  • Early Prevention of Severe Acute Pancreatitis Complicated with Infection

    Objective To explore the early prevention measures of severe acute pancreatitis (SAP) complicated with infection. Methods The foreign and native related literatures of SAP complicated with infection in recent 10 yearswere retrieved, and the effect of sooner measures for SAP complicated with infection were discussed. Results It was beneficial to reduce the incidence of infection and improve the outcome in SAP by using antibiotics of apt through the blood-pancreatic barrier, using proton pump inhibitor properly, and using enteral nutrition, hemofiltration, and hyperbaric oxygen as soon as possible, and then combined with traditional Chinese medicine. Conclusions SAP complicated with infection is the one of the main reasons of death. It is a must to take effective and comprehensive measures in the early stage in order to reduce the rate of infection. However, there are several unclear problems to probe in the future on this issue.

  • Research of TAP Induction The Pancreatic Acinar Cells to Release HMGB1 in Rat

    Objective To explore the secretion law of high mobility group box 1(HMGB1)in rat pancreatic acinar cells induced by trypsin activation peptide(TAP)and release of HMGB1 affected by ethyl pyruvate(EP). Methods The experiment was performed in 12 SD rats. The pancreatic acinar cells of rats were taken out and then separated into three groups:control group, TAP group, and EP group. TAP was added into TAP group and EP group(keep TAP at a final concentration of 3 nmol/L), respectively, but EP was added into EP group only (keep EP at a final concentration of 28 mmol/L). The expressions of HMGB1 mRNA and protein were detected by using real-time quantitative reverse transcription polymerase chain reaction(RT-PCR)or Western blot at 3 h, 6 h, 12 h, and 24 h time point, respectively. The relationship between HMGB1 and TAP action time was explored by rank correlation. Results Compared with control group, the expressions of HMGB1 mRNA and protein were increased with prolongation of the TAP action in TAP group and EP group(P<0.05). Compared with TAP group, the expressions of HMGB1 mRNA and protein were decreased in EP group(P<0.05). The expressions of HMGB1 mRNA and protein were increased with prolongation of the TAP action(P<0.05), and were highest at 12 h time point(P<0.01)in TAP group. There were positive correlation between the expressions of HMGB1 mRNA and protein and TAP action time(rs=0.971, P<0.01;rs=0.966, P<0.01).Conclusions TAP can induce the release of HMGB1 in pancreatic acinar cells. There is positive relationship between TAP in early stage and HMGB1 in later period of acute pancreatitis. EP can inhibit the release of HMGB1.

  • Progress on reconstruction of proximal femur in the hemiarthroplasty for intertrochanteric fracture with distal fixated long stem

    ObjectiveTo summarize the application of distal fixated long stem in the treatment of intertrochanteric fracture (ITF) in the sequence of proximal femoral reconstruction, fixation materials, and other details after operation, in order to improve doctor’s attention to the reconstruction of the proximal femur and reduce complications.MethodsThe related literature about the application of distal fixated long stem in the treatment of ITF was extensively reviewed, summarized, and analyzed.ResultsThe sequence of reconstruction is divided into fracture reconstruction priority and prosthesis reconstruction priority. The former is mainly to provide an anatomical reference for the placement of joint prostheses, the latter is mainly to provide support for fracture fixation. The distal fixated cement long stem and cementless long stem have their own characteristics, and materials of reconstruction are used in combination. There is no uniform standard for the sequence and materials of reconstruction.ConclusionAlthough the stability of the distal fixated long stem depends on the distal femur, the accurate reconstruction of the proximal femur is still worthy of attention.

  • Construction of influence evaluation index system for key medical disciplines/specialties in Sichuan Province

    ObjectiveTo establish an evaluation index system suitable for key medical disciplines/specialties in Sichuan Province, with the coverage of tertiary-level comprehensive and tertiary specialty hospitals, and the provincial medical key disciplines/specialties as the starting point, so as to promote the development of key medical disciplines in Sichuan Province.MethodsThe literature method, expert meeting method, Delphi method, and analytic hierarchy process were used to establish the index system and the corresponding weights of each index.ResultsAn evaluation index system for the influence ranking of key medical disciplines/specialties in Sichuan Province was established, covering scientific and technological inputs and outputs, clinical services, and industry influence. The entire evaluation index system included 3 first-level indicators, 8 second-level indicators, 19 third-level indicators and corresponding weights.ConclusionThis evaluation index system has applied to rank the influence of key medical disciplines/specialties in Sichuan Province, laying a solid foundation for the influence evaluation of the key medical disciplines/specialties and the future platform construction in Sichuan Province.

  • Visualization analysis of microfluidics research status

    Microfluidics is the science and technology to manipulate small amounts of fluids in micro/nano-scale space. Multiple modules could be integrated into microfluidic device, and due to its advantages of microminiaturization and controllability, microfluidics has drawn extensive attention since its birth. In this paper, the literature data related to microfluidics research from January 1, 2006 to December 31, 2021 were obtained from Web of Science Core Collection database. CiteSpace 5.8.R3 software was used for bibliometrics analysis, so as to explore the research progress and development trends of microfluidics research at home and abroad. Based on the analysis of 50 129 articles, it could be seen that microfluidics was a hot topic of global concern, and the United States had a certain degree of authority in this field. Massachusetts Institute of Technology and Harvard University not only had a high number of publications, but also had strong influence and extensive cooperation network. Combined with ultrasonic, surface modification and sensor technology, researchers constructed paper-based microfluidic, droplet microfluidic and digital microfluidic platforms, which were applied in the field of immediate diagnosis, nucleic acid and circulating tumor cell analysis of in vitro diagnosis and organ-on-a-chip. China was one of the countries with a high level of research in the field of microfluidics, while the industrialization of high-end products needed to be improved. As people’s demand for disease risk prediction and health management increased, promoting microfluidic technological innovation and achievement transformation is of great significance to safeguard people’s life and health.

  • Short-term effectiveness of TiRobot combined with O-arm navigation system in minimally invasive treatment of hindfoot fracture

    Objective To investigate the short-term effectiveness of TiRobot combined with O-arm navigation system in the minimally invasive treatment of hindfoot fracture. Methods Between March 2019 and March 2021, 25 patients with hindfoot fractures were admitted. There were 14 males and 11 females, with an average age of 51.7 years (range, 19-76 years). The causes of injuries included falling from height in 17 cases and traffic accident in 8 cases. The interval between injury and operation was 1-3 days (mean, 2.1 days). There were 16 cases of calcaneus fracture, 7 cases of talus fracture, and 2 cases of calcaneus and talus fractures. According to Sanders classification criteria, the calcaneus fractures were classified as type Ⅱ in 10 cases and type Ⅲ in 8 cases; according to the Hawkins classification criteria, the talus fractures were classified as type Ⅱ in 4 cases and type Ⅲ in 5 cases. Preoperative American Orthopaedic Foot and Ankle Society (AOFAS) ankle-hindfoot score was 48.1±9.1. During operation, the fractures were fixed with the percutaneous cannulated screws with the assistance of the TiRobot combined with the O-arm navigation system. The operation time, hospital stay, and the occurrence of related complications were recorded. X-ray films were reviewed to evaluate the fracture healing and the occurrence of talus osteonecrosis, and the width, length, height, Böhler’s angle, and Gissane’s angle of the calcaneus were measured; AOFAS ankle-hindfoot score was used to evaluated the foot function. Results The operation time ranged from 47 to 71 minutes (mean, 60.5 minutes). The length of hospital stay ranged from 2 to 5 days (mean, 3.4 days). All incisions healed by first intention. All patients were followed up 12-24 months (mean, 17.3 months). One patient demonstrated hypoesthesia on the lateral side of foot after operation and recovered after symptomatic treatment. All fractures healed confirmed by X-ray films and the healing time ranged from 10 to 16 weeks (mean, 11.8 weeks). No talus osteonecrosis occurred during follow-up. There were significant differences in the width, length, height, Böhler’s angle, and Gissane’s angle of the calcaneus between pre-operation and at last follow-up (P<0.05). At last follow-up, AOFAS ankle-hindfoot score was 91.2±5.0, the difference was significant when compared with preoperative score (t=22.169, P<0.001). The results were excellent in 16 cases and good in 9 cases, with an excellent and good rate of 100%. Conclusion TiRobot combined with O-arm navigation system for minimally invasive treatment of hindfoot fractures can obtain the satisfactory short-term effectiveness, with the advantages of less surgical trauma, precise fixation, and fewer complications.
