ObjectiveTo compare the cerebral protective effect of unilateral and bilateral antegrade selective cerebral perfusion during total aortic arch replacement, particularly with respect to neuropsychological outcome.MethodsFrom June 2003 to March 2004, 16 patients who underwent total aortic arch replacement were randomly allocated to one of two methods of brain protection: unilateral antegrade selective cerebral perfusion (unilateral group, n =8) or bilateral antegrade cerebral perfusion (bilateral group, n =8). Preoperative and postoperative neurological examination, brain computed tomography(CT) scan, and cognitive function tests were performed.ResultsAll patients survived the operations and were discharged from hospital. No new brain infarction occurred. Transient neurologic dysfunction occurred in 1 patient of each group. There were no intergroup differences in the scores of preoperative and post operative cognitive function ( P gt;0.05).ConclusionBoth methods of brain protection for patients undergoing total aortic arch replacement result in favorable and similar effect of brain protection in term of cognitive function provided the circle of Willis is patent and collateral flow is adequate.
ObjectiveTo discuss the efficacy of type Ⅱ hybrid aortic arch repair for type A aortic dissection in patients of different age groups.MethodsWe retrospectively analyzed the clinical data of 126 patients with type A aortic dissection admitted to the Fuwai Hospital between January 2016 and December 2018, including 78 (61.9%) males and 48 (38.1%) females, with an average age of 61.8±6.9 years. The patients were divided into an elderly group (≥60 years, n=82) and a non-elderly group (<60 years, n=44). The preoperative, intraoperative and postoperative data of patients in the two groups were compared.ResultsThe age between the elderly and non-elderly group was significantly different (65.9±4.1 years vs. 54.3±4.1 years, P<0.010), and no significant difference was found between the two groups in other preoperative baseline data. There were 6 (4.8%) patients of early death, 3 (2.4%) patients of stroke and 2 (1.6%) patients of paralysis. A total of 194 stents were implanted, and the average dimeter of the stents was 33.6±1.8 mm and the average length was 199.0±6.7 mm. The non-elderly group had shorter mechanical ventilation time (31.9±41.7 h vs. 61.0±89.2 h, P=0.043) and ICU stay time (77.8±51.4 h vs. 143.1±114.4 h, P<0.001) than the elderly group. There was no significant difference in in-hospital mortality rate, reoperation rate or survival rate between the two groups (P>0.05). Follow-up time was 1-43 (22.6±10.8) months, and 3 patients were lost. There were 104 (82.5%) patients of complete thrombus formation of false lumen in stent and endoleak was reported in 11 (9.2%) patients.ConclusionType Ⅱ hybrid aortic arch repair offers an alternative approach to acute type A aortic dissection with acceptable early and mid-term clinical effects. The non-elderly patients have a similar early treatment effect to the elderly patients, but have a better mid-term outcome.
ObjectiveTo explore the risk factors and predictive value of acute kidney injury (AKI) after total aortic arch replacement.MethodsThe clinical data of patients undergoing total aortic arch replacement in our hospital from January 2018 to June 2019 were retrospectively analyzed, and patients receiving preoperative renal replacement therapy and missing creatinine values were excluded. According to whether postoperative AKI occurred, patients were divided into an AKI group and a control group. The univariate and multivariate analyses (logistic regression) were used to explore the independent risk factors of AKI. The receiver operating characteristic curve was used to analyze the significant factors in predicting the occurrence of AKI after total aortic arch replacement.ResultsA total of 162 patients were included in the study, including 135 (83.3%) males and 27 (16.7%) females, with an average age of 52.61±9.90 years (range: 22 to 73 years). The incidence of AKI was 68.5% (n=111). The results of univariate and multivariate analyses showed that the postoperative serum cystatin C level (OR=76.145, 95%CI 15.575-372.260, P<0.01) was an independent risk factor for AKI after total aortic arch replacement. When its cut-off value was above 1.08 mg/L, the specificity for predicting postoperative AKI was 70.59%, and the sensitivity was 85.59%.ConclusionThe postoperative cystatin C level is an independent risk factor for AKI after total aortic arch replacement and has predictive value.
Objective To investigate the correlation between D-dimer level within 2 hours on admission and early in-hospital major adverse events (MAEs) in patients with acute type A aortic dissection undergoing arch replacement and the frozen elephant trunk (FET) implantation. Methods The patients with acute type A aortic dissection undergoing arch replacement and the FET implantation, who hospitalized in our hospital from September 2017 to December 2022, were included in this retrospective study. Grouping based on the occurrence of in-hospital major adverse events (MAEs) after total arch replacement and FET implantation, with no in-hospital MAEs as a control group and in-hospital MAEs as an observation group. The perioperative data were compared between the two groups. Univariate and multivariate analyses were used to investigate the risk factors for MAEs (in-hospital mortality, gastrointestinal bleeding, paraplegia, acute kidney failure, reopening the chest, low cardiac output syndrome, cerebrovascular accident, respiratory insufficiency, multiple organ dysfunctionsyndrome, gastrointestinal bleeding, and severe infection). Receiver operating characteristic (ROC) curve was used to evaluate the predictive value of the prediction area under the ROC curve (AUC). Results Finally 218 patients were collected, including 157 males and 61 females with an average age of 51.54±9.79 years. There were 152 patients in the control group and 66 patients in the observation group. In-hospital mortality was 2.8% (6/218). The level of D-dimer, lactic acid, cardiopulmonary bypass time, aortic cross-clamping time, ventilator-assisted time and ICU stay in the observation group were higher or longer than those in the control group (P=0.013). Multivariate logistic analysis showed that D-dimer (OR=1.077, 95%CI 1.020-1.137, P<0.05) was an independent risk factor for MAEs in hospital. The level of D-dimer within 2 hours admission predicted that the AUC of MAEs in hospital was 0.83 (95%CI 0.736-0.870, P<0.05), and the optimal critical point was 2.2 μg/mL, with sensitivity and specificity of 84.8% and 73.0%, respectively. Conclusion Increased D-dimer levels at admission are associated with early in-hospital MAEs in the patients with acute type A aortic dissection undergoing arch replacement and FET. These results may help clinicians optimize the risk evaluation and perioperative clinical management to reduce early adverse events.
Objective To investigate the surgical approach and efficacy of reconstruction of the isolated left vertebral artery (ILVA) in single upper hemisternotomy for total aortic arch replacement. Methods From March 2017 to October 2023, patients who underwent total aortic arch replacement under single upper hemisternotomy in General Hospital of Northern Theater Command were selected. According to the presence of ILVA, they were divided into a conventional group and an ILVA group. All the ILVA group underwent intraoperative ILVA reconstruction. The perioperative clinical data between two groups were compared. Results A total of 504 patients were collected, including 471 males and 31 females, with an average age of 50.4±11.4 years. There was no increase in the duration of cardiopulmonary bypass or postoperative hospitalization in the ILVA group (n=31) compared to the conventional group (n=473), and the rates of perioperative complications and in-hospital mortality were not significantly different between the two groups. There were 2 (6.45%) patients of acute cerebral infarction and 2 (6.45%) patients of perioperative death in the ILVA group, with no spinal cord injuries. Conclusion ILVA reconstruction during total aortic arch replacement in single upper hemisternotomy is feasible, safe, and effective, and prioritizing off pump ILVA-left common carotid artery transposition.
