  • Effects of Modified Ultrafiltration on Blood Rheology in Infants after Open Cardiac Surgery

    Abstract:Objective To evaluate the effects of modified uhrafihration (MUF)on blood rheology in infants after open cardiac surgery. Methods According to admission number, 22 infants of body weight less than 10 kilograms with ventricular septal defect (VSD) and pulmonary hypertension (PH) were divided into control group (10 infants, the mantissa of their admission number was odd number) and experimental group (12 infants, the mantissa of their admission number was even number). Cases in control group didn't undergo MUF at the end of cardiopulmonary bypass (CPB), while cases in experimental group underwent MUF; the flow rate of MUF ranged from 10 ml/min · kg to 15 ml/min · kg. MUF lasting for 10-15 minutes. Blood samples were repeatedly collected as following time: before operation, at the end of CPB, 15 minutes after CPB or the end of MUF, 2, 24 h after operation. Blood sample of 2. 5 ml was collected from the radial artery with hepathrom test-tube. The changes of relative indexes of the blood rheology were observed by MDK-3200 completely automatic dual pathways blood rheology testing analysator at 37±1 C. Results Hemoglobin, hematocrit, red cell count, blood yielding stress, plasma viscosity, the whole blood viscosity at high shear rate, the whole blood viscosity at middle shear rate and low shear rate, the whole blood reduction viscosity at high shear rate and middle shear rate, the whole blood reduction viscosity at low shear rate and Casson viscosity in experimental group at the end of MUF were significantly higher than those in control group at 15 minutes after CPB (P〈0. 05). There was no significant difference in red cell aggregation index and red cell deformity between two groups at each moment (P 〉 0.05 ). Conclusion Hemoglobin, hematocrit and red cell count are significantly elevated through MUF after CPB. Whole blood viscosity in infants undergone open cardiac surgery after CPB with MUF is higher than those who didn't undergo MUF.

  • EARLY DIAGNOSIS OF LOWER LIMB DEEP VEIN THROMBOSIS AFTER MAJOR ORTHOPEDIC SURGERIES

    Objective To investigate the early diagnosis of lower l imb deep vein thrombosis (DVT) after major orthopedic surgeries. Methods From October 2005 to June 2009, color doppler sonography and hemorheology detection were carried out for 62 cases undergoing first total hip arthroplasty (THA), 14 cases undergoing total knee arthroplasty (TKA), and 86 cases undergoing hip fractures surgery (HFS) before operation and 1, 7, 14 days after operation. The plasma D-dimerlevels of the DVT were also examined for the THA patients before operation and 3 days after operation. Of all cases, therewere 89 males and 73 females, aged from 34 to 74 years (51.5 years on average). After operation, all the patients were treated with the regular low molecular weight heparin sodium against DVT. Results After operation, 17 cases (10.5%) developed DVT, including 8 THA cases, 1 TKA case, and 8 HFS cases. Preoperative color doppler sonography showed no abnormal echo, normal b blood flow signal, and normal periodical variation in vein blood flow without regurgitation. Postoperative examinations showed that the vascular occlusion of common femoral vein and popl iteal vein occurred in DVT patients. There were significant differences (P lt; 0.05) in whole blood viscosity between patients with DVT and without DVT after 1, 14 days and no significant difference (P gt; 0.05) before operation and 7 days after operation. There was no significant difference (P gt; 0.05) in plasma viscosity and erythrocyte aggregation index, between patients with DVT and without DVT pre- and postoperation. There was significant difference (P lt; 0.05) in erythrocyte deformation index between patients with DVT and without DVT 14 days after operation. The preoperative plasma D-dimer levels of patients with DVT and without DVT were (372.00 ± 148.62) ng/mL and (369.00 ± 141.03) ng/mL, respectively, showing no significant difference (P gt; 0.05); the 3 days postoperatively levels were (574.00 ± 217.29) ng/mL and (391.00 ± 120.16) ng/mL, respectively, showing significant difference (P lt; 0.05). Conclusion Color doppler sonography in combination of hemorheology and plasma D-dimer examination can be beneficial for the early diagnosis of DVT in major orthopedic surgeries.

  • HEMORHEOLOGY OF ISLAND FLAP AFTER ISCHEMIA-REPERFUSION INJURY AND MODULATION OF DEXAMETHASONE

    OBJECTIVE: To study the hemorheology of island flap after ischemia-reperfusion injury and modulation of dexamethasone. METHODS: Sixty Wister rats were made ischemia-reperfusion injury model, and divided into two groups randomly(Group I: intraperitoneal injection of normal saline 2 ml/kg as control group; Group II: intraperitoneal injection of dexamethasone 5 mg/kg as experimental group). Flap survived areas were measured and neutrophil necrosis numbers in flaps were counted. Erythrocytes and neutrophil hemorheology were observed. RESULTS: Area survived flap in group II was larger than that in group I. Neutrophil necrosis numbers were less in group II than in group I (P lt; 0.05). Whole blood hyposhear viscosity, erythrocyte aggregation, Casson yield stress and nerutrophil adhesion ability were higher in group I than in group II (P lt; 0.05); and the neutrophil deformability was lower in group I than in group II. CONCLUSION: Flap inchemia-reperfusion can increase erythrocyte aggregation index and neutrophil adhesion ability. Dexamethasone can improve these and decrease neutrophil necrosis numbers, so as to prevent flap from ischemia-reperfusion injury.

  • Application of laser-Doppler retinal blood flowmeter in diabetic retinopathy

    Objective To observe the effect of laser-Doppler retinal blood flowmeter in diabetic retinopathy. Methods The blood flow volume (VOL), the blood flow velocity (FLW) and the erythrocyte flow velocity (VEL) of peri-papillary retina were measured with the non-invasive Heidelberg Retinal Flowmeter (HRF) in 108 patients (216 eyes) with diabetes and 32 patients (64 eyes) in normal control group. The patients with diabetes were divided into non-diabetic retinopathy (NDR) group (27 patients, 54 eyes) and non-proliferative diabetic retinopathy (NPDR) group (81 patients, 162 eyes). NPDR group were subdivided into 3 groups: 26 patients (52 eyes) in mild group, 24 patients (48 eyes) in moderate group, and 31 patients (62 eyes) in severe group. The foveal avascular zone area (FAZ) was measured by fundus flourescein angiography (FFA) in patients with diabetes and some in the control group. The data in each group were statistically analyzed. Results The parameter of retinal blood flow of temporal and nasal peri-papillary retina in NDR and NPDR group was significantly lower than that in the control group (Plt;0.05). With the degree of retinopathy becoming more severe, the FLW and VEL of temporal and nasal peri-papillary retina in NDR and mild NPDR group presented ascending tendency, reached the peak in moderate NPDR group, and then decreased. The change tendency of the FLW was more obviously. The VOL of temporal and nasal peri-papillary retina in moderate NPDR group was obviously higher than that in the other groups (Plt;0.01).The FLW and the VEL of temporal and nasal peri-papillary retina in moderate NPDR group were significantly higher than that in mild NPDR and NDR group (Plt;0.01). The FLW and the VEL of temporal peri-papillary retina in severe NPDR group were obviously higher than that in NDR group (Plt;0.01). Blood sugar value positively correlated with degree of diabetic retinopathy (r=0.172,P=0.046). The FLW and the VEL of temporal peri-papillary retina positively correlated with FAZ area in patients with diabetes (r=0.268, P=0.000;r=0.275, P=0.000). The FAZ area positively correlated with the degree of macular degeneration in patients with diabetes (r=0.559, P=0.000). Conclusion As a non-invasive method for measurement of retinal blood flow, HRF has important value in revealing the mechanism and degree of pathological changes and choice of treatment for diabetic retinopathy.   • The effect of combined treatment of xue-shuan-tong and isovalaemic haemodilution on activities of fibrinolysis and hemorrheology in patients with retinal vein occlusion

    Objective To explore the effect of xue-shuan-tong(panax notoginsang saponins,PNS)or isovalaemic haemodilution(IHD)and PNS combining IHD treatment on activities of fibrinolysis and hemorrheology in patients with retinal vein occlusion (RVO). Methods Seventy-three patients with RVO were allocated at random to 3 groups which were treated with PNS,IHD and PNS combining IHD.The activities of t-PA and PAI,rheological parameters and visual acuity before and after treatment were observed. Results At the end of treatment,significantly increased activity of t-PA and decrease of PAI was found in combined treatment group and PNS group,but the difference before and after treatment was not significant in IHD group.Furthermore,except the plasma viscosity in IHD group,the other hemorrheological parameters in all the petients of 3 groups revealed to be improving.One month after treatment,the parameters return completely to normal in both PNS and IHD groups; while the whole blood apparent relative viscosity in low shear rate,RBC aggregation and RBC deformability maintained still in lower level,and also the visual acuity resumed better and quicker in combined group. Conclusion Combined treatment of PNS and IHD can both regulate the activity of fibrinolysis and decrease the blood viscosity of patients with RVO for a period of relatively long time and increase the effect of treatment.   • THE FEATURES OF HEMORRHEOLOGY IN PATIENTS WITH UVEITIS AND BEHCET' SYNDROME

    Using paired t-test statistical analysis,authors studied the features of hemorrheology in 13 cases of anterior uveitis,16 cases of pan-uveitis,and 15 cases of Behccedil;et' syndrome.The study showed that serous viscocity(at 230 s-1)in anterior uveitis patient group was higher than that of the control group,P<0.02; that blood viscocity at high shear rate(230 s-1)in pan-uneitis patient group was markedly elevated and the difference between patient and control group was sighificant,P<0.005; and that blood viscocity at both high(230 s-1)and low shear rate(5.75 s-1)in Behcet' syndrome grou was elevated,and the differences were significant as compared to the controls(P<0.01,P<0.05).the high blood viscocity in uveitis and Behcet' syndrome patients was analyzed and discussed in the paper.We thought that the treatment for uveitis and Behcet' syndrome should include lowering blood viscocity and improving the plasticity of red blood cells.   • Effect of Hemorheology on Ultrasonic Doppler Blood Flow Spectrum Diagram

    The present research aims to point out the long-existing defect of analyzing the spectrum diagram only from the perspective of haemodynamics instead of haemorheology. In the light of the theories of haemodynamics and haemorheology, the causes of spectrum diagram formation of carotid artery blood at the rapid and slow flow can be clarified completely and accurately. Four conclusions have been drawn in the end. As long as the velocity gradient is large enough, obvious red blood cells concentrate to the shaft even in the big or bigger blood vessels; the spectrum diagram is the powerful proof of the two phase flow model of blood; the spectrum diagram can be completely and accurately analyzed only by combining haemodynamics with haemorheology; and only when the red blood cells concentrate to the shaft, the big or bigger blood vessels can be regarded as haemogeneous fluid.

  • Research on Hemorheology in Rats with Acute Hyperuricemia

    Hyperuricemia is a risk factor for various diseases, but knowledge on acute hyperuricemia is still not sufficient. The present study was aimed to investigate the effect of acute hyperuricemia on red blood cells from hemorheological point of view, and to provide the reference for clinical treatment. The rats were gavaged with 500 mg/kg hypoxanthine and intraperitoneally injected with 100 mg/kg oxonate to induce the model of acute hyperuricemia. The same volume of blood samples were drawn within time period of 0, 1, 2, 3 and 6 h, respectively, from the inner canthus of rats to measure the serum uric acid, hemorheological parameters and the malondialdehyde level. It was found that in each period of 1, 2 and 3 h, the rats had significantly higher levels of uric acid. The integrated deformation index and relax index were increased. The hemolysis rate was significantly reduced. The plasma malondialdehyde level was obviously decreased at the end of 2 h. The results suggested that short-term elevated uric acid could improve the hemorheological parameters and the lipid oxidative level in red blood cells.

  • Influence of Fe2O3 nanoparticles on the rhological properties of simulated asthma airway mucus

    The properties of mucus in a person with asthma can alter with disease process so that it may lead to the airway embolism. Fe2O3 nanoparticles can be used for drug delivery. Up till now, however, little is known about how the Fe2O3 nanoparticles influence the properties of airway mucus. In this study, Fe2O3 nanoparticles were dispersed with ultrasound, and the morphological properties were measured with scanning electron microscope, atomic force microscope and nanometer laser particle size and zeta potential analyzer. Then the dispersed Fe2O3 nanoparticles were added to the simulated asthma airway mucus with different final concentration (0.03, 0.3, and 0.4 mg/mL). The measurements of flow curve, yield stress, large amplitude oscillatory shear (LAOS) and shock scanning were carried out with a rotational rheometer. Experimental results showed that the Fe2O3 nanoparticles reduced the zero shear viscosity of simulated asthma airway mucus. With increase of shear rate, the wind speed of mucus was reduced. The yield stress of simulated asthma airway mucus was 19.0 Pa, but the yield stresses of experimental group (0.03, 0.3 and 0.4 mg/mL) were 17.0, 0.99, and 0.7 Pa, respectively. The results showed that the viscoelastic modulus of asthma airway mucus treated with Fe2O3 nanoparticles were changed obviously as measured with large amplitude scanning and frequency scanning. By adopting the method of optical phase microscopy, we found that different structures of simulated airway mucus were absorbed. The results showed Fe2O3 nanoparticles distroyed mucus structure. The experimental results proved that Fe2O3 nanoparticles could change the rheological characteristics of simulated asthma airway mucus. This experimental result would lay a foundation for the further development of airway mucus sticky agent based on the function of Fe2O3 nanoparticles.
