ObjectiveTo analyze the clinical characteristics and geographical distribution of Keshan disease in Chongqing city for prevention and disease control. MethodsWe collected the clinical data of patients with Keshan disease from 2008 to 2012 in Liangping, Shizhu, Fengdu and Dianjiang counties as well as Wanzhou district of Chongqing city including the medical history, physical examination, results of laboratory tests to analyze the clinical characteristics and geographical distribution. ResultsFifty-eight patients were included from Liangping (n=21), Shizhu (n=25), Fengdu (n=11) and Dianjiang (n=1). The number of patients with potential and chronic Keshan disease was 16 and 42, respectively. The average age of patients was 54.91±15.53 years. The proportion above age 60 was 32.76% and below age 10 was 3.45%. The patients had main clinical signs as heart enlargement (36.76%), low-weak first heart sound (22.41%), systolic murmur (10.34%), arrhythmia (8.62%), etc. Abnormal ECG detection rate was 98.28%, with common types followed by sinus rhythm (37.93%), complete right bundle branch block (25.86%), ST-T changes (24.14%), left ventricular hypertrophy (15.52%), atrial fibrillation (13.79%), occasional ventricular premature (10.34%), T changes (10.34%), sinus bradycardia (8.62%), and incomplete right bundle branch block (6.90%). X-ray results showed that heart enlargement accounted for 82.76%. The ratios of mild, moderate and significant expansion of the heart were 46.55%, 27.59%, and 8.62%, respectively. ConclusionIn recent years, most patients with Keshan disease in Chongqing are chronic type at older age. The main clinical symptom is heart enlargement with high abnormal ECG detection rate.
ObjectiveTo understand the prevention and control effects of iodine deficiency disorders and its progression trend of disease condition in Chongqing. MethodsOne primary school was randomly selected as longitudinal surveillance sites in each of 4 counties in Chongqing city. A total of 90 children aged 8 to 10 years were randomly selected in each school with goiter, salt iodine, urinary iodine and intelligence detected. Meanwhile, in the four sites, urinary iodine of pregnant women and nursing women was tested. The goiter was examined by B-ultrasonograghy, urinary iodine by Ce arsenic acid digestion, and salt iodine by direct drop methods. ResultsA total of 360 children aged 8 to 10 years were investigated surveillance sites. The total goiter rate was 5.28%. There was no significant difference in the total goiter rate among different counties (P > 0.05). The median of iodized salt was 26.65 mg/kg. The coverage rate of iodized salt, consumption rate of qualified iodized salt and intake rate of qualified iodized salt was 99.72%, 91.09% and 97.04%, respectively. The medians of urine iodine of children, pregnant and nursing women were 238.15 μg/L, 218.15 μg/L and 252.90 μg/L, respectively. The cases with urine iodine lower than 100 μg/L accounted for 11.94%, 32.93% and 13.75%, respectively. The cases with urine iodine above 300 μg/L accounted for 34.17%, 13.41% and 43.75%, respectively. The mean intelligence quotient of children was 112.57. ConclusionThe goiter rate of schoolchildren in Chongqing city is relatively high and the population's iodine nutrition level is stable. However, the shortage and overdose of iodine still exist in Chongqing city.
ObjectiveTo study effect of expression levels of serum inflammatory factors and insulin receptor substrate(IRS)-1/2 in visceral adipose tissue after Roux-en-Y gastric bypass(RYGB) on type 2 diabetes mellitus(T2DM) rats, and explore possible mechanism in treatment of T2DM. MethodsThe T2DM rats models were established, which were divided into 3 groups by intervention: T2MD-RYGB group(n=14), T2MD-sham operation(T2MD-SO) group(n=10), and T2MD group(n=10), and 10 normal rats were selected as control group. The rats of the T2MD-RYGB group were received the RYGB, and of the T2MD-SO group were received transection and reanastomosis of the gastroin-testinal tract. The fasting plasma glucose(FPG), fasting insulin(FINS), C-reaction protein(CRP), tumor necrosis factor-α(TNF-α), free fatty acid(FFA), homestasis model assessment for insulin resistance(HOMA-IR), adipose tissue insulin resistance(Adipo-IR) were tested respectively before operation and on week 1, 4, 8 after operation(synchronous detec-tion of rats with or without surgical intervention). The IRS-1 and IRS-2 protein contents of the rat epididymal adipose tissue were tested on week 8 after operation. ResultsThe FPG, FINS, CRP, TNF-α, FFA levels, and HOMA-IR, Adipo-IR indexes in the T2DM rats were significantly higher than those in the normal rats(P < 0.05) before operation, the above indicators on week 4, 8 after operation were significantly lower than those before operation in the T2MD-RYGB group(P < 0.05). The differences of changes among the other groups were not statistically significant(P > 0.05). The IRS-1 and IRS-2 protein expressions in the adipose tissue of the rats were significantly increased in the T2MD-RYGB group as compared with these indicators in the T2MD group and T2MD-SO group(P < 0.05), but which were significantly lower than those in the control group(P < 0.05). ConclusionsRYGB could increase IRS-1/2 expression levels in adipose tissue, which could enhance insulin sensitivity, decrease serum inflammatory factors levels, and improve insulin resistance ultimately. This might be one of the mechanisms in treatment of T2DM.
