  • Methodological Evaluation of Clinical Trials in Radioactive Iodine Therapy for Thyroglobulin Positive and Radioactive Iodine Negative Metastases

    Objective To evaluate the quality of controlled trials on 131I therapy for thyroglobulin positive and radioactive iodine negative metastases(131I WBS-/Tg+). Methods We electronically searched MEDLINE (1966 to Mar. 2004), EMBASE (1984 to 2003), The Cochrane Central Register of Controlled Trial, CENTRAL (Issue 2, 2004), CBMdisc (1978 to 2004) and CNKI (1994 to 2004), and handsearched 5 radiotherapy and endocrinology journals (Jan. 1980 to Apr. 2003). The methodological quality of included studies was assessed by using quality assessment criteria of the Cochrane systematic review guideline. Results Three non-randomized controlled trials were included. One was historical controlled trial, and two prospective trials. The sample sizes of three trials were 26, 60 and 70 respectively without mentioning the calculation base. The comparability of baseline was mentioned but not comparable across the trials. No double blind studies were used in the assessment of 131I efficacy. The time of follow-up varied from 2 to 15 years. None studies mentioned the side effects of 131I therapy for 131I WBS-/Tg+ metastases. Only two studies applied statistical methods properly. Conclusions Problems about allocation of groups, double blind, sample size, and follow-up period continue to exist in published controlled trials. RCT or prospective controlled trials of high quality are urgently needed in order to define the efficacy and safety of 131I therapy for 131I WBS-/Tg+ metastases.

  • Diagnosis Value of Serum Thyroglobulin in Patients with Thyroid Diseases: A Systematic Review

    摘要:目的:采用Meta分析的方法评价甲状腺球蛋白在甲状腺良性疾病和甲状腺癌中的临床意义。方法:通过检索MEDLINE、EMBASE、The Cochrane Library, 中国生物医学文献数据库、中国学术期刊全文数据库和其他方式广泛收集文献。根据QUADAS质量评价标准评价纳入文献的质量,用MetaDisc软件对其敏感度、特异度、阳性似然比、阴性似然比等进行合并分析,并进行异质性检验,绘制综合受试者工作特征曲线(summary receiver operator characteristic curve,SROC)。结果:最终纳入5篇文献。合并敏感度0.60,合并特异度0.83,合并比值比2.68, SROC下面积(AUC)=0.645 4。结论:现有研究证实:甲状腺球蛋白在甲状腺癌中的阳性率是甲状腺良性疾病中的2.68倍,有统计学差异,但敏感度不高。尚需更多设计严谨、科学的临床试验进一步证实。Abstract: Objective: To evaluate the quality of the current studies involving the value of serum thyroglobulin in the diagnosis of thyroid benign diseases and thyroid carcinoma. Methods: We comprehensive collected current studies about serum thyroglobulin in thyroid benign diseases and thyroid carcinoma by computer and manual searches. QUADAS items were used for quality assessment in our systematic review. Metadisc software was used to analyze pooled sensitivity, pooled specificity,pooled positive likelihood ratio and pooled negative likelihood ratio,pooled diagnostic test odds ratio and heterogeneity test,and draw summary receiver operator characteristic curve (SROC). Results: Totally 5 studies were included. To identify thyroid benign diseases and thyroid carcinoma, pooled sensitivity was 0.60, pooled specificity was 0.83,pooled odds ratio was 2.68, the area under curve (AUC) was 0.645 4.Conclusion: The results of statistic alanalysis showed that the positive rate of thyroglobulin in thyroid carcinoma is 2.68 times more than in benign thyroid diseases. There was significant difference. But sensitivity was not high and reporting quality of the studies was relatively poor. The conclusion still need more clinical trials to confirm.

  • Metastatic Pattern and Influencing Factors of Cervical Lymph Node in Papillary Thyroid Carcinoma

    Objective To explore the pattern and clinical influencing factors of cervical lymph node metastasis in papillary thyroid carcinoma (PTC), and provide a basis for the choice of surgical approach for the PTC neck lymph node processing. Methods The clinical data of 98 patients with PTC treated in Affiliated Hospital of Guiyang Medical College from Jan. 2009 to Dec. 2011 were collected, and the pattern and clinical influencing factors of cervical lymph node metastasis were analyzed. Results Ninety eight consecutive patients underwent neck dissection in a total of 114 sides. The lymph node metastasis rate of cervical lymph node, districtⅥ, districtⅡ+Ⅲ+Ⅳ, and districtⅤwas 77.55% (76/98), 74.49% (73/98), 42.86% (42/98), and 5.10% (5/98), respectively. Results of univariate analysis showed that lymph node metastasis rates were higher in patients with diameter of tumor greater than 1 cm, tumor invaded thyroid capsule, multi-focal tumor, and old than 45 years (P<0.05). Results of multivariate analysis showed that the age of patients, diameter of tumor, tumor invaded thyroid capsule, and multifocal tumor were independent risk factors of cervical lymph node metastasis (P<0.05). Tumor invaded thyroid capsule, multifocal tumor, combined with districtⅥmetastasis, and combined with districtⅡ+Ⅲ+Ⅳ metastasis were independent risk factors of prelaryngeal lymph node metastasis (P<0.05). Tumor invaded thyroid capsule and multifocal tumor were independent risk factors of skip lymph node metastasis (P<0.05). Conclusions DistrictⅥ is found to be the predominant site for lymph node metastasis of PTC, the districtⅢ and the districtⅣinvolved in addition, so it is necessary to clean lymph nodes at districtⅥ routinely. The regularity of cervical lymph node metastasis can provide the basis for surgeon to choose a reasonable type of neck dissection.

  • Relationship Between Methylation of MiR-34b Gene Promoter and Papillary Thyroid Carcinoma

    Objective To investigate the expression level and methylation level of micro RNA-34b(miR-34b) gene in papillary thyroid carcinoma (PTC), and to analyze the relationship between methylation and clinicopathological characters of PTC. Methods PTC tissues and tumor adjacent tissues were collected from 25 patients with PTC who underwent operation in Huai’an First People’s Hospital of Nanjing Medical University from Sep. 2008 to Oct. 2010. Expression of miR-34b gene and level of methylation in gene promoter were detected by real time PCR and methylation-specific PCR in the 2 kinds of tissues, respectively. Results The expression value of miR-34b mRNA in PTC tissues was 0.85±0.05, which was significantly lower than those of tumor adjacent tissues (1.62±0.09), P=0.030. There were methylation in 18 (72%,18/25) PTC tissues, and 10 (40%,10/25) in tumor adjacent tissues, and the ratio of methylation was higher in PTC tissues (P=0.021). In PTC tissues, methylation was not related to age, gender, tumor size, TNM stage, and invasion of the capsule (P>0.05), but was related to lymph node metastasis (P<0.05). Ratio of methylation in patients with lymph node metastasis was significantly higher than those of patients with no lymph node metastasis. Conclusion Methylation of miR-34b gene promoter is one of the reasons for inactivation of PTC, and it may be related to the development and metastasis of PTC, which needs to be further investigated.

  • The Applied Significance of Carbon Nanoparticles in Central Compartment Lymph Node Dissection in Treatment of cN0 Papillary Thyroid Carcinoma

    Objective To assess the applied significance of carbon nanoparticles in central compartment lymph node dissection in treatment of cN0 papillary thyroid carcinoma. Methods Sixty-eight patients with cN0 papillary thyroid carcinoma who were treated in Tongji Hospital of Tongji Medical College from May. to Oct. in 2012 were randomly allocated to the control group (n=32) and the carbon nanoparticles trace group (tracer group, n=36), receiving non-carbon nanoparticles trace and carbon nanoparticles trace respectively. All patients were received total resection of thyroid plus the affected side and (or) contralateral side central compartment lymph node dissection. The lymph node-related indexes(including number of dissected lymph node at Ⅵarea and lymph node metastasis rate at Ⅵarea) and operative indexs (including operation time, blood loss, drainage time, complication, and hospital stay) were collected and compared between the 2 groups. Results There were 205 and 324 dissected lymph node at central compartment in control group and tracer group respectively. The results of postoperative pathology showed that the number of lymph node in central compartment of the tracer group was much more than those of control group (8.99±2.24 vs. 6.41±1.56, P<0.001). The metastasis rate of central compartment lymph node were 40.6% (13/32) in control group and 47.2% (17/36) in tracer group, but there was no significant difference between the 2 groups (P=0.762). But in medial area of laryngeal recurrent nerve, the metastasis rate in the tracer group (38.9%, 14/36) was much higher than those of control group (12.5%, 4/32), P=0.029. There were no significant differences in the operation time, blood loss, drainage time, hospital stay, and complication incidence such as bleeding, temporary hypocalcemia, and injury of superior laryngeal nerve between 2 groups (P>0.05). All the patients in 2 groups had followed-up for 6 months without death, recurrence, and metastasis.Conclusions The lymphatic tracer technique of carbon nanoparticles may improve the number of dissected lymph nodes in central region of cN0 papillary thyroid carcinoma, without increasing (or prolonging) operation time, intraoperative blood loss, and postoperative hospital stay, and can accurately represent the metastasis of lymph node, thus to make the staging of the tumor accurately and guide postoperative treatment.

  • The Reoperative Techniques of Differentiated Thyroid Carcinoma

    Objective To explore the reoperative techniques of differentiated thyroid carcinoma. Methods Clinical data of 56 patients who treated in The First Affiliated Hospital of General Hospital of PLA and General Hospital of PLA from Feb. 2011 to Feb. 2013 were analyzed retrospectively. Results All performed surgeries were successful. Surgeries took 90-150 minutes with an average of 120 minutes. Bleeding during surgeries was 70-200 mL with an average of 120 mL. Postoperative drainage was 90-210 mL with an average of 100 mL. The pathological diagnosis of the second surgery in 44 cases were as the same as the first, but there were no malignant tumor tissues of dissected glands in 12 cases. All patients had no postoperative bleeding and bucking, but 8 patients experienced hand and foot numbness, and 5 patientsexperienced transient hoarseness. Fifty patients were followed-up for 6-30 months (average 10.8 months) from the reoper-taion and 18-66 months (average 45.2 months) from the first operation, and rate of postoperative followed-up was 89.3%(50/56). During the followed-up, 1 patient with papillary thyroid carcinoma and 1 patient with follicular thyroid carcinoma died in 44 months and 38 months respectively, 3 patients suffered lymph node metastasis at non-Ⅵ region ofaffected side, no one suffered recurrence. Conclusions For differentiated thyroid carcinoma patients who are undergoingthe second surgery, thorough whole body condition analysis should be performed and appropriate type of surgery should be chosen. By using recurrent laryngeal nerve monitoring, carbon nanoparticles for lymph node clearance, and protecting parathyroid gland to lower the possibility of postoperative complication, to improve survival rate and life quality.

  • Progress of Diagnosis and Treatment for Differentiated Thyroid Carcinoma in Pregnancy

    Objective To explore the progress of diagnosis and treatment for differentiated thyroid carcinoma (DTC) in pregnancy. Methods The literatures on studying the diagnosis and treatment of DTC in pregnancy were reviewed and analyzed retrospectively. Results Radionuclide scanning and radioiodine (131I) administration during pregnancy were contraindicated. Surgery during the second trimester was considered safe. Monitoring of pregnancy must be strict during each trimester if surgery was delayed until after delivery. Conclusions Pregnancy makes the diagnosis and treatment of DTC become more complicated,the diagnosis and treatment of DTC during pregnancy present a challenging situation for the endocrinologists,surgeon,and obstetrician.

  • Evaluation of Multi-Slice Spiral CT for The Papillary Thyroid Carcinoma

    Objective To explore the manifestations and features of multi-slice spiral CT (MSCT) in the diagnosisof papillary thyroid carcinoma (PTC). Methods Preoperative MSCT data of 35 cases of PTC proved by operation and pathology in our hospital form May. to Jun. in 2013 were observed retrospectively, to analyze the manifestations and characteristics of MSCT for it. Results Of 35 patients with PTC, MSCT totally showed 48 lesions, 68.6% (24/35) of patients with single lesion, 31.4% (11/35) of patients with 2-3 lesions, and 62.9% (22/35) of patients with lymph node metastasis. Of the 48 lesions, 29.2% (14/48) of lesions located in the left lobe, 70.8% (34/48) of lesions located in the right lobe;the lesions’ maximum diameter were 0.4-5.8cm, with the average maximum diameter of 1.3cm. There were 39.6% (19/48) of lesions with uneven density, 25.0% (12/48) of lesions with irregular shape, 47.9% (23/48) of lesions with blurred edges, 18.8% (9/48) of lesions had papillary enhanced tumor nodules, 10.4% (5/48) of lesions had peritumoral incomplete enhanced ring sign, 22.9% (11/48) of lesions invaded surrounding tissue or organs. There were 35.4% (17/48) of lesions had calcification, in which 76.4% (13/17) of lesions were fine granular calcification, 11.8% (2/17) of lesions were mixed calcification, and 11.8% (2/17) of lesions were coarse calcification. Conclusion MSCT manifestations of PTC have certain characteristics, which can provide imaging basis for clinical treatment options.

  • IgG4 Related Thyroid Diseases

    Objective To summarize the relationship between IgG4 and IgG4 related thyroid diseases. Methods Domestic and international publications involving the pathological features of IgG4-related thyroid diseases and relationship with IgG4 were retrieved and reviewed. Results IgG4-related disease was a newly recognized class of chronic and systemic lymphocytes disease, which may be solitary or involving multiple body organs, as well as thyroid. The expression of IgG4 was found in leisons of Hashimoto thyroiditis, Riedel thyroiditis, and papillary thyroid carcinoma. Conclusions IgG4-related thyroid disease is a new concept of thyroiditis. The knowledge of this new disease will provide appropriate treatment for patients with thyroiditis.

  • Discussion about Reasons of Reoperation of Papillary Thyroid Carcinoma

    Objective To discuss the resection extent of primary surgery for papillary thyroid carcinoma (PTC) based on the analysis of the remaining thyroid gland residue and lymph nodes metastasis. Methods The clinical data of 163 patients with PTC received reoperation from January 2009 to September 2011 in our hospital were analyzed retrospectively. Results There were 24 males and 139 females in these patients. The age was 10-75 years old with (38.22±14.57) years old. Among 131 patients received residual thyroid thyroidectomy,88 patients were below 45 years old,and the cancer residual rate was 60.23% (53/88);the others were over 45 years old,and the cancer residual rate was 76.74% (33/43),which was no significant difference (P?=?0.062).The lymph nodes metastasis rate was 77.14% (81/105) in the patients below 45 years old and 81.13%?(43/53) in the patients over 45 years old among 158 patients received cervical lymph nodes dissection,which was no significant difference (P?=?0.958).The cancer residual rate was 74.36%?(58/78),53.66%?(22/41),50.00%?(6/12),and 0 (0/32) in the patients with the tissue less than one lobe resection,lateral lobe (isthmus) resection,lateral lobe plus opposite side subtotal resection,total or subtotal resection,respectively. The total cancer residual rate was 65.65% (86/131) and the bilateral cancer residual rate was 52.76%?(86/163) after reoperation.79.75%?(130/163) of the patients didn’t received lymph nodes dissection in the primary surgery. The lymph nodes metastasis rate was 71.21%?(94/132) and 80.58% (83/103) in the reoperation patients received central region lymph nodes dissection and cervical lateral lymph nodes dissection, respectively. Conclusions  Multifocus is an obvious character of PTC. Patients over 45 years old have more opportunities of tumor residual than that of the youngers (below 45 years old),but the result was no statistic value. For the most patients with PTC,a smaller resection extent suggests a larger risk for cancer residual,and total thyroidectomy or subtotal thyroidectomy leads to the least possibility for the risk. Cervical lymph node is a major type for its metastasis. Proper lymph nodes dissection is an essential way to reduce recurrence and reoperation. The Revised American Thyroid Association Management Guidelines for Patients with Thyroid Nodules and Differentiated Thyroid Cancer (2009) could be a best choice for therapy of PTC. And it is proposed to take central region lymph nodes dissection during primary surgery.
