  • Clinical Effect of Laparoscopic Cholecystectomy Combined with Laparoscopic Common Bile Duct Exploration Between Elderly and Non-Elderly Patients with Cholecystolithiasis and Choledocholithiasis

    ObjectiveTo compare the effect of laparoscopic cholecystectomy (LC) combined with laparoscopic common bile duct exploration (LCBDE) in the treatment of cholecystolithiasis combined with choledocholithiasis in elderly patients and non-elderly patients. MethodsThe clinical data of 185 cases of cholecystolithiasis combined with choledocholithiasis who treated in our hospital from September 2010 to November 2015 were analyzed retrospectively. Then the 185 cases of holecystolithiasis combined with choledocholithiasis were divided into elderly patients group (n=74) and the non-elderly patients group (n=111). The operative time, intraoperative blood loss, postoperative exhaust time, postoperative activity time, abdominal drainage time, postoperative hospital stay, total hospital stay, hospitalization cost, incidence of complications, unplanned analgesia, stone-free rate, rate of conversion to laparotomy, recurrence of stone, and mortality were compared between the two groups. Results① Intraoperative and postoperative indexes. No significant difference was noted in operative time and intraoperative blood loss (P > 0.050), but the postoperative exhaust time, postoperative activity time, abdominal drainage time, postoperative hospital stay, total hospital stay, hospitalization cost, and ratio of indwelling T tube of elderly patients group were all higher or longer than corresponding index of non-elderly patients group (P < 0.050). ② Postoperative complications and unplanned analgesia. There was no striking discrepancy in incidence of complications (including biliary leakage, peritonitis, haemorrhage, vomit, ectoralgia, and fever), and Clavien-Dindo grade (P > 0.050), except unplanned analgesia (P=0.007), the rate of unplanned analgesia in elderly patients group was higher than that of non-elderly patients group. ③Surgical effect. There was no significant difference in the stone-free rate, rate of conversion to laparotomy, and rate of recurrence of stone between the 2 groups (P > 0.050). ConclusionLC+LCBDE is also safe and effective in treatment of cholecystolithiasis combined with choledocholithiasis in elderly patients, it's worthy to be expanded and be used broadly.

  • Clinical Observation on Effect of Acupuncture in Treating Primary Mild and Moderate Pruritus Ani

    ObjectiveTo observe the clinical efficacy of acupuncture treatment on primary mild and moderate pruritus ani. MethodsAccording to the diagnostic and inclusion criteria, 50 outpatients between March 2013 and March 2014 were randomly divided into acupuncture group and drug control group, with 25 cases in each. The acupuncture treatment group were treated by perianal local surrounding needling mostly with body acupuncture treatment for 14 times in 4 weeks. Drug control group were given oral antihistamine drugs and traditional Chinese medicine, pevisone cream rub for 4 weeks. Itching index scores, dermatology life quality index scores and treatment satisfaction were observed before and after treatment, and referral number and average number of pruritus of the two groups were observed in six months after the end of treatment. ResultsAfter 4 weeks of treatment, itching index score of patients in the acupuncture treatment group was significantly lower than the drug control group; the dermatology life quality index scores and treatment satisfaction were significantly better than the drug control group, and in the six months after the end of treatment, referral number and the average number of pruritus were lower than the drug control group. The differences were statistically significant (P<0.05). ConclusionClinical efficacy of acupuncture treatment on primary mild and moderate pruritus ani is significant, and the long term effect is stable.

  • The Advantage of Ambulatory Laparoscopic Cholecystectomy

    ObjectiveTo investigate the feasibility, safety, cost, and patient satisfaction of ambulatory laparo-scopic cholecystectomy(ALC). MethodsThe clinical data of patients who divided into ALC group(678 cases) and in-patient laparoscopic cholecystectomy(IPLC) group(1 534 cases) in our hospital from April 2011 to December 2012 were retrospectively analyzed. The operative time, conversion rate, complication rate, hospitalization time, cost of hospi-talization, rehospitalization rate, and patient satisfaction were analyzed and evaluated. ResultsThere were no significant differences of the operative time, postoperative complication rate, and rehospitalization rate between the 2 groups(P > 0.05). The conversion rate(0.44%), and hospitalization time[(1.2±0.5)d] of the ALC group were significantly lower or shorter than those of IPLC group[3.19%, (4.8±1.3) d], P < 0.05. The direct, indirect health care costs, and the total costs of the ALC group were (6 555.6±738.7), (230.0±48.0), and (8 856.0±636.0) yuan, respec-tively; and lower than those of the IPLC group[(7 863.71, 014.6), (973.0±136.5), and(8 856.0±636.0)yuan], P < 0.05. ConclusionALC is safe and feasible, and could shorten the hospitalization time, lower the medical cost, speed up the bed turnover, and increase the efficiency in the use of health resource.

  • EXTENDED NASOLABIAL FLAPS IN MANAGEMENT OF ANTERIOR BUCCAL MUCOSAL DEFECTS

    ObjectiveTo investigate the feasibility of the extended nasolabial flap in repairing small or medium anterior buccal mucosal defects. MethodsBetween March 2013 and April 2014, 10 patients with anterior buccal mucosal defects were treated with extended nasolabial flaps. There were 8 males and 2 females with the average age of 47.2 years (range, 39-62 years). The left side was involved in 4 cases and the right side in 6 cases. The pathological types included 3 cases of oral leukoplakia (OLK), 3 cases of OLK with malignant changes, 1 case of malignant oral lichen planus, and 3 cases of papilloma. The clinical course ranged from 2 to 15 months (mean, 7.1 months). The resection was restricted to the mucosa and little buccinators without cheek penetration, and the defects ranged from 2.5 to 4.0 cm in width and 3.5 to 5.5 cm in length. The distance between defect and the corner of the mouth was 0.5 to 1.5 cm. A falcate flap was designed along the nasolabial fold with a pedicle lateral beside the corner of the mouth. The flap was lifted in the plane of the superficial muscular aponeurotic system from both terminal points to the region of the central pedicle. Then the flap was transposed intraorally through a transbuccal tunnel to cover the mucosal defect while the extra-oral incision was closed directly. ResultsAll flaps completely survived and all wounds healed primarily. All patients were followed up 6 to 18 months (mean, 10.4 months). All patients regained symmetrical appearances and normal mouth commissure only with linear scars hidden in the nasolabial folds. The mouth opening was 2.7 to 3.5 cm (mean, 3.1 cm) at last follow-up. The intraoral flaps healed perfectly with thin and flat outlooks. No cheek biting or fish-mouth deformity was observed. ConclusionThe extended nasolabial flap can be used to repair small or medium anterior buccal mucosal defects because it has the advantages of reliable blood supply, flexibility in design, simplicity in harvesting, and hidden donor site scars.

  • Video-assisted Thoracoscopic 3D versus 2D Mode Operation for Solitary Pulmonary Nodules: A Case Control Study

    ObjectiveTo compare solitary pulmonary nodule resection via thoracoscopic 3D mode or 2D mode and to further evaluate the clinical application value of thoracoscopic 3D mode. MethodsWe retrospectively analyzed the clinical data of 120 patients with solitary pulmonary nodule between March 2013 and March 2014 in the First Hospital Affiliated to Xiamen University. The patients were allocated into two groups including a 3D-VATS group (50 patients) and a 2D-VATS group (70 patients). Pulmonary partial resection was performed firstly. Pulmonary lobectomy would be conducted or not on the basis of intra operative rapid pathological results. ResultsTwenty three patients were performed 3D-VATS in the 3D-VATS group. Twenty-nine patients were diagnosed as pathological malignancy underwent lobectomy plus partial dissection. There were statistical differences between the 3D-VATS group and the 2D-VATS group in operative time (t=1.967, P<0.05), intra operative blood loss (t=7.85, P<0.05), drainage volume 24 h after operation (t=6.18, P<0.05), postoperative chest tube retention time (t=7.1, P<0.05), and postoperative hospital stay (t=2.35, P<0.05). Following-up time in the 3D-VATS group was 6.3 (2-12) months. Complications occurred in 3 patients, including 2 patients with postoperative pneumonia and 1 patient with paroxysmal atrial fibrillation in the 3D-VATS group. The following-up time in the 2D-VATS group was 8.2 (2-15) months. Complications occurred in 4 patients, including 1 patient with chylous hydrothorax, 2 patients with pneumothorax, 1 patient with delayed pulling up the chest closed drainage in the 2D-VATS group. The patients in both groups with complications were cured by appropriate treatment. Conclusion3D-VAST for SPN is a new operation mode choice. It is safe and feasible with low incidence of postoperative complications. Surgery vision, stereo feeling, the operation of adaptation, and postoperative recovery have certain advantages. It is worthy popularizing.

  • Quality Control Circle Activities on Reducing the Risk of Observed Patients in Emergency Room

    ObjectiveTo study the effect of quality control circle activities on reducing the risk of observed patients. MethodBy carrying out the quality control circle, it was done that confirming the subject, grasping the risk of observed patients, setting a goal, analyzing the risk factors of existing problems, finding out the real reasons, drawing up and actualizing the countermeasures. ResultsAfter carrying out the quality control circle, the main risk factor, emergency rescue, of observed patients was ameliorated significantly (P < 0.05). The risk was declined from 0.41% to 0.14% (P < 0.05). The self-evaluation of circle members was improved (P < 0.001). ConclusionBy the quality control circle, it is actualized effectively that reducing the risk of observed patients and improving the overall qualities of nurses. This thing is helpful to improve the quality of nursing.

  • BIPADDLED SPLIT PECTORALIS MAJOR MYOCUTANEOUS FLAPS FOR IMMEDIATE RECONSTRUCTION OF ORAL MUCOSAL DEFECTS AND NECK DEFECTS AFTER RESECTION OF RECURRENT ORAL CANCER

    ObjectiveTo investigate the feasibility of the bipaddled split pectoralis major myocutaneous flap for immediate reconstruction of oral mucosal defects and neck defects after resection of recurrent oral cancer. MethodsSix patients with oral mucosal defects combined with neck defects after recurrent oral cancer resection were treated with bipaddled split pectoralis major myocutaneous flap between September 2013 and September 2014. There were 5 males and 1 female with an average age of 54.7 years (range, 45-62 years), including 4 cases of recurrent tongue cancer, 1 case of recurrent mandibular gingival cancer, and 1 case of mouth floor carcinoma. All patients underwent local recurrence at 8 to 14 months after first operation, with no distant metastasis. The defects of the intraoral mucosa was 4.0 cm×2.5 cm to 6.5 cm×3.5 cm and the defect of the neck skin was 5.5 cm×3.5 cm to 7.5 cm×5.0 cm. The pectoralis major myocutaneous flaps (14.0 cm×3.5 cm to 17.0 cm×5.5 cm) were incised at the level of the 3rd to the 4th rib, and then split down along the muscle fiber till about 2 cm away from the thoracoacromial vessels, forming 2 independent skin paddles with 1-2 branch vessels to the pedicles of the distal ones. The distal skin paddles were used for oral reconstruction while the proximal paddles for repair of neck defects. The chest donor sites were sutured directly. ResultsCervical haematoma and infection happened in 1 patient respectively after operation, and were cured after symptomatic treatment. All 6 split pectoralis major myocutaneous flaps with 12 skin paddles completely survived. All patients were followed up 6 to 18 months (mean, 11 months). One patient died of pulmonary metastasis at 8 months after operation and the other 5 survived without relapse or metastasis during follow-up. The intraoral paddles showed good shape with satisfactory speech function and swallowing recovery. The paddles also healed perfectly on the neck with flat outlooks, and all patients obtained full appearance and free movement of the neck. No fistula formed on the submandibular region and neck. ConclusionThe bipaddled split pectoralis major myocutaneous flap can complete simultaneous immediate reconstruction of oral mucosal defect and neck defect. It is very useful in the treatment of recurrent oral cancer.

  • Effect of Preoperative Oral Administration of Olive Oil to Prevent Chylothorax after Thoracoscopic Resection of Esophageal Carcinoma

    ObjectiveTo study the effect of preoperative oral administration of olive oil in the patients with laparoscopic resection of esophageal carcinoma and to expose the advantages of alimentary duct in operation. MethodsWe retrospectively analyzed the clinical data of 136 patients in our hospital from June 2013 through June 2015 year. There were 83 males and 53 females at age of 58.3±7.6 years. The patients were given oral olive oil 100 ml at preoperative 12 hours. ResultsAll patients completed surgery successfully without transfer to open chest. Operation of thoracic duct filling was transparent, milky white, and with clear exposure. Thoracic duct was retained successfully in the 131 patients. It was found that intraoperative injury during separation of thoracic duct due to tumor invasion in 5 patients. These patients were treated with by titanium clipping. All the patients were with chest tube drainage for 3-5 d. Average drainage volume at postoperative 24 hours was 150±35 ml. Postoperative total drainage volume was 500±130 ml. None of postoperative chylothorax was found. Postoperative average hospitalization time was 9±2 d. ConclusionEsophageal cancer preoperative oral administration of olive oil is simple, safe, effective, no injury, and with clear exposure in thoracic duct in operation. Injury of the thoracic duct is reduced. The integrity of thoracic duct is preserved. Normal glucose and lipid metabolism is retained. Therefore, it is a good choice in preoperative preparation for esophageal cancer patients. It is worth popularization and application.
