  • Comparison of fundus fluorescein angiography and indocyanine green angiography for imaging of subretinal hemorrhage in age-related macular degeneration

    ObjectiveTo compare the effects of fundus fluorescein angiography (FFA) and indocyanine green angiography (ICGA) on classifications and locations of choroidal neovascularization (CNV) located under subretinal hemorrhage in age-related macular degeneration, and on the areameasuring of subretinal hemorrhage.MethodsThe medical record of 177 patients with subretinal hemorrhage associated with AMD confirmed by photochrome of ocular fundus, FFA and ICGA examinations were retrospectively reviewed. The locations and classifications of CNV on the images of FFA and ICGA in the 177 patients and the area of subretinal hemorrhage of 30 patients randomly selected from the 177 patients were analyzed and measured. ResultsOn the images of FFA, the locations and classifications of CNV could be defined in 160 patients (90.4%), and CNV was considered eligible for laser therapy in 24 patients (13.6%). On the images of ICGA, the locations and classifications of CNV could be defined in 175 patients (98.9%),and CNV was considered eligible for laser therapy in 51 patients (28.8%). There was no statistical difference of the locations(Z=-0.383,P=0.701) and classifications ( χ2=2.993,P=0.810) of CNV on the images of FFA and ICGA. The areas of blocked fluorescence measured on the images of FFA (x-=26.610 mm2 and M=13.548 mm2) were larger than those of ICGA (x-=24.714 mm2,M=12.875 mm2) with statistical differences (Z=-3.000,P=0.003) between FFA and ICGA. ConclusionsICGA is beneficial for imaging CNV located under subretinal hemorrhage, and may increase the number of the patients who are considered eligible for laser treatment. The effect of measurement of the area of subretinal hemorrhage in AMD evaluated by FFA is better. (Chin J Ocul Fundus Dis,2003,19:149-151)

  • Effect of age-related maculopathy on the quality of life

    Objective To assess the vision-related quality of life(VRQL) in patients with age-related maculopathy (AMD) and the related factors in Shanghai. Methods The VRQL of 105 patiens with AMD and 105 without (the control group) in Shanghai was investigated via Chinese-version low vision quality of life questionnaire. Resutls The Cronbach α coefficient of the questionnaire is 0.97 in the patients with AMD, whose scores of the various quality of life were significantly lower than those in the control group. The more the exudative defect happened, or the longer the disease course developed, the lower the sccores were. All the scores of the scales and the total questionnaire were significantly related to the better or worse monocular visual acuity. The better monocular visual acuity was the first independent risk factor affecting all aspects of VRQL in pateints with AMD. Conclusions Decreased visual acutiy may gradually impair the VRQL in patients with AMD. Chinese-version questionnaire of low vision quality of life can provide the general information of the VRQL in patients with AMD.(Chin J Ocul Fundus Dis,2004,20:303-306)

  • Characteristics of exudative age-related macular degeneration optical coherence tomography

    Objective To verifying the characteristics of optical coherence tomography(OCT) in exudative age-related macular degeneration (AMD). Methods The patients being investigated in this series included 16 cases (19 eyes) of exudative age-related macular degeneration diagnosed by FFA and OCT examinations, among them 4 cases (6 eyes) were examined with ICGA. The color photographs of ocular fundi, FFA, ICGA and OCT were investigated by contrasting each other. Results As compared with the FFA and ICGA examinations, the characteristic findings found in OCT in patients with exudative AMD in this series were as the following:①serous detachment of neurosensory epithelium in 11 eyes,②retinal hemorrhage in 2 eyes,③serous detachment of retinal pigment epithelium in 5 eyes,④hemorrhagic detachment of retinal pigment epithelium in 10 eyes,⑤disciform scar in 4 eyes,⑥fibrovascular pigment epithelial detachment and occult CNV in 12 eyes. Conclusion OCT can supply a comprehensive survey of exudative AMD, in making the diagnosis as an important complementary examination of FFA and ICGA. (Chin J Ocul Fundus Dis,2000,16:220-223)

  • Histopathologic changes of macula lutea retinae in elderly Chinese autopsy eyes

    Purpose To investigate the histopathologic changes of macula lutea retinae in the elderly Chinese population,and to provide information for the cause of visiual disturbance in an autopsy study. Methods Two hundred and twelve eyes from 108 consecutive cases of postmortem (mean age of 78.4 years old) sections of the area of macula lutea retinae were studied by histopathology. Results Among the 212 eyes,hard drusen were found in 36.3% of eyes and soft drusen in 19.3% of eyes.The eyes with both of the above mentioned 2 types of drusens were found to be combined with RPE atrophy and RPE detachment;and subretinal neovascularization were found in 5.7% of these eyes and they were associated with obvious RPE atrophy and photoreceptor loss;3.2% eyes had posterior scleral staphyloma showing thinning of the sclera and choroid,RPE atrophy and apparent photoreceptor loss;2.4%eyes showed CME with the history of cataract surgery. Conclusion The macular affections,age-related macular degeneration,myopic macular degeneration and post-perative cystoid macular edema are varying in number in the above mentioned order successively,in elderly Chinese at an autopsy study.   • CLASSIFICATION AND AGE OF COATS DISEASE

    PURPOSE:Toinvestigate the classification and the age incidence of Coats disease. METHODS:75 cases(79 eyes)of Coats disease were analysed,which were diagnosed by fundus fluorescein angiograpy(FFA)between Jan.1981 and Dec.1994. RESULTS:The patients included 59 males and 16 famales.Average age of the first presentation was 26 years old,and there were 45(60%)cases aged from 19 to 60 years old.In 5 eyes,the ophthalmoscopic examination revealed no obviously abnormal retinal blood vessel,but a number of dilated retinal vessels and microaneuryms were demonstrated by FFA. CONCLUSIONS:The autors consider that there is no essential distinction between type Ⅰand type ⅡCoats disease and it is no longer suitable to apply the traditional classification.Coats disease affects not only juvenile patients but also persons of all ages.   • 玻璃体视网膜交界面超微结构及其年龄相关性改变

    玻璃体视网膜交界面由玻璃体基底部、玻璃体后皮质与视网膜内界膜构成。玻璃体基底部借Ⅱ型胶原垂直插入内界膜形成紧密连接,玻璃体后皮质区则经"分子胶"模型、新糖蛋白模型及核纤层蛋白细胞模型形成相对松散的连接。随着年龄增长,玻璃体基底部后缘会逐渐向后延伸形成新的紧密连接,而玻璃体后皮质区则会由于内界膜增厚、基质降解酶浓度升高、自由基累积等致使玻璃体视网膜交界面粘连作用减弱,甚至形成玻璃体后脱离。玻璃体黄斑牵拉综合征、黄斑裂孔、孔源性视网膜脱离等玻璃体视网膜交界面疾病均被证实与玻璃体视网膜交界面状态密切相关。正确认识玻璃体视网膜交界面超微结构及年龄相关性改变是了解玻璃体视网膜交界面疾病的基础。

  • 青少年儿童黄斑前膜与成人黄斑前膜的比较

    黄斑前膜(MEM)多发生在50岁以上人群;青少年儿童MEM较为罕见。MEM根据病因分为特发性MEM(IMEM)和继发性MEM(SMEM)。成人MEM以IMEM多见;青少年儿童MEM以SMEM多见。在玻璃纸样反光(CMR)型和黄斑前纤维增生(PMF)型MEM中, 青少年儿童MEM中PMF型较CMR型多见。光相干断层扫描发现青少年儿童前膜组织与黄斑分离的比例低于成人。目前, 对于青少年儿童和成人MEM治疗尚无统一标准, 对于明显视力下降、严重视物变形者多采用玻璃体切割手术, 手术后多数患眼视力提高。手术后MEM复发原因成人多为前膜组织剥除不完全, 而青少年儿童MEM的复发机制尚无文献报道。进一步探索青少年儿童EME复发机制, 改进治疗方式是今后努力的方向。

  • Correlation study between retinal thickness and age, gender of normal children in the urban of Beijing

    ObjectiveTo determine the retinal thickness of normal children 3-6 years old and its relationship with the age and gender. MethodsIn a cross-sectional study, 480 eyes of 240 normal preschool children including 115 male and 125 female, ages 3 to 6 years in the urban of Beijing, China were included. The average age was (4.93±0.77) years old. The visual acuity, slit-lamp microscopy and frequency domain optical coherence tomography (FD-OCT, Optvue, Inc. USA) were examined. The retinal thickness of the macular fovea and 500, 750, 1500 μm from temporal and nasal side around the fovea were measured. 32 eyes were excluded from the study because they couldn't cooperate. Pearson correlation analysis was used to determine the correlation between age and macular retinal thickness. Independent samples group t-test was used to compare the differences between boys and girls. ResultsThe mean thickness of macular fovea was (169.10±20.587) μm. The mean macular thickness of boys was significantly higher than girls (t=-4.549, -6.167, -5.492, -5.163, -6.749, -7.494, -6.874; P≤0.001). The mean thickness of 500 μm and 750 μm from nasal side of macular fovea were significantly higher than temporal side (t=5.594, 15.778, 7.678, 18.180; P < 0.001). There was no significant relevance between macular thickness and age. ConclusionsThe mean macular thickness of boys is significantly higher than girls in normal children in the urban of Beijing. There is no significant relevance between macular thickness and age.

  • Correlation between macular blood flow density, structure and age in normal eyes

    ObjectiveTo observe the changes of blood flow density in the macular area of normal eyes, and to analyze its correlation with age. MethodsA cross-sectional study. Two hundred and fifty normal healthy subjects (125 males and 125 females, aged 44.76±14.77) in routine ophthalmologic examination at the Department of Ophtalmology of Guangdong Provincial People’s Hospital during June 2017 to June 2018 were enrolled. Among them, 20 to 29, 30 to 39, 40 to 49, 50 to 59, and ≥ 60 years old were 50 subjects (50 eyes) in each. BCVA, slit lamp microscope, indirect ophthalmoscope, OCT angiography (OCTA) examinations were conducted for all eyes. The subjects were examined by both eyes, and the data of 1 eye was selected by EXCEL to generate random numbers, including 126 right eyes and 124 left eyes. The range of 6 mm × 6 mm in the macular area was scanned using a frequency domain OCTA instrument. The software automatically divides it into three concentric circles centered on the macular fovea, which were foveal area with a diameter of 1 mm, parafoveal area of 1 to 3 mm, and foveal peripheral area of 3 to 6 mm. The blood flow density of superficial capillary vessel, deep capillary vessel and foveal avascular area (FAZ) within a 300 μm width (FD-300), FAZ area, perimeter (PERIM), non-circularity index, center retinal thickness (CRT) were measured. The relationship between the blood flow density in macula, CRT, FAZ and age was analyzed by Pearson correlation analysis. ResultsThe mean blood flow density of superficial capillary vessel and deep capillary vessel were (51.61±2.54)% and (54.04±5.46)%, respectively. The average FD-300, CRT, PERIM and non-circularity index were (285.55±12.13) μm, (2.150±0.367) mm, 1.10±0.04, respectively. The relevance of the results showed that the age was negatively correlated with the blood flow density of whole area (r=−0.335, −0.279; P<0.01), parafoveal area (r=−0.255, −0.368; P<0.01), foveal peripheral area (r=−0.330, −0.269; P<0.01) in superficial capillary vessel and deep capillary vessel as well as FD-300 (r=−0.311, P<0.01), but not correlated with the blood flow density of foveal area (r=−0.071, −0.118; P=0.264, 0.064). There was no relationship between the age and the FAZ area, PERIM, non-circularity index (r=−0.070, −0.055, 0.074; P=0.267, 0.385, 0.142). The age was negatively correlated with the average CRT (r=−0.217, P<0.01), but not correlated with the CRT in foveal area (r=0.115, P=0.068). The CRT was positively correlated with the blood flow density of superficial capillary vessel and deep capillary vessel in foveal area (r=0.715, 0.653; P<0.01), but negatively correlated with the FAZ area (r=−0.669, P<0.01). ConclusionThe capillary blood flow density of macular area in the normal eyes decreases with age.
