 2, L3 in 2, L4 in 3, T1, 2 in 1, T3, 4 in 1, T7, 8 in 1, T11, 12 in 1, T7-L1 in 1, and T12-L4 in 1. Nine patients had vertebral compression fracture with a vertebral compression rate below 75%. Results All patients were successfully performed PKP. There was no serious adverse reactions in cardiopulmonary and brain vascular systems and no perioperative death. The biopsy results showed that all were metastatic adenocarcinoma. All patients were followed up 9-18 months mean, 14 months). Complete pain rel ief was achieved in 14 cases and partial rel ief in 2 cases 6 months after operation according to World Health Organization criterion, with a pain-rel ief rate of 87.5%. The VAS score was 1.8 ± 0.6 at 6 months postoperatively, showing significant difference when compared with the preoperative score (P lt; 0.05). Two patients had cement leakages in 3 vertebrae with no symptoms at 6 months postoperatively. During follow-up, 12 patients died and the others survived with tumor. Conclusion For patients with thoracolumbar metastatic tumors who can not tolerate anesthesia and open operation, improved PKP has the advantages such as minimal invasion, high diagnostic rate, and early improvement of pain in the biopsy and treatment. It can improve patient’s qual ity of l ife in the combination of radiotherapy or chemotherapy.

  • Selectivity Individualization of Curing Lumbar Intervertebral Disc Protrusion by Combined Minimally Invasive Techniques

    摘要:目的:探讨联合应用激光汽化减压(percutaneous laser disc discompression,PLDD)、射频热凝靶点消融、臭氧注射治疗腰椎间盘突出症的的个体化选择。方法: 自2006年6月,在CT引导下选择性联合应用PLDD、射频和臭氧治疗腰椎间盘突出症患者267例,突出椎间盘的特点个体化选择穿刺路径和治疗方法;其中PLDD联合臭氧治疗92例(A组),射频联合臭氧治疗67例(B组),PLDD、射频和臭氧三者联合治疗108例(C组)。结果:所有患者均顺利完成手术,于术后1周、1个月,3个月及6个月随访记录VAS评分和Macanab优良率。三组患者VAS评分经方差分析,手术前、后有显著性差异(Plt;0.05),术后1周至6个月的VAS评分统计无显著性差异(Pgt;0.05);术后三组间VAS评分、Macanab优良率比较无显著性差异(Pgt;0.05)。结论: 选择性联合应用微创技术进行个体化的立体治疗,具有扩大微创手术适应症、提高手术疗效的优势,值得推广和利用。Abstract: Objective: To investigate the selectivity and individualization of using percutaneous laser disc discompression(PLDD) and ozone injection combined with radiofrequency thermocoagulation and target ablation curing lumbar intervertebral disc protrusion. Methods: From June 2006, 267 lumbar disc herniation cases were operated that guided by CT, the characteristic of the liable disc was confirmed by magnetic resonance imaging and CT before the procedure. 92 cases (A group) were treated by PLDD combined with ozone injection,67 case were treated by radiofrequency thermocoagulation and target ablation combined with ozone injection, 108 cases were treated by PLDD and ozone injection combined with radiofrequency thermocoagulation and target ablation. Results: All case been successfully operated, the theraptic effect was evaluated by comparing the value of VAS and excellent and good rate of therapy at preoperation and at 1 week, 1month,3 months, 6 months after operation. The value of VAS in three groups at postoperation were remarkably lower than preoperation (Plt;0.05). The excellent and good rate of therapy at 6 months was respectively 94.5% in group A,94.0% in group B and 95.4% in group C,no significant difference was observed between the three groups(Pgt;0.05).Conclusion: The selectivity and individualization of using PLDD and ozone injection combined with radiofrequency thermocoagulation and target ablation curing lumbar intervertebral disc protrusion can enlarge the indication and improve the clinical curative effect, it should be spreaded in clinic.

  • SHORT-TERM EFFECTIVENESS OF MINIMALLY INVASIVE PERCUTANEOUS PLATE OSTEOSYNTHESIS IN TREATMENT OF ANTERIOR PELVIC RING FRACTURES

    ObjectiveTo investigate the short-term effectiveness of minimally invasive percutaneous plate osteosynthesis (MIPPO) in treatment of anterior pelvic ring fractures. MethodsBetween January 2012 and October 2013, 16 patients with anterior pelvic ring fractures were treated with MIPPO. There were 10 males and 6 females at the age of 20-63 years (mean, 41 years). The causes of injury were traffic accident in 9 cases and falling from height in 7 cases. The duration of injury to admission was 2 hours to 5 days (mean, 1 day). According to Tile classification, 8 cases were rated as type B2, 4 cases as type B3, 2 cases as type C1, and 2 cases as type C2. Of them, 2 cases had iliac wing fracture, and 4 cases had pelvic posterior ring fracture. The time from admission to operation was 3-12 days (mean, 6 days). ResultsThe bleeding volume was 60-120 mL (mean, 70 mL). All wounds healed by first intention. No postoperative complication of deep venous thrombosis or long-term continuous pain occurred. All cases were followed up 5-27 months (mean, 11.5 months). No clinical manifestation of lateral femoral cutaneous nerve injury or spermatic cord injury was found, and cremasteric reflex existed in males. All cases obtained bony union, and the healing time was 12-16 weeks (mean, 13 weeks). During the follow-up period, no loss of fracture reduction and no internal fixation loosening or broken were observed. According to Matta radiological evaluation criterion, 16 cases had anatomical reduction, and 3 cases had satisfactory reduction; according to Majeed scoring system of pelvic fracture, the results were excellent in 12 cases and good in 4 cases. ConclusionMIPPO for treatment of anterior pelvic ring fractures has the advantages of less intraoperative blood loss, few soft tissue complications, and low infection rate, and can get satisfactory short-term effectiveness.

  • RESEARCH PROGRESS OF SUPERCAPSULAR PERCUTANEOUSLY ASSISTED TOTAL HIP IN MINIMALLY INVASIVE TOTAL HIP ARTHROPLASTY

    ObjectiveTo summarize the research progress of Supercapsular percutaneously assisted total hip (SuperPath) in minimally invasive total hip arthroplasty (THA). MethodsThe related literature on SuperPath in terms of its history, indications, contraindications, surgical technique, and curative effect was summarized and analyzed. ResultsSuperPath for minimally invasive THA is an approach that makes use of the interval between the gluteus medius and piriformis, it can eliminate the need to cut through any muscle and tendon. In comparison to conventional surgery, the SuperPath has the advantages of less soft tissue injury, quicker post-operative recovery, shorter hospitalization days, and the short-term curative effect is good. But there are also some disadvantages of narrower indications, need of special equipment and higher prosthesis malposition. ConclusionThe SuperPath can reduce surgical trauma and the risk of postoperative dislocation and other complications, so the purpose of early rapid rehabilitation is achieved. However, the time of its clinical application is still short, so the long-term effectiveness and related complications should be further observed.

  • Diagnostic Value of CT-Guided Percutaneous Lung Biopsy in Pulmonary Diseases

    ObjectiveTo evaluate the diagnostic value of computer tomography (CT)-guided percutaneous lung biopsy and tissue culture in pulmonary diseases. MethodsAfter the preoperative evaluation, 48 patients underwent CT-guided percutaneous lung biopsy were analyzed, and 16 patients accepted tissue culture. The clinical significance of combined diagnostic rate of histopathology and tissue culture was investigated. ResultsAll patients were punctured successfully. The pathological diagnosis confirmed 26 (54.2%) cases with malignant lesions and 16 (33.3%) cases with benign lesions. Six cases could not be pathological diagnosed. The pathological diagnostic rate was 87.5%. Sixteen patients accepted tissue culture. Among them 3 cases with benign lesions were cultured. The positive rate of tissue culture was 18.8%, and improved to 30.0% in the patients with benign lesion. The combined diagnostic rate of histopathology and tissue culture was 89.6%. In addition, the accuracy rate of pathological diagnosis in the group of lung nodules was 95.0%, higher than the others. The accuracy rate of pathological diagnosis in lung nodules of 2cm~3cm in diameter was 100.0%, and the positive rate of tissue culture in lung lump of 3cm~5cm in diameter was 25.0%. The incidence of pneumothorax and hemorrhage was 14.6%. ConclusionsCT-guided percutaneous lung biopsy is a safe and effective diagnostic method. For pulmonary infectious diseases, combination of CT-guided percutaneous lung biopsy and tissue culture can improve the early diagnostic rate.

  • The Impact of Social Resources on Self-management in Patients Undergoing Coronary Stent Implantation

    ObjectiveTo analyze the impact of social resources on self-management in patients undergoing coronary stent implantation, and provide evidence for nurses to predict patients' self-management behavior and take intervening measures. MethodsA convenience sample of 359 patients undergoing coronary stent implantation between July and December 2013 was selected according to the criteria. Three instruments were used to collect data including general data scale, chronic illness resources survey (CIRS) and coronary artery disease self-management scale (CSMS). ResultsThe score of CIRS among 359 patients was 2.77±0.60, and the scores of six subscales in descending order were support of neighborhood/community, media and policy, and family and friends, and personal coping, and support of physician/health care team, and organizations. The total score of CSMS was 55.58±10.59, and the scores of three subscales in descending order were:daily management, emotion management, and medical management. The total self-management score and factor scores of CIRS were significantly correlated with the total and factor scores of CSMS (r=0.183-0.663, P<0.05), while the correlation of support of physician/health care team and family and friends with daily life management was negative (r=-0.215, -0.294; P<0.05). ConclusionIn this study, patients' total social resources are at a low level; the standard of self-management is at a moderate level, while medical management has the lowest score; good social resources can promote self-management, but supports from medical staff, family and friends are bad for patients' daily life management.
