  • Relationship between postoperative kinesiophobia and functional recovery in patients with lumbar disc herniation

    Objective To explore the correlation between postoperative kinesiophobia and rehabilitation outcome in patients with lumbar disc herniation (LDH). Methods This study was a cross-sectional survey. The patients who underwent lumbar discectomy due to LDH in West China Hospital of Sichuan University between June and October 2019 were included consecutively. According to the Tampa Scale for Kinesiophobia (TSK) score, patients were divided into two groups: the kinesiophobia group and the non-kinesiophobia group. The patients’ scores of the TSK, the Visual Analogue Scale (VAS), the Oswestry Disability Index (ODI), and the Japanese Orthopaedic Association Scores (JOA) at different time points were collected. The relationship between postoperative kinesiophobia, pain, and lumbar function in patients was analyzed. Results A total of 201 patients were included. Among them, there were 128 cases in the kinesiophobia group and 73 cases in the non-kinesiophobia group. The incidence of kinesiophobia was 63.68%. Pearson correlation test analysis showed that TSK score was positively correlated with VAS score (r=0.218, P<0.01) and ODI score (r=0.263, P<0.01) at 24 h after surgery. At one month after surgery, there was a positive correlation with VAS score (r=0.296, P<0.01) and a negative correlation with JOA score (r=−0.182, P<0.05). The results of multiple linear regression showed that postoperative age, pain, lumbar function, and postoperative kinesiophobia were interrelated (P<0.05). Conclusions Patients with LDH had a higher incidence of kinesiophobia 24 h after surgery. Postoperative kinesiophobia was correlated with the recovery outcome of patients with LDH.

  • Analysis of self-perceived burden status and its influencing factors in patients undergoing spine surgery

    Objective To investigate the status of self-perceived burden (SPB) in patients undergoing spine surgery and to explore its influencing factors, in ordering to provide a basis for formulating corresponding nursing interventions. Methods A cross-sectional survey was conducted on patients undergoing spine surgery in Department of Orthopedic Surgery, West China Hospital of Sichuan University between May and August 2024. The patient general information questionnaire, the patient SPB Scale, the Barthel Index, the Medical Coping Modes Questionnaire, and the Social Support Rating Scale were used to investigate the SPB status and its influencing factors in patients undergoing spine surgery. Results A total of 230 patients were included. There were 113 cases in the non-SPB group and 117 cases in the SPB group. There were statistically significant differences in age, marital status, occupation, payment method, the number of family, disease diagnosis, Barthel Index score, caregiver identity, and caregiver gender between the two groups of patients (P<0.05). Among 117 patients with SPB, 83 (36.09%) had mild SPB, 27 (11.74%) had moderate SPB, and 7 (3.04%) had severe SPB. The average SPB scores for mild, moderate, and severe patients were (24.06±2.92), (33.07±2.87), and (44.86±4.56) points, respectively. The results of binary logistic stepwise regression analysis showed that the patient’s marital status, disease diagnosis, and caregiver gender were independent influencing factors for SPB in patients undergoing spine surgery (P<0.05). Conclusion The SPB of patients undergoing spine surgery is at a mild to moderate level, which is affected by factors such as marital status, disease diagnosis, and caregiver gender.

  • Effect of two different follow-up intervention modes on the rehabilitation of patients after total hip arthroplasty

    Objective To investigate the effects of online follow-up mode and online + offline follow-up mode on rehabilitation after total hip arthroplasty. Methods Patients who underwent total hip arthroplasty in the Department of Orthopedic Surgery, West China Hospital of Sichuan University between August and December 2022 were selected. According to personal preference, the included patients were divided into an observation group and a control group. The observation group underwent comprehensive follow-up mode, while the control group underwent simple online follow-up mode. Joint function, daily living ability, Huaxi Emotional-distress Index, follow-up satisfaction, complications and readmission were compared between the two groups at 3, 8, 26 and 52 weeks after surgery. Results A total of 83 patients were included. Among them, there were 41 cases in the control group and 42 cases in the observation group. There was no statistically significant difference in personal basic information and disease-related data between the two groups (P>0.05). The joint function and daily living ability of the observation group were better than those of the control group at 8, 26, and 52 weeks after surgery (P<0.05). There was no significant difference between the two groups in Huaxi Emotional-distress Index and follow-up satisfaction at 52 weeks after operation (P>0.05). The incidence of complications in the observation group was lower than that in the control group (P<0.05). Conclusions The comprehensive follow-up model has more advantages in direct physical assessment, wound assessment, in-depth communication, timely feedback and adjustment, which can improve patients’ postoperative joint function, enhance their daily life ability, and reduce the occurrence of postoperative complications.

This paper explores the current applications of wearable devices in the field of clinical nursing, with a focus on their significant roles in real-time monitoring of physiological parameters, disease management, functional rehabilitation exercises. Additionally, it analyzes the challenges these devices face, such as the need for standardized development, data security and privacy protection, and cost-benefit analysis. This paper also proposes measures to address these challenges, including enhancing policy formulation, promoting standardization, and fostering technological innovation, with the aim of providing valuable insights for the advancement of high-quality clinical nursing practices.

  • The West China Hospital program of nutritional management under accelerated perioperative recovery in cervical spondylosis

    In the context of accelerated rehabilitation, nutritional support for patients with orthopedic cervical spondylosis is an important condition for lessening postoperative stress response, reducing postoperative complications, shortening patient’s length of hospital stay, lowering medical expenses, and promoting early recovery of patients. Based on this, West China Hospital of Sichuan University explored and established the West China Hospital program for nutritional management in cervical spondylosis from the aspects of team building, nutritional assessment and monitoring, and nutritional intervention.

  • Construction of quality standards for joint surgery specialist nursing based on three-dimensional quality evaluation model

    Objective To explore the nursing quality standards of joint surgery based on “structure-process-outcome” three-dimensional quality evaluation model, and provide a reference for improving the quality of joint surgery nursing. Methods From March to July 2023, based on the “structure-process-outcome” three-dimensional quality structure model as the theoretical framework, combined with systematic literature review and semi-structured interviews, the quality standards of joint surgery specialist nursing were preliminarily formulated. The Delphi method was used for screening, demonstration, and correction, then the analytic hierarchy process was used to determine the weights. Results The recovery rates of the two rounds of expert letter consultation were 93.75% and 100.00%, respectively. The expert authority coefficients of both rounds were 0.95. The positive coefficients of experts in the two rounds were 0.93 and 1.00, respectively. The coefficients of variation after the second round of correspondence were 0-0.20. In the second round of correspondence inquiry, the Kendall harmony coefficients of primary standards, secondary standards and tertiary standards were 0.156, 0.115 and 0.285, respectively, with statistical significance (P<0.05). Finally, the quality standards of joint specialty nursing included 3 primary standards, 23 secondary standards and 99 tertiary standards. Conclusion The construction process of nursing quality standards of joint surgery specialty based on “structure-process-outcome” three-dimensional quality structure model is scientific and reliable, the index evaluation results are less volatile, and the opinions are concentrated, which can provide a basis for comprehensively, accurately and pertinently improving the quality of joint surgery specialty nursing.
