  • Diagnostic Accuracy of 18F-FDG PET Dual Time Point Scan in Identifying Benign and Malignant Lung Lesions: A Meta-Analysis

    Objective To systematically review the diagnostic accuracy of 18F-FDG PET dual time point scan in identifying benign and malignant lung lesions, in order to necessity and clinical value of dual time point scan. Methods We electronically searched PubMed, EMbase, The Cochrane Library, WanFang Data, CNKI and CBM for diagnostic tests on 18F-FDG PET dual time point scan vs. surgery or needle biopsy (gold standard) from January 1990 to November 2013. Two reviewers independently screened literature according to the inclusion and exclusion criteria, extracted data, and assessed the methodological quality of included studies. Then statistical analysis was performed to calculate pooled effect sizes of sensitivity (SEN) and specificity (SPE), and area under the curve (AUC) of summary receiver operating characteristics (SROC), followed by sensitive analysis and subgroup analysis. Results A total of 19 domestic and foreign studies were totally included, involving 1 225 lesions. The results of meta-analysis showed SEN 0.82 (95%CI 0.79 to 0.85) and SPE 0.74 (95%CI 0.71 to 0.78) regarding 18F-FDG PET dual time point scan in identifying benign and malignant lung lesions. The results of sensitive analysis showed that: a) after eliminating studies in which tuberculosis in the benign lesions accounted for more than 50%, it showed pooled SEN 0.81 (95%CI 0.77 to 0.84), pooled SPE 0.76 (95%CI 0.72 to 0.80), and AUC 0.850 3; b) after eliminating studies in which sample size was less than 50 cases, it showed pooled SEN 0.78 (95%CI 0.74 to 0.82), pooled SPE 0.78 (95%CI 0.74 to 0.82), and AUC 0.814 1; and c) after eliminating studies in which iSUV was more than 2.5, it showed pooled SEN 0.67 (95%CI 0.55 to 0.78), pooled SPE 0.66 (95%CI 0.54 to 0.77), and AUC 0.779 8. Conclusion 18F-FDG PET dual time point scan has intermediate value in identifying benign and malignant lung lesions, which is almost as good as single time point scan, so it’s unnecessary to apply it as a clinical routine test.

  • Effectiveness and Safety of Fluoxetine for Premature Ejaculation: A Meta-Analysis

    Objective To systematically evaluate the effectiveness and safety of fluoxetine in treating premature ejaculation (PE). Methods All randomized controlled trials (RCTs) on fluoxetine treating PE published from July 1996 to May 2012 were collected in the following databases: MEDLINE, EMbase, PubMed, Ovid, The Cochrane Central Register of Controlled Trials, CBM and CKNI. According to the inclusion and exclusion criteria, literature screening, data extraction and quality assessment were conducted independently by two reviewers. Then meta-analysis was performed using RevMan 5.0 software. Results A total of 6 RCTs involving 221 patients were included finally. The results of meta-analysis showed that, as for effectiveness, there was no significant difference in the intravaginal ejaculatory latency time (IELT) between the two groups before the treatment (WMD=–0.21, 95%CI −4.79 to 4.37, P=0.93), but the IELT of the fluoxetine group was obviously longer than that of the control group after the treatment, with a significant difference (WMD=134.54, 95%CI 79.78 to 189.30, Plt;0.000 01). The results of sensitivity analysis indicated that the IELT of the fluoxetine group was longer than that of the control group, with a significant difference (WMD=155.19, 95%CI 130.64 to 179.75, Plt;0.000 01). As for safety, the fluoxetine group was higher in the incidence of adverse reaction than the control group, with a significant difference (OR=5.49, 95%CI 2.43 to 12.38, Plt;0.000 1). Conclusion Current evidence indicates that fluoxetine can improve the symptoms of PE patients, obviously prolong the IELT, and improve the quality of sexual life; and it is tolerable to patients with mild adverse reactions and is suitable for long-term intake. For the limited quantity of the included studies, we herein believe that, to obtain more evidence, it is necessary to further confirm the diagnosis and therapeutic criteria of PE, to design and conduct more multicenter and large scale clinical studies by adopting the internationally recognized indexes, and to perform a long-term follow-up.

  • Identify Micrometastasis in Lymph Nodes of Gastric Cancer Use qRT-PCR Assay and Evaluate The Clinical Significance of Micrometastasis

    Objective To identify micrometastasis in regional lymph nodes of gastric cancer by quantitative real-time reverse transcription-PCR (qRT-PCR) assay and to evaluate the clinical significance of micrometastasis. Methods To study 320 lymph nodes collected from January 2010 to June 2010, 281 of which were from 40 patients with gastric cancer who had undergone a standard gastrectomy with lymphadenectomy, and other 39 of which were from 10 patients with gastroduodenal ulcer. Made CEA, CK-19, and CK-20 as primers, and used qRT-PCR assay in addition to hematoxylin and eosin staining to detect the micrometastasis, and to analyze the clinicopathologic characteristics.Results Totally, micrometastasis were detected by qRT-PCR assay in 31 (15.34%,31/202) lymph nodes of 28 (70.00%, 28/40) patients. Thirty-nine lymph nodes from 10 patients with gastroduodenal ulcer were negative by qRT-PCR and HE staining. The degree of differentiation, depth of gastric mural invasion, and clinical stage had statistically significant correlation with the incidence of lymph node micrometastasis (P<0.05). Conclusions qRT-PCR assay is a sensitive and specific method to detect lymph node micrometastasis in gastric cancer patients,and it has importantly clinical significance in evaluating clinical staging,prognosis and treatment prescription.

  • Clinical Significance of Screening Serum Protein Fingerprint in Gastric Cancer

    Objective To detect the serum protein fingerprint in gastric cancer patients by using the surface-enhanced laser desorption-ionization time-of-flight mass spectrometry (SELDI-TOF-MS) and protein chip array technology, screen biomarker candites, build diagnostic models and evaluate its clinical significance. Methods The serum proteomic patterns were detected in 40 patients with gastric cancer, 20 patients with gastric ulcer and 20 healthy blood donors. The diagnostic models were developed and valited by discriminant analysis. Results The peak intensity of differential expression proteins was not found in healthy blood donors, and 1 case was found in patient with gastric ulcer (m/z: 5 910,4 095). The peak intensity of 5 329, 4 095, 5 910, 8 691 and 3 300 (m/z) proteins were significantly higher in 40 gastric cancer patients than those in 20 gastric ulcer patients and 20 healthy blood donors ( P <0.05). Three differential expression proteins were set up a diagnostic model together to diagnose gastric cancer. The diagnostic model made up of the differential expression proteins of 4 095, 5 910 and 8 691 had a sensitivity of 92.5% and a specificity of 97.5% . Conclusion Using SELDI-TOF-MS shows great potential to detect, and screen novel and better biomarkers for gastric cancer.

  • SYNCHRONOUS RESECTION OF BOTH COLONIC CARCINOMA AND ITS HEPATIC METASTASIS (REPORT OF 1 CASE)

    Objective To investigate the feasibility and indication of synchronous resection of colonic carcinoma and its hepatic metastasis. Methods Radical sigmoidectomy and right hemi-hepatectomy plus left lateral segment resection were performed at the same time in a 71-year-old patient with sigmoid carcinoma and multiple hepatic metastasis. Results The operation lasted for 5 hours and 10 minutes with 300ml blood lost during the procedure. The patient recovered smoothly and was discharged 2 weeks after operation. Follow-up showed no reoccurrence up to the day of this presentation(4 months).Conclusion The operation could be performed safely by experienced surgeon in good-equipment hospital.

  • Gene expression of β-defensin-4 and β-defensin-6 in lung tissue of acute lung injury mouse

    Objective To investigate the gene expression of beta-defensin-4 (mBD-4) and mBD-6 in acute lung injury (ALI) mouse.Methods Sixty adult mice were randomly divided into a control group and a ALI group.ALI was induced by intraperitoneal injection of lipopolysaccharide (LPS) in the ALI group.The control group was treated with same dose of normal saline.The lung tissues were harvested at different time point after stimulation.The expression of mBD-4 and mBD-6 mRNA was measured by real-time quantitative reverse transcription polymerase chain reaction.DNA sequencing was used to confirm the specificity of mBD-4 and mBD-6 cDNA fragment.Results There were no obvious mBD-4 and mBD-6 mRNA expression in mouse lung in the control group at all time points and ALI 6 h group.In the ALI group a marked increasing expression was found on 12 h,1 d and 3 d after LPS stimulation.The mBD-4 mRNA expression was significant higher in the ALI groups of 1 d and 3 d points than that of ALI 12 h group with no obvious difference between each other.There were no significant differences of mBD-6 mRNA expression between ALI groups of 12 h,1 d and 3 d points Conclusion mBD-4 and mBD-6 mRNA is not constitutive expressed in mouse lung and show a up-regulative expression pattern after ALI.

  • Relationships Between Health-Related Quality of Life and Social Support in Patients with Obstructive Sleep Apnea-Hypopnea Syndrome

    Objective To study the relationships among health-related quality of life( HRQL) ,social support, excessive daytime sleepiness ( EDS) and PSG parameters in patients with obstructive sleep apnea-hypopnea syndrome ( OSAHS) . Methods Eighty-five patients were recruited who were diagnosed as OSAHS by overnight polysomnography from August 2007 through November 2007 in West China Hospital.The Calgary sleep apnea quality of life index ( SAQLI) was used for HRQL, social support rating scale ( SSRS) was used for social support, and Epworth sleepiness scale( ESS) was used for EDS. The Pearson linear correlation and stepwise multiple regression analysis were used to analyze the correlation among SAQLI, SSRS, ESS, and PSG. Results The SAQLI was correlated with SSRS score ( r =0. 402, P lt;0. 01) ;ESS score ( r = - 0. 505, P lt;0. 01) ; apnea-hypopnea index ( AHI) ( r = - 0. 269, P lt; 0. 05) and lowest artery oxygen saturation ( LSaO2) ( r = 0. 226, P lt; 0. 05) . Stepwise multiple regression analysis determined two variables, the SSRS and ESS score, as independent factors for predicting the total score of SAQLI which accounted for 37. 3% of the total variance in the total score on SAQLI ( R2 = 0. 373, P lt; 0.001) .Conclusions The HRQL of patients with OSAHS was correlated with the SSRS score, ESS score and PSG parameters. The former two were the more important factors to affect the HRQL of patients with OSAHS.

  • Effect of Different Inspiratory Rise Time on Work of Breathing in Patients with Acute Exacerbation of COPD during Noninvasive Positive Pressure Ventilation

    Objective To investigate the effects of different inspiratory rise time during noninvasive positive pressure ventilation ( NPPV) on work of breathing in patients with acute exacerbation of chronic obstructive pulmonary disease ( COPD) . Methods Eleven patients with acute exacerbation of COPD received different inspiratory rise time ( 0. 1sec, 0. 3sec, 0. 5sec) during NPPV. The changes of inspiratory muscle effort and breathing pattern of the patients were observed. Results The average respiratory rate,minute ventilation, and tidal volume were higher during NPPV compared with spontaneous breathing. But the changes of average minute ventilation and tidal volume were not significant ( P gt; 0. 05) . The pressure time product ( PTP) , transdiaphragmatic pressure ( Pdi) , and work of breathing of inspiratory muscle reduced significantly during different inspiratory rise time as compared with spontaneous breathing ( P lt;0. 01) . PTP,Pdi, and work of breathing reduced 59. 2% , 62. 7% , and 49% respectively when inspiratory rise time was 0. 1sec. They reduced more significantly during inspiratory rise time of 0. 1sec. Conclusions The present study confirms NPPV can unload inspiratory muscles in patients with acute exacerbation of COPD. It is more effective to reduce inspiratory load when inspiratory rise time is set at 0. 1sec while the patients feel most comfortable.

  • Intraoperative Transit Time Flow Measurement during Sequential Coronary Artery Bypass Grafting

    Objective To investigate the use of intraoperative transit time flow measurement (TTFM) to accuratelyevaluate graft patency during sequential coronary artery bypass grafting (CABG). Methods Clinical data of 131 patientsundergoing sequential off-pump coronary artery bypass grafting (OPCAB) with the great saphenous vein (with or without internal mammary artery) as graft vessels in Beijing Anzhen Hospital from April 2012 to January 2013 were retrospectivelyanalyzed. There were 92 male and 39 female patients with their age of 61.35±8.24 years. During the operation,2 methods were used to measure mean blood flow volume,pulsatility index (PI) and diastolic filling (DF) of the graft vessels. For thenon-blocking method,blood flow in graft vessels was maintained,and TTFM was applied 2 cm proximal to the anastomoticsite in graft vessels to record above parameters. For the blocking method,blood flow in graft vessels was temporally blockedby clipping distal graft vessels with an atraumatic bulldog clamp,and TTFM was applied 2 cm proximal to the anastomotic site in graft vessels to record above parameters. Results Blood flow volumes of the diagonal branch (27.43±15.22 ml/minvs. 59.28±30.13 ml/min),obtuse marginal branch (26.14±19.74 ml/min vs. 47.19±24.27 ml/min) and posterior left ventr-icular branch (19.16±8.92 ml/min vs. 38.83±20.11 ml/min) measured by the blocking method were significantly smallerthan those measured by the non-blocking method (P<0.05) . PI values of the diagonal branch (2.93±1.30 vs. 2.31±0.91),obtuse marginal branch (2.62±1.17 vs. 2.01±0.87) and posterior left ventricular branch (2.33±0.92 vs. 1.80±0.73) meas-ured by the blocking method were significantly higher than those measured by the non-blocking method (P<0.05). There was no statistical difference in DF values measured by 2 methods at respective anastomotic sites,and all DF values were higher than 50%. Non-blocking method identified 1 anastomotic site and blocking method identified 3 anastomotic sites that were not patent,and these anastomotic sites became patent after graft reconstruction. Conclusion Blood flow velocity of sequential grafts is higher than that of single grafts,indicating that sequential grafts have the advantages of high blood flow volume and low risk of thrombosis. Blocking method can more accurately evaluate graft patency at the anastomotic sites and sensitively identify graft vessels that are not patent,which is helpful for anastomotic site reconstruction in time and enhancement of successful rate of CABG.

  • Real-time Three Dimensional Echocardiography Guided Closure of Atrial Septal Defect through a RightMinithoracotomy in Comparison with Traditional Surgical Repair under Cardiopulmonary Bypass

    Objective To compare surgical results between real-time three dimensional echocardiography(RT-3DE) guided closure of atrial septal defect (ASD) through a right minithoracotomy and traditional surgical repair under cardiopulmonary bypass (CPB). Methods Sixty-four patients with secundum ASD received surgical repair in the First People’s Hospital of Honghe Autonomous Prefecture from April 2009 to April 2012. According to different surgical approach, all the patients were divided into group A and B. In group A, 35 patients underwent traditional ASD repair under CPB including 20males and 15 females with their age of 12-56 (16.4±4.0) years. In group B, 29 patients received real-time RT-3DE guidedASD closure through a right minithoracotomy without CPB, including 20 males and 15 females with their age of 15-50 (18.5±0.2) years. Operation time,postoperative mechanical ventilation time,hospital stay,chest drainage,mortality,morbidity and follow-up outcomes were compared between the 2 groups. Results Operation time (110.47±35.90 minutesvs. 159.32±20.60 minutes),postoperative mechanical ventilation time (10.40±22.30 hours vs. 16.40±12.20 hours),chestdrainage (106.71±85.20 ml vs. 146.70±75.63 ml)and postoperative hospital stay (4.0±1.0 days vs. 7.0±1.0 days)ofgroup B were significantly shorter or less than those of group A. In group A, 1 patient died postoperatively and 7 patientshad postoperative complications. In group B, there was no in-hospital mortality and 3 patients had postoperative complications.Postoperative morbidity of group A was significantly higher than that of group B (20.0% vs. 10.3%,P<0.05) . ConclusionFor ASD patients with definite surgical indications,RT-3DE guided ASD closure through a right minithoracotomy has more advantages over traditional surgical repair under CBP.
