Objective To study the correlation between preoperative serum thyroid stimulating hormone (TSH) level and the malignant risk of thyroid nodules. Methods This study retrospectively analyzed the medical records of 959 patients with thyroid nodules. Ststistical analysis was conducted by SPSS 17.0 software. Results There were 959 patients with thyroid nodules, of which 746 cases were benign, and 213 cases were diagnosed as thyroid papillary carcinoma (PTC). The preoperative TSH level of PTC patients was higher than that with benign nodules [(2.32±1.65) mU/L vs. (1.76±1.20) mU/L, P<0.001]. Moreover, the higher preoperative TSH level was, the higher risk of diagnosed as PTC would be. There was no correlation between the preoperative TSH level and tumor diameter, number of lesions, and lymph nodes metastasis in PTC (P>0.05). Logistic regression analysis showed that, the preoperative TSH level was an independent risk factor for PTC [OR=1.315, 95% CI was (1.171, 1.477), P<0.001]. The best critical value of TSH in the PTC diagnosis was 1.575 mU/L. At this point, the sensitivity was 62.0%, the specificity was 53.4%, and the area under the receiver operating characteristic (ROC) curve was 0.602 (P<0.001). Conclusion There is a certain correlation between preoperative TSH level and malignant risk of thyroid nodules, and the risk increases with the raise of preoperative TSH level.
ObjectiveTo explore the feasibility strategy for patients’ fast track after the operation of thyroid papillary carcinoma with no drainage tube application.MethodsPatients undergone the operation of thyroid papillary carcinoma from June 2017 to March 2018 were enrolled in this retrospective study. All patients were from the same medical teams composed with the same attending doctors. They were divided into two groups according to the drainage tube applied or not. Finally the incidence of postoperative hematoma, incisional infection, and subcutaneous effusion were compared between the two groups, while the length of stay, pain score after operation, and satisfaction of patients were also analyzed.ResultsThe incidences of postoperative hematoma, incisional infection, and subcutaneous effusion were totally similar between the two groups. While the length of stay and pain score were significantly shorter or lower, satisfaction of patients were higher in group of no drainage tube applied (P<0.05).ConclusionsThe operation of total thyroidectomy plus bilateral central lymph node dissection for papillary thyroid cancer without drainage tube will not increase the probability of complications such as hematoma, incisional infection and subcutaneous effusion. On the contrary, it can shorten hospitalization time, reduce wound pain and improve patient satisfaction in the concept of rapid rehabilitation.
ObjectiveTo summarize the characteristics of cervical lymph node metastasis (LNM) in the papillary thyroid carcinoma (PTC) coexisting with Hashimoto thyroiditis (HT). MethodThe literatures related to cervical LNM of PTC coexisting with HT in recent years were collected and summarized. ResultsCompared with the PTC patients without HT, the more enlarged lymph nodes could be detected, and the cervival central LNM rate was lower, but there was still controversy about cervival lateral LNM in the patients with PTC coexisting with HT. The male, young, large tumor diameter, extraglandular invasion, multifocal cancer, BRAF gene mutation, and higher thyroid peroxidase antibody and thyroglobulin antibody levels, as well as the ultrasound features such as thyroid nodule aspect ratio >1, extremely low echo, calcification, and lymph node calcification, liquefaction, and disappearance of hilar lymph nodes could be used to evaluate the risk factors of cervical LNM for the patients with PTC coexisting with HT. ConclusionsFrom the results of this review, it is suggests that the rate of central LNM is lower in patients with PTC coexisting with HT, but the status of LNM in the cervical lateral region remains to be explored. The relevant risk factors in combination with ultrasonic characteristics could help evaluate cervical lymph node status, could provide basis for early detection of metastatic lymph nodes and the formulation of individualized surgical plans, and improve the prognosis of patients.
