  • 局部麻醉药在腹腔镜中胆囊切除术的应用

    摘要:目的:探讨通过腹腔内注射局部麻醉药在腹腔镜胆囊切除术中的镇痛效果。方法:52例患者随机分为4组,Ⅰ组术前腹腔内喷洒0.75%罗哌卡因20 mL;Ⅱ组术后腹腔内喷洒0.75%罗哌卡因20 mL;Ⅲ组为术后腹腔内喷洒0.5%布比卡因20 mL;Ⅳ组术后腹腔内喷洒生理盐水20 mL。麻醉方法均为全凭静脉麻醉。术后1、2、3、4 h 4个时间点记录患者术后疼痛视觉模拟评分(VAS)。并观察4组术后使用镇痛药物的例数和肩背痛、恶心呕吐发生率。结果:术后1、2、3、4 h VAS评分Ⅱ组lt;Ⅲ组lt;Ⅰ组lt;Ⅳ组(Plt;0.05)。Ⅳ组使用镇痛药物的例数明显高于Ⅰ组、Ⅱ组和Ⅲ组(Plt;0.05)。4组术后肩背痛、恶心呕吐发生率差异无显著性(Pgt;0.05)。结论:经腹腔给局麻药镇痛效果明显,术毕给药镇痛效果优于术前给药,罗哌卡因镇痛效果优于布比卡因。Abstract: Objective: To investigate the effect of intraperitoneal local anesthetic on patients undergone laparoscopiccho1ecystectomy.Methods:Fiftytwo patients were randomly divided into four groups. Group Ⅰ received preoperational anesthetic spary with 20 mL of 0.75% ropivacaine. Group Ⅱ was given the anesthetic ata same dosage after the operation. Group Ⅲ received preoperational anesthetic spary with 20 mL of 0.5% bupivacaine. Group Ⅳ received preoperational anesthetic spary with 20 mL saline. The LC was completed under general anesthesia.After the operation,visual analog scale (VAS)was recorded at 1,2,3 and 4 hours to evaluate the degree of postoperative pain.Meanwhile,the number of the patients who received anesthetics after the surgery,as well as the incidence rates shoulder or back pain and nausea or vomiting were recorded. Results: Postoperative VAS of the group Ⅱ was significantly lower than that of the other three groups, while the VAS of group Ⅲ was significantly lower than that in group Ⅰ(both Plt;0.05).Compared to groups Ⅰ, Ⅱ and Ⅲ,more patients in the group Ⅳ needed anesthetics after the operation (Plt;0.05).No significant diference was noticed in the incidence rates of shoulder or back pain and nausea or vomiting among the four groups (Pgt;0.05).Conclusions: Intraperitoneal local anesthetic can significantly reduce postoperative pain after LC. It is more effective to give local anesthetic at the end of the procedure than using it before operation. The effect of ropivacaine is better than bupivacaine.

  • Meta-Analysis of Clinical Efficacy and Safety about Hyperthermic Intraperitoneal Perfusion Chemotherapy in Treatment of Advanced Colorectal Cancer

    Objective To investigate the clinical efficacy and safety of hyperthermic intraperitoneal perfusion chemotherapy (HIPC) in treatment of advanced colorectal cancer.Methods The Meta-analysis was applied to analyze 8 randomized controlled quantitative studies published at domestic and abroad. These patients treated by HIPC after radical operation with colorectal cancer were included for the treatment group, and those treated only by radical operation with colorectal cancer for the control group. Relative risk (RR) of outcome variable of 3-year and 5-year survival rate and safety between the two groups were compared. Results There were 8 selected literatures, including 1 501 cases, in which 765 cases for treatment group, and 736 cases for control group. RR of 5-year survival rate of the total patients was 2.39 (95% CI: 1.66-3.45). RR of 3-year survival rate of the total patients was 2.13 (95% CI: 1.45-3.13). The results demonstrated that HIPC could improve 5-year and 3-year survival rate, and sensitivity analysis confirmed the conclusions more reliable. The security was described in 5 literatures, the available information showed smaller potential security issue. Conclusions HIPC after radical operation of advanced colorectal cancer can increase 5-year and 3-year survival rate of patients, improve the prognosis of patients. Whether patients with increased incidence of postoperative complications related to the HIPC is no clear-cut conclusions for lack of related research.

  • Comparison of Laparoscopic Total Peritoneum Intraperitoneal Onlay Mesh and Trans-Abdominal Preperitoneal Laparoscopic Mesh in Inguinal Hernioplasty

    Objective To study the superiority and efficiency of total peritoneum intraperitoneal onlay mesh (TPIPOM) in laparoscopic inguinal hernioplasty. Methods One hundred and five cases of laparoscopic inguinal hernioplasty with TPIPOM and 34 cases of inguinal hernioplasty with trans-abdominal preperitoneal laparoscopic mesh repair (TAPP) were performed from January 2002 to August 2005. Perioperative data and follow-up results were collected and compared in two groups. Results The laparoscopic hernioplasty was successfully performed in all patients. The total operation time, hospital stay, average off-bed time, duration of pain in TPIPOM group were significant shorter than those in TAPP group 〔(30.8±10.3) min vs (68.4±22.4) min, (3.8±1.3) d vs (4.3±1.5) d, (1.2±0.5) d vs (1.8±0.7) d, (1.0±0.5) d vs (1.6±0.9) d, respectively〕, P<0.01, the total hospital cost was RMB 5 000.8±800.5 in TPIPOM group and that was RMB 8 000.5±950.6 in TAPP group (P<0.01). No significant scrotal edema was observed postoperatively and no recurrence reported during (18.6±8.9) months follow-up in both groups. Conclusion Laparoscopic inguinal hernioplasty with TPIPOM is safe and efficacy with advantages of mini-invasion, simple procedures, shorter operation time, no complications and better recovery.

  • Design and Fabrication of a Device for Intraperitoneal Treatment of Pediatric Hernia

    For treatment of pediatric inguinal hernia, we fabricated a device, i.e. so called "filling type pediatric hernia sac", which treats the problem from the abdominal cavity, through the abdominal and is a self-adaptive closer, using synthetic material. The device includes filling rack, self-adaptive umbrella support bar, bottom piece, outside pulling line and device fixing lines. The filling rack is composed of 2 concentric circles of 3.0 cm diameter with peripherally fixed together and can be pulled into the shapes of a ball or an olive. The supporting bar is structured of 3 pieces with 0.5 cm wide, 4.0 cm long, cross-fixed on top of the filling rack. The bottom piece is in a circular structure with a diameter of 3.0 cm, and it is connected to the filling rack bottom. Adjust positioning stay outside the fixed on the top of the device are connected at one end, and the other end free through filling the top frame connected with the bottom slice of central fixation. By using this device, we treated 37 pediatric inguinal hernia cases with 38 side-inguinal hernia successfully. The mean duration of post-operation follow-ups was 14.6±5.89 months, without hernia recurrence, obvious scar and hard sections of inguinal region. This device could provide a convenient, safe and effective plugging technology for children's pediatric hernia.

  • Clinical Effect of Combination of Cytoreductive Surgery and Hyperthermic Intraperi-toneal Chemotherapy for Treating Hepatocellular Carcinoma with Peritoneal Metastasis

    ObjectiveTo investigate the curative effects of cytoreductive surgery (CRS) combined with hyper-thermic intraperitoneal chemotherapy (HIPEC) for treating the hepatocellular carcinoma with peritoneal metastasis. MethodsThe clinical data of 80 cases of hepatocellular carcinoma with peritoneal metastasis who were treated in our hospital from January 2004 to January 2012 were collected, and were classified into observation group (n=40) and control group (n=40) according to the treatment. Case of observation group received CRS+HIPEC, and cases of control group received CRS+conventional intraperitoneal injection of chemotherapy. Then the comparison of clinical effect and prog-nosis was performed. ResultsIn observation group, the results of completeness of cytoreduction (CC) grade was:31 cases in grade 0, 6 cases in grade 1, and 3 cases in grade 2. In control group, the results of CC grade was:32 cases in grade 0, 6 cases in grade 1, and 2 cases in grade 3. There was no significant difference between 2 groups in the CC grade (P=0.213). In addition, there were no significant differences between observation group and control group in operation time (6.8 hours vs. 6.5 hours), hospital stay (17.3 days vs.18.7 days), and incidence of adverse reactions[70.0% (28/40) vs. 60.0% (24/40)], P>0.05. All of the cases of observation group and control group were followed up for 10-61 months (average of 42.5 months) and 6-49 months (average of 30.2 months) respectively. During the follow up period, in observation group, there were 18 cases died, 12 cases suffered from recurrence, 5 cases suffered from metastasis, and the rest of 5 cases survived; in the control group, there were 26 cases died, 9 cases suffered from recurrence, and 5 cases suffered from metastasis. However, the survival situation was better in observation group than that of control group (P<0.01). ConclusionCombining CRS and HIPEC for treating hepatocellular carcinoma with peritoneal metastasis was safe and effective, which would be widely applied.

  • Updates of European clinical trials in hyperthermic intraperitoneal chemotherapy for gastric and colorectal cancers

    Hyperthermic intraperitoneal chemotherapy (HIPEC) has been used in clinical setting, and is one of the optional treatment for peritoneal surface tumors. It can be used as adjuvant therapy to prevent peritoneal recurrence after gastric or colorectal cancer resection, or to treat those diseases with peritoneal metastasis alone through cytoreductive surgery +HIPEC or HIPEC alone, based on a multidisciplinary model. The updates of European HIPEC-related clinical trials, GASTRIPEC, GASTRICHIP, PRODIGE 7, PROPHYLOCHIP, COLOPEC, COMBATAC, were reported at the 11th International Workshop on Peritoneal Surface Malignancy. In those trials, there was no definitive result surporting that HIPEC treatment might bring survival benefits to patients with gastric or colorectal cancer. However, long-term follow-up results remain to be seen, and some studies are still recruiting. Although several studies were designed as phase Ⅲ trials, the overall sample size was small-scaled. In addition, in the trials, diagnostic laparoscopy were widely used in gastric or colorectal cancer patients, which was helpful to improve staging accuracy and optimizing treatment strategies. The indications for HIPEC therapy (peritoneal cancer index) and technical issues (duration, temperature, approach, and agents) need further investigate.

  • Amylase value in drains after pancreaticoduodenectomy as predictive factor of grade B and C postoperative pancreatic fistula

    ObjectiveTo investigate predictive value of amylase level in intraperitoneal drainage fluid after pancreaticoduodenectomy (PD) for grade B and C pancreatic fistula (PF).MethodThe clinical data of 106 patients underwent PD in the First Affiliated Hospital of Xinjiang Medical University from June 2014 to June 2018 were retrospectively analyzed.ResultsTwenty-four cases (22.6%) had the PF after the PD, in which of 18 cases of grade A, 4 cases of grade B, and 2 cases of grade C. The age, tumor diameter, pancreatic texture, and main pancreatic duct diameter were associated with the PF after the PD by the univariate analysis (P<0.050), the main pancreatic duct diameter (OR=4.856, P=0.004) was its independent risk factor. The amylase levels of intraperitoneal drainage fluid on day 5, 7, and 10 after the PD could better predict the occurrence of PF (AUC was 0.910, 0.951, 0.948, respectively), especially which on day 10 after the PD could better predict the occurrence of grade B and C PF (AUC was 0.938).ConclusionElevated amylase levels in peritoneal drainage fluid on day 5, 7, and 10 after PD have better diagnostic effects on PF, especially which on day 10 after PD has a better diagnostic effect on grade B and C PF.

  • MDT discussion of a case of intraperitoneal mass after associating liver partition and portal vein ligation for staged hepatectomy

    ObjectiveTo summarize experiences of diagnosis and treatment of intraperitoneal mass after associating liver partition and portal vein ligation for staged hepatectomy (ALPPS) in treatment of hepatocellular carcinoma (HCC).MethodThe clinicopathologic data of a 40 years old case of HCC with intraperitoneal mass after ALPPS in the First Affiliated Hospital of Guangxi Medical University were retrospectively analyzed.ResultsThe patient was admitted to this hospital because of abdominal pain and abdominal distension for 5 d. The intraperitoneal mass was found and its nature was not clear on year 1 after ALPPS on admission. After discussion of multidisciplinary team (MDT), the exploratory laparotomy and abdominal tumor resection were planned to perform. The intraperitoneal mass and appendix were removed, the resected tissues were diagnosed as chronic inflammation and retroperitoneal abscess caused by perforation of suppurative appendicitis, respectively. The anti-infection and symptomatic support treatment were strengthened to perform after operation, the patient discharged after recovery. The patient was followed up so far, the general condition was good, and there was no clinical recurrence.ConclusionsFor patient underwent ALPPS, regular follow-up should be paid attention to. If intraperitoneal mass is found and nature is not clear, MDT discussion should be performed so as to make a more reasonable treatment plan. After exclusion of contraindications, surgical treatment should be carried out to furthest benefit patients.

    In recent years, cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) have been increasingly used for the treatment of peritoneal metastases. Imaging examination plays an important role in the process of CRS+HIPEC in treatment of peritoneal metastasis. This article briefly introduces the preoperative imaging evaluation, postoperative imaging evaluation, and current limitations of CRS+HIPEC in the treatment of peritoneal metastases.

  • Current situation and analysis of factors affecting non-mechanical tube blockage during hyperthermic intraperitoneal chemotherapy

    ObjectiveTo understand the current situation and factors affecting tube blockage (non-mechanical) during hyperthermic intraperitoneal chemotherapy (HIPEC). MethodsAccording to the inclusion and exclusion criteria, the patients with malignant tumors who underwent HIPEC in the Department of Gastroenterology of West China Hospital of Sichuan University from May 2019 to May 2021 were retrospectively gathered. The information about the patient and the occurrence of occlusion during HIPEC were obtained by consulting electronic medical records and perfusion records. The logistic regression analysis was performed to analyze the factors influencing non-mechanical tube blockage during HIPEC. ResultsA total of 240 patients with malignant tumors were gathered. During HIPEC, the non-mechanical tube blockage occurred in 88 patients with malignant tumors, with the incidence of 36.7%. The multivariate analysis results by logistic regression showed that the probabilities of non-mechanical tube blockage during HIPEC were higher in the patients with age≥65 years (OR=2.142, P=0.016), diabetes mellitus (OR=2.326, P=0.007), perfusion speed of 300–450 mL/min (OR=2.778, P=0.001), ascites (OR=2.192, P=0.020), and PCI ≥20 points (OR=4.380, P<0.001). ConclusionsPatients with malignant tumors treated with HIPEC are prone to non-mechanical tube blockage. The patients with middle-aged and elderly, diabetes, low perfusion speed, ascites, and high PCI score need to be of great concern, so as to prevent and deal with tube blockage in time.
