  • Evidences-based thyroid disease

    Objective To review all published evidence about thyroid disease based on Meta-analysis. Methods We thoroughly searched the MEDLINE and Cochrane Library combined with Meta-analysis and thyroid disease as key words. Results Total 22 articles were obtained but only 14 papers reached our aim. Conclusion These studies ranged from etiology of thyroid cancer, levothyroxine treatment, to Graves’ disease and have provided excellent evidences about some clinical questions. But there still are a lot of questions need us study further.

  • The Change of Spectrum of Thyroid Diseases Within Ten Years Implementation of Universal Salt Iodization in Guangxi Zhuang Autonomous Region

    Objective To reveal the relationship between the iodine nutrition and the change of spectrum of thyroiddiseases by analyzing the change of spectrum of thyroid diseases in different iodine environments before and after the implementation of universal salt iodization (USI). Methods To compare the urinary iodine concentration between the normal people (1 000 cases) and the patients with thyroid diseases (5 998 cases) by surgical operations who were from 4 cities of Guangxi Zhuang Autonomous Region, covering the iodine deficient areas and the iodine rich areas. Results After the USI was put into practice, the proportions of nodular goiter decreased, but the proportions of toxic nodular goiter, thyroid papillary carcinoma, and Hashimoto thyroiditis were higher than those before USI (P<0.05). The urinaryiodine concentrations of nodular goiter, Graves disease, toxic nodular goiter, thyroid papillary carcinoma, and Hashimotothyroiditis were higher than those before the measure was taken (P<0.05). The urinary iodine concentration of patients with thyroid was higher than that of normal people (P<0.05), and the urinary iodine concentration of patients with thyroidand normal people was higher than those before the USI (P<0.05). Conclusions The change of spectrum of thyroid diseases in Guangxi Zhuang Autonomous Region is obvious within 10 years after USI had been taken. The excessive intake of iodine may be one of the dangerous factors leading to toxic nodular goiter, thyroid papillary carcinoma, and Hashimoto thyroiditis.

  • Parathyroid Micro Vascular Anatomy and Thyroid Lobectomy with Capsular Technique

    Objective To explore the significance of parathyroid micro vascular anatomy in thyroid lobectomy with capsular technique. Methods The pertinent literatures in recent thirty years were screened with key words “parathyroid micro vascular anatomy, capsular technique, and protection”and reviewed. Results There were many types of number, origin, and length of parathyroid vascular, and specific measurements should be taken in thyroid lobectomy with capsular technique. Conclusion Fully awareness of parathyroid micro vascular anatomy will benefit to ensure preservation of their function during thyroid lobectomy with capsular technique.

  • Application on The Approach of Transecting Anterior Cervical Muscle Group at Sternal End for Removal of Bilat-eral Huge Thyroid Neoplasm

    Objective To explore the advantage of transection the cervical muscles at sternal end and flip fixed in therapeutic bilateral huge thyroid surgery. Methods The transection of the cervical muscles at sternal end and flip fixed in 53 cases was observation group, 44 cases of the neck white line incision thyroid surgery completed for the control group. The completion of the surgery by the same group of physicians. The operative time, operation field of exposing effect, amount of bleeding in operation, postoperative complications, and postoperative drainage volume were compared between two groups. Results There was no statistically differences of sex, age, disease composition, and tumor size between two groups (P>0.05). Operative time, amount of bleeding, and postoperative drainage volume in observation group were shorter (less) than that in control group (P<0.01). The postoperative complication rate in observation group was lower than that in control group(P=0.04). Surgical field exposure in observation group was better than that in control group (P<0.01). Conclusions The huge bilateral thyroid surgery with the sternal end approach is feasible and simple. The operation field exposure is better than the white line neck incision, complications after operation is less. It is worthy of clinical application.

  • Diagnosis and Treatment for Primary Hyperparathyroidism with Concomitant Thyroid Diseases

    Objective To investigate the clinical diagnosis and treatment for primary hyperparathyroidism with concomitant thyroid diseases. Methods The clinical data of 40 cases diagnosed as primary hyperparathyroidism with concomitant thyroid diseases including manifestation, preoperative qualitation and localization, and surgical treatment and results were retrospectively analyzed. Results The 40 cases were composed of 4 parathyroid adenomas with thyroid papillary carcinomas, 28 parathyroid adenomas with nodular goiters, 6 parathyroid adenomas with thyroid adenomas, 1 parathyroid hyperplasia with nodular goiter, and 1 parathyroid carcinoma with thyroid adenoma. The diagnostic sensitivities of localization for primary hyperparathyroidism with concomitant thyroid diseases by ultrasound, by computerized tomography (CT), and by radioisotope (99Tcm) scanning were 82.5% (33/40), 80.0% (32/40) and 90.0% (36/40), respectively, and the combined sensitivity was 97.5% (39/40). The surgical treatments included resection of parathyroid adenoma with subtotal thyroidectomy in 34 cases, resection of parathyroid adenoma with total thyroidectomy in 3 cases, bilateral exploration of parathyroid with subtotal thyroidectomy in 1 case, and unilateral parathyroidectomy with thyroidectomy and neck lymphonodes clearance in 2 cases. ConclusionFor primary hyperparathyroidism with concomitant thyroid diseases, the sensitivity of preoperative localization could be raised by combining ultrasound, CT with radioisotope scanning, and surgical resection is the main treatment, which includes the main operation of resection of parathyroid adenoma with subtotal or total thyroidectomy.

  • Effects of DNA Electrotransfer in Muscle on the Implanted Tumor Growth in Nude Mice

    【Abstract】ObjectiveTo study the antitumor effects of DNA electrotransfer in muscle (ETM) by using established animal model for human anaplastic thyroid cancer cell line TA-K.MethodsNude mice with implanted TA-K were divided into five groups including: control group, pcDNA-3 plasmid electrotransfered into muscle (pcDNA ETM group), TIMP-3 plasmid injected into muscle (TIMP-3 IM group), TIMP-3 plasmid electrotransfered into muscle (TIMP-3 ETM group), TIMP-3 plasmid electrotransfered into implanted tumor (TIMP-3 ETT group). Electrical parameters used in electrotransfer were: 200 V/cm, 20 ms/pulse; 8 pulses, 1 Hz in muscle and 600 V/cm, 20 ms/pulse; 1 pulses, 1 Hz in implanted tumor respectively. ResultsThe growth of TA-K was inhibited more significantly in the groups of TIMP-3 plasmid electrotransfered into muscle and TIMP-3 plasmid electrotransfered into implanted tumor than the other groups (P<0.05), and the content of TIMP-3 protein in tumor tissues was higher in these two groups . ConclusionAnti-oncogene has the antitumor effects by DNA electrotransfer in muscle.

  • Experimental Study on Allotransplantation of Cultured Parathyroid Cells in Rats

    【Abstract】Objective To study the influence of transplantation of cultured parathyroid cells on the survival of the allografts in rats. Methods Parathyroid cells digested with collagenase and trypsin were cultured and transplanted under the left renal capsule. The survival time of the allografts was recorded and the allografts were examined by transmission electron microscopy.Results In fresh parathyroid cells group, the mean survival time was (9.25±3.45) days. While in cultured parathyroid cells group, the survival time was (46.25±7.44) days (P<0.01). During the 50 days of observation, serum calcium and PTH remained normal in 6 of 8 rats. There were intact parathyroid cells in the allografts which had abundant rough endoplasmic reticula,mitochondria and secretory granules. Conclusion Transplantation of cultured parathyroid cells in rats can prolong the survival time of allografts and is a potent way to cure hypoparathyroidism.

  • Studies on Induction of Experimental Autoimmune Thyroiditis with Excessive Iodine and Thyroglobulin in Rats

    【Abstract】Objective To study the relation between iodine and experimental autoimmune thyroiditis(EAT). Methods Establishment of animal model was performed with iodine and thyroglobulin(TG).The rats were randomly divided into 5 groups: normal control group(NC), low iodine group (LI,500 μg/L), high iodine group(HI,500 mg/L), TG+Freund adjuvant group(TG) and TG+Freund adjuvant+HI group(TG+HI). The rats in TG group and TG+HI group were rejected hypodermically with TG emulsified by complete Freund adjuvant, and strengthen immunity was conducted with TG emulsified by incomplete Freund adjuvant on 15 days. After that, strengthen immunity was done weekly till the end of the experiment. Serum TGAb and TPOAb were measured by radioimmunoassay. Observation of the pathological changes of thyroid gland was also done. Results Thyroid follicular destruction and lymphocytic infiltration in the TG+HI group (3.83±1.72) and HI group (3.00±0.89) were significantly higher than that of the NC group(0.67±0.82),P<0.05. The results of the TG group were higher compared with the NC group, but there were no significant differences between them(Pgt;0.05). The levels of TGAb in the TG+HI (4.990±1.505),HI (3.589±1.240) and TG group (4.883±1.198) were significant higher than those of the NC group (0.642±0.454) and the LI group (0.707±0.240),P<0.01. The levels of TPOAb in TG+HI group (1.475±0.523) and TG group (1.316±0.606) were significantly higher than those of the NC group (0.365±0.196) and the LI group(P<0.01). Serum TGAb and TPOAb levels were positively correlated with the histological grades of lymphocytic thyroiditis(r=0.9,P<0.05). Conclusion Excessive iodine intake may induce the occurrence of EAT. The induction of EAT with excessive iodine and TG may be more efficient.

  • Total Thyroidectomy for Benign Thyroid Disease

    【Abstract】Objective To investigate the safety and clinical significance of total thyroidectomy performed for benign thyroid disease. Methods Eighty-eight patients with benign thyroid disease were treated with total thyroidectomy. The postoperative complications were analyzed. Results With primary total thyroidectomy, the incidences of transient hypocalcemia and transient recurrent laryngeal nerve paralysis were 2.5% and 1.2% respectively. The incidences after reoperation were 28.6%(P<0.05)and 28.6%(P<0.01)respectively. No patients had permanent hypoparathyroidism and permanent recurrent laryngeal nerve injury. Conclusion Total thyroidectomy can be performed safely. It can avoid reoperation for the recurrence caused by the remainder thyroid.

  • INTERVENTIONAL THERAPY FOR HYPERTHYROIDISM

    Objective To evaluate the safety and effectiveness of interventional therapy for hyperthyroidism. Methods From 1995 to 2000, 38 cases underwent bilateral super-selective superior thyroid arteries embolization with brown-algae microballs.Results There was no misembolization and mortality. Hyperthyroidism crisis developed in 2 cases. Thirty eight cases were followed-up for 0.5~5 years (the median time was 2.2 years). Medications were needed only in one patient because of relapse and the others were cured. Conclusion This procedure features miniinvasive trauma, less complications and quick recovery. It may be a safe and rational treatment for hyperthyroidism.
