  • Clinical Applications of Plasma Shock Wave Lithotripsy Technique via Choledochoscope. 

    Objective To investigate the clinical applications of plasma shock wave lithotripsy (PSWL) in the treatment of calculi via choledochoscope. Methods Between 2004 and 2009, 56 huge residual calculi (54 cases) were underwent PSWL via choledochoscope treatment in our hospital. Calculi size: diameter ≤10 mm in 9 calculi, 10 mm lt;diameter ≤15 mm in 24 calculi, 15 mm lt;diameter ≤20 mm in 17 calculi, and diameter gt; 20 mm in 6 calculi. Twenty four cases of these 54 patients had bile duct stricture. Procedure: Firstly found the calculus, and then the lithotriptor wire was introduced to the place 0.5-1.0 mm far away from the calculus surface through choledochoscope channel, and powered on, fired, destructed. Results Forty-eight cases of 54 patients were cured by PSWL with 1 times, 2 cases with 2 times, 2 cases with 3 times, and 2 cases with 4 times. Shock 4-300 times were exported per PSWL, with an average of 65 times. Crushing each stone needed shock for 4-680 times, with an average of 77 times. The calculi were ruptured in different degrees by PSWL, of which 20 cases were completely ruptured. All calculi were completely extracted. Except one case with bile duct stricture was found a small amount of bile duct bleeding during operation, all other patients had no operation complications. Conclusion PSWL technique plays an important role in lithotripsy. It is easy to deal with intrahepatic impacted calculi by PSWL, especially the pigment stones with rough surface.

  • CHANGES AND ITS SIGNIFICANCE OF MOTILIN, VASOACTIVE INTESTINAL PEPTIDE IN PLASMA, GALLBLADDER TISSUS AND BILE IN PATIENTS WITH GALLSTONE DISEASE

    Objective To discuss the relationship between motilin, vasoactive intestinal peptide and the gallstone formation. Methods The level of motilin, vasoactive intestinal peptide in plasma, bile and gallbladder tissue of 48 cases of chololithiasis before operation and the first, third, seventh day after cholecystectomy were mesured by radioimmunoassay. Results The level of motilin in plasma was markedly increased in patients with chololithiasis before cholecystectomy and the first day after cholecystectomy. The level of motilin, vasoactive intestinal peptide in bile and gallbladder tissue were significantly increased in patients and motilin was positively correlated with vasoactive intestinal peptide in the gallbladder tissue. Conclusion Motilin, vasoactive intestinal peptide might affect the gallstone formation by affecting the motility of gallbladder.

  • CLINICAL STUDY ON SERUM COMPLEMENT-3 AND PLASMA FIBRONECTIN OF PATIENTS WITH OBSTRUCTIVE JAUNDICE PRE- AND POST-OPERATION

    Thirty patients with obstructive jaundice were investigated for serum complement-3 (C3) and plasma fibronectin (FN).The levels of C3 and FN of the juandiced patients were higher than that of thirty patients without obstructive jaundice (P<0.01). As compared to pre-operation, the level of C3 of the jaundiced patients decreased obviously within two weeks after operation(P<0.01), and recovered in the third week after operation. The level of FN of the juandice patients decreased evidently within one week(P<0.01), and recovered in the second week after operation. However, the levels of C3 and FN of the patients without obstructive jaundice changed slightly after operation (P<0.05). The high levels of C3 and FN of jaundiced patients may be relative to the latent infection. Consumption and immune imparing may be the reasons of C3 and FN to decrease.

  • Comparison of Clinical Applications Between Two Different Extracorporeal Membrane Oxygenation Systems

    Abstract: Objective To summarize the clinical experiences of performing extracorporeal membrane oxygenation (ECMO) on patients with heart and respiratory failure, and compare the clinical outcomes of Medtronic system and Quadrox PLS system. Methods We retrospectively analyzed the clinical data of 121 heart and respiratory failure patients who underwent ECMO treatment in Fu Wai Hospital from December 2004 to December 2009. Based on the different systems used, 121 patients were divided into two groups. In group M, there were 65 patients including 41 males and 24 females, with an age of 26.6±25.9 years; and they accepted Medtronic ECMO system treatment. In group Q, there were 56 patients including 39 males and 17 females, with an age of 32.4±23.9 years and they underwent Quadrox PLS ECMO system treatment. The evaluation of the clinical outcomes of these two different systems was based on the comparison of transmembrane pressure drop (TMPD), anticoagulation, hemolysis, plasma leakage, organ function, complication and hospital mortality between the two groups. Results Compared with group M, ECMO with Quadrox PLS system was associated with lower TMPD (at the beginning of ECMO: 15.0±6.0 mm Hg vs.28.0±5.0 mm Hg, P=0.000; 24 hours later: 16.0±5.0 mm Hg vs. 30.0±7.0 mm Hg, P=0.000) and less thrombus formation(7.1% vs.23.1%, P=0.037), less plasma leakage (0.0% vs. 50.8%, P=0.000), less hemolysis (14.3% vs. 29.2%, P=0.047). There were no significant differences between the two groups in support duration, complication rate, and hospital mortality(Pgt;0.05). Conclusion Both devices have similar effects for safe clinical application, but Quadrox PLS ECMO system has a relatively high biocompatibility with lower TMPD, less plasma leakage, and thrombus formation.

  • Plasma Exchange for Severe Haemolysis During ReCardiopulmonary Bypass to Patients with Cardiac Surgery

    Abstract: Objective To summarize the clinical experience of plasma exchange (PE) during recardiopulmonary bypass (CPB) of patients with severe haemolysis in cardiac surgery. Methods Between January 2001 and December 2005, five patients required PE for severe haemolysis after cardiac surgery. There were periprosthetic leakage and infective endocarditis in 3 patients, congenital heart disease of pulmonary artery stenosis with unsatisfied right ventricular outflow tract patching in 1 patient and thrombosis during extracorporeal membrane oxygenation (ECMO) in 1 patient. They all need blood purification to avoid acute renal failure. Results Five patients were successfully treated with PE during CPB without major complications. The amount of plasma and blood transfused in the 5 patients were 2.2±0.8L and 0.6±0.3L respectively. The volume of plasma exchange and ultrafiltrate were 3.9±1.8L and 2.4±1.3L respectively.The electrolytes and bloodgas analysis in all patients were maintained at the normal levels. The hemodynamics was stable. After heart resuscitation CPB stopped smoothly. Disappearance of periprosthetic leakage and satisfaction of right ventricular outflow tract patching were observed by echocardiograms after peration.Extubation was performed 24h after the operation in 5 patients, and they were discharged 12 to 53 d after the operation with fully recovery. The urine was clear and the body temperature was normal. Before they left thehospital, the concentration of free hemoglobin was tested in 3 patients. The concentration of free hemoglobin was slightly higher in 1 patient (68mg/L), and normal in 2 patients (lt;40mg/L). Conclusion PE during CPB in severe haemolysis is a safe technique which can effectively prevent acute renal failure caused by severe mechanical haemolysis after cardiac surgery.

  • EARLY DIAGNOSIS OF LOWER LIMB DEEP VEIN THROMBOSIS AFTER MAJOR ORTHOPEDIC SURGERIES

    Objective To investigate the early diagnosis of lower l imb deep vein thrombosis (DVT) after major orthopedic surgeries. Methods From October 2005 to June 2009, color doppler sonography and hemorheology detection were carried out for 62 cases undergoing first total hip arthroplasty (THA), 14 cases undergoing total knee arthroplasty (TKA), and 86 cases undergoing hip fractures surgery (HFS) before operation and 1, 7, 14 days after operation. The plasma D-dimerlevels of the DVT were also examined for the THA patients before operation and 3 days after operation. Of all cases, therewere 89 males and 73 females, aged from 34 to 74 years (51.5 years on average). After operation, all the patients were treated with the regular low molecular weight heparin sodium against DVT. Results After operation, 17 cases (10.5%) developed DVT, including 8 THA cases, 1 TKA case, and 8 HFS cases. Preoperative color doppler sonography showed no abnormal echo, normal b blood flow signal, and normal periodical variation in vein blood flow without regurgitation. Postoperative examinations showed that the vascular occlusion of common femoral vein and popl iteal vein occurred in DVT patients. There were significant differences (P lt; 0.05) in whole blood viscosity between patients with DVT and without DVT after 1, 14 days and no significant difference (P gt; 0.05) before operation and 7 days after operation. There was no significant difference (P gt; 0.05) in plasma viscosity and erythrocyte aggregation index, between patients with DVT and without DVT pre- and postoperation. There was significant difference (P lt; 0.05) in erythrocyte deformation index between patients with DVT and without DVT 14 days after operation. The preoperative plasma D-dimer levels of patients with DVT and without DVT were (372.00 ± 148.62) ng/mL and (369.00 ± 141.03) ng/mL, respectively, showing no significant difference (P gt; 0.05); the 3 days postoperatively levels were (574.00 ± 217.29) ng/mL and (391.00 ± 120.16) ng/mL, respectively, showing significant difference (P lt; 0.05). Conclusion Color doppler sonography in combination of hemorheology and plasma D-dimer examination can be beneficial for the early diagnosis of DVT in major orthopedic surgeries.

  • TISSUE ENGINEERED NERVE BASED ON PLASMA TREATMENT AND BMSCs

    Objective To evaluate the effect of the plasma treated PLGA nerve conduits seeded BMSCs on repairing SD rat sciatic nerve defects. Methods BMSCs were acquired from 30 newborn SD rats. After ampl ified and passaged for 3 times, PLGA nerve conduits were prepared and some of them were treated with plasma. A 1-cm-length sciatic nerve defect wasmade in 30 4-week-old SD rats, then they were randomly divided into 3 groups for three different nerve defects reconstruction methods (n=10). In the experimental group, defect was repaired by plasma treatment and PGLA nerve conduits seeded with BMSCs; in the control group, by normal PLGA nerve conduits seeded with BMSCs; and in the autologous group, by autologous nerve. At 6 weeks after the surgery, the dynamic walking pattern was recorded and the sciatic function index (SFI) was calculated; the electrophysiological test was taken; the gastrocnemius wet weight recovery rate was calculated; and the image analysis of regenerated nerve was made. Results All rats survived after the surgery and l ived to the end of the experiment. At 6 weeks after the surgery, the dynamic walking pattern of the experimental group and autologous group was better than that of the control group. The SFI value of the experimental, control and autologous groups was —51.02 ± 6.54, —58.73 ± 7.87 and —48.73 ± 3.95, respectively, showing statistically significant differences among the experimental group, control group and autologous group (P lt; 0.05). The results of the motor nerve conduction velocity and wave ampl itude showed that there were statistically significant differences between the experimental group and the control group (P lt; 0.05), and between the control group and the autologous group (Plt; 0.01); but no significant difference between the experimental group and autologous group(Pgt; 0.05); The gastrocnemius wet weight recovery rate of the experimental, control and autologous groups was 56.13% ± 4.27%, 43.14% ± 6.52%, 59.47% ± 3.85%, respectively; showing statistically significant differences among experimental group, control group and autologous group (P lt; 0.05). The density, diameter of regenerated nerve fiber as well as neural sheath thickness of the experimental group were all higher than those of the control group (P lt; 0.05) and lower than those of the autologous nerve group (P lt; 0.05); there was significant difference between the control group and the autologous group (P lt; 0.01). Conclusion Plasma treated PLGA nerve conduits seeded with BMSCs can effectively repair sciatic nerve defects and provide a new strategy for the development of tissue engineered nerve to repair the peripheral nerve defects.

  • Association between Plasma Homocysteine Level and the Prevalence of Coronary Heart Disease: A Meta-analysis

    Objective To evaluate the association between coronary heart disease (CHD) and plasma homocysteine level, and to provide additional information for prevention and management of CHD. Methods We searched CBM, CNKI, WanFang, and VIP databases. Case-control studies about the association between CHD and plasma homocysteine level published in China were identified. Meta-analysis was performed using RevMan 4.2 software. Results The result of meta-analysis showed the plasma homocysteine level in the CHD group was higher than that of the control group (WMD=4.88, 95%CI 4.40 to 5.35, Plt;0.000 01), and the loss of safety coefficient was 1 339. Conclusion High plasma homocysteine level is associated with increasing morbidity of CHD.

  • Detection and Clinical Significance of Methylation of Peripheral Plasma DNA in Patients with Hepatocellular Carcinoma

    Objective To evaluate the effect of methylation determination about the peripheral plasma DNA in diagnose of hepatocellular carcinoma (HCC) and select the highly sensitive and specific methylated cancer suppressor genes. Methods Methylation-specific PCR (MSP) was used to detect the degree of methylation about SLIT2 and DAPK genes in peripheral plasma and associated cancer tissues of 34 patients with HCC confirmed by pathology, then analyzed their relationship to clinicopathologic feature. Results The positive rate of the promoter methylation of SLIT2 and DAPK genes in cancer tissues in 34 cases were 70.6% (24/34) and 79.4% (27/34), while the relevant promoter methylation rate in plasma were 44.1% (15/34) and 50.0% (17/34) correspondingly. The sensitivity of detection of DNA methylation about SLIT2 and DAPK genes in plasma was 62.5% and 63.0%, respectively;both of the specificity for them were 100%. The negative predicted value was 52.6% and 41.2%, respectively;while both of the positive predicted value were 100%. There were no significant correlation between the clinicopathologic features and the methylation rate in cancer tissues and plasma (P>0.05). In plasma of patients whose AFP<400 μg/L, the positive rate of combined detection of DNA methylation of SLIT2 and DAPK was 61.1% (11/18). Conclusions The detection rate of DNA methylation of SLIT2 and DAPK genes in plasma is higher, and there is a significant correlation between the DNA methylation in HCC tissue and plasma, based on MSP method. DNA methylation in plasma, as an non-invasive method, could be used to diagnose HCC, especially for the patients whose AFP is negative. HBV infection may be only associate with DNA methylation of part gene.

  • Role of Vaspin in Roux-en-Y Gastric Bypass on Type 2 Diabetes Mellitus Rats

    Objective To study the therapeutic effect of Roux-en-Y gastric bypass (RYGB) on type 2 diabetes mellitus (T2DM) rats and explore the possible mechanism of vaspin in RYGB on T2DM. Methods Twenty SD rats with T2DM and 20 age- and sex-matched normal SD rats were randomly divided into 4 groups according to the random digits table:T2DM-RYGB group, T2DM-sham operation (SO) group,RYGB group,and SO group,10 rats in each group. Fasting plasma glucose (FPG) level,serum insulin (INS) level,vaspin level,and homeostasis model of insulin resistance (HOMA-IR) were determined before operation and on week 4,8 after operation,respectively.At the same time,the correlation between vaspin and the indicators (FPG,INS,or HOMA-IR) was analyzed.Results Compared the indicators after operation with before operation,the FPG level,INS level,vaspin level,and HOMA-IR were not significantly different between the T2DM-RYGB group and T2DM-SO group (P>0.05) or between the RYGB group and SO group (P>0.05),but the FPG level,INS level,vaspin level,and HOMA-IR in the T2DM-RYGB group and T2DM-SO group were significantly higher than those in the RYGB group (P<0.05) and SO group (P<0.05),respectively. On week 4 after operation,the FPG level,INS level,vaspin level,and HOMA-IR decreased in the T2DM-RYGB group,except for the FPG level,the other indexes had no significant differences as compared with the values before operation. On week 8 after operation,the FPG level,INS level,vaspin level,and HOMA-IR further decreased in the T2DM-RYGB group,there were significant differences of these indicators between before operation and on week 8 after operation. Compared the indicators after operation with before operation,the FPG level,INS level,vaspin level,and HOMA-IR were not statistically significant (P>0.05) in the T2DM-SO group,RYGB group,or SO group. The changes in serum vaspin level correlated positively with those in INS and HOMA-IR before operaion and on week 4,8 after operaion in the T2DM-RYGB group and T2DM SO group rats (P<0.05),respectively. Conclusions RYGB surgery has a therapeutic effect on T2DM rats,and serum vaspin level decreases and insulin resistance is improved after RYGB surgery,which may be one of the mechanisms of the treatment for T2DM.
