  • Effectiveness and Cost of Laparoscopic versus Traditional Abdominal Myomectomy in China: A Systematic Review

    Objective To systematically evaluate the effectiveness and cost of laparoscopic myomectomy (LM) vs. traditional abdominal myomectomy (TAM) in treating Chinese patients with hysteromyoma. Methods Such databases as The Cochrane Library (Issue 4, 2012), PubMed, EMbase, CNKI, CBM and WanFang Data were searched from their inception to September, 2012 to collect the randomized controlled trials (RCTs) about LM vs. TAM in treating Chinese patients with hysteromyoma, and the references of the included studies were also retrieved. Two reviewers independently screened the literature according to the inclusion and exclusion criteria, extracted the data and assessed the methodological quality. Then meta-analysis was conducted using RevMan 5.2 software. Results A total of 8 RCTs involving 1 000 Chinese patients were included. The results of meta-analysis showed that, LM was superior to TAM in postoperative exhaust time (WMD= ?15.21, 95%CI ?20.19 to ?10.24, Plt;0.000 01) and postoperative hospital stay (WMD= ?3.07, 95%CI ?4.25 to ?1.90, Plt;0.000 01), with significant differences. But it was inferior to TAM in operation time (WMD=28.33, 95%CI 18.07 to 38.59, Plt;0.000 01) and hospital costs (WMD=2 028.87, 95%CI 1 190.75 to 2 866.98, Plt;0.000 01), with a significant difference. There was no significant difference in intraoperative bleeding amount between the two groups (WMD= ?2.78, 95%CI ?41.56 to 36.00, P=0.89). Conclusion This study shows LM is superior to TAM in fastening postoperative recovery and shortening hospital stay, but it is longer in operation time and higher in cost. The intraoperative bleeding amount is similar in the two groups. Due to low methodological quality and small sample size of the included studies, this conclusion has to be further proved by more high-quality RCTs.

  • Laparoscopic Hepatectomy versus Open Hepatectomy for Hepatocellular Carcinoma: A Meta-Analysis

    Objective To systematically evaluate the effectiveness and safety of laparoscopic hepatectomy (LH) versus open hepatectomy (OH) for hepatocellular carcinoma (HCC). Methods Databases including PubMed, EMbase, MEDLINE, SCI, CNKI, CBM, WanFang Data and The Cochrane Library (Issue 3, 2012) were searched to collect the randomized controlled trails (RCTs) and non-RCTs about LH versus OH for HCC. The retrieval time was from inception to August 2012. The studies were screened according to the inclusion and exclusion criteria, the data were extracted and the quality was evaluated by 2 reviewers independently. Then the meta-analysis was conducted using RevMan 5.1 software. Results A total of 13 non-RCTs involving 701 patients were included. The results of meta-analysis showed that: Compared with OH, LH had lesser amount of intraoperative bleeding (MD=?144.09, 95%CI ?194.25 to ?93.94, Plt;0.000 01), shorter hospital stay (MD=?5.48, 95%CI ?7.10 to ?3.85, Plt;0.000 01), and lower postoperative complications (OR=0.43, 95%CI 0.27 to 0.66, P=0.000 1). But there were no differences between the 2 groups in operation time (MD=?0.64, 95%CI ?22.95 to 21.68, P=0.96), perioperative death rate, 3-5 year survival rate, and tumor free survival rate. Conclusion LH is superior to OH in treating HCC for it is associated with smaller wound, lesser operative blood loss, shorter hospital stay, and lower postoperative complications. And it is similar as OH in operation time, perioperative death rate and 3-5 year survival rate. So LH is safe and feasible for treating HCC when its indications are strictly controlled. However, for the quantity and quality limitation of the included studies, this conclusion still requires to be further proved by performing large scale and high quality RCTs. It suggests that doctors should choose a best therapy for HCC patients according to an integrative disease assessment.

  • Clinical Effectiveness and Safety of Laparoscopy versus Laparotomy for Endometrial Cancer: A Meta-Analysis

    Objective To assess the clinical effectiveness and safety of laparoscopy versus laparotomy for endometrial cancer. Methods The databases such as The Cochrane Library, PubMed, EMbase, Ovid, CNKI, WanFang Data, and VIP were searched to collect the randomized control trials (RCTs) about the clinical effectiveness and safety of laparoscopy and laparotomy for endometrial cancer. The retrieval time was from January 1998 to September 2012. Two reviewers independently screened the literature according to the inclusive and exclusive criteria, extracted the data, and assessed the methodological quality of included studies. Then the meta-analysis was performed by using RevMan 5.0 software. Results A total of 10 RCTs involving 6 993 patients were included. Meta-analysis showed that, compared with laparotomy, laparoscopy had lesser amount of intraoperative bleeding, lower decrease of hemoglobin before and 1-day after operation, shorter time of both waiting for postoperative gas and hospital stay, lower incidence of postoperative complications, longer operation time, and higher incidence of intraoperative complications. Additionally, there were no differences between the 2 groups in the number of pelvic and para-aortic lymph nodes removed during operation, as well as the postoperative recurrence and mortality rates in 3-5 year follow-up. Conclusion Compared with laparotomy, laparoscopy shows lesser amount of intraoperative bleeding, lower decrease of hemoglobin before and 1-day after operation, shorter time of both waiting for postoperative gas and hospital stay, lower incidence of postoperative complications. But laparotomy shows lower incidences of intraoperative complications, and shorter operation time. Both operations are similar in the number of pelvic and para-aortic lymph nodes removed during operation, as well as the postoperative recurrence and mortality rates in 3-5 year follow-up. For quantity limitation and low methodological quality of included studies, this conclusion still needs to be further proved by performing more high-quality and large scale RCTs.

  • Application of Laparoscopic Transabdominal Preperitoneal Hernia Repair for Recurrent Inguinal Hernia

    Objective To explore the surgical techniques and the clinical effect of laparoscopic transabdominal preperitoneal (TAPP) hernia repair for recurrent inguinal hernia. Methods Clinical data of 130 cases of recurrent inguinal hernia who underwent TAPP hernia repair from Mar. 2009 to Dec. 2012 in Beijing Chao-Yang Hospital of Capital Medical University were retrospectively analyzed. Results Operations were completed successfully in 129 cases and 1 case was converted to open surgery. The operation time was (54.5±16.1) min (30-100min) and the hospital stay was (4.5±2.1) d (2-11d). The rates of postoperative pain, hydrocele, and urinary retention were 3.8% (5/130),11.5% (15/130), and1.5% (2/130) respectively. There were no complications such as foreign body sensation,wound infection,and intestinal obstruction after operation. All cases were followed-up for 7-50 months 〔(24.3±11.3) months)〕 with no recurrence was observed. Conclusions Laparoscopic TAPP hernia repair for recurrent inguinal hernia has advantages of minimal invasion and few complications,who is safe and effective.

  • Comparison of Total Extraperitoneal and Transabdominal Preperitoneal in Laparoscopic Inguinal Herniorrhaphy

    Objective To compare the feasibility, safety, and efficiency of laparoscopic total extraperitoneal (TEP)hernia repair surgery and laparoscopic transabdominal preperitoneal (TAPP) hernia repair surgery. Methods The clinical data of 95 patients with inguinal hernia who underwent laparoscopic TEP hernia repair surgery (TEP group) and TAPP hernia repair surgery (TAPP group) from Mar. 2010 to Oct. 2013 in our hospital were retrospectively analyzed, and clinical parameters including operation time, intraoperative blood loss, postoperative hospital stay, postoperative comp-lication, and operation cost of 2 groups were compared. Results All the procedures were successful, none of them was converted to open surgery. There was no significant difference between TEP group and TAPP group when considering operation time 〔(65±16) min vs.(68±17) min〕, intraoperative blood loss 〔(7.0±1.2) mL vs. (8.0±1.4) mL〕, visual pain analogue scale 〔(2.0±1.1) score vs. (1.8±1.1) score〕, postoperative hospital stay 〔(3.1±1.4) d vs. (3.3±1.2) d〕,and time to release to regular activities 〔(4.2±1.0) d vs. (4.5±1.2) d〕, P>0.05. But the operation cost of TEP group was significantly lower than that of TAPP group 〔(8 033±536) yuan vs. (9 632±643) yuan, P=0.007〕. There were 6 atients (6.3%, 6/95) suffered complications, 3 cases in TEP group and 3 cases in TAPP group, including 3 cases of seroma or hematoma in scrotum, 1 case of transient neurapraxia, and 3 cases of urinary retention. There was no signi-ficant difference in incidence rate of postoperative complication between the 2 groups (P=1.000). All patients were followed-up for 1-35 months 〔(20.0±10.2) months〕 without recurrence and chronic pain. Conclusions TEP and TAPP hernia repair surgery are feasible, safe, effective, and minimally invasive technique for inguinal hernioplasty. There are advantages and disadvantages of both TAPP and TEP hernia repair surgery, but there is no statistically significant difference regarding intraoperative and postoperative complications.

  • Liver Function Alterations and Its Influencing Factors after Laparoscopic-Assisted Radical Gastrectomy for Gastric Cancer

    Objective To evaluate the changes of liver function after laparoscopic-assisted radical gastrectomy for gastric cancer and analyze related impact factors. Methods Patients with gastric cancer or colon cancer, who underwent radical gastrectomy or hemicolectomy between Jun 2008 and Jun 2010 in General Hospital of PLA, were included in this study. These patients were divided into open gastrectomy (OG group, n=43), laparoscopic-assisted gastrectomy (LAG group, n=35), and laparoscopic-assisted hemicolectomy (LAC group, n=23). The serum AST, ALT, TB, and ALP levels of all patients enrolled on the preoperative day and post operative day (POD) 1, POD3, POD5, and POD7, and related impact factors were analyzed. Results Compared with the preoperative results, serum AST and ALT levels of patients increased until POD5 in both LAG and OG groups (P<0.05), and there was no changes in liver function after operation of patients in LAC group (P>0.05). In addition to that serum AST and ALT levels of patients in LAG group in POD1 were significantly higher than those in OG group (P=0.035 and P=0.041), and that serum ALT level of patients in LAG group was significantly lower than that in OG group in POD3 (P=0.048), serum AST and ALT levels of patients in two groups in the remaining time points were not statistically significant difference (P>0.05). The serum AST and ALT levels of patients in LAG group were significantly higher than those in LAC group during 5 d after operation (P<0.05).There was no significant changes of serum ALP and TB levels of patients in LAG and OG groups on the before and after operation (P>0.05). The increased serum ALT level of gastric cancer patients after operation related to body mass index (BMI, P=0.038), operative time (P=0.011), intraoperative hepatic injury (P=0.035), and abnormal ligation of hepatic artery (P=0.048), instead of the type of operation (OG vs.LAG, P>0.05). Conclusions Gastric cancer patients who underwent radical gastrectomy have transient liver dysfunction, which attributes mainly to direct liver manipulation or abnormal ligation of hepatic artery, but not CO2 pneumoperitoneum. Laparoscopic-assisted radical gastrectomy is feasible and safe to the patients without serious liver damage or other vital organs disorders.

  • Research Progress of CO2 Pneumoperitoneum Impacts on Invasiveness of Cancer Cells

    Objective To summarize the research progress of CO2 pneumoperitoneum impacts on invasiveness of cancer cells. Methods Currently published experimental and clinical researches related to the effect of CO2 pneumoperitoneum on invasiveness of cancer cells were reviewed. Results CO2 pneumoperitoneum may affect the invasiveness of cancer cell through several ways, such as changing the structure and function of mesothelial cell, changing microenvironment of peritoneum, influencing the expression of oncogen, affecting the secretion of cell factor, and changing the adhesion of cancer cell. Conclusions The consequences of these alterations to cancer cell and the microenvironment are not well understood, but they may facilitate tumor invasion and implantation. Further investigations in this area are very urgent.

  • Advances of Lymphatic Metastasis and Lymphadenectomy in Gastric Cancer

    Objective To summarize and analyze the different views on the lymph node metastasis and the extent of lymphadenectomy in gastric cancer.Methods The current domestic and foreign reports on lymph node metastasis and lymphadenectomy in gastric cancer were reviewed.Results Lymph node dissection of gastric cancer is based on clinical stage and the location of the tumor. Laparoscopic-assisted gastrectomy in treatment of gastric cancer is a safe, feasible, effective, and minimally invasive technique with good outcomes for patients. Sentinel lymph node(SLN) in the clinical assessment of early gastric cancer is feasible,besides with high accuracy and sensitivity. Lymphatic mapping is an effective, easy, and safe method to guide lymphadenectomy in gastric cancer. Evidence-based lymphadenectomy in gastric cancer provide a new perspective to the extent. Conclusions It is difficult to evaluate those methods exactly. Researchers over the world should learn from each other and explore further in order to develop guiding principles in the end.

  • Value of Laparoscopy in Treatment of Acute Inguinal Incarcerated Hernia (Report of 11 Cases )

    Objective To investigate the value of laparoscopy in resetting and repairing of inguinal incarcerated hernia. Methods Clinical data of 11 patients with acute inguinal incarcerated hernia treated with laparoscopy from March 2007 to February 2008 were reviewed, including 3 cases of direct inguinal hernia and 8 cases of oblique inguinal hernia. Results There were 7 patients with incarcerated hernia who were reset automatically after general anaesthesia, and 4 patients who were reset with laparoscopy. All the hernias were repaired with the transabdominal preperitoneal (TAPP) method after observed or improved the blood cycle of the incarcerated intestinal tract under laparoscopy. Average follow-up time was 6.6 months (2-12 months) after operation. Good recover and no recurrence were showed in all patients. Conclusion Laparoscopy is a safe technology in resetting and repairing of inguinal incarcerated hernia

  • Effects of Hand Assistant Laparoscopic Splenectomy Plus Pericardial Devascularization on Systemic Stress Responses

    Objective To investigate the effects of hand assistant laporoscopic splenectomy plus pericardial devascularization on systemic stress responses. Methods Forty patients with cirrhotic portal hypertension were selected, 20 cases of which were underwent hand assistant laparoscopic splenectomy plus pericardial devascularization (LAP group), and the other 20 were underwent open splenectomy plus pericardial devascularization (OP group). The levels of blood glucose (BG), insulin (Ins), triiodothyronine (T3), tetraiodothyronine (T4), corticosteroid (CS) and other related clinical data were measured before operation and on day 1-3 after operation, which were compared between two groups. Results There was no statistical significance between two groups on those levels before operation. On day 1 after operation, BG and CS level in both two groups were higher than those before operation (P<0.05), but they were recovered on day 2 after operation in LAP group (Pgt;0.05), and on day 3 after operation in OP group (Pgt;0.05). BG and CS level in OP group were markedly higher than those in LAP group on day 2 after operation (P<0.05). On day 1 after operation, Ins, T3 and T4 level of two groups were lower than those before operation (P<0.05), but they were recovered on day 2 after operation in LAP group (Pgt;0.05) and on day 3 after operation in OP group (Pgt;0.05). Ins, T3 and T4 level in OP group were lower than those in LAP group on day 2 after operation (P<0.05). There was no significant difference in operation time between two groups (Pgt;0.05). But laparoscopic surgery had more advantages than conventional open surgery such as reducing bleeding quantity in operation, shortening recovery time of bowel and urinary bladder function and the length of stay. Conclusion Compared with laparotomy, the laparoscope not only imposes less impact on physical stress system, but also makes recovery after operation more quickly.
