  • PROGRESS IN TREATMENT OF TERRIBLE TRIAD OF ELBOW

    Objective To review the injury mechanism and the treatment progress of terrible triad of the elbow, and to analyze the direction of further research. Methods Related literature concerning terrible triad of the elbow was extensively reviewed and comprehensively analyzed. Results The main treatment of terrible triad of the elbow is operation. The ultimate goal of treatment is to reconstruct sufficient stability of the elbow. The treatment includes fixation of the coronoid by suture, screw or plate; fixation of radial head by screw and plate, partial or complete replacement of the radial head; fixation of lateral collateral ligament and the medial collateral ligament by bone suture or anchors and the application of the external fixator. These surgical treatments have their own indications and advantages, most get satisfactory results. Conclusion Generally, surgery is needed to maintain the stability of the elbow for patients of terrible triad elbow. However, medial ligament repair or not, the choice of approach, and mechanism of injury still need further study.

  • RESEARCH PROGRESS OF POSTEROLATERAL ROTATORY INSTABILITY OF THE ELBOW

    【Abstract】 Objective To review the progress in pathoanatomy, diagnosis, and treatment of posterolateral rotatory instability (PLRI) of the elbow. Methods Related literature concerning PLRI of the elbow was extensively reviewed, comprehensive analysis was done. Results The lateral collateral ligament complex (LCLC), radial head, capitellum, and coronoid process are important constraints to PLRI. Muscle groups that cross the lateral elbow are secondary constraints to PLRI. Clinical examination includes lateral pivot-shift test, lateral pivot-shift apprehension test, chair sign, active floor push-up sign, tabletop relocation test, and posterolateral rotatory drawer test. Radiology, arthroscopy, and ultrasound can help diagnosis of PLRI. Reconstruction of bony fixation or soft tissue fixation can be used for treatment of injured LCLC. Conclusion The primary constraints to PLRI is LCLC. Ultrasound imaging is accurate for identification and measurement of normal LCLC. Therefore, ultrasound may prove valuable in assessment of abnormal lateral ulnar collateral ligaments. Reconstruction of soft tissue fixation, which can avoid iatrogenic fracture, is a selective treatment method.

  • PROGRESS OF LIPOSOMES AS ANTIBIOTIC CARRIERS

    Objective To reviewe the research progress of liposomes as antibiotic carriers. Methods Domestic and abroad literature concerning liposomes as antibiotic carriers was reviewed and analyzed thoroughly. Results Liposomes as antibiotic carriers can significantly improve drug distribution, enhance antibacterial activity, and reduce the side effects of antibiotics during treatment. But it also has some problems, such as poor physical and chemical stabilities and low encapsulation efficiency. Conclusion Liposomes as antibiotic carriers can reduce the drug toxicity, improve drug biodistribution, and pharmacokinetics, and bring the dawn to completely curing infections disease.

  • PROGRESS IN DIAGNOSIS AND TREATMENT OF TARSAL COALITION

    Objective To review the progress in the diagnosis and treatment of tarsal coal ition. Methods Recent l iterature concerning the diagnosis and treatment of tarsal coal ition was reviewed. Results Tarsal coal ition is a bridge between the tarsal bones of the foot. The most common types are talocalcaneal and calcaneonavicular coal itions. Calcaneonavicular coal itions can be diagnosed with an oblique radiograph of the hindfoot. Most talocalcaneal coal itions require computer tomography for diagnostic confirmation. Magnetic resonance imaging may be useful for diagnoses of cartilaginous andfibrous coal ition. Casting is the usual initial treatment for the symptomatic individual. For patients with treatment failure and no degenerative changes, resection of the coal ition can be performed with good results. Isolated subtalar fusion may be performed for patients with failure of talocalcaneal resections. For patients undergoing failure of subtalar fusions and calcaneonavicular resection, tri ple arthrodesis may be performed. During minimally invasive operation, operation indications should be strictly controlled. Conclusion The diagnosis method of tarsal coal ition is clear. The cl inical manifestation combined with imaging examination can improve the diagnosis rate. The surgical indication of tarsal coal ition remains controversial, the randomized prospective studies are still required.

  • TREATMENT AND RESEARCH PROGRESS OF MASSIVE ROTATOR CUFF TEARS

    Objective To review the progress in the treatment and research of massive rotator cuff tears. Methods Recent l iteratures about the treatment and research of massive rotator cuff tears were reviewed. Results Treatment options of massive rotator cuff tears include nonoperative treatment, debridement, direct repair, tendon transfer, and repair with various substitutes, but the outcome is unpredictable. Recently, many experimental studies on the treatment of massive rotator cuff tears, such as gene therapy, cell therapy, and tissue engineering techniques, can provide cl inicians with new treatment strategies. Conclusion The treatment of massive rotator cuff tears pose a distinct cl inical challenge for the orthopaedist, depending on the overall presentation. The effect of traditional operation method to repair massive rotator cuff tears is limited. The treatment and research of massive rotator cuff tears still need to be studied.

  • RESEARCH PROGRESS OF ARTIFICIAL TRACHEAL PROSTHESIS

    Objective To review the research of the artificial tracheal prosthesis in the past decade so as to provide theoretical references for the development of the artificial tracheal prosthesis. Methods The l iterature about the artificial tracheal prosthesis was extensively reviewed and analyzed. Results Many new materials are used for the research of artificialtracheal prosthesis which have excellent biocompatibil ity and stabil ity of the structural characteristics. And many compl ications such as migration, obstruction, and infection have been resolved, but so far none of the new materials has been used for cl inical treatment successfully. Conclusion The choice of the materials for artificial tracheal prosthesis is the key to succeed. Biodegradable polymer materials with its unique biological properties become the new direction of the tracheal prosthesis research.

  • PROGRESS IN TREATMENT OF Kienböck’s DISEASE

    Objective To review the progress in the cl inical treatment of Kienbouml;ck’s disease. Methods Related l iterature concerning the treatment of Kienbouml;ck’s disease was reviewed, and comprehensive analysis was done. Results The treatment methods of Kienbouml;ck’s disease include non-surgical treatment and surgical treatment, which are primarily guided bythe anatomic factor and Lichtman stage. Non-surgical treatment methods should be selected in patients of children and at stage I. Surgical treatment methods include vascularized bone graft transfer into the cored-out lunate, radial shortening osteotomy, radial lateral wedge osteotomy, etc. All surgical treatment methods have advantages and disadvantages. Conclusion For the treatment of Kienbouml;ck’s disease, none of procedure is superior to another. In short, choosing different treatment strategies based on different patients can achieve the desired outcome.

  • APPLICATION AND RESEARCH PROGRESS OF ARTIFICIAL CHORDAE TENDINEAE

    Object ive To summar ize recent advance in the appl icat ion and research of ar t i f icial chordae tendineae. Methods The cl inical and experimental research l iterature was extensively reviewed and analyzed. Results The follow-up results showed that artificial chordae tendineae replacement was superior to other operation methods in valve repair. But, it was compl icated and difficult-to-learn. In recent years with the development of many surgical skills and new techniques, good cl inical results were achieved. Conclusion With the development of surgical equi pment, chordae material, and implanting skills, artificial chordae tendineae implanting will be easier and the scope of appl ication will be larger.

  • PROGRESS IN AMPUTATION TECHNIQUE OF DIABETIC FOOT

    Objective To review the progress in amputation technique of diabetic foot. Methods Recent l iterature concerning the amputation technique of diabetic foot was reviewed and analyzed. Results According to the different levels of the amputation, the diabetic foot’s amputation can be classified as major amputation and minor amputation, and differentkinds of methods can derive from these styles. Different factors should be considered when the style and method of amputation are chosen. Conclusion To the diabetic foot amputation, the general rule is to l imit the amputation level on the premise of the good cl inical effect. The health state, the region of the diabetic foot, the tissue perfusion, the susceptibil ity to infection in local tissue, and the abil ity of wound healing are important factors in selecting the styles and methods of diabetes-related amputations. Importance should be attached to the synthesis treatments of diabetes to prevent the higher level amputation.

  • RESEARCH PROGRESS OF VISCOSUPPLEMENTATION

    Objective To carry out a comprehensive and objective evaluation of viscosupplementation, so as to provide the actual judgments of effectiveness, development trends, and direction. Methods The related l iterature was reviewed, and the keys were the assessments from the international relevant advisory bodies and insurance companies and the evidence-based medicine on cl inic. Results Viscosupplementation has been gradually accepted. Although there are many different cl inical views of the viscosupplementation in the molecular weight of hyaluronan, injection dose, and the indications, intra-articular injection of hyaluronan for the treatment of osteoarthritis is well accepted. ConclusionViscosupplementation is one of the methods of treating osteoarthritis. As it has been already verified that hyaluronan iseffective in treating osteoarthritis of the knee and the periarthritis of shoulder, the hip joint injection is gradually recognized,and other small joints are going gradually.
