  • Changes of urotensin Ⅱ level in induced sputum of patients with asthma

    Objective To investigate the role of urotensin Ⅱ(UⅡ) in the pathogenesis of asthma.Methods Lung function,differential cell count and level of UⅡin induced sputum were studied in 26 asthmatic patients in acute exacerbation and in clinical remission.Results Induced sputum UⅡ level from acute asthma was higher than that of remissed asthma [(58.88±47.38)pg/mL vs (12.69±12.78)pg/mL,Plt;0.01].Induced sputum UⅡ levels of asthma patients in acute exacerbation had a tendency to increase as disease deteriorated,which negatively correlated with FEV1% predicted (r=-0.326,Plt;0.05),but did not with sputum total cell and neutrophil counts(Pgt;0.05).No significant difference of induced sputum UⅡ levels was found between male and female,smokers and non-smokers.Conclusion UⅡ may play a role in acute exacerbation of asthma

  • Simvastatin Improves Pulmonary Function and Vascular Endothelial Growth Factor Level in Induced Sputum of Patients with COPD Exacerbation

    Objective To investigate the effects of simvastatin on pulmonary function and vascular endothelial growth factor ( VEGF) levels in induced sputumof patients with COPD exacerbation( AECOPD) .Methods Thirty-eight patients with AECOPD were divided into two groups randomly, ie. a routine medical treatment( RT) group( n =30) and a routine + statin medical treatment( RST) group( n =28) . The VEGF levels in serumand induced sputum were detected by ELISA on the first day and after a week treatment in hospital, respectively. Meanwhile, the pulmonary function measurements were performed. Results There were no significant differences in the pulmonary function ( FEV1% pred and FEV1 /FVC) and VEGF levels in induced sputumbetween the two groups before treatment( P gt;0. 05) . The RT group showed no significantchanges in any parameters before and after a week treatment( P gt; 0. 05) . FEV1% pread, FEV1 /FVC and VEGF levels in induced sputum in the RST group after a week treatment significantly increased compared with those before treatment and the RT group( P lt;0. 01, P lt;0. 01, P lt;0. 05) . But There were no significant differences in serumVEGF levels between the two groups before and after a week treatment. The VEGF levels in induced sputum were positively correlated to FEV1% pread and FEV1 /FVC after a week treatment( r =0. 430, P lt;0. 05; r = 0. 388, P lt; 0. 05) . Conclusions Simvastatin may reduce the decline in pulmonary function and decrease the levels of VEGF in induced sputum of patients with AECOPD. Improvement in pulmonary function may be related to down-expression of lung VEGF

  • Characteristics of Neutrophilic Asthma:A Clinical Investigation

    Objective To investigate the clinical characteristics of neutrophilic asthma ( NA) .Methods NA patients were collected from the out-patient and in-patient departments of Respiratory Diseases of Xinqiao Hospital between January 2010 and December 2010. The results of the medical records,pulmonary function tests, and induced sputum cytology were analyzed retrospectively. Results The NA patients with neutrophil percent ≥ 61% accounted for 33. 1% ( 51 /154 ) of all the asthmatics patients detected by induced sputumin the same period, and 45 cases with complete records were included. Of the 45 cases recruited, 20 cases ( 44. 4% ) were in-patients,2 cases ( 4. 4% ) were with controlled asthma, 3 cases ( 6. 7% ) were with cough variant asthma, 30 cases ( 66. 7% ) were female, 12 cases ( 26. 7% ) were atopic patients, and 27 cases ( 60% ) had acute exacerbation. The age of onset of 35 patients ( 77. 8% ) were after 12 years. FEV1% pred lt; 60% and gt; 80% was obtained in 55. 9% ( 19/34) and 14. 7% ( 5 /34) of patients respectively. The result of bronchodilator test was positive in 64% ( 16/25) of patients, and mean increase in FEV1 was 11. 7% . The percentage of neutrophil and eosinophil was ( 74. 5 ±9. 1) % and ( 2. 4 ±2. 5) % respectively in induced sputum, and 35. 6% ( 16/45) of the patients had increased eosinophil percentage ( gt;3% ) . Conclusions In our study, most of NA is severe and acute exacerbation asthma, and its clinical features are various. The mechanismand clinical significance of increased neutrophils in asthmatic patients are unclear and more studies are needed.

  • Sputum induction:methodologyand clinicalapplicationsinassessment of airways inflammation

    诱导痰(Is)是以高渗盐水或其他诱导物雾化吸入诱导无痰或少痰受检者产生足量痰液,以对气道分泌物的细胞及其他液相成分进行分析研究的一种检测方法,具有经济、无创、准确、重复性较好等优点,能客观地反映气道状态,而且与气道活检的病理改变基本一致,能早期敏感地发现病情变化,并可借此深入研究呼吸系统疾病发病机制及治疗干预。目前,这种检测技术已较成熟,并被广泛应用。 1958年Bickerman等首次建立了诱导痰检测方法,对不能自然咳痰的患者进行高渗盐水雾化获得痰液,利用二硫苏糖醇(DTr)等黏液裂解剂对痰进行处理,消除黏液的影响,提高了肺部肿瘤患者癌细胞的检出率。随后又应用于结核和条件致病菌引发的肺部炎症的诊断。1992年,Pin等首次将诱导痰技术应用于哮喘气道炎症的研究,随后该技术在 COPD、慢性咳嗽、肺间质纤维化等临床研究中得到进一步应用,分析项目也从单一的细胞和病原体等有型成分的检测发展为对痰液液相成分的检测。近年来,越来越多的研究利用诱导痰技术对多种呼吸道疾病的发病机制、诊断与治疗进行研究,逐渐扩大了该技术的应用范围。本文主要阐述诱导痰细胞学检查技术的方法学及临床应用。

  • Changes of γ-glutamyleysteine synthetase an d reduced glutathione in patients with bronchial asthma

    Objective To explore changes of 3’-glutamylcysteine synthetase( γ-GCS)and reduced glutathione(GSH)in patients with bronchial asthma.Methods Ten patients with acute asthma were enrolled and treated for six weeks according to guideline recommendations.Levels of -GCS,GSH and malondialdehyde(MDA)in total cells in induced sputum and GSH,MDA,reactive oxygen(ROS)in selum were measured and compared before and after therapy.Ten healthy volunteers were as normal contro1.Meanwhile,the pulmonary function(FEVl%pred)was measured and asthmatic symptoms were quantified using Hogg’s way.Results A.In serum and sputum of the asthma patients,GSH were lower and MDA were higher before treatment than those of the control(Plt;0.01).And -GCS in induced sputumwere higher before treatment than those of contro1.B.After treated for six weeks.levels of GSH in serum and sputum of the asthma patients increased copmpared to baseline(all Plt;0.01),but were still lower than that of control(Plt;0.05).Activities of MDA in serum and sputum and -GCS in sputum were elevated compared to baseline(Plt;0.01),but still higher than that of control(all Plt;0.05).C.Levels of GSH in serum of all patients were correlated negatively witll asthmatic symptom scores and levels of MDA and ROS(r=-0.701,-0.901,-0.878;Plt;0.05,lt;0.01,lt;0.01).There was a positive relationship between levels ofGSH in serum and FEV1%pred(r=0.854,Plt;0.01).In induced sputum,activities of 3’-GCS in all patients was correlated positively with their asthmatic symptom scores and level of MDA f r=0.804,0.926;Plt;0.05,lt;0.叭).Conclusion γ-GCS and GSH may participate the reaction of

  • Cytologic Profile of Induced Sputum and Its Relationship with Treatment Response in Patients with Chronic Obstructive Pulmonary Disease

    ObjectiveTo explore the cytologic profile of induced sputum and its relationship with the treatment response in patients with chronic obstructive pulmonary disease (COPD). MethodsSixty-five treatment-naive patients with COPD and 26 normal subjects were recruited for the study. Sputums induced by the inhalation of hypertonic saline were collected, and the associations of differential cell counting were analyzed with pulmonary function, modified Medical Research Council dyspnea scale, St. George's Respiratory Questionnaire score (SGRQ) before and after the treatment with inhaled corticosteroid and long-acting β2-agonist. ResultsThe cell percentages of neutrophil (Neu), macrophage, eosinophil (Eos) and lymphocyte in induced sputum of the COPD patients were (86.24±15.04)%, (5.75±6.96)%, (4.71±4.79)%, and (1.30±1.09)%, respectively. The eosinophil percentage (Eos%) was≥3% in 31 patients (60.78%). The neutrophil percentage (Neu%) was inversely correlated with forced expiratory volume in 1 second (FEV1), percent of predicted value of FEV1 (FEV1% pred), forced vital capacity (FVC), and percent of predicted value of FVC (FVC% pred) (P < 0.01, respectively), and positively correlated with the SGRQ symptom score (r=0.304, P=0.034). The Eos% was inversely correlated with FEV1/FVC ratio (r=-0.399, P=0.004). The patients with Eos%≥3% improved significantly in FEV1 and symptom score (P < 0.05, respectively) than the patients with Eos% < 3%. ConclusionsAn eosinophilic airway inflammation is present in a subgroup of COPD. The Eos% is inversely correlated with pulmonary function and may be a predictive indicator of response to treatment with inhaled corticosteroids and long-acting β2-agonists.

  • 16S rRNA-base analysis of bacterial diversity in the induced sputum of patients with acute exacerbation of chronic obstructive pulmonary disease

    ObjectiveTo explore the characteristics of induced sputum microbiome in the patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD).MethodsInduced sputum samples from 55 patients with AECOPD and 45 patients with stable COPD were analyzed by sequencing of 16S rRNA gene. Microbiota was measured by alpha diversity, beta diversity and LDA effect size analysis (LefSe).ResultsThe microbiome diversity of induced sputum in the AECOPD group was lower than that in the stable COPD group. The microbiome richness in the AECOPD group was higher than that in the stable COPD group. The microbiome structure changed in the AECOPD group compared with the stable COPD group. The proportion of some common pathogens got enriched. The levels of hypersensitive C reactive protein (hs-CRP), interleukin-8 (IL-8), tumor necrosis factor alpha (TNF-α) and Global Initative for Chronic Obstructive Lung Disease (GOLD) grade were negatively related to the diversity of microbiome in the AECOPD group.ConclusionsThe microbiome diversity of induced sputum in AECOPD patients is decreased, and is negatively correlated with the levels of hs-CRP, IL-8, TNF-α and GOLD grade. There are differences in the microbiome structure between AECOPD and stable COPD patients. Some enrichment of common pathogens are found in the induced sputum of patients with AECOPD. These results suggest that there is a significant bacterial dysbiosis in patients with AECOPD.
