  • Application of Remifentanil Combined with Propofol and Remifentanil Combined with Etomidate for General Anesthesia During Gynecologic Laparoscopy

    目的:比较瑞芬太尼联合异丙酚或依托咪酯全麻在腹腔镜妇科手术中的临床效果。方法:择期腹腔镜妇科手术80例,随机分成瑞芬太尼异丙酚组(RP组)和瑞芬太尼依托咪酯组(RE组),各40例。两组全麻诱导用药相同,维持麻醉RP组采用瑞芬太尼联合异丙酚、RE组采用瑞芬太尼联合依托咪酯。记录基础值、诱导后、插管后1 min、3 min、气腹时、气腹后10、20、30、40 min的动脉收缩压(SBP)、舒张压(DBP)、心率(HR);记录停药至自主呼吸恢复、睁眼、拔除气管导管、恢复定向能力的时间;记录清醒即刻及清醒后1、2、4、8、12、16、20、24 h患者疼痛程度,采用VAS评分;记录24 h内不良反应发生情况。 结果: 两组SBP、DBP均在诱导后明显低于基础值(Plt;001), 插管后恢复,气腹开始后趋于平稳;两组HR均在诱导后减慢(Plt;001),插管后及气腹开始时恢复。RP组自主呼吸恢复、呼之能睁眼、拔除气管导管及恢复定向能力的时间均明显短于RE组(Plt;001)。麻醉清醒即刻、清醒后1、2、4、8 h VAS评分RE组明显低于RP组(Plt;005),12、16、20、24 h VAS评分两组比较无显著性差异(Pgt;005)。术后发生恶心呕吐患者数RP组明显减少(Plt;005)。 结论:全麻行腹腔镜妇科手术时,瑞芬太尼联合异丙酚或依托咪酯都能缓解气腹及手术引起的血流动力学变化,瑞芬太尼联合异丙酚术后苏醒快且能明显降低术后恶心呕吐的发生率,但术后疼痛较为严重。

  • Etomidate Combined with Sufentanil for Laryngeal Mask Airway Insertion under the General Anaesthesia

    【摘要】 目的 观察依托咪酯乳剂复合舒芬太尼用于全麻下喉罩置入的血流动力学变化。 方法 选择2009年4月-2010年2月间,46例需全麻手术、适合使用喉罩,美国麻醉医师协会(ASA)Ⅰ~Ⅱ级,年龄18~60岁的患者,随机分为两组:依托咪酯乳剂组(E组)23 例,静脉推注咪达唑仑0.05 mg/kg,依托咪酯乳剂0.3 mg/kg;依托咪酯乳剂+舒芬太尼组(ES组)23 例,静脉推注咪达唑仑0.05 mg/kg,依托咪酯乳剂0.15 mg/kg,加舒芬太尼0.5 mg/kg,诱导后置入喉罩,记录患者诱导前、用药后1 min、喉罩置入后1 min的心率(HR)、平均动脉压(MAP)以及评估喉罩置入条件的6项指标(张口困难分级、置入喉罩困难分级、舌咽反射、干咳干呕反射、肢动反应及喉痉挛分级),同时记录呼吸暂停时间。 结果 ES组能提供更好的喉罩置入条件,且能减少舌咽反射和肢体反应, 更能保证喉罩置入时血流动力学的稳定。 结论 依托咪酯乳剂复合舒芬太尼能为全麻喉罩置入时提供更好的条件,且能保证更好的血流动力学稳定。【Abstract】 Objective To investigate the hemodynamics changes when etomidate combined with sufentanil was applied for laryngeal mask airway insertion under the general anaesthesia. Methods From April 2009 to February 2010, 46 patients requiring general anesthesia using laryngeal mask airway (LMA) (American Society of Anesthesiologists (ASA)Ⅰ-Ⅱ) aged 18-60 were randomly divided into two groups: 23 in etomidate emulsion group (group E) underwent the intravenous injection with midazolm (0.3 mg/kg) and etomidate (0.05 mg/kg); 23 in etomidate emulsion + sufentanil group (group ES) underwent the intravenous injection with etomidate (0.15 mg/kg), midazolm (0.05 mg/kg), and sufentanil 0.5 mg/kg. The patients were evaluated by six indexes of LMA insertion (mouth opening, swallowing reflex, cough reflex,vomiting reflex, body motion, and laryngospasm classification). After the anesthesia induction, LMA was inserted. The blood pressure (BP), heart rate (HR), and mean arterial pressure (MAP) were recorded before anesthesia induction one minute after the injection and one minute after LMA insertion. Meanwhile, the apnea time was recorded. Results Compared with group E, group ES offered better anesthesia for LMA insertion, less swallowing reflex and body motion, and more stable haemodynamics. Conclusion Etomidate combined with sufentanil provides good condition for LMA insertion under the general anaesthesia with steady haemodynamics.

  • Efficacy of Pretreatment of Vecuronium Combined with Dilution of Etomidate on Etomidateinduced Myoclonus

    目的:旨在评价预注维库溴铵联合稀释依托咪酯减轻依托咪酯全麻诱导中肌阵挛的效果。方法:本研究为前瞻性、双盲、随机对照研究。80名ASA I-II级、年龄18~60岁、拟行胆囊切除术的全麻患者被随机分为4组:组1,预注维库溴铵0.01 mg/kg;组2,依托咪酯用生理盐水由2 mg/mL 稀释为1 mg/mL;组3,预注维库溴铵联合稀释依托咪酯;组4,生理盐水对照组+非稀释依托咪酯组。根据分组给予患者预注维库溴铵0.01 mg/kg或同等量的生理盐水,观察并询问患者有无呼吸困难,并记录呼吸频率和脉搏氧饱和度(SpO2)。3分钟后推注稀释或无稀释依托咪酯0.3 mg/kg,询问患者有无注射痛并做疼痛评分,观察有无肌阵挛并评估其程度。2分钟后给予维库溴铵0.1 mg/kg、芬太尼3 μg/kg和丙泊酚1 mg/kg行气管插管。实验期间同时记录无创动脉血压(BP)和心率(HR)。结果:组1和组4分别有2例(10%)和3例(15%)患者述轻度注射疼痛,而组2和组3无患者述注射疼痛。组1、组2和组3肌阵挛的发生率明显低于组4(30%、40%和15% vs 70%,Plt;0.05)。且组1、组2和组3肌阵挛的程度多为轻中度,而组4多为中重度。四组患者均未述任何呼吸困难,呼吸频率无明显降低,SpO2无明显变化。四组患者BP和HR变化一致,无明显差别。结论:预注小剂量维库溴铵或稀释依托咪酯可明显降低依托咪酯引起肌阵挛的发生率并减轻其程度。且这两种方法联合应用比单独应用效果更佳,具有一定程度的协同作用。

  • The Association of BIS and HRV in TargetControlled Infusion of Etomidate Used in Fasttracking Anesthesia of the Elderly

    目的:研究老年患者依托咪酯靶控输注时不同BIS值(脑电双频指数)的HRV(心率变异性)的变化情况,探讨不同镇静深度与HRV之间的关系。方法:选择65岁以上行门诊胃镜检查患者30例,随机分为3组,A组BIS45~55,B组55~65,C组65~75,各组均在麻醉前、麻醉诱导后,术中、术毕监测BIS、HRV及血液动力学指标。结果:A组各监测HRV明显降低(Plt;0.05),B组仅有轻度下降(Pgt;0.05),C组明显升高(Plt;0.05)。结论:患者镇静深度BIS55~65时,即可明显抑制内镜操作刺激所致的HRV变化,是临床较为合适的镇静深度,可显著降低老年患者交感神经活性、交感/迷走神经均衡性和自主神经总张力,利于机体血液动力学稳定。

  • A Clinical Trial of Etomidate Continous Infusion Used in General Anesthesia Induction and Maintenance

    目的:观察依托咪酯持续输注用于麻醉诱导和维持对肾上腺皮质功能的抑制程度以及评价是否会造成严重不良后果。方法:择期行普外手术患者60例,随机分成依托咪酯组(E组)和异丙酚组(P组)。两组患者常规麻醉诱导后,E组和P组分别采用瑞芬太尼0.1~0.4 μg/kg·min,复合依托咪酯或异丙酚维持,据BIS值调整依托咪酯或异丙酚的输注速率。分别测定麻醉前,手术结束时,术后24 h,术后48 h患者血清皮质醇浓度,记录各组的血压,心率,睁眼时间,拔管时间和术后不良反应。结果:与麻醉前比较,E组和P组患者在手术结束时皮质醇浓度均降低,但E组降低更明显(Plt;0.05),48 h后恢复正常甚至高于麻醉前水平;与诱导前相比,异丙酚组患者诱导后平均动脉压(MAP)明显低于诱导前,而依托咪酯组无明显变化;E组的拔管时间比P组长(Plt;0.05),而睁眼时间无明显差异(Pgt;0.05).结论:1依托咪酯和异丙酚均能抑制患者肾上腺皮质功能,且依托咪酯组更为明显,在术后48 h基本恢复正常,甚至高于术前水平。2依托咪酯在麻醉诱导和维持中比异丙酚具有血流动力学稳定性。3依托咪酯持续输注用于全身麻醉维持并不造成围术期严重不良后果。

  • Application of Dexmedetomidine Combined with Etomidate for Elderly Patient Undergoing thyroidectomy

    ObjectiveTo observe the effect of dexmedetomidine combined with etomidate on the clinical safety and the tracheal extubation response after general anesthesⅠa in elderly patient undergoing thyroidectomy. MethodsFifty patients (aged between 65 and 75 years, ASAⅠor Ⅱ) scheduled for thyroid surgery between July 2012 and January 2013 were randomly divided into two groups:dexmedetomidine group (group D) and control group (group C) with 25 patients in each group. Group D received dexmedetomidine of 0.5 μg/(kg·h) through intravenous infusion after anesthesia induction, and the intravenous infusion was stopped five minutes before the end of surgery. Normal saline was infused at the same volume in group C at the same time. Patients were induced with etomidate at 0.2 mg/kg for anesthesia, and etomidate and remifentanil were used for the anesthesia maintenance during the operation. Heart rate (HR), systolic blood pressure SBP), diastolic blood pressure (DBP) and bispectral index (BIS) were recorded 5 (T1), 15 (T2) and 30 (T3) minutes after the beginning of the operation, and 15 (T4) and 5 (T5) minutes before the end of the operation. Moreover, the time of eye opening, time of extubation, the number of patients with restlessness and etomidate requirement were recorded. ResultsCompared with group C, HR and MAP at the time points of T2 and T5 in group D did not obviously change, but the number of restlessness patients in group D was significantly less than in group C (P<0.05). There was no statistically significant difference in time of eye opening and time of extubation between the two groups (P>0.05). Etomidate requirement in group D was[(8.6±2.1) μg/(kg·min)], which was significantly lower than that in group C[(14.4±3.4) μg/(kg·min)] (P<0.05). ConclusionDexmedetomidine combined with etomidate is efficient and safe for elderly patients undergoing thyroidectomy, and this method can effectively reduce cardiovascular responses to tracheal extubation, decrease the incidence of postoperative restlessness, and reduce the requirement of etomidate during the operation.

  • Effects of Propofol, Etomidate and Ethanol on GPCR mRNA Expression in Daphnia pulex

    The mechanisms of general anesthesia, which was introduced about 170 years ago, remain poorly understood. Even less well understood are the effects of general anesthesia on the human body. Recently we identified 18 G-protein coupled receptor (GPCR) genes of Daphnia pulex, an invertebrate model organism. Phylogenetic analysis identified these genes to be the homologs of the human γ-aminobutyric acid, type B (GABAB) receptor, metabotropic glutamate receptors (mGluR), adrenergic receptor, serotonin (5-HT) receptor, dopamine receptor and muscarinic acetylcholine receptor (mAChR). Using reverse transcription and quantitative PCR techniques, we systematically measured the effects of propofol, etomidate and ethanol on these 18 GPCR mRNA expressions in Daphnia pulex.

  • Effects of Etomidate on mRNA Expression of Ion Channels in Daphnia Pulex

    Ion channels are involved in the mechanism of anesthetic action and side effect. The transcription and expression of ion channel genes can be modulated by general anesthetics. The adverse effect of continuous infusion of etomidate has been concerned. However, the effects of etomidate on mRNA expressions of ion channel genes remain unclear. In this study, we exposed Daphnia pulex in 250 μmol/L of etomidate for 240 min and observed the change of heart rate, phototactic behavior and blood glucose during the period of exposure, as well as the mRNA expressions of 120 ion channel genes at the end of the experiment. Compared to the controls, heart rate, phototactic behavior and blood glucose were not influenced by 250 μmol/L of etomidate. According to the quantitative PCR results, 18 of 120 Daphnia pulex ion channel genes transcripts were affected by persistent 240 min exposure to 250 μmol/L of etomidate: 2 genes were upregulated and 16 genes were down-regulated, suggesting that etomidate showed effects on many different ion channels in transcription level. Systematical exploration of transcriptional changes of ion channels could contribute to understanding of the pharmacological mechanism of etomidate.

  • Efficacy and Safety of Midazolam in the Prevention of Etomidate-induced Myoclonus: A Meta-analysis

    ObjectiveTo evaluate the efficacy and safety of midazolam in the prevention of etomidate-induced myoclonus. MethodsDatabases including PubMed, EMbase, The Cochrane Library (Issue 8, 2015), CBM, WanFang Data, VIP, and CNKI were electronically searched to collect randomized controlled trials (RCTs) about midazolam in the prevention of etomidate-induced myoclonus from inception to August, 2015. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies. Then, meta-analysis was performed by RevMan 5.2 and Stata 12.0 softwares. ResultsA total of 14 RCTs involving 1 274 patients were included. The results of metaanalysis showed that, compared with placebo, pretreatment with midazolam injection could reduce the incidence of myoclonus (RR=0.28, 95%CI 0.19 to 0.42, P<0.000 01). The sub-group analysis based on different doses of midazolam showed that all three different doses of midazolam (0.015 mg/kg, 0.03 mg/kg and 0.05-0.1 mg/kg) could reduce the incidence of myoclonus effectively (all P values <0.05). ConclusionPretreatment with midazolam injection can reduce the incidence of etomidate-induced myoclonus without increasing the risk of recovery latency and over sedation. Due to the limited quality of included studies, the above conclusion needs to be further verified by more high quality studies.
