In order to satisfy demands of massive and heterogeneous tumor clinical data processing and the multi-center collaborative diagnosis and treatment for tumor diseases, a Tumor Data Interacted System (TDIS) was established based on grid platform, so that an implementing virtualization platform of tumor diagnosis service was realized, sharing tumor information in real time and carrying on standardized management. The system adopts Globus Toolkit 4.0 tools to build the open grid service framework and encapsulats data resources based on Web Services Resource Framework (WSRF). The system uses the middleware technology to provide unified access interface for heterogeneous data interaction, which could optimize interactive process with virtualized service to query and call tumor information resources flexibly. For massive amounts of heterogeneous tumor data, the federated stored and multiple authorized mode is selected as security services mechanism, real-time monitoring and balancing load. The system can cooperatively manage multi-center heterogeneous tumor data to realize the tumor patient data query, sharing and analysis, and compare and match resources in typical clinical database or clinical information database in other service node, thus it can assist doctors in consulting similar case and making up multidisciplinary treatment plan for tumors. Consequently, the system can improve efficiency of diagnosis and treatment for tumor, and promote the development of collaborative tumor diagnosis model.
ObjectiveTo review the current situation of total knee arthroplasty (TKA) used in young patients with osteoarthritis. MethodsThe recent literature in the treatment of osteoarthritis with TKA in young patients was extensively reviewed. The characteristics, curative effect, and postoperative satisfaction degree of TKA in young patients were analyzed and summarized. ResultsYoung patients have longer life expectancy and higher activity, which may lead to much higher expectation of the TKA. Comparing with elderly patients, young patients obtain equal or better effectiveness after TKA, but they are likely to not be satisfied with the effects because of low survival rate of the prosthesis. At present, continuous development of implant design, prosthesis material, and operation technique are in progress to reduce wear, hence to prolong the implant survivorship so as to meet the desire of young patients. However, the studies of the big samples and long-term follow-up are required to confirm the clinical advantages of such developments. ConclusionWith the trend of TKA in young patients, surgeons should have a good understanding of the patients' demands, select suitable prosthesis and give appropriate preoperative counseling to build up an objective expectation of curative effect, which will lead to a better doctor-patient relationship.
Multi-layer perceptron (MLP) neural network belongs to multi-layer feedforward neural network, and has the ability and characteristics of high intelligence. It can realize the complex nonlinear mapping by its own learning through the network. Bipolar disorder is a serious mental illness with high recurrence rate, high self-harm rate and high suicide rate. Most of the onset of the bipolar disorder starts with depressive episode, which can be easily misdiagnosed as unipolar depression and lead to a delayed treatment so as to influence the prognosis. The early identification of bipolar disorder is of great importance for patients with bipolar disorder. Due to the fact that the process of early identification of bipolar disorder is nonlinear, we in this paper discuss the MLP neural network application in early identification of bipolar disorder. This study covered 250 cases, including 143 cases with recurrent depression and 107 cases with bipolar disorder, and clinical features were statistically analyzed between the two groups. A total of 42 variables with significant differences were screened as the input variables of the neural network. Part of the samples were randomly selected as the learning sample, and the other as the test sample. By choosing different neural network structures, all results of the identification of bipolar disorder were relatively good, which showed that MLP neural network could be used in the early identification of bipolar disorder.
ObjectiveTo investigate clinical outcomes and prognostic factors of surgical resection of pulmonary metastases after esophagectomy. MethodsClinical data of 15 patients who underwent surgical resection of pulmonary metastases after esophagectomy from March 1994 to May 2008 were retrospectively analyzed. There were 10 males and 5 females with their age of 43-72 (65.0±8.8) years. Surgical procedures included partial lung resection, pulmonary wedge resection, segmental resection and lobectomy. Follow-up duration was 60 months after surgical resection of pulmonary metastases. The influence of number and size of pulmonary metastases, TNM staging of primary esophageal cancer, and disease-free interval (DFI) after esophagectomy on postoperative survival rate after pulmonary metastasectomy was analyzed. ResultsTwelve, 24 and 60 months survival rates after pulmonary metastasectomy were 80.0%, 66.7% and 6.7%, respec-tively. Median DFI was 30 months. Survival rate after pulmonary metastasectomy of patients whose DFI was longer than 24 months was significantly longer than that of patients whose DFI was shorter than 24 months (χ2=5.144, P=0.023). Survival rate after pulmonary metastasectomy of patients with solitary pulmonary metastasis was significantly longer than that of patients with multiple pulmonary metastases (χ2=3.990, P=0.046).The size of pulmonary metastases and TNM staging of primary esophageal cancer didn't have significant impact on survival rate after pulmonary metastasectomy (P > 0.05). Cox proportional hazards model showed that DFI after esophagectomy was the main factor affecting survival rate after pulmonary metastasectomy (P=0.026). ConclusionSurgical resection is a therapeutic strategy for the treatment of pulmonary metas-tases after esophagectomy, and may achieve good clinical outcomes for patients with solitary pulmonary metastasis and patients whose DFI is longer than 24 months.
