  • Study on the reengineering application of tumor radiotherapy process in a tertiary hospital

    ObjectiveGiven the relatively limited resources available for tumor radiotherapy, the reengineering theory to the tumor radiotherapy process of a tertiary hospital is applied to improve the efficiency of medical service, shorten patient waiting time and improve patient satisfaction. MethodsThe tumor radiotherapy process of a tertiary hospital was studied from January 2017 to September 2018. The indicators such as efficiency and satisfaction were analyzed before reconstruction (from January to December 2017) and after reconstruction (from January to September 2018). ResultsAfter radiotherapy process reengineering, on the one hand, the medical efficiency was improved: the number of new patients for radiotherapy per month rose by 16.58% (P<0.05), and the number of daily radiotherapy increased by 5.80% (P<0.05). On the other hand, the patient treatment process became more concise: the preparation time was shortened from 2-3 days to 1 day, while the waiting time for radiotherapy was shortened by nearly 10 days, and the overall satisfaction of patients increased from 64.17% to 83.55%. ConclusionIt can improve the operation efficiency of tumor radiotherapy and improve patient satisfaction under the condition of relatively fixed resources through the reconstruction of the information-based tumor radiotherapy process.

  • A comparative study of evaluation indicators of different clinical departments before and after the reform of diagnosis-related group payment method under total amount control

    Objective To explore the impact of diagnosis-related group (DRG) payment method reform under total amount control on neurology and neurosurgery departments. Methods The DRG grouping data of the Department of Neurology and the Department of Neurosurgery of Panzhihua Central Hospital from January 2018 to December 2020 were collected, and the mature DRG evaluation indexes in China were selected. Using the interrupt time series analysis method, the DRG-related indexes of the two departments before and after the introduction of the performance appraisal plan in July 2019 were compared, to evaluate the intervention effects on the two departments. Results Both neurology and neurosurgery departments showed a slow downward trend in the overall medical service capacity under the DRG payment. The efficiency of medical services showed a slow upward trend and the consumption of medical expenses showed a slow downward trend in the Department of Neurology, while the efficiency of medical services showed a slow downward trend and the consumption of medical expenses showed a slow upward trend in the Department of Neurosurgery. According to the results of interrupt time series analysis, in the Department of Neurosurgery, the total weight showed a significant downward trend before intervention (β1=−5.526, P=0.003), and the downward trend became sluggish after intervention, with a statistically significant slope difference before and after intervention (β3=4.546, P=0.047); the case-mix index showed a downward trend before intervention (β1=−0.050, P<0.001), and no obvious trend after intervention, with a statistically significant slope difference before and after intervention (β3=0.052, P=0.001); the cost consumption index showed no obvious downward trend before intervention (β1=−0.006, P=0.258), and an upward trend after intervention, with a statistically significant slope difference before and after intervention (β3=0.027, P=0.032). The impact of this assessment plan on the Department of Neurology was not statistically significant (P>0.05), needing further observation. Conclusions The reform of DRG payment method under total amount control has different effects on the evaluation indicators of clinical departments of different natures. It is recommended to implement classified management and assessment for clinical departments of different natures.

  • Analysis on Satisfaction Degree for MBA Program of Hospital Management in Sichuan University and Its Enlightenment

    【摘要】 目的 调查四川大学医院管理MBA项目的学员满意度,分析其影响因素,寻求相关启示,为现代医院管理者决策提供参考依据。 方法 以2006年-2010年四川大学医院管理MBA项目学员为研究对象,采用自制的调查问卷,对参与培训的336名学员进行统一的问卷调查,对结果采用logistic回归分析。 结果 发放问卷336份,回收有效问卷320份,有效回收率95%。86.2%的学员表示对培训项目的效果满意。课程内容的实践性、培训组织管理模式、是否有丰富的个案分析等因素与学员满意度有关(Plt;0.05)。 结论 要做好医院管理MBA的培训项目,需要关注影响学员满意度的因素,需在课程内容设计、培训模式改进、案例资料库的甄选等方面努力,从而设计更符合现代医院管理需要的MBA培训课程。【Abstract】 Objective To investigate the satisfaction degree of students in the MBA programs of Sichuan university, analyze the influential factors for the satisfaction degree. Methods Self-made questionnaire was applied in the investigation on 336 students who attended the MBA program of Sichuan University from 2006 to 2010. The investigation results were collected and analyzed by using logistic regression analysis. Results A total of 320 (89.65%) valid questionnaires were retrieved among all the 336 questionnaires assigned. In all the students, 86.2% were contented with the current situation of scientific training program. The content of the course practice, management of training mode and whether there was a rich case analysis were influential factors for students′ satisfaction degree (Plt;0.05). Conclusions The influential factors for students’ satisfaction degree should be taken into consideration in order to better carry out the MBA hospital management training program. More emphasis should be paid on the course design, training mode adjustment and selection of cases, in order to cater for modern hospital managers.

  • Exploration and construction of “West China Hospital-Mabian model” of targeted health poverty alleviation in ethnic minority areas

    This paper analyzes the health status and predicament of Mabian Yi Autonomous County before accepting assistance from West China Hospital of Sichuan University in 2016, and expounds the measures and achievements of West China Hospital of Sichuan University in building a “West China Hospital-Mabian model” of precision health poverty alleviation combining its own advantages and oriented by the needs of health poverty alleviation of Mabian Yi Autonomous County. The main contents of the model are putting an emphasis on the application and promotion of appropriate technologies, paying equal attention to technical and management help, combining “blood transfusion” and “hematopoiesis”, and taking the construction of a chimeric medical association as the carrier. The “West China Hospital-Mabian model” of precision health poverty alleviation provides a reference for continuously and systematically improving the technical level, service capacity and management level of grassroots hospitals in ethnic minority areas.

  • Analysis of 875 Procedures of Earthquake Related Patients in West China Hospital of Sichuan University after the Wenchuan Earthquake

    Objective To analyze retrospectively the 875 procedures of earthquake related patient in West China Hospital of Sichuan University after the Wenchuan earthquake, so as to provide reference for the improvement of medical rescue for the disaster of earthquake and the establishment of state-level regional medical centers. Methods The analysis was based on the data provided by the Department of Information of the Hospital up until July 11. The software of Microsoft EXCEL was used for data input and statistical analysis. Results Up to July 11, 1 265 operations in earthquake injuries have been performed in the hospital, of which 875 were done in operating rooms. Initial peak of admission to hospital was positively correlated to peak of surgery performed, which the type of the operation was all emergency operation and most of which were amputation (43%). Second peak of surgery was 2 days delayed following admission to hospital. The type of operation mainly was elective surgeries. Most patients received second stage procedure-open reduction and internal fixation (61%). Conclusion  Our hospital played a key part in rescue effort following such huge natural disaster. Our prompt response and effective leadership demonstrated our hospital’s flexibility and capability in the state-level hospital near the epicenter.

  • Effectiveness and Performance Evaluation of Medical Rescue during 2 Weeks after Lushan Earthquake

    Objective We evaluated effectiveness and performance of medical rescue after Lushan earthquake during 2 weeks, and enriched Wenchuan lessons to provide useful references for emergency medical rescue (EMR) after similar earthquake worldwide. Methods We collected and analyzed official information, public documents, news release, and relevant information from websites, and then we systematically reviewed and descriptively analyzed all included literature of EMR after earthquake (domestic and foreign). Results Learned from Wenchuan earthquake, EMR for Lushan earthquake were characterized as: a) Assess the situations of quake damage and injuries were scientifically assessed; human resources, funds and materials were reasonably distributed; and the EMR relied mainly on regional rescue power of Sichuan province. b) Patients’ with critical injuries were treated using “Four concentration treatment principles”, which resulted in a new medical record of zero death, 14 days after the earthquake. c) The experience of EMR after Lushan earthquake verified, enriched and improved lessons from Wenchuan, Yushu and Yiliang earthquake, which provided first-hand references of evidence-based decision making for earthquake EMR worldwide.
