  • Human Recombinant Activated Protein C for Severe Sepsis: A Meta-Analysis

    Objective To systemically review the effectiveness and safety of human recombinant activated protein C (rhAPC) for severe sepsis. Methods Such databases as MEDLINE, EMbase, The Cochrane Library, VIP, CNKI and CBM were electronically searched for comprehensively collecting randomized controlled trials (RCTs) on the effectiveness and safety of human recombinant activated protein C (rhAPC) for severe sepsis from inception to July 2012. References of included studies were also retrieved. Two reviewers independently screened literature according to the inclusion and exclusion criteria, extracted data, and assessed the methodological quality of included studies. Then, meta-analysis was performed using RevMan 5.0 software. Results Totally, five RCTs involving 6 307 patients were included. The results of meta-analysis showed that, no significant difference was found in 28-day mortality (RR=1.00, 95%CI 0.84 to 1.19, P=1.00) and 90-day mortality (RR=1.00, 95%CI 0.87 to 1.14, P=0.96) between the rhAPC group and the placebo group. The results of subgroup analysis showed that, the two groups were similar in the 28-day mortality of patients with different Acute Physiology and Chronic Health Evaluation II (APACHE II) scores (APACHE II scorelt;25: RR=1.06, 95%CI 0.93 to 1.21, P=0.37; APACHE II score≥25: RR=0.93, 95%CI 0.69 to 1.24, P=0.60), and in the 28-day mortality by protein C deficiency class (APC deficiencylt;80%: RR=0.96, 95%CI 0.56 to 1.65, P=0.89; APC deficiencygt;80%: RR=0.61, 95%CI 0.34 to 1.08, P=0.09). Besides, bleeding risk in the rhAPC group was 1.62 fold more than that in the placebo group (RR=1.62, 95%CI 1.17 to 2.23, P=0.004). No significant difference was found in the incidence of adverse reaction (RR=1.04, 95%CI 0.92 to 1.18, P=0.53). Conclusion Current evidence suggests that, rhAPC could not improve the prognosis of patients with severe sepsis, but it significantly increases bleeding risk.

  • Surgical Treatment of Total Anomalous Pulmonary Venous Connection in Thirtyone Cases

    Objective To summarize the experience of the surgical treatment of total anomalous pulmonary venous connection (TAPVC) in 31 cases,so as to elevate the operative effect. Methods The total corrected operation under moderate hypothermic cardiopulmonary bypass were performed in 31 cases with TAPVC. Site of drainage was supracardiac (n=16),cardiac(n=13)and mixed(n=2). Other accompanying congenital cardiac malformations were diagnosed such as atrial septal defect(n=31), patent ductus arteriosus(n=4), pulmonic stenosis(n=1), mitral regurgitation(n=1), tricuspid regurgitation (n=15),etc. Results No hospital death was observed in the early period of operation (30 d). Complication occurred in 8 cases (25.8%), such as paroxysm of nodal rhythm(n=1), Ⅰ° atrial ventricular block(n=1), frequently atrial premature beats and paroxysm of auricular tachycardia(n=1),pulmonary infection(n=2),atelectasis(n=1), pneumatothorax (n=1), left diaphragmatic paralysis and pulmonary infection with tracheal reintubation (n=1),etc. All of them discharged after active therapy. All of 31 cases was followed up, followup time was 2.8±1.5 years. 30 cases were living (96.7%) and 1 case died of heart failure 8 months after operation. 2 cases had auricular arrhythmia with a bad medical effect. 28 cases(93.3%) had normal heart function and they could work and live normally after discharged. Conclusion Surgical procedure should be based on individual abnormality for TAPVC.Surgeons should take care not only of the stenosis of anastomotic stoma,but also of the arrhythmia after operation.Satisfied correction of the abnormality is important and the curative effect of those cases is approving. 

  • Measurement of Normal Renal and Related Effect Analysis: Study of CT/MRI in Vivo Adults

    Objective To explore the effect factors on the related measurement guidelines of renal area and renal cortex thickness by measurement of CT/MRI radiography in vivo kidney in adults. Methods Thickness of renal cortex (TC), cortical area (CA), parenchymal area (PA), as well as cortical faction (CF, cortical/parenchymal area) of 164 cases (106 cases with enhanced CT abdomen and 58 cases with MRI abdomen scanning) without renal disease was calculated bilaterally. All data were analyzed by SPSS 11.5 (the mean of two groups and multi-groups was compared by t test and analysis of variance, respectively).Results ① In CT scan, the mean value and 95% confidence interval of TC,CA,PA and CF were 0.62 (0.44 to 0.80) cm, 7.2 (4.1 to 10.2) cm2, 18.2 (10.7 to 25.7) cm2, 39.3 (30.3 to 48.3) % on the left, and 0.63 (0.43 to 0.83) cm, 7.3 (4 to 11) cm2, 18.1 (11 to 25.3) cm2, 39.9 (32 to 48) % on the right, respectively. Likewise, in MRI, those were 0.58 (0.33 to 0.83) cm, 7.5 (3.5 to 11.3) cm2, 14.8 (8.5 to 21.1) cm2, 50.2 (32.8 to 67.6) % on the left, and 0.55 (0.31 to 0.79) cm, 7.3 (4.4 to 10.3) cm2, 15.6 (10.1 to 21.1) cm2, 47.3 (30 to 65) % on the right. ② There was a significant difference in the value of TC, CA, PA between different gender and age groups, and were decreased with the age increaseing. ③ Most of the values measured by MRI were less than those by CT. Conclusions The study suggests that the values of TC, CA, PA and CF can well represent the renal size and function, and may offer a practical and significant normal standard in the radiological diagnosis.

  • The Expression of Inhibitor of Differentiation-1 in Hepatoma Tissues and Its Correlation with Prognosis

    目的 检测分化抑制因子1(Id-1)在肝细胞肝癌组织中和正常肝组织中的表达情况,了解Id-1与肝细胞肝癌患者预后关系。 方法 对获得的19例肝细胞肝癌组织标本和8例正常肝组织进行免疫组织化学染色,并对染色结果进行分级。借助SPSS软件,分析肝细胞肝癌组织标本与正常肝组织标本中的Id-1表达强度区别,Id-1与甲胎蛋白(AFP)相关性,及Id-1表达强度与肝癌患者预后之间的关系。 结果 免疫组织化学染色结果显示所有标本均表达为阳性,其中3例(+),7例(++),10例(+++)7例(++++),肝细胞肝癌组织标本与正常肝组织标本差异有统计学意义(P<0.05);Id-1表达与血液中AFP水平相关不显著(r=−0.121,P=0.621);Spearman等级相关分析显示患者生存时间与Id-1表达呈负相关(r=−0.567,P=0.011)。 结论 Id-1在肝癌组织中表达增高 ,Id-1表达水平与生存时间呈负相关,但和AFP无明显相关。

  • Effect of Laparoscopic and Open Abdominal Miles Surgeries on Perineal Wound Healing for Postoperative Patients with Rectal Cancer

    Objective To compare the effects between laparoscopic and conventional open abdominal approaches on perineal wound healing for patients with rectal cancer. Methods The clinical data of 54 patients undergoing laparoscopic abdominal perineal resection (laparoscopy group) and 48 patients undergoing open abdominal perineal resection (open group) between January 2008 and December 2009 in this hospital were collected. Parameters including class A healing rate of perineal wound, duration of surgery, duration of perineal resection, blood loss during operation, anterosacral drainage, and serum albumin level on day 3 after surgery were compared between two groups. Results There was no significant difference of gender, age, tumor Dukes stage, preoperative albumin, or preoperative complications between two groups (Pgt;0.05). The class A healing rate of perineal wound (χ2=5.54, Plt;0.05) and serum albumin level on the third day after surgery (t=3.92, Plt;0.01) in the laparoscopy group was significantly higher than those in the open group. In the laparoscopy group, duration of perineal resection (t=6.64, Plt;0.01), blood loss during operation (t=6.05, Plt;0.01), and anterosacral drainage (t=12.86, Plt;0.01) were less than those in the open group. Conclusion The higher class A healing rate of laparoscopic approach for the patients with rectal cancer might be associated with the minimal invasiveness, less blood loss, and shorter duration.

  • EXPERIMENTAL STUDY ON SURGICAL OPERATION AND HYPOPHOSPHATEMIA

    Objective To investigate the relationship between surgical operation and hypophosphatemia, to observe the possible damage of hypophosphatemia and to assess the value of postoperative phosphate supplementation. Methods Sixty four male SD rats were randomly divided into 2 groups, Group Ⅰ, drinking a specially prepared solution to reduce their phosphate storage, Group Ⅱ, drinking water as a control. All received common bile duct ligation 3 weeks later. The serum biochemical data including phosphate level were obtained before and after operation. Then half of rats in each group were supplied with NaH2PO4 5-day survival rates were analyzed with statistic methods and their vital organs were observed under electron microscope. Results The phosphate level of each group was descended after operation. The group with phosphate shortage before operation (group Ⅰ) had a greatest fall of phosphate and average arterial pressure. The phosphate-supplied rats had a minor change of vital organs under electron microscope scan and higher 5-day survival rate compared to others in this group.Conclusion Abdominal surgery may induce postoperative hypophosphatemia, especially when the phosphate has been lacking before operation. Severe hypophosphatemia, superimposed on surgical trauma, enhances the damage to the body. Prompt supplement of phosphate will improve the prognosis of surgical operation.

  • CT Features and Anatomic-Pathologic Bases of Secondary Pyogenic Peritonitis

    【Abstract】ObjectiveTo evaluate the main CT features, the morbidity of CT signs and the anatomic-pathologic bases of secondary pyogenic peritonitis (SPP). MethodsTwentyfour patients of the SPP were retrospectively studied. Emphasis was placed on the spiral CT manifestations of the SPP correlating with their anatomic-pathologic bases and the occurrence as well as the signs of primary lesions which resulted in the SPP. ResultsThe main CT manifestations of SPP revealed as follows: the thickened peritoneum, 16 in 24 cases (66.7%), of which 14 cases were smooth and 2 cases were irregular; the ascites, 15 in 24 cases (62.5%); the free air within peritoneal cavity, 9 in 24 cases (37.5%); the edema and thickening involved in the greater omentum, 8 in 24 cases (33.3%); the small bowel mesentery, 5 in 24 cases (20.8%); and the bowels’ wall, 5 in 24 cases (20.8%); the adhesions of bowels, 6 in 24 cases (25.0%). The CT manifestation of the promary lesions, which caused SPP, and the complications were shown as follows: the signs of primary lesion, 13 cases (54.2%); the inflammatory changes in retroperitoneal cavity 13 cases (54.2%); the involvements of chest 13 cases (54.2%); and the abscess in peritoneal and pelvic cavity 6 cases (25.0%). ConclusionThe main significant CT signs of SPP could be concluded as follows: thickened peritoneum, ascites, free air within peritoneal cavity, edematous and thickened greater omentum, the small bowel mesentery, and the bowels’ wall, as well as the adhesions of bowels. So, the CT scan can present plenty of CT signs, which are significant and very helpful for making an appropriate diagnosis of SPP.

  • Surgical treatment of tracheal or bronchial stenosis related to severe congenital heart disease

    Objective To explore a new surgical treatment of patients with severe congenital heart disease associated with tracheal or bronchial stenosis. Methods We retrospectively analyzed clinical data of 32 patients with severe pulmonary blood increased congenital heart disease complicated with tracheal or bronchial stenosis in our hospital between June 2010 and June 2014. There were 18 males and 13 females with an average age of 14.6±4.0 months and a weight of 8.8±3.0 kg. Results Average hospital stay was 12 days. Duration of intraoperative cardiopulmonary bypass was 65.0±21.0 min. Duration of aortic interruption was 42.0±16.0 min. Duration of postoperative ventilator was 25.0±18.0 h. ICU retention time was 4.0±1.8 d. All patients were survived. On the 9th day after surgery, the X-ray chest film revealed that the cardio-thoracic ratio was reduced compared with pre-operation but no statistical significance (P>0.05). Left ventricular end-diastolic dimension (LVEDD) was decreased (P<0.05). Pulmonary artery pressure was lower (P<0.01). The velocity of bloodstream in pulmonary artery was approximately normal. The computed tomography angiography (CTA) on the 9th day after surgery revealed that diameter of pulmonary artery in all patients reduced compared with that of pre-operation (P<0.01). Tracheal diameter of 28 patients returned to normal level and 3 patients had residual mild tracheal stenosis. All preoperative atelectasis recovered to normal level. All patients recovered completely. When the patients were followed up for 6 months to 4 years, the patients grew healthily and no abnormity occurred. Conclusion For the patients with tracheal or bronchial stenosis related to severe congenital heart disease,it is necessary to adopt surgical procedure to relieve the pressure of trachea or bronchus.The bronchial decompression surgery is a perfect and effective method for the patients with tracheal or bronchial stenosis related to severe congenital heart disease.
